103. Glaukomversorgung in Deutschland – Ergebnisse einer Mitgliederumfrage von DOG und BVA – Teil 1: Diagnostik

Abstract

Hintergrund: Die Möglichkeiten in der Glaukomversorgung sind in den letzten Jahren immer vielfältiger geworden. Unter den Mitgliedern der Deutschen Ophthalmologischen Gesellschaft (DOG) und des Berufsverbands der Augenärzte Deutschlands (BVA) wurde eine anonymisierte Befragung durchgeführt, um zu erfassen, welche Behandlungswege in der Versorgung von Glaukompatienten gewählt werden, welche diagnostischen Parameter den Augenärztinnen und Augenärzten wichtig sind und welche Rolle Behandlungsleitlinien in der Alltagspraxis zukommt. Ziel der Arbeit: Meinungsbild unter der deutschen Augenärzteschaft über die aktuelle Glaukomversorgung, zentrale diagnostische Parameter und ihre Anwendung in der Alltagspraxis. Material und Methoden: Die Befragung wurde durch einen Online-Fragebogen mit insgesamt 26 Fragen (107 Items) zur Versorgungspraxis in der Glaukomdiagnostik und der Glaukomtherapie durchgeführt. Vollständig ausgefüllte Fragebögen lagen von 1361 Personen vor. Ergebnisse: Die Papillenbeurteilung an der Spaltlampe hat für Augenärzte weiterhin den höchsten diagnostischen Stellenwert. Auch der Rolle der optischen Kohärenztomographie (OCT) kommt eine sehr hohe diagnostische Bewertung zu. Bei der Interpretation verschiedener diagnostischer Parameter ergibt sich eine höhere Sicherheit für papillennahe Parameter unter den Befragten. Eine leitliniengemäße Versorgung wird nach Selbstauskunft der Augenärzte weitgehend betrieben. Etwa zwei Drittel der Befragten führen im ersten Behandlungsjahr zwei oder mehr Gesichtsfelduntersuchungen durch und ebenso eine strukturelle Papillendarstellung einmal im Jahr. Die Rolle der Gonioskopie wird kontrovers gesehen und nicht einheitlich intensiv praktiziert. Diskussion: Die Diagnosestellung beim Glaukom ist eine große klinische Herausforderung. Die verschiedenen diagnostischen Parameter haben einen unterschiedlich hohen Wert für Augenärzte. Morphometrische Verfahren haben eine sehr hohe Bedeutung gewonnen und ermöglichen eine assistierte, jedoch keine automatisierte Diagnostik. Mehr Behandlungsleitlinien und Standards bei der Glaukomversorgung werden gewünscht und sollten in Aus- und Weiterbildung und in den Behandlungsalltag implementiert sein.

104. Notfälle in der Augenheilkunde: Vermittlung anhand interaktiver Key-feature-Fälle für Medizinstudierende

Abstract

Hintergrund: Wichtiges Ziel eines Curriculums für Medizinstudierende ist, die Fähigkeit zum selbstständigen Erkennen und Einordnen von Notfällen zu vermitteln. Die Augenheilkunde steht hierbei aufgrund fachspezifischer „red flags“, also Warnsymptomen und -zeichen, vor der Herausforderung, dass solche selten von anderen Organsystemen hierauf übertragen werden können. Um Medizinstudierende dabei zu fördern, die „red flags“ der Augenheilkunde in ihrer späteren Tätigkeit zu erkennen, entwickelten wir für unser eLearning-Angebot leitsymptomorientierte interaktive Fallvignetten. Material und Methoden: Es wurden 7 interaktive Fallvignetten zu potenziell bedrohlichen ophthalmologischen Symptomen und Zeichen wie „schmerzloser Visusverlust“ oder „rotes Auge“ entwickelt. Hierbei werden Studierende mit Bild und Text durch einen Fall geführt und zu entscheidenden Aspekten („key features“) mit verschiedenen Frageformaten geprüft. Die interaktiven Fälle wurden mithilfe von eLearning-Authoring-Software umgesetzt und als Lernmodule in der Learning-Management-Präsenz der Augenklinik integriert. Die Patientenfälle waren Teil unseres Praktikums der Augenheilkunde. Die Fälle wurden im Anschluss von den Studierenden evaluiert. Ergebnisse: Die Fälle wurden im Mittel mit einer Note von 1,51 ± 0,68 (Mittelwert ± Standardabweichung) bewertet (n= 163). Auf einer Likert-Skala wurden sie mit 1,60 ± 0,81 als hilfreich für das eigene Lernen empfunden (1 = sehr hilfreich, 7 = gar nicht hilfreich; n= 164). Die Informationsmenge und Auswahl der Szenarien wurden ebenfalls positiv evaluiert. Diskussion: Um Studierenden im engen zeitlichen Rahmen eines Kurses mehr Sicherheit im Erkennen und der primären Versorgung von augenärztlichen Notfällen verschaffen zu können, können praxisorientierte Key-feature-Fälle Bestandteil eines eLearning-Angebotes sein.

