- Abstract
This research paper offers information, activities, and material to anyone who is interested in applying role-play activities based on Task-based language method (TBL) to help students, English language major, improve their speaking ability. This research highlights the necessity to enhance the speaking skills through strategies which provide learners with plenty of opportunities for oral communication. Role-play is a suitable strategy to improve speaking skills since it gives students the chance to interact with each other in a classroom using language for communicative purposes based on daily life situations. To develop the role-play activities proposed in this project, the TBL method has been considered since it as role-play is centered on students’ interaction. Besides, this method emphasizes on the use of authentic language through meaningful tasks such as problem solving, finding solutions, giving advice, sharing personal experiences, among others. Role-play activities have been developed in form of lesson plans as a part of the sequence of an English lesson.

María Eugenia Ibarra Melo presenta en este libro parte de los hallazgos teóricos y metodológicos de su tesis doctoral Transformaciones identitarias de las mujeres como resultado de su participación política en las guerrillas y en las acciones colectivas por la paz en Colombia (Universidad Complutense de Madrid, 2007). La autora analiza en esta investigación la vinculación de mujeres colombianas a los grupos insurgentes (ELN, EPL, CRS y M-19) durante el periodo 1979-1990, e interpreta un proceso de reconfiguración de la identidad femenina en la emergencia de las mujeres como actores políticos. Una investigación que intenta recuperar las voces de las excombatientes y aportar a un proceso de levantamiento del velo de invisibilidad que recae sobre la participación de la mujer colombiana en los procesos de guerra y paz. En el abordaje de la problemática la autora opta teóricamente por un enfoque constructivista y acude a teorías feministas que reconocen la existencia de múltiples opciones en el posicionamiento de ser mujer, lo que permite ampliar el espectro de análisis de los modos de subjetividades. Metodológicamente la autora asume la opción cualitativa y se sirve de las estrategias etnográficas, de la observación directa y de entrevistas semi-estructuradas en profundidad para recoger textos que serán significados en una intersección de la historia personal con la historia de la sociedad.

Qualitative Verfahren werden in der Sozial- und Praxisforschung meist verwandt, weil sie eine intensive Analyse von Kausalfaktoren und die Entwicklung alternativer Handlungsoptionen im Fall sozialer Problemstellungen eher unterstützen als quantitative Verfahren. Ausgehend von den Ergebnisse aus drei Studien im Feld der Politischen und Geschlechterpsychologie beschäftigt sich dieser Artikel mit Teilnehmender Aktionsforschung als einem sinnvollen qualitativen Ansatz zum Umgehen mit sozialen Phänomenen wie Rassismus, Gewalt gegen Frauen oder mit Kindern, die aufgrund bewaffneter Konflikte gezwungen sind, ihre Heimat zu verlassen. Hierzu werden drei Bereiche angesprochen: 1. Es wird kurz in die Geschichte und in den theoretischen Bezugsrahmen des Paradigmas der Teilnehmenden Aktionsforschung eingeführt. 2. Es werden aktuelle Entwicklungen in den USA, Deutschland und Lateinamerika zusammengeführt, dies insbesondere auch mit Blick auf Frauenforschung. 3. Based on the findings of three studies in the field of political and gender psychology, this article focuses on Participant Action Research (PAR) as a useful qualitative approach to deal with social phenomena, such as racism, violence against women, and the problem of children and youth who have been dislocated as the result of armed conflict and sheltered by the Colombian government's program for persons relocated to civil society. This article is composed of three parts. The first part offers historical and theoretical background to the Action Research (AR) paradigm, its validation criteria and their meaning for the development of the Latin American rendering of Participant Action Research (PAR). The second part synthesizes trends in the AR approach in the United States and Germany, discusses feminist research and compares these to trends in PAR in Latin America. The third part is a description of Participant Action Research as an intervention method, including features, models, goals, and concepts. 112. Características ginecológicas y psicosociales del dolor pélvico crónico

