Abstract

The pivotal LYM-3002 study compared frontline rituximab plus cyclophosphamide, doxorubicin, vincristine and prednisone (R-CHOP) with bortezomib, rituximab, cyclophosphamide, doxorubicin and prednisone (VR-CAP) in newly diagnosed mantle cell lymphoma (MCL) patients for whom stem cell transplantation was not an option. This post hoc subanalysis of the VR-CAP data from LYM-3002 evaluated the effect of bortezomib dose intensity on OS in patients who completed ≥6 cycles of treatment. From the end of cycle 6, patients receiving ≥4.6 mg/m2/cycle of bortezomib had significantly longer OS (but not PFS) compared with those receiving <4.6 mg/m2/cycle by univariate analysis (HR 0.43 [95% CI: 0.23-0.80]; p = .0059). This association remained significant in multivariate analysis adjusting for baseline patient and disease characteristics (HR 0.40 [95% CI: 0.20-0.79]; p = .008]. Abstract

On the basis of a remote effect, a series of acetoacetamide ligands and corresponding nickel complexes N-(R-phenyl) acetoacetamide Ni(CH2Ph) (PMe3) (R = H, 1; R = 2-methyl, 2; R = 2,6-dimethyl, 3; R = 2,6-diisopropyl, 4; R = 4-NO2, 5) were synthesized and characterized. The solid structure of complex 3 was confirmed by X-ray single-crystal analysis to be of cis form. 1H and 31P NMR spectroscopy confirmed that cis and trans isomers of nickel complexes were present in solution. Norbornene with acetoacetamide nickel complexes activated with modified methylaluminoxane (MMAO) were investigated in detail. Remote steric and electronic effects of acetoacetamide ligand on and molecular weight of polynorbornenes (PNBs) were observed. 147. A sensitive method for determination of furanodiene in rat plasma using liquid chromatography/tandem mass spectrometry and its application to a pharmacokinetic study.

Abstract

ABSTRACT Furanodiene, a sesquiterpene component extracted from the essential oil of the rhizome of Curcuma wenyujin Y.H. Chen et C. Ling (Wen Ezhu), is widely used in traditional Chinese medicine. A sensitive analytical method was established and validated for furanodiene in rat plasma, which was further applied to assess the pharmacokinetics of furanodiene in rats receiving a single dose of furanodiene. Liquid chromatography tandem mass spectrometry (LC/MS/MS) in multiple reaction monitoring mode was used in the method and costundide was used as internal standard. A simple protein precipitation based on methanol was employed. The simple sample cleanup increased the throughput of the method substantially. The method was validated over the range of 1-1000 ng/mL with a correlation coefficient >0.99. The lower limit of quantification was 1 ng/mL for furanodiene in plasma. Intra- and inter-day accuracies for furanodiene were 88-115 and 102-107%, and the inter-day precision less than 14.4%. After a single oral dose of 10 mg/kg of furanodiene, the mean peak plasma concentration of furanodiene was 66.9 ± 23.4 ng/mL at 1 h, the area under the plasma concentration-time curve (AUC0-10 h) was 220 ± 47.8 h ng/mL, and the elimination half-life was 1.53 ± 0.06 h. After an intravenous adminstration of furanodiene at a dosage of 5 mg/kg, the area under the plasma concentration-time curve was 225 ± 76.1 h⋅ng/mL, and the elimination half-life was 2.40 ± 1.18 h. Abstract

We introduce a dose-finding algorithm to be used to identify a level of dose that corresponds to some given targeted response. Our motivation arises from problems where the response is a continuously measured quantity, typically some pharmacokinetic parameter. We consider the case where an agreed level of response has been determined from earlier studies on some population and the purpose of the current trial is to obtain the same, or a comparable, level of response in a new population. This relates to bridging studies. The example driving our interest comes from studies on drugs for HIV that have already been evaluated in adults and where the new studies are to be carried out in children. These drugs have the ability to produce some given mean pharmacokinetic response in the adult population, and the goal is to calibrate the dose in order to obtain a comparable response in the childhood population. In practice, it may turn out that the dose producing some desired mean response is also associated with an unacceptable rate of toxicity. In this case, we may need to reevaluate the target response and this is readily achieved. In simulations, the algorithm can be seen to work very well. In the most challenging situations for the method, those where the targeted response corresponds to a region of the dose–response curve that is relatively flat, the algorithm can still perform satisfactorily. Abstract