119. Association of Birth Weight with Peripapillary Retinal Nerve Fiber Layer Thickness in Adulthood-Results from a Population-Based Study

Abstract

Purpose Low birth weight is associated with altered retinal development in childhood, including reduced peripapillary retinal nerve fiber layer (pRNFL) thickness. However, to the best of our knowledge, no population-based study has analyzed the relationship of low birth weight to pRNFL thickness in adulthood. The purpose of this study was to investigate whether birth weight has a long-term effect on pRNFL thickness in adulthood. Methods In the German population-based Gutenberg Health Study (GHS), participants were examined with spectral-domain optical coherence tomography using a peripapillary scan and automated measurement of pRNFL thickness as a global parameter and in six sectors. The association between self-reported birth weight and the different pRNFL sectors were analyzed with multivariable linear regression, adjusted for potential confounders including sex, age, axial length, self-reported age-related macular degeneration, and glaucoma. Results In 3,028 participants, self-reported birth weight was documented and pRNFL measurements were successfully performed (1632 females, ages 54.9 ± 10.0 years). After adjustment for several confounders in the multivariable model, a positive association was observed between birth weight and pRNFL thickness in the global sector (β = 0.13 µm/100 g; 95% CI, 0.08–0.18; P < 0.001; R2 = 0.007) and especially in the inferotemporal sector (β = 0.22 µm/100 g; 95% CI, 0.15–0.29; P < 0.001; R2 = 0.008) and inferonasal sector (β = 0.28 µm/100 g; 95% CI, 0.17–0.39; P < 0.001; R2 = 0.005). Conclusions Our data show that there is a weak relationship between birth weight and pRNFL thickness in adulthood. This weak association is particularly present in the inferior part of the optic nerve head. Therefore, low birth weight may have an impact on optic nerve head development and potentially on ocular disease development.

132. Postoperative results after iris reconstruction with an Artificialiris®implant with consideration of the cause of the iris defects

Abstract

Purpose To test the hypothesis that severely damaged eyes are more likely to experience postoperative complications after an ArtificialIris®(AI) implantation than other iris defects.Methods Patients after iris reconstruction with an AI were included in this consecutive case series of the Department of Ophthalmology, University of Mainz, Germany. Best-corrected visual acuity (BCVA), objective refraction (KR 8900 Topcon, Tokyo, Japan) and Scheimpflug imaging of the anterior segment with Pentacam®(Oculus, Wetzlar, Germany)and postoperative complications were evaluated.Results Thirty-two eyes of 32 patients after AI implantation were included, separated in partial or total aniridia in a closed globe (group 1, n = 16) and after perforating globe injury (group 2, n = 16). Two or more previous surgeries in the corresponding eye were found in two patients (12.5%) in group 1 and eleven patients (69%) in group 2 (p = 0.001). The time span between initial trauma or iris defect and AI implantation was not significantly related to the occurrence and severity of postoperative complications (p= 0.89). Postoperative complications were classified into mild and severe and showed no differences between group 1 and group 2 (mild: 1 vs. 0; p= 0.52; severe: 5 vs. 6 p= 0.8).Conclusion AI implantation is a treatment option for various iris defects. An individual, case-based decision should be made with strict indication also considering other possible methods for pupil reconstruction. The postoperative outcome is not affected by the time point of AI implantation. So, an implantation is already possible as early as six weeks after the previous trauma.

Abstract

Background By identifying diseases of the anterior segment of the eye associated with exposure to UV light, recommendations for action can be derived. Aim After reading this review, the reader should be familiar with UV light-associated diseases of the anterior segment of the eye. Method Using a selective literature search, UV light-associated diseases of the anterior segment of the eye were identified and protective mechanisms are described. Results The UV light-associated lesions of the anterior segment of the eye include basal cell and squamous cell carcinomas, malignant melanoma of the eyelids and conjunctiva, pterygium, keratoconjunctivitis photoelectrica and climatic droplet keratopathy as well as cortical cataract. Conclