Abstract

Introduction: Haemophilia management and patients’ quality of life significantly improved. However, data on current patients’, caregivers’ and clinicians’ satisfaction and limitations of treatments and haemophilia management are limited. Aim: Assessing the management satisfaction and unmet needs from the perspective of Italian patients with haemophilia (PWH) without inhibitors (or caregivers if children) and of specialist physicians. Methods: Surveys (for patients≥18 years, caregivers of children and haemophilia specialists) were developed by a multidisciplinary working group and conducted from November 2019 to June 2020. Results: Among 275 participants, 120 (43.6%) were PWH without inhibitors, 79 (28.7%) caregivers and 37 (13.4%) clinicians. Patients and caregivers perceived a higher control of the disease compared to clinicians. However, more than 40% of patients and caregivers reported to feel significantly conditioned by the risk of bleeding during their daily life. PWH reported a 6-month mean/median (range) of bleeds 2.3/.0 (0-24) and caregivers 1.3/.0 (0-16) in children. The treatment burden (frequency of administration) was not satisfactory for more than half adults and caregivers of children treated with prophylaxis. A good access to treatment, haemophilia centres and medical service was reported, with issues associated to the multidisciplinary approach and treatment at emergency department. Conclusions: This large national study provides an updated overview of haemophilia care in Italy from different points of views, highlighting positive aspects and unmet needs. This information can guide future interventions to improve haemophilia management and the assessment of impact of new treatment options.

Abstract

Objectives: The aim of this study was to assess the incidence of being overweight and metabolic syndrome (MS) among people living with HIV (PHIV) in three different cross-sectional studies conducted over three different periods: 2005, 2011 and 2015. Methods: This was a multi-centre, nationwide study. Data were collected in three studies from the CISAI group – SIMOne, HIV-HY and STOPSHIV – and included a total of 3014 PHIV. Logistic regression [odds ratio (OR), 95% confidence interval (CI)] was used to account for age and gender difference among three groups when comparing MS prevalence and being overweight; potential confounders were accounted for by including them in the regression equation. Results: Overall, the mean age was 46.9 ± 10.2 years, and men comprised 73.3% of participants. Comparing 2005, 2011 and 2015, MS was present in 34.5%, 33.0% and 29.3% of PHIV, respectively. Adjusted OR for MS was 0.64 (95% CI: 0.52–0.78) in 2011 and 0.56 (95% CI: 0.46–0.69) in 2015 compared with 2005, while BMI (kg/m2) increased from 23.6 in 2005, 24.5 in 2011 and 24.5 in 2015, with a concomitant increase of being overweight from 29.4% to 39.5% to 39.6% (p < 0.0001). Conclusions: In recent years, PHIV have had a significantly improved metabolic profile compared with previously, despite increasing weight and BMI.

Abstract

Currently, cognitive stimulation exercises conducted with Mild Cognitive Impairment (MCI)-affected older adults are typically administered by care professionals through exercises, which require a high degree of involvement by professional therapists, and are often perceived as tedious by the older adults. We aim to offer novel digital training through serious games designed using personally relevant material from the older adults' life. They will be based on memories associated with their biography, thus making interactions personalised, relevant, and more engaging. The serious games can be accessed through humanoid robots, which can make the training exercise more engaging because of their human-like behaviour. Abstract

The importance of guaranteeing accessible Web applications is becoming widely recognised, and supported by national and international legislation. This implies an increasing need for large scale validations, which can be achieved only through automatic support. Thus, there is a need for a new generation of accessibility validation tools able to check against the continuously evolving guidelines and report any issues revealed, considering that their results will be used by many people with varied backgrounds and goals. Such tools should be able to support monitoring of Web sites' accessibility, provide user-friendly information suitable for various purposes, and be transparent in terms of what they are actually able to evaluate in order to elicit the right expectations from their users. They should also consider how the technologies for implementing Web sites have evolved in recent years. In this paper, we provide a description of the requirements and design dimensions that characterise such tools in order to address emerging needs, providing indications on whether and how several existing validation tools support them, and present how we have addressed and implemented them in a specific tool. We also report on a first usability test of such tool, which has provided encouraging feedback.

