Introduction: Bacteriuria of pregnancy is a common condition. Case report: Patient, 30-years, pregnant woman. During pregnancy, E. coli infection recurred in 4 times, applied Cephalexin and Ceftriaxone. The delivery was terminated by CS, GW 38; girl infant, AS 9. After the period of lactation: secretory status - the patient was a secretor of A and H blood type substance; ultrasonography and contrast radiography - presence of the third kidney. The therapy was added by vaccine UroVaxom, and there was no E. coli infection during 2 years follow up period. The Child is now 7 years old girl, having brilliant psychomotorical development. Possible child brain damage, lung damage, mental diseases are the reason for necessity E. coli infection treatment during pregnancy. Conclusion: All pregnant women should be screened for bacteriuria. E. coli is most commonly sensitive to group B antibiotics (cephalexin and amoxicillin), safe to be included in pregnancy. Long-term follow up of infants born from mothers having bacterial infection during pregnancy is necessary.

Since the beginning of 90s of last century both the petroleum industry and petrochemical industry have operated in difficult circumstances. In particularly, margins of petroleum and petrochemical industry were exacerbated during global economic crisis in 2008-2009 years. At that time, as one option that could be the solution, the global analysts had started to more intense investigate the benefits of Refining-Petrochemical Integration. Shortly afterwards, more and more petroleum refineries and petrochemical manufacturers began to see the future in this kind of operational, managerial, marketing and commercial connection. This paper evaluates, in particular, the achieved level of integration of refinery and petrochemical businesses in Central and South-Eastern Europe. And specifically, the paper identifies current capabilities and future chances of linking this kind of integration between Serbian refining and petrochemical players. The viability of integration between possible actors and benefits of every single refining-petrochemical interface in Serbia depend on many factors, and therefore each integrated system is unique and requires prior serious Cost Benefit Analysis.

BACKGROUND: The severity classification of functional mitral regurgitation (FMR) remains controversial despite adverse prognosis and rapidly evolving interventions. Furthermore, it is unclear if quantitative assessment with cardiac magnetic resonance can provide incremental risk stratification for patients with ischemic cardiomyopathy (ICM) or non-ICM (NICM) in terms of FMR and late gadolinium enhancement (LGE). We evaluated the impact of quantitative cardiac magnetic resonance parameters on event-free survival separately for ICM and NICM, to assess prognostic FMR thresholds and interactions with LGE quantification. METHODS: Patients (n=1414) undergoing cardiac magnetic resonance for cardiomyopathy (ejection fraction<50%) assessment from April 1, 2001 to December 31, 2017 were evaluated. The primary end point was all-cause death, heart transplant, or left ventricular assist device implantation during follow-up. Multivariable Cox analyses were conducted to determine the impact of FMR, LGE, and their interactions with event-free survival. RESULTS: There were 510 primary end points, 395/782 (50.5%) in ICM and 114/632 (18.0%) in NICM. Mitral regurgitationfraction per 5% increase was independently associated with the primary end point, hazards ratios (95% CIs) of 1.04 (1.01-1.07; P=0.034) in ICM and 1.09 (1.02-1.16; P=0.011) in NICM. Optimal mitral regurgitation-fraction threshold for moderate and severe FMR were =20% and =35%, respectively, in both ICM and NICM, based on the prediction of the primary outcome. Similarly, optimal LGE thresholds were =5% in ICM and =2% in NICM. Mitral regurgitation-fraction×LGE emerged as a significant interaction for the primary end point in ICM (P=0.006), but not in NICM (P=0.971). CONCLUSIONS: Mitral regurgitation-fraction and LGE are key quantitative cardiac magnetic resonance biomarkers with differential associations with adverse outcomes in ICM and NICM. Optimal prognostic thresholds may provide important clinical risk prognostication and may further facilitate the ability to derive selection criteria to guide therapeutic decision-making. Background/Aim. There are few tests for evaluation of functional abilities of patients surgically treated for hip fractures or osteoarthritis during early rehabilition period. The aim of this study was to investigate reliability (interobserver reproducibility and internal consistency) of the A-test, an original test for functional recovery evaluation during early rehabilitation of patients in an orthopedic ward. Methods. The investigation included 105 patients (55 patients with hip osteoarthritis that underwent arthroplasty and 50 surgically treated patients with hip fracture). It was conducted in an orthopedic ward during early inpatient rehabilitation (from 1st to 5th day). For their functional recovery evaluation during early rehabilitation we used the A-test, a performance-based test with 10 items for assessing basic activities by six level ordinal scale (0-5). For internal consistency of the test the Cronbach coefficient alpha was calculated for the A-test results collected during early rehabilitation for all patients (105 patients x 5 days = 525 measures) and separately for the results of patients with hip osteoarthritis (275 measures) and hip fracture (250 measures). Values of this coefficient > 0.7 imply good internal consistency of the test. Interobserver reproducibility was estimated as follows: two physiotherapists together conducted physical therapy with the patients, and then, separately, rated the performance of each activity from the test (78 measures). The agreement between their estimations was expressed by the linear weighted kappa coefficient (for very good agreement values of kappa coefficient have to be in the range 0.81-1). Results. The Cronbach coefficient alpha was 0.98 (the results of all the patients and the results of the patients with hip osteoarthritis) and 0.97 (the results of the patients with hip fracture). The values of kappa coefficient were in the range 0.81-0.92 for all items. Conclusion. The A-test is a reliable instrument for everyday evaluation of functional recovery during early rehabilitation of patients surgically treated in an orthopedic ward.

Background/Aim. The A-test was designed for assessment of functional recovery during early rehabilitation of patients in an orthopedic ward. This performance-based test consists of 10 items for assessing basic activities by a six level ordinal scale (0-5). Total scores can range from 0 to 50, i.e. from inability to perform any activity despite the help of therapists to complete independence and safety in performing all activities. The aim of this study was to examine the A-test validity. Methods. This prospective study was conducted in an orthopedic ward and included 120 patients [60 patients with hip osteoarthritis that underwent arthroplasty and 60 surgically treated patients with hip fracture (HF)] during early inpatient rehabilitation (1st-5th day). Validity was examined through 3 aspects: content validity - floor and ceiling effect, range, skewness; criterion validity - concurrent validity [correlation with the University of Iowa Level of Assistance Scale (ILAS) for patients with hip osteoarthritis, and with the Cumulated Ambulation Score (CAS) for patients with HF, Spearman rank correlation] and predictive validity [the New Mobility Score (NMS) 4 weeks after surgery, Mann-Whitney U test]; construct validity - 4 hypotheses: 1) on the fifth day of rehabilitation in patients underwent arthroplasty due to hip osteoarthritis, the A-test results will strongly correlate with those of ILAS, while the correlation with the Harris hip score will be less strong; 2) in patients with HF, the A-test results will be significantly better in those with allowed weight bearing as compared to patients whom weight bearing is not allowed while walking; 3) results of the A-test will be significantly better in patients with hip osetoarthritis than in those with HF; 4) the A-test results will be significantly better in patients younger than 65 years than in those aged 65 years and older. Results. The obtained results were: low floor (1%) and ceiling (2%) effect, range 0-50, skewness 0.57, strong correlation with ILAS for the patients with hip osteoarthritis (r = -0.97, p = 0.000) and with CAS for the patients with hip fracture (r = 0.91, p = 0.000) The patients with the A-test score 35 and more on the fifth day of rehabilitation (n = 46, Md = 4) had significantly higher NMS rank 4 weeks after surgery than the patients with the A-test score less than 35 (n = 59, Md = 2), (U = 379, z = -6.47, p = 0.000, r = 0.63). All 4 hypotheses were confirmed. Conclusion. The A-test is simple and valid instrument for everyday evaluation of pace and degree of functional recovery during early rehabilitation of patients surgically treated in an orthopedic ward.

Background/Aim. One of the possible complications after implantation of a cement hip-joint endoprosthesis is fracture in the endoprosthesis body. Fractures arise from overload or material fatigue of which an implant is made. The purpose of this research was to define the intensity of maximum stress and the positions of a critical cross-section in the endoprosthesis body. Methods. Unilaterally changing forces which act on the hip joint during walking as well as the loads result in flexible deformations of the endoprosthesis body. Biomechanical analysis of the forces acting on the hip joint determine their direction and intensity, whereas on the basis of Gruen’s classification of the endoprosthesis body loosening the level of fixation is established. The bodies of cement hip joint endoprosthesis are made of cobalt-chromiummolybdenum (CoCrMo) alloy, suitable for vacuum casting, are submitted to the analysis. Analysis of the critical stress in the endoprosthesis body was performed on the endoprosthesis body by means of the finite element method. The experimental verification of the obtained results was carried out on the physical prototype under laboratory conditions. Results. Computer analysis, by means of the finite element method, determined the stress state by calculation of the maximum Von Mises stress and critical cross-sections for different angles of the resultant force action. The results obtained by the computer and experimental method correlate and are comparable to the results of similar analyses conducted on various endoprosthesis types. Conclusion. The analyses described in the paper make the basis for improving the process designing of hip joint endoprostheses and their customization to each individual patient (custom made). Trichinellosis (trichinellosis) is a disease common for both people and animals, which was mentioned in even some several centuries old notes. As well as domestic pigs, wild boars, being omnivores and the animals that broadly cover the territory of Serbia, could definitely be considered as one of possible trichinellosis indicators, and that is the main reason for starting this investigation with the objective to perceive the level of infection of wild boars in different hunting areas at the territory of the Republic of Serbia. In this work it is presented the distibution of T. spiralis in wild boars at the territory of Serbia in the period from 2006. to 2010. Besides the distribution of trichinellosis in wild boars, we have also calculated the tendency of changing in the number of infected wild boars at the territories of different hunting associations, in the period from 2006. to 2010. The distribution of T. spiralis in shot wild boars showed that trichinellosis appeared at the total of 24 hunting associations territories during the investigated period. Only at the territories of Pirot and Dimitrivgrad hunting associations, there was diagnosed at least one case of trichinellosis in shot wild boars in all five years. Out of the total of shot and inspected wild boars (20.250) in 123 of them, there was diagnosed the presence of T. spiralis infectious forms (0,61%). The greatest number (29) of the infected wild boars was shot in 2007. By analysing the change in number of positive wild boars in the interval from 2006. to 2010. it was found out that there is a constant tendency of growth, namely 1,1 head of boar per year. Change in number of trichinellosis wild boars by years of observation is a second degree polynomial (Y=-0,6+17,81xi-2,79xi2) with correlation coefficient of 0,69. On the basis of the obtained analysis, that were carried out, it can be concluded that the occurence of trichinellosis is most frequent in border areas towards Bulgaria, Romania, Hungary, Croatia and Bosnia and Hercegovina. In this paper, performance analysis of diversity technique with equal gain combining method (EGC) with two branches for the detection of signals in wireless communication systems is presented. In the following analysis, it is assumed that the fading via channels is Nakagami-m correlated. The first order statistical characteristics of the system are analysed. Useful formulae for the probability density function (pdf) and cumulative distribution function (CDF) of EGC output SIR are derived, and the effects of the fading severity on the output signal are observed.

Background/Aim. Infected total knee arthroplasty (TKA) is a topic of great importance, because its diagnosing and treatment requires a lot of resources, and often has an unsatisfactory outcome. The aim of this study was to analyze the outcome of the treatment of infection developed following TKA. Methods. This retrospective study of infected TKAs was performed in the period from 1998 to 2008 in the Orthopedics & Traumatology Clinic of the Military Medical Academy (MMA) in Belgrade. A total of 654 primary and revised TKAs were performed in the said period. We registered and surgically treated 28 infected TKAs (primary TKAs: MMA - 22, other institutions - 6). The incidence of TKA infection in the MMA was 3.36%. The most common pathogens were: Staphylococcus aureus - 14 (50%) cases, and Staph. epidermidis - 3 (10.7%) cases. Other isolated pathogens were: Enterococcus faecalis, Klebsiella pneum., Klebsiella spp., Streptoccocus viridans, Seratia spp, Micrococcus luteus and Peptostreptococcus spp. In one case we had mixed anaerobic flora, and in 3 cases cultures were negative. We analyzed diagnostic challenges, risk factors (such as age and previous viscosupplementation) and treatment outcomes in our series of infected TKAs. Results. In our series 2 infections healed after iv antibiotics and debridement, 1 patient responded to open debridement with component retention, 4 patients responded fully to one-stage reimplantation, 10 cases responded fully to two-stage reimplantation, 11 patients ended with arthrodesis and we had 1 patient with above knee amputation. Conclusion. Two-stage reimplantation remains gold standard for treatment of infected TKA, and we recommend it as treatment of choice for eradication of infection. The antibiotic loaded spacer prothesis concept in most cases allows infection eradication, good function and high patient satisfaction.