El dolor pélvico crónico es un síndrome prevalente controvertido en cuanto a su definición y a su relación con patología pélvica, problemas psicosocioculturales y su abordaje primario, diagnóstico y terapéutico. Objetivo: describir el perfil sociodemográfico, las patologías orgánicas más frecuentemente observadas, y la asociación con síntomas de ansiedad y depresión en mujeres con dolor pélvico crónico en Cali, Colombia. Métodos: se realizó un estudio descriptivo en mujeres con diagnóstico de dolor pélvico crónico que iban a ser sometidas a laparoscopia diagnóstica. Resultados: se estudiaron 30 mujeres que cumplían los criterios de inclusión y aceptaron participar en el estudio. La edad promedio fue de 34,2 + 6,2 años con duración del dolor pélvico crónico entre uno y diez años. El 76% de las mujeres vivían en pareja (casadas 53%, unión libre 23%), el 40% pertenecían al estrato socioeconómico 3 (rango I-6), el 53% eran de raza mestiza, el 50% tenía escolaridad universitaria. Todas las pacientes presentaron patología pélvica en la laparoscopia; las más frecuentes fueron las adherencias 26,6%, adenomiosis 20%, endometriosis 10% y ovario poliquístico 10%, con un 43,4% de combinaciones de múltiples patologías. Se observaron adherencias densas en intestino y pared lateral en el 30% de las pacientes. La ansiedad evaluada por la Escala de Ansiedad y Depresión en el Hospital presentó un porcentaje alto 30%, en contraste con una baja proporción de depresión 3,3%. Conclusiones: todas las pacientes estudiadas con dolor pélvico crónico presentaron patología pélvica, principalmente adherencias densas en intestino y pared lateral 30%, con una mayor proporción de ansiedad que depresión. Estas características ginecológicas y psicosociales son propias de la población estudiada y difieren de estudios reportados previamente en otros grupos poblacionales. Chronic pelvic pain (CPP) is one of the chronic pain syndromes where controversy exists in its definition, accompanying pelvic pathology, psychosocial and cultural associated factors, diagnosis, and therapeutic approaches. Objective: To describe the socio-demographic profile of women with CPP, pelvic pathology by laparoscopy, and the association with anxiety or depressive symptoms in the woman. Methods: A descriptive study was performed in thirty women with CPP and diagnostic laparoscopy. Results: The mean age was of 34,2 + 6,2 years old. Long duration of the CPP (range 1-10 years) was observed. The most of the women 40% were of middle socioeconomic status (level 3, range 1-6), mixed ethnic backgrounds 53%, university education 50%, and stable marital state (married 53%, common marriage in law 23%. All women had pelvic pathology by laparoscopy. The most frequent were adherences 36,6%, adenomyosis 20%, endometriosis 10%, polycystic ovary (10%), and the remaining 43.4% were mixed pelvic pathologies. A high proportion of dense adherences with the intestine 30% was observed. Anxiety, characterized by Hospital Anxiety Depression Scale (HADS), was observed in high proportion (30%) as well, in contrast with a low proportion of depression 3,3%. Conclusion: The studied women had a high frequency of anxiety and low frequency of depression. All cases were associated with pelvic pathology with a high proportion (30%) of dense adherences with the intestine. This gynecologic and psychosocial characteristics were different in relation to others populations.

Este artículo aporta a la discusión sobre procesos resilientes y al signifi cado de estos procesos en el desarrollo de niños en condición de adoptabilidad. La situación de adopción de niños colombianos se caracteriza desde datos estadísticos y normativas políticas y jurídicas (ICBF, 2003; 2006; 2009; Presidencia de la República, 1960; 1984; 2004; 2006). Se discuten aportes sobre la resiliencia como proceso psicológico desde autores europeos, norteamericanos y latinoamericanos (Cyrulnik, 1999; 2001; 2002; 2003; Ungar, 2008; Bowlby, 1992; Villalobos, 2003; Crawford & Wrigt y Masten 2005; Colmenares, 2002; 2006; Soarez de Souza y de Oliveira, 2006; entre otros). - Abstract
Objective: since the revision of some criteria of the Participatory Action Research (par): the researcher`s influence, agrees, transparency and coupling, we try to point out the significance that this approach is qualitative research to address problems like abuse and some forms of political violence that affect children and young people. Issues that are cross to the interests of investigative work and intervention of a political psychology and psychology of gender, since the focus of a critical social psychology. Methodology: the goals of this task requiring the deployment of methodologies analyzes recognize the pattern of meaning as a space in which meanings emerge on triggers of the problems and factors that enable the development of alternative solution. The empirical component consists of some research findings on racism, women's identity in subjects with experience of abuse, the construction of subjectivities and the phenomenon of political participation in children and young people demobilized from illegal armed groups. The text shows dates of abuse and the untying of young children and armed conflict as a way of forms of political violence and some thoughts about the commitment of psychologists in handling these problems. Results: a summary of some historical background to the par and its significant elements as proposed critical and qualitative research and intervention, and ends the text to mean scientific criteria of the par, the influence of the researcher, agreement and transparency, based on empirical findings of the component., Objetivo: a partir de la revisión de algunos criterios de la Investigación Acción Participativa (iap): influencia del investigador, acuerdo, transparencia y acoplamiento, se intenta señalar el significado que de este enfoque de investigación cualitativa para el abordaje de problemáticas como el maltrato y algunas formas de violencia política que afectan a niños, niñas y jóvenes, problemáticas que son transversales a los intereses del quehacer investigativo y de intervención de una Psicología política y una Psicología de género, desde el enfoque de una psicología social crítica. Metodología: las metas emancipatorias de ese quehacer exigen de la implementación de metodologías que reconozcan el análisis de tramas de sentido, en el cual emergen significados sobre factores desencadenantes de los problemas y factores que posibilitan el desarrollo de alternativas de solución. El componente empírico lo constituyen algunos hallazgos de investigaciones sobre el racismo, la identidad femenina en sujetos con experiencia de maltrato, la construcción de subjetividades y el fenómeno de participación política en niños y en jóvenes desvinculados de grupos armados irregulares. Se introduce el texto con cifras sobre el maltrato y la desvinculación de niños y jóvenes del conflicto armado como forma de violencia política y algunas reflexiones sobre el compromiso de los psicólogos en el manejo de estas problemáticas. Resultados: se resumen algunos antecedentes históricos de la iap y sus elementos significativos como propuesta crítica y cualitativa de investigación e intervención y se finaliza el texto, al significar criterios científicos de la iap -la influencia del investigador, el acuerdo y la transparencia- con base en hallazgos del componente empírico.

- Abstract
Objectives: To determine if children with congenital heart disease (CHD) have lower newborn T-cell receptor excision circles (TREC) levels than the general population and to evaluate if low TREC levels in newborns with CHD are associated with clinical complications such as hospitalization for infection.Study Design: The Connecticut Newborn Screening Program reported TREC levels for newborns with CHD delivered between October 2011 and September 2016 at 2 major Connecticut children's hospitals. TREC levels for children with CHD were compared with the general population. TREC levels and outcome measures, including hospitalization for infection, were compared.Results: We enrolled 575 participants with CHD in the study. The median TREC level for newborns with CHD was lower than the general population (180.1 copies/μL vs 312.5 copies/μL; P < .01). patients with CHD requiring hospitalization for infection had lower median TREC levels than their counterparts (143.0 copies/μL vs 186.7 copies/μL; P < .01). The combination of prematurity and low TREC level had a strong relationship to hospitalization for infection (area under the receiver operative characteristic curve of 0.89). There was no association between TREC level and CHD severity.Conclusions: Newborns with CHD demonstrated lower TREC levels than the general population. Low TREC levels were associated with hospitalization for infection in preterm children with CHD. Study limitations include that this was a retrospective chart review. These findings may help to identify newborns with CHD at highest risk for infection, allowing for potential opportunities for intervention. [ABSTRACT FROM AUTHOR]- Published

El presente trabajo realiza una comparación entre la violencia del narcotráfico de Perú y Colombia,siendo estos dos países los mayores productoresde cocaína en el mundo, pero teniendo niveles distintos de violencia. En el caso de Perú los niveles de violencia son bajos y en el caso de Colombia notablemente elevados. El trabajo plantea que el cultivo y producción de droga en sí mismo no es fuente de altos niveles de violencia,sino que la intensidad de esta depende de la presencia de más variables en juego.