112. Towards understanding the transparency of automations in daily environments

Abstract

This paper outlines a proposal for how to address transparency of automations in daily environments, such as smart homes, based on experiences carried out in previous projects. The trigger-action programming paradigm has been used to describe and implement such automations in both commercial and research tools. Such automations can be generated through machine learning techniques or directly by the end users or through an interaction between an intelligent agent and the user. When they are executed the resulting behaviour does not always result in the desired actions, and users may have difficulties in understanding and controlling them. Thus, there is a need for design criteria and associated tools that help people to understand and control what happens with the automations active in the environments where they live, and explain how they work and can be modified to better meet their needs.

Cognitive developmental disorders are common in children and affect the language or attention area, such as attention deficit and hyperactivity. In these cases, the individual is unable to engage and interact effectively with the environment, resulting in difficulties in learning and autonomously carrying out daily activities. For these reasons, this work proposes a solution that aims to stimulate children in the relevant cognitive aspects, which are attention, planning tasks and error monitoring. The proposed solution is PlayToPickUp, a serious game that supports tasks that replicate the activities of daily life, such as preparing the backpack, recognising money or emotions.

Abstract

One of the goals of Ambient Assisted Living (AAL) solutions is to be able to stimulate the cognitive resources of older adults. An innovative way to address such stimulation is the use of serious games delivered through humanoid robots. Such devices are based on Internet of Things technologies since they are collections of sensors and actuators in a human shape. Serious games delivered through humanoid robots can provide an engaging way to perform exercises useful for training human memory, attention, processing, and planning activities. This paper presents an approach to supporting cognitive stimulation based on personal memories. The humanoid robot can exhibit different behaviours through different modalities, and propose the games in a way personalized to specific individuals' requirements, preferences, abilities, and motivations, which can vary among older adults, and even dynamically evolve over time for the same person depending on changing user needs and health conditions.

Abstract

Background: The ISTH Bleeding Assessment Tool (ISTH-BAT) has been validated for clinical screening of suspected von Willebrand disease (VWD) and for bleeding prediction. Recently it has been validated for subjects with inherited platelet disorders (IPD) (BAT-VAL study). Objectives: To determine whether the ISTH-BAT bleeding score (BS) predicts subsequent bleeding events requiring treatment in IPD patients. Methods: Patients with IPD, type 1 VWD (VWD-1) and age- and sex-matched healthy controls enrolled in the BAT-VAL study were prospectively followed-up for 2years and bleeding episodes requiring treatment were recorded. Results: Of the 1098 subjects initially enrolled, 955 were followed-up and 124 suffered hemorrhages during follow-up, 60% of whom had inherited platelet function disorders (IPFD). Total number of events was significantly higher in IPFD (n=235) than VWD-1 (n=52) or inherited thrombocytopenia (IT; n=20). Abstract

Aortic stenosis is the most frequent valvular disease to require intervention in the western world and has always been featured as a progressive disease. The rate of progression can be assessed by carefully performed Doppler echocardiography and can vary greatly between individuals with a profound impact on prognosis. Unfortunately, the determinants of disease progression had been insufficiently studied and remain challenging to define, particularly in the outpatient setting. Multiple factors have been proposed and tested, but at present, there are no proven therapies to slow the course of the stenotic process. Heart valve clinics may be particularly important to define the progression rate and tailor follow-up and management at an individual level. This review enlightens knowledge and gaps regarding the progression-rate of aortic valve stenosis, from the historical perspective to the molecular one.

Abstract

Background - Although the widespread use of factor VIII/IX replacement therapy has significantly reduced the severity of arthropathy in persons with haemophilia (PWH), some develop degenerative joint changes, associated with significant pain. The aim of this survey was to investigate the management and perception of pain among Italian physicians who treat PWH. Materials and methods - Between September and October 2017, a questionnaire was distributed to 35 Italian haemophilia treatment centres (60 physicians). Results - Fifty-three haemophilia specialists completed the survey. We found that there was good agreement (98.1%) on the need to investigate pain at each clinical visit, but there was heterogeneity in the opinions of haemophilia specialists with regards to the availability of validated guidelines (35.8%) and whether pain specialists should be a part of the comprehensive care team in daily clinical practice (58.5%). Haemophilia specialists also agreed pain should be evaluated using a rating scale validated in PWH (88.7%). Pain was mainly managed by the haemophilia specialists themselves, supported by a physiatrist and physiotherapist, while a pain specialist was only involved in 26.4% of cases. The combination of paracetamol with tramadol or codeine was the most common first-line treatment, while cyclo-oxygenase-2 inhibitors, non-steroidal anti-inflammatory drugs, and opioids were less commonly used. Discussion - There are some unmet needs in Italy regarding pain management for PWH and the management of pain in these patients by haemophilia specialists. There is a lack of evidence-based guidelines for these specialists to use, as well as a reluctance to involve pain specialists. The lack of spontaneous reporting of pain by PWH, despite using pain relief, highlights the need for clinicians to actively ask patients about any pain they may be experiencing.