The gastrointestinal (GI) tract is an organ actively involved in mechanical processes, where it detects forces via a mechanosensation mechanism. Mechanosensation relies on specialized cells termed mechanoreceptors, which convert mechanical forces into electrochemical signals via mechanosensors. The mechanosensitive Piezo1 and Piezo2 are widely expressed in various mechanosensitive cells that respond to GI mechanical forces by altering transmembrane ionic currents, such as epithelial cells, enterochromaffin cells, and intrinsic and extrinsic enteric neurons. This review highlights recent research advances on mechanosensitive Piezo channels in GI physiology and pathology. Specifically, the latest insights on the role of Piezo channels in the intestinal barrier, GI motility, and intestinal mechanosensation are summarized. Additionally, an overview of Piezo channels in the pathogenesis of GI disorders, including irritable bowel syndrome, inflammatory bowel disease, and GI cancers, is provided. Abstract

8034 Background: Tec is a B-cell maturation antigen × CD3 bispecific antibody currently being evaluated in the single-arm, phase 1/2 MajesTEC-1 trial (NCT04557098) in pts with RRMM who had received ≥3 prior lines of therapy (LOT) and were TCE to an immunomodulatory agent, a proteasome inhibitor, and an anti-CD38 monoclonal antibody. The aim of this study is to evaluate the comparative efficacy of tec versus physician’s choice of therapy, as no head-to-head trials have been conducted. Methods: An external control arm for MajesTEC-1 was created from pts in the long-term follow-up of 4 clinical trials of daratumumab (CASTOR, POLLUX, EQUULEUS, and APOLLO) who met the eligibility criteria for MajesTEC-1. These pts (N = 427) were subsequently treated with physician’s choice of therapy after discontinuing trial txs, and disease progression and best tx response were based on the investigator’s assessment. Individual pt-level data from MajesTEC-1 pts who received tec (1.5 mg/kg weekly) at a clinical cutoff of Sep 7, 2021 were included. Inverse probability of tx weighting (IPTW) with average tx effect on the treated population was used to adjust for imbalances in baseline covariates of prognostic significance: refractory status, progression on last LOT, cytogenetic risk, International Staging System stage, number of prior LOT, extramedullary plasmacytoma, time since diagnosis, age, and hemoglobin. Overall response rate (ORR), rate of complete response or better (≥CR), rate of very good partial response or better (≥VGPR), progression-free survival (PFS), time to next tx (TTNT), and overall survival (OS) were assessed. For binary endpoints (ORR, ≥CR rate, ≥VGPR rate), the relative effect of tec vs physician’s choice of therapy was estimated with an odds ratio (OR) and 95% confidence interval (CI) derived from a weighted logistic regression analysis. A weighted Cox proportional hazards model was used to compute hazard ratios (HRs) and 95% CIs for time-to-event endpoints (PFS, OS, and TTNT). Several sensitivity analyses were conducted. Results: After IPTW, baseline characteristics were comparable between the 2 cohorts. Pts treated with tec had improved outcomes vs physician’s choice of therapy: ORR (OR 4.58; 95% CI 2.83–7.53; P < 0.0001); ≥CR rate (OR 12.62; 95% CI 5.20–38.55; P< 0.0001); ≥VGPR rate (OR 11.64; 95% CI 6.49–21.98; P< 0.0001); PFS (HR 0.62; 95%CI 0.45–0.84; P= 0.0024); TTNT (HR 0.38; 95% CI 0.27–0.52; P< 0.0001); and OS (HR 0.47; 95% CI 0.32–0.69; P= 0.0001). Results were similar for all sensitivity analyses. Conclusions: Tec showed improved efficacy versus physician’s choice of therapy in all clinical outcomes, highlighting its therapeutic potential to address unmet needs in pts with TCE RRMM who received ≥3 prior LOT.