- Abstract
Background:Patients status post tetralogy of Fallot (TOF) repair experience RV volume load due to pulmonary insufficiency. We previously demonstrated RV end diastolic volume index (EDVi) inversely correlates with LV septal segmental displacement, Figure 1. We sought to evaluate recovery of segmental displacement and decrease in RVEDVi following pulmonary valve replacement (PVR) and its effect on RV/LV global and regional function.Methods:From 2011 to 2016, pre and post cardiac MR (CMR) studies were obtained in 16 TOF patients on average 5.5 years following PVR. RV/LV myocardial deformation and segmental displacement were quantified using CMR Tissue Tracking software (2D CPA MR, TomTec). Correlations of segmental displacement were performed with RV EF, LV EF, RVEDVi, pulmonary regurgitant fraction (RF), and strain.Results:Pulmonary RF and RVEDVi decreased post PVR in 91.7% and 93.3% of patients. On average, RVEDVi decreased from 154.4 ml/m2to 111.2 ml/m2post-op, Figure 2. Overall, LV EF and RV EF declined in 81.3% and 75% of patients. Patients with lower RVEDVi showed improved RV septal wall strain magnitude (r = 0.618, P = 0.01) and improved RV EF (r = -0.609, P = 0.01). Patients with recovered LV septal segmental displacement had an increase in LV septal wall strain magnitude (r= 0.617, P = 0.01).Conclusions:Overall,RVEDVi and pulmonary RF improved after PVR, however, LV EF and RV EF continued to decline. RV remodeling with recovery of septal displacement and smaller RVEDVi correlated with improved RV/LV global and regional functional parameters. Future longitudinal studies may help elucidate the use of septal segmental displacement in determining optimal timing for PVR.

139. Abstract 13482: Change From Baseline and Initial Post-Chemotherapy Echocardiographic Shortening Fraction Predicts Long-Term Cardiac Dysfunction in Patients With Anthracycline Chemotherapy Exposure

Introduction:Anthracycline induced cardiomyopathy (AIC) and cardiac dysfunction is a major cause of mortality and morbidity among pediatric cancer survivors. We hypothesized that a shortening fraction (SF) decline of > 10% from pre-treatment to initial post-treatment echocardiogram correlated with long term cardiac dysfunction defined as SF < 29%.Methods:Inclusion criteria included exposure to anthracycline chemotherapy with ? 3 serial echocardiograms. Patients were stratified into 4 groups based on pre-treatment SF and change to initial post-treatment study (Figure 1). Statistical analysis was performed using Student?s t-test.Results:There was no difference in the demographics or cumulative anthracycline dose between the Groups. Group A had the smallest percent change in SF between baseline and initial post-treatment SF and the highest post-treatment SF of 36.7? 4.7, in comparison to 34.9? 4.2 in Group B (p<0.0001), 28.4?9.3 in Group C (p<0.0001), and 29.3?5.3 in Group D (p<0.0001). Group C had a larger percent decline in SF 24?13.5 versus Group B which had increased SF of 5.4?12.2 (p<0.0001). 17/27 (63%) of subjects in Group C had follow-up SF < 29% compared to 11/31 (34%) in Group B on follow-up echocardiogram (p < 0.001). Long term follow up SF values depicted in Figure 2.Conclusions:Both the baseline pre-treatment SF and a decline of > 10% between the pre-treatment and initial post-treatment study correlates with long term cardiac dysfunction defined as SF < 29%, and increased mortality. These findings provide an insight to the importance of acute CTRCD monitoring and its impact on long term cardiac dysfunction secondary to AIC.

140. Abstract 14533: Role of Cardiorespiratory Fitness to Modulate Cardiotoxicity in Long Term Cancer Survivors Exposed to Anthracycline Therapy

Introduction:Cardiotoxicity is a frequent adverse complication of anthracycline therapy (AC). Low cardiorespiratory fitness (CRF) is the strongest predictor of mortality in patients with cardiovascular disease than other established risk factors.Hypothesis:We hypothesize that VO2max as a measure of CRF is correlated with higher LV mass and increased stroke volume(SV) irrespective of cardiotoxicity risk factors.Methods:Thirty-four subjects, 21?6.2 yr. with cumulative AC doses 205.8 ? 99.6 g/m2underwent a cardiopulmonary stress test 7.9?6.2 years after cancer treatment. Volumetric parameters were computed by cardiac MRI. Patients were stratified based on CV risk factors (cancer treatment and traditional risk factors) and given a composite risk score.Results:Pediatric cancer survivors have lower levels of CRF when compared with predicted values for a similarly aged general population. Fifteen (44%) subjects demonstrated VO2max ?30 mL/Kg/min and only 4 subjects achieved VO2max above predicted. Lower VO2max was inversely correlated with elevated BMI (R-0.76, p<.0001) and higher VO2max with improved volumetric and functional cardiac MRI parameters (Table 1). High risk score correlated with lower functional cardiac MRI parameters. Multiple regression analysis demonstrated the impact of VO2max relative to risk score (Table 2).Conclusions:LV hypertrophy and the ability to generate a large SV are direct results of CRF irrespective of cardiotoxicity risk in long-term cancer survivors of pediatric cancer.

- Abstract
Objectives: Myocardial fibrosis in noninfarcted myocardium is emerging as a principal phenotype of vulnerability to adverse events such as mortality and hospitalization for heart failure (HHF), but its optimal noninvasive measurement remains uncertain despite consistently robust histologic validation data for extracellular volume fraction (ECV). We therefore compared ECV, native T1, post contrast T1, the gadolinium contrast partition coefficient (lambda), and the presence of nonischemic scar in their associations with mortality and HHF outcomes. Method: To quantify of myocardial fibrosis, we performed T1 mapping (MOLLI) in basal and mid short axis slices with cardiovascular magnetic resonance (CMR) before contrast and 12-30 minutes post contrast bolus in 1185 consecutive patients without amyloidosis, hypertrophic or stress cardiomyopathy. We assessed associations with outcomes using Kaplan-Meier plots and chi square values from univariable Cox regression models. All standard T1 mapping parameters were obtained: native and post contrast myocardial T1, the partition coefficient lambda, and ECV. ECV = (1-hematocrit) · [ΔR1myocardium]/[ΔR1bloodpool], where R1 = 1/T1 Late gadolinium enhancement imaging with phase sensitive reconstruction identified nonischemic scar. Results: Over a median of 1.7 years, 111 individuals experienced events after CMR: 55 HHF events and 74 deaths. ECV yielded better separation of Kaplan-Meier curves in a dose dependent fashion (Figure) and also stronger associations with the combined endpoint of death or HHF. The ECV chi square (77.3, p < 0.001) was at least twice as large as the Native T1 chi square (37.5, p < 0.001), the lambda chi square (34.8, p < 0.001) and nonischemic scar (chi square = 20.5, p<0.001). Post-contrast T1 was not associated with outcomes, even when adjusting further for time after contrast bolus, renal function, and patient weight (chi square <3, p >0.10). Conclusion: Analogous to histologic previously published validation data, quantitative ECV myocardial fibrosis measures associated with outcomes far stronger than other surrogate measures outcome measures such as native T1, post contrast T1 and nonischemic scar on LGE images. - Abstract
Introduction: Intravenous rescue analgesia in the postoperative anesthesia care unit (PACU) is the most effective method for reducing postoperative pain (POP) when perioperative multimodal analgesia fails to control it. Appropriate analgesia during these first postoperative hours may prevent morbidity associated with pain. Objective: To compare the effectiveness of intravenous morphine versus fentanyl in the PACU for reducing severe POP. Methods: Randomized, prospective, double blind trial that included patients with severe POP using VAS in the PACU. Rescue was performed on one group with 0,1 mg/Kg morphine and with another with 1 μg/Kg of fentanyl every 5 minutes intravenously until pain was reduced from severe to mild (VAS < 4). Thirty patients were included in both groups. Results: There were no significant differences in the percentage of patients with reduction of severe POP to mild 5 minutes after the injection of morphine or fentanyl, or in the subsequent rescue analgesia intervals (P > .05). Similarly, there were no significant differences in mean VAS (95% CI) in morphine or fentanyl groups beginning 5 minutes after the first analgesic dose (P > .05) between the groups. There were no significant differences in side effects such as respiratory depression, nausea, vomiting or pruritus (P = 1,0). There was a high satisfaction in both groups (P > ,05). Conclusions: Morphine and fentanyl were equally effective in treating severe POP after 5 minutes and following intervals after rescue analgesia was initiated, during 25 minutes at PACU, with no differences in efficacy or adverse effects between groups (Register # NCT02145975 clinicaltrials.gov, prospective). The diagnosis of coarctation of the aorta (CoA) prior to birth can be challenging due to the physiologic changes during postnatal transition. Prenatal risk stratification can standardize postnatal management and improve outcome. CT Children’s Fetal Cardiology created the Antenatal Risk of CoA in Hartford (ARCH) clinical pathway defining four distinct postnatal order sets based on degree of suspicion for ductal dependency on fetal evaluation: low, low-moderate, moderate-high, and high risk. This study aims to evaluate safety and efficacy of the ARCH pathway in neonates with suspected CoA. This study was a single-center, retrospective chart review evaluating maternal-infant dyads with findings concerning for CoA between July 2004 and July 2021, before and after ARCH pathway implementation. Neonates were evaluated for the presence or absence of critical CoA and postnatal clinical data were collected. Statistical analysis was performed using chi square and Fisher’s exact test. There were 108 maternal-infant dyads studied, comprising 53 non-pathway patients and 55 ARCH pathway participants. Thirty-three neonates had critical CoA, comprising 23 non-pathway and 10 ARCH pathway subjects. Patients categorized in the high-risk group were highly associated with critical CoA (
P = 0.003). Non-pathway neonates with CoA demonstrated higher likelihood of hospital transfer compared to ARCH pathway neonates (56.5% vs. 10.0%,P = 0.021). NICU admission, prostaglandin administration, and intubation were not significantly different between before and after ARCH implementation (P < 0.05). More echocardiograms were performed in ARCH pathway neonates without CoA than their non-pathway counterparts (1.586 vs. 2.133,P = 0.049). The ARCH pathway is a safe, reliable prenatal risk stratification system to help guide management of patients with critical CoA. Outpatient cardiologists provide longitudinal care for Fontan patients. As these patients age, they face mounting morbidities, necessitating challenging conversations about prognosis and goals of care. We created a novel survey to evaluate cardiologists’ attitudes surrounding risk counseling for patients/caregivers. Cardiologists were recruited during concomitant outpatient enrollment of individuals with Fontan operation > age 10. Physician demographic data, expectations of timing in discussing adverse event risk, and perceived barriers were collected. Barriers were analyzed using a thematic approach. 40 cardiologists (9 institutions) responded regarding 155 patients (mean age 21.2 years, SD 7.7). Physicians were mostly male (58%) with mean practice of 21 years post-fellowship (SD 12). Most felt the time was right to have a conversation with patient (55%) and family (62%), and majority thought patient (53%) and family (75%) were ready for such a conversation. Most had previously discussed prognosis with patient (72%) and family (75%). Providers were inclined to discuss risk with caregivers earlier (mean patient age 9 years, SD 11) than patients (mean patient age 17 years, SD 6.4). Nevertheless, 42% of physicians perceived significant barriers and provided 58 narrative comments categorized into 4 major themes: (1) Patient-related (53.4%), including cognitive limitations and mental health; (2) Provider-related (16.4%), including lack of familiarity, preservation of happiness, and discomfort; (3) Family related (12.3%), including protection/denial and psychosocial stressors; (4) Other (26%), including social barriers. Experienced cardiologists are willing to have difficult conversations; nearly half reported largely patient-related barriers. Background: Understanding the clinical course and short-term outcomes of suspected myocarditis after the coronavirus disease 2019 (COVID-19) vaccination has important public health implications in the decision to vaccinate youth.Methods: We retrospectively collected data on patients <21 years old presenting before July 4, 2021, with suspected myocarditis within 30 days of COVID-19 vaccination. Lake Louise criteria were used for cardiac MRI findings. Myocarditis cases were classified as confirmed or probable on the basis of the Centers for Disease Control and Prevention definitions.Results: We report on 139 adolescents and young adults with 140 episodes of suspected myocarditis (49 confirmed, 91 probable) at 26 centers. Most patients were male (n=126, 90.6%) and White (n=92, 66.2%); 29 (20.9%) were Hispanic; and the median age was 15.8 years (range, 12.1-20.3; interquartile range [IQR], 14.5-17.0). Suspected myocarditis occurred in 136 patients (97.8%) after the mRNA vaccine, with 131 (94.2%) after the Pfizer-BioNTech vaccine; 128 (91.4%) occurred after the second dose. Symptoms started at a median of 2 days (range, 0-22; IQR, 1-3) after vaccination. The most common symptom was chest pain (99.3%). Patients were treated with nonsteroidal anti-inflammatory drugs (81.3%), intravenous immunoglobulin (21.6%), glucocorticoids (21.6%), colchicine (7.9%), or no anti-inflammatory therapies (8.6%). Twenty-six patients (18.7%) were in the intensive care unit, 2 were treated with inotropic/vasoactive support, and none required extracorporeal membrane oxygenation or died. Median hospital stay was 2 days (range, 0-10; IQR, 2-3). All patients had elevated troponin I (n=111, 8.12 ng/mL; IQR, 3.50-15.90) or T (n=28, 0.61 ng/mL; IQR, 0.25-1.30); 69.8% had abnormal ECGs and arrhythmias (7 with nonsustained ventricular tachycardia); and 18.7% had left ventricular ejection fraction <55% on echocardiogram. Of 97 patients who underwent cardiac MRI at a median 5 days (range, 0-88; IQR, 3-17) from symptom onset, 75 (77.3%) had abnormal findings: 74 (76.3%) had late gadolinium enhancement, 54 (55.7%) had myocardial edema, and 49 (50.5%) met Lake Louise criteria. Among 26 patients with left ventricular ejection fraction <55% on echocardiogram, all with follow-up had normalized function (n=25).Conclusions: Most cases of suspected COVID-19 vaccine myocarditis occurring in persons <21 years have a mild clinical course with rapid resolution of symptoms. We report two cases of prenatally diagnosed double aortic arch with dominant right arch and a left-sided ductus arteriosus, consistent with a complete vascular ring. Postnatal transthoracic echocardiogram and cardiac magnetic resonance imaging demonstrated a spontaneous closure of the ductus arteriosus and obliteration of the left aortic arch distal to the origin of the left subclavian artery in both cases. Spontaneous closure of the ductus arteriosus involving extended ductal tissue in the left aortic arch likely led to obliteration of the distal left arch after birth. One patient presented with recurrent symptoms suggestive of dysphagia and underwent a successful surgical repair of the vascular ring with resolution of symptoms. Purpose To investigate right ventricular (RV) strain in patients without identified cardiac pathology using cardiac magnetic resonance tissue tracking (CMR TT). Methods A total of 50 consecutive patients with no identified cardiac pathology were analyzed. RV longitudinal and circumferential strain was assessed by CMR TT. The age range was 4–81 years with a median of 32 years (interquartile range, 15 to 56 years). Results Analysis time per patient was < 5 min. The peak longitudinal strain (E ll ) was − 22.11 ± 3.51%. The peak circumferential strains (E cc ) for global, basal, mid-cavity and apical segments were as follows: − 11.69 ± 2.25%, − 11.00 ± 2.45%, − 11.17 ± 3.36%, − 12.90 ± 3.34%. There were significant gender differences in peak E cc at the base ( P = 0.04) and the mid-cavity ( P = 0.03) with greater deformation in females than in males. On Bland-Altman analysis, peak E ll (mean bias, 0.22 ± 1.67; 95% CI − 3.05 to 3.49) and mid-cavity E cc (mean bias, 0.036 ± 1.75; 95% CI, − 3.39 to 3.47) had the best intra-observer agreement and inter-observer agreement, respectively. Conclusions RV longitudinal and circumferential strains can be quickly assessed with good intra-observer and inter-observer variability using TT. [ABSTRACT FROM AUTHOR]

Bicuspid aortic valve is the most common congenital heart defect, and its association with proximal aortic dilation (aortopathy) increases risk for aortic aneurysm and dissection in adults. Maximal aortic root and ascending aortic dimensions are predictors of adverse cardiovascular outcomes. Recent single center data demonstrate medical therapy with beta or angiotensin II receptor blockers tempers aortic growth rates. Although randomized clinical trials may not be feasible, larger population studies would better inform bicuspid aortic valve preventive cardiovascular care. Improve regional pediatric bicuspid aortopathy collaboration with standardized medical management guidelines and patient care registry. The New England Congenital Cardiology Association (NECCA) developed consensus-based prophylactic treatment guidelines for bicuspid aortopathy and designed a regional clinical registry (NECCA BAR) to study cardiovascular practice variation and short-term outcomes in subjects with moderate to severe bicuspid aortopathy. Individuals 6 months to 25 years of age with bicuspid aortic valve (all variants) and body surface area-adjusted aortic root or ascending aorta z score ≥ 4.0 SD, and/or absolute dimension ≥4.0 cm, are eligible for inclusion. Standardized bicuspid aortopathy medical management guidelines were established among New England congenital cardiology practices. The primary aims are to compare cardiovascular practice variation by guideline adherence (medical therapy and athletic participation), and evaluate medication effectiveness by comparing rates of annual change in aortic root and ascending aorta dimensions/z scores over 3 years. Additional endpoints include aortic valve disease progression (stenosis/regurgitation), incidence of aortic dissection, surgery, death, and incidence of adverse drug events. Recruitment in progress; data analysis pending. We describe the development of regional pediatric clinical care guidelines and a patient care registry designed to refine medical management of bicuspid aortopathy. • Bicuspid aortopathy carries risk for aortic aneurysm, surgery, dissection and death. • Losartan or atenolol reduced aortic growth rates in a small group of young patients. • Standard clinical recommendations can improve regional management and collaboration. • Larger registry data are needed to track bicuspid aortopathy patients and outcomes.