Abstract

BACKGROUND: In this work, a small-scale ceramic microbial fuel cell (MFC) with a novel type of metal–carbon-derived electrocatalyst containing iron and nicarbazin (Fe-NCB) was developed, to enhance electricity generation from neat human urine. Substrate oxidation at the anode provides energy for the separation of ions and recovery from urine without any chemical or external power additions. RESULTS: The catalyst was shown to be effective in clear electrolyte synthesis of high pH, compared with a range of carbon-based metal-free materials. Polarisation curves of tested MFCs showed up to 53% improvement (44.8 W m−3) in performance with the use of Fe-NCB catalyst. Catholyte production rate and pH directly increased with power performance while the conductivity decreased showing visually clear extracted liquid in the best-performing MFCs. CONCLUSIONS: Iron based catalyst Fe-NCB was shown to be a suitable electrocatalyst for the air-breathing cathode, improving power production from urine-fed MFCs. The results suggest electrochemical treatment through electro-osmotic drag while the electricity is produced and not consumed. Electro-osmotic production of clear catholyte is shown to extract water from urine against osmotic pressure. Recovering valuable resources from urine would help to transform energy intensive treatments to resource production, and will create opportunities for new technology development. © 2018 The Authors. Abstract

A self-stratified microbial fuel cell fed with human urine with a total internal volume of 0.55 ml was investigated as an internal supercapacitor, for the first time. The internal self-stratification allowed the development of two zones within the cell volume. The oxidation reaction occurred on the bottom electrode (anode) and the reduction reaction on the top electrode (cathode). The electrodes were discharged galvanostatically at different currents and the two electrodes were able to recover their initial voltage value due to their red-ox reactions. Anode and cathode apparent capacitance was increased after introducing high surface area activated carbon embedded within the electrodes. Peak power produced was 1.20 ± 0.04 mW (2.19 ± 0.06 mW ml−1) for a pulse time of 0.01 s that decreased to 0.65 ± 0.02 mW (1.18 ± 0.04 mW ml−1) for longer pulse periods (5 s). Durability tests were conducted over 44 h with ≈2600 discharge/recharge cycles. In this relatively long-term test, the equivalent series resistance increased only by 10% and the apparent capacitance decreased by 18%.

Abstract

Power generation of bioelectrochemical systems (BESs) is a very important electrochemical parameter to consider particularly when the output has to be harvested for practical applications. This work studies the effect of cathode immersion on the performance of a self-stratified membraneless microbial fuel cell (SSM-MFC) fuelled with human urine. Four different electrolyte immersion heights, i.e. [Formula presented], [Formula presented], [Formula presented] and fully submerged were considered. The SSM-MFC performance improved with increased immersion up to [Formula presented]. The output dropped drastically when the cathode was fully submerged with the conditions becoming fully anaerobic. SSM-MFC with [Formula presented] submerged cathode had a maximum power output of 3.0 mW followed by 2.4 mW, 2.0 mW, and 0.2 mW for the [Formula presented], [Formula presented] and fully submerged conditions. Durability tests were run on the best performing SSM-MFC with [Formula presented] cathode immersed and showed an additional increase in the electrochemical output by 17% from 3.0 mW to 3.5 mW. Abstract

Background: Among people living with HIV (PLWH), the prevalence of non-HIV related co-morbidities is increasing. Aim of the present study is to describe co-morbidity and multi-morbidity, their clustering mode and the potential disease-disease interactions in a cohort of Italian HIV patients. Methods: Cross-sectional analysis conducted by the Coordinamento Italiano per lo Studio di Allergia e Infezioni da HIV (CISAI) on adult subjects attending HIV-outpatient facilities. Non-HIV co-morbidities included: cardiovascular disease, diabetes mellitus, hypertension, oncologic diseases, osteoporosis, probable case of chronic obstructive pulmonary disease (COPD), hepatitis C virus (HCV) infection, psychiatric illness, kidney disease. Multi-morbidity was defined as the presence of two or more co-morbidities. Results: One thousand and eighty-seven patients were enrolled in the study (mean age 47.9 ± 10.8). One hundred-ninety patients (17.5%) had no co-morbidity, whereas 285 (26.2%) had one condition and 612 (56.3%) were multi-morbid. The most recurrent associations were: 1) dyslipidemia + hypertension (237, 21.8%); 2) dyslipidemia + COPD (188, 17.3%); 3) COPD + HCV-Ab+ (141, 12.9%). Multi-morbidity was associated with older age, higher body mass index, current and former smoking, CDC stage C and longer ART duration. Conclusions: More than 50% of PLHW were multi-morbid and about 30% had three or more concurrent comorbidities. The identification of common patterns of comorbidities address the combined risks of multiple drug and disease-disease interactions.

Abstract

Background: Alcohol consumption is a known risk factor for liver disease in HIV-infected populations. Therefore, knowledge of alcohol consumption behaviour and risk of disease progression associated with hazardous drinking are important in the overall management of HIV disease. We aimed at assessing the usefulness of routine data collected on alcohol consumption in predicting risk of severe liver disease (SLD) among people living with HIV (PLWHIV) with or without hepatitis C infection seen for routine clinical care in Italy. Methods: We included PLWHIV from two observational cohorts in Italy (ICONA and HepaICONA). Alcohol consumption was assessed by physician interview and categorized according to the National Institute for Food and Nutrition Italian guidelines into four categories: abstainer; moderate; hazardous and unknown. SLD was defined as presence of FIB4 > 3.25 or a clinical diagnosis of liver disease or liver-related death. Cox regression analysis was used to evaluate the association between level of alcohol consumption at baseline and risk of SLD. Results: Among 9542 included PLWHIV the distribution of alcohol consumption categories was: abstainers 3422 (36%), moderate drinkers 2279 (23%), hazardous drinkers 637 (7%) and unknown 3204 (34%). Compared to moderate drinkers, hazardous drinking was associated with higher risk of SLD (adjusted hazard ratio, aHR = 1.45; 95% CI: 1.03-2.03). After additionally controlling for mode of HIV transmission, HCV infection and smoking, the association was attenuated (aHR = 1.32; 95% CI: 0.94-1.85). There was no evidence that the association was stronger when restricting to the HIV/HCV co-infected population. Conclusions: Using a brief physician interview, we found evidence for an association between hazardous alcohol consumption and subsequent risk of SLD among PLWHIV, but this was not independent of HIV mode of transmission, HCV-infection and smoking. Abstract

The effects of the synthesis steps of a platinum group metal-free (PGM-free) catalyst on the surface chemistry, morphology, and electrochemical activity in acidic and alkaline media toward the oxygen reduction reaction (ORR) were studied. Each step exhibits a positive impact on catalyst activity. In acid media, etching of the silica template is the major contributor to the enhancement of the half-wave potential, as the ORR active sites formed during the first pyrolysis become more accessible. Further processing steps result even in higher accessibility and utilization of the 4e– transfer sites. In alkaline media, the second pyrolysis is a critical step that favors the complete reduction of oxygen to water, as the peroxide production is significantly diminished. The large heterogeneity in the porosity at each synthesis step indicates that this parameter needs to be further studied to attain better control of the morphology of the PGM-free catalyst, as it is an important factor that contributes to the active site utilization. The acid etching and second pyrolysis increase the meso- and macroporosity. Understanding the effects of each of the synthesis steps on the chemical composition, morphology and ORR activity of the PGM-free catalyst provides the necessary feedback for the design of synthetic schemes that increase the catalysts’ performance.

Abstract

We report the detection of volatile organic compound 1-cyclohexyl-2-pyrrolidone (CHP), in a bioelectrochemical system inoculated with Escherichia coli and supplemented with redox mediator 2-hydroxy-1,4-naphthoquinone (2-HNQ), using a functionalized carbon nanotube-coated screen-printed electrode (CNT-SPE) modified with –COOH and -NH2 functional groups. Addition of CHP decreased current output of the bioelectrochemical system, thus enabling rapid detection of CHP concentration. Under optimal conditions, the H2N-CNT-SPE was the best performing and a limit of detection (LOD) of 0.4 mg L−1 was achieved. The proposed sensor showed low interference from other inorganic and organic components. These results outline a viable strategy for detecting organic contaminants in environmental monitoring applications.

Abstract

BackgroundAlcohol consumption is a known risk factor for liver disease in HIV-infected populations. Therefore, knowledge of alcohol consumption behaviour and risk of disease progression associated with hazardous drinking are important in the overall management of HIV disease. We aimed at assessing the usefulness of routine data collected on alcohol consumption in predicting risk of severe liver disease (SLD) among people living with HIV (PLWHIV) with or without hepatitis C infection seen for routine clinical care in Italy.MethodsWe included PLWHIV from two observational cohorts in Italy (ICONA and HepaICONA). Alcohol consumption was assessed by physician interview and categorized according to the National Institute for Food and Nutrition Italian guidelines into four categories: abstainer; moderate; hazardous and unknown. SLD was defined as presence of FIB4 > 3.25 or a clinical diagnosis of liver disease or liver-related death. Cox regression analysis was used to evaluate the association between level of alcohol consumption at baseline and risk of SLD.ResultsAmong 9542 included PLWHIV the distribution of alcohol consumption categories was: abstainers 3422 (36%), moderate drinkers 2279 (23%), hazardous drinkers 637 (7%) and unknown 3204 (34%). Compared to moderate drinkers, hazardous drinking was associated with higher risk of SLD (adjusted hazard ratio, aHR = 1.45; 95% CI: 1.03–2.03). After additionally controlling for mode of HIV transmission, HCV infection and smoking, the association was attenuated (aHR = 1.32; 95% CI: 0.94–1.85). There was no evidence that the association was stronger when restricting to the HIV/HCV co-infected population.ConclusionsUsing a brief physician interview, we found evidence for an association between hazardous alcohol consumption and subsequent risk of SLD among PLWHIV, but this was not independent of HIV mode of transmission, HCV-infection and smoking. More efforts should be made to improve quality and validity of data on alcohol consumption in cohorts of HIV/HCV-infected individuals.

Abstract

Antifibrinolytics combined with aPCC are not routinely administered to patients with acquired hemophilia A due to increased thrombotic risk. This association normalizes clot stability, and improves the efficacy of therapy, but can increase the risk of severe side effects. Due to these premises it has always raised doubts and perplexities in the clinics. We now report the data of the "FEIBA® on acquired haemophilia A Italian Registry (FAIR Registry)", a retrospective-prospective study that included 56 patients. This is the first study that assessed the clinical response of the combination of aPCC and antifibrinolytic agents in patients with acquired haemophilia A. A total of 101 acute bleeds were treated with aPCC. Antifibrinolytic agents were used in the treatment of 39.6% of total bleeds, based on both, a clinical assessment and an evaluation of bleeding. Twenty-five of the 30 patients (57.1%) treated with antifibrinolytic drugs showed serious co-morbidity. Among them, 40% presented severe cardiovascular diseases. All bleeds treated with combined therapy had a shorter duration of treatment (mean reduction 16.3%). All the treated patients presented a good tolerability and no arterial or venous thromboembolic events were reported. In our retrospective registry the combination of antifibrinolytics and aPCC appears safe and effective in the treatment of patients with AHA, especially in the case of severe and life-threatening bleeding, but this hypothesis needs to be confirmed in adequate, larger clinical trials.

Abstract

Objectives: Our aim was to investigate the durability of different initial regimens in patients starting ART with CD4+ counts 5 log10 copies/mL. Methods: This was a retrospective study of HIV-infected patients prospectively followed in the ICONA cohort. Those who started ART with boosted protease inhibitors (bPIs), NNRTIs or integrase strand transfer inhibitors (InSTIs), with CD4+ 5 log10 copies/mL, were included. The primary endpoint was treatment failure (TF), a composite endpoint defined as virological failure (VF, first of two consecutive HIV-RNA >50 copies/mL after 6 months of treatment), discontinuation of class of the anchor drug or death. Independent associations were investigated by Poisson regression analysis in a model including age, gender, mode of HIV transmission, CDC stage, HCV and HBV co-infection, pre-treatment HIV-RNA, CD4+ count and CD4+/CD8+ ratio, ongoing opportunistic disease, fibrosis FIB-4 index, estimated glomerular filtration rate, haemoglobin, platelets, neutrophils, calendar year of ART initiation, anchor drug class (treatment group) and nucleos(t)ide backbone. Results: A total of 1195 patients fulfilled the inclusion criteria: 696 started ART with a bPI, 315 with an InSTI and 184 with an NNRTI. During 2759 person-years of follow up, 642 patients experienced TF. Starting ART with bPIs [adjusted incidence rate ratio (aIRR) (95% CI) 1.62 (1.29-2.03) versus starting with NNRTIs; P5 log10 HIV-RNA copies/mL, the durability of regimens based on InSTIs was longer than that of NNRTI- and bPI-based regimens.

Abstract

The proportion of new diagnoses of HIV infection in immigrants residing in Italy raised from 11% in 1992 to 29.7% in 2018. To investigate the HIV clades circulating in this community a retrospective study was performed in 557 HIV-infected immigrants living in 12 Italian cities. Immigrants originated from East-Europe and Central-Asia (11.7%), North Africa and Middle East (7.3%), South and South-East Asia (7.2%), Latin America and the Caribbean (14.4%), and sub-Saharan Africa (59.4%). More than 87% of immigrants were on antiretroviral therapy (ART), although 26.6% of them were viremic. A 22.0% of immigrants had hepatitis (HBV and/or HCV) and/or tuberculosis. HIV phylogenetic analysis on sequences from 192 immigrants showed the presence of clades B (23.4%), G (16.1%), C (10.4%), A1 (9.4%), F1 (5.2%), D (1.6%) and Circulating Recombinant Forms (CRFs) (33.9%). CRF02_AG represented 72.3% of the total CRFs. Clusters between immigrants and Italian natives were also present. Drug resistance mutations to NRTI, NNRTI, and PI drug classes occurred in 29.1% of ART-treated and in 12.9% of ART-naïve individuals. These data highlight the need for tailored public health interventions in immigrants to avoid spreading in Italy of HIV genetic forms and ART-resistant variants, as well as HIV co-morbidities.

Abstract

Background - In Italy, the National Register of Congenital Coagulopathies (NRCC) collects epidemiological and therapeutic data from patients affected by haemophilia A (HA), haemophilia B (HB), von Willebrand's disease (vWD) and other rare coagulation disorders. Here we present data from the 2016 annual survey. Materials and methods - Data are provided by the Italian Haemophilia Centres, on a voluntary basis. Information flows from every Centre to a web-based platform of the Italian Association of Haemophilia Centres, shared with the Italian National Institute of Health, in accordance with current privacy laws. Patients are classified by diagnosis, disease severity, age, gender and treatment-related complications. Results - In 2016, the total number of patients with congenital coagulopathies in the NRCC was 10,360: 39.8% of these patients had HA, 31.5% had vWD, 8.5% had HB, and 20.2% had less common factor deficiencies. The overall prevalence of HA and HB was 13.9/100,000 males and 3.0/100,000 males, respectively. The overall prevalence of vWD was 5.4/100,000 inhabitants. During 2016, 126 patients had current alloantibodies to factor VIII (FVIII) or factor IX (FIX) and were under treatment with bypassing agents and/or immune tolerance induction. Overall, 388 patients with a history of alloantibodies were recorded in the NRCC of whom 337 with severe HA and 12 with severe HB. Coagulation factor use, evaluated from treatment plans, was approximately 451,000,000 IU of FVIII for HA patients (7.5 IU/inhabitant), and approximately 53,000,000 IU of FIX for HB patients (0.9 IU/inhabitant). Discussion - The prevalences of HA and HB fall within the ranges reported in more developed countries; the consumption of FVIII and FIX was in line with that of other European countries (France, United Kingdom) and Canada. The NRCC, with its bleeding disorder dataset, is a helpful tool for shaping public health policies, as well as planning clinical and epidemiological research projects.

Abstract

Microbial fuel cells (MFCs), despite representing a promising technology, suffer from low power generation that hinders, in most of the cases, their application as power sources. In fact, MFCs are usually coupled with supercapacitors or batteries and these storage units accumulate the energy harvested by MFCs and deliver it on demand. In this work, the electrodes of a MFC are used as electrodes of an internal supercapacitor and discharges and self-recharges are performed and investigated. Discharges between 1.5 mA and 4 mA were presented produc- ing a maximum power of 1.59 mW. Discharges between 1 mA and 100 mA and recharges are systematically studied for three commercial supercapacitors (SCs) with different capacitances of 1 F, 3 F, and 6 F. The MFC was also connected in parallel with external SCs and discharged galvanostatically. The SC was self- recharged by the MFC without any additional external power sources. At lower current pulses, the MFC contributed to the overall capacitance, probably owing to its faradaic component. At higher current pulses, the use of SCs enables the energy to be harvested by MFCs at power levels that could not be achieved with the MFC alone. This study demonstrates that, through the proper connection and operation mode of the MFC and SC, it is possible to improve and maximize the performance of every single unit. Understanding the MFCSC combination is important for identifying the right practical application for which the combination is suitable.

Abstract

Emicizumab has been approved in several countries for regular prophylaxis in patients with congenital haemophilia A and FVIII inhibitors because it substantially reduces their bleeding risk and improves quality of life. However, although significantly less frequent, some breakthrough bleeds may still occur while on emicizumab, requiring treatment with bypassing or other haemostatic agents. Thrombotic complications have been reported with the associated use of activated prothrombin complex concentrates. In addition, when surgery/invasive procedures are needed while on emicizumab, their management requires multidisciplinary competences and direct supervision by experts in the use of this agent. Given this, and in order to expand the current knowledge on the use of emicizumab and concomitant haemostatic agents, and reduce the risk of complications in this setting, the Italian Association of Haemophilia Centres (AICE) here provides guidance on the management of breakthrough bleeds and surgery in emergency situations in patients with haemophilia A and inhibitors on emicizumab prophylaxis. This paper has been shared with other National Scientific Societies involved in the field.

Abstract

Platinum group metal-free (PGM-free) catalysts are the most promising materials for substituting expensive platinum catalyst for efficient oxygen reduction reaction (ORR), particularly for microbial fuel cells. For these devices contamination due to wastewater is one of the major issues due to the presence of various poisoning anions. The known nitrite contamination effect over PGM-free was studied through rotating disk electrode (RDE) technique in neutral media to understand its patterns. The results were then compared to other contaminants commonly found in wastewater such as chloride (Cl-), perchlorate (ClO4-), and nitrate (NO3-) in the concentration range of 0.05-50 mM. Onset potential (Eonset), half wave potential (E1/2), limiting disk current density (Jlim) and Tafel slopes variations were the parameters exploited to identify specific or unspecific adsorbed contaminants. Chloride and nitrate had no negative effect on ORR performance, while perchlorate slightly reduced the catalyst function with no permanent issues. Durability tests (1000 cycles) were also performed to ensure the stability of the catalyst for relatively long time.

Abstract

Quaternary ammonium poly(2,6-dimethyl 1,4-phenylene oxide) (QAPPO) anion exchange membranes (AEMs) with topographically patterned surfaces were assessed in a microbial desalination cell (MDC) system. The MDC results with these QAPPO AEMs were benchmarked against a commercially available AEM. The MDC with the non-patterned QAPPO AEM (Q1) displayed the best desalination rate (a reduction of salinity by 53 ± 2.7%) and power generation (189 ± 5 mW m− 2) when compared against the commercially available AEM and the patterned AEMs. The enhanced performance with the Q1 AEM was attributed to its higher ionic conductivity and smaller thickness leading to a reduced area specific resistance. It is important to note that Real Pacific Ocean seawater and activated sludge were used into the desalination chamber and anode chamber respectively for the MDC – which mimicked realistic conditions. Although the non-patterned QAPPO AEM displayed better performance over the patterned QAPPO AEMs, it was observed that the anodic overpotential was smaller when the MDCs featured QAPPO AEMs with larger lateral feature sizes. The results from this study have important implications for the continuous improvements necessary for developing cheaper and better performing membranes in order to optimize the MDC., Highlights • Novel anion exchange membranes were used to develop effective MDCs. • AEMs with different topographical patterns were investigated. • Maximum power achievement was 189 ± 5 mW m− 2. • Maximum desalination achieved was 53 ± 2.7% over 3 days. • QAPPO AEM membrane outperformed commercial membrane.

Abstract

In this work, a platinum group metal-free (PGM-free) catalyst based on iron as transitional metal and Nicarbazin (NCB) as low cost organic precursor was synthesized using Sacrificial Support Method (SSM). The catalyst was then incorporated into a large area air-breathing cathode fabricated by pressing with a large diameter pellet die. The electrochemical tests in abiotic conditions revealed that after a couple of weeks of successful operation, the electrode experienced drop in performances in reason of electrolyte leakage, which was not an issue with the smaller electrodes. A decrease in the hydrophobic properties over time and a consequent cathode flooding was suspected to be the cause. On the other side, in the present work, for the first time, it was demonstrated the proof of principle and provided initial guidance for manufacturing MFC electrodes with large geometric areas. The tests in MFCs showed a maximum power density of 1.85 W m−2. The MFCs performances due to the addition of Fe-NCB were much higher compared to the iron-free material. Abstract

Takenouchi-Kosaki syndrome (TKS) is a recently delineated syndromic form of thrombocytopenia strictly related to an hot-spot missense variant, p.Tyr64Cys, in CDC42 (Cell Division Control protein 42). Herein we report an additional patient with the p.Tyr64Cys aminoacidic substitution who showed the well-defined phenotypical TKS features and an intermittent, very mild, macrothrombocytopenia at 10.7 years of age (93,000/mL), that was only retrospectively valorized. Outside of this value the PLT count had always been higher than 100,000/mL. We also review literature data from patients carrying this recurrent variant. Our female patient presented with prenatal onset of short stature and microcephaly, camptodactyly, heart defects, typical facial gestalt, developmental delay, and not specific brain abnormalities. After several genetic investigations (karyotype, CGH-Array, targeted NGS analysis for short stature genes), by whole exome sequencing we identified the p.Tyr64Cys in CDC42, occurring de novo. The case presented here provides further evidence that macrothrombocytopenia can be intermittent and thus it might escape attention of clinicians. Without this key feature, TKS clinical presentation can overlap other syndromic forms of short stature. Immunodeficiency, autoimmunity, and malignancies were recently reported in patients with the p.Tyr64Cys substitution, making imperative an early diagnosis of Takenouchi-Kosaki syndrome to organize the most proper follow-up of these pediatric patients. The whole exome sequencing can be a solving tool in the challenge to the rare diseases.

Abstract

: Introduction: There is paucity of data related to potential gender differences in the use of interventions to prevent and treat cardiovascular disease (CVD) among HIV‐positive individuals. We investigated whether such differences exist in the observational D:A:D cohort study. Methods: Participants were followed from study enrolment until the earliest of death, six months after last visit or February 1, 2015. Initiation of CVD interventions [lipid‐lowering drugs (LLDs), angiotensin‐converting enzyme inhibitors (ACEIs), anti‐hypertensives, invasive cardiovascular procedures (ICPs) were investigated and Poisson regression models calculated whether rates were lower among women than men, adjusting for potential confounders. Results: Women (n = 12,955) were generally at lower CVD risk than men (n = 36,094). Overall, initiation rates of CVD interventions were lower in women than men; LLDs: incidence rate 1.28 [1.21, 1.35] vs. 2.40 [2.34, 2.46]; ACEIs: 0.88 [0.82, 0.93] vs. 1.43 [1.39, 1.48]; anti‐hypertensives: 1.40 [1.33, 1.47] vs. 1.72 [1.68, 1.77] and ICPs: 0.08 [0.06, 0.10] vs. 0.30 [0.28, 0.32], and this was also true for most CVD interventions when exclusively considering periods of follow‐up for which individuals were at high CVD risk. In fully adjusted models, women were less likely to receive CVD interventions than men (LLDs: relative rate 0.83 [0.78, 0.88]; ACEIs: 0.93 [0.86, 1.01]; ICPs: 0.54 [0.43, 0.68]), except for the receipt of anti‐hypertensives (1.17 [1.10, 1.25]). Conclusion: The use of most CVD interventions was lower among women than men.
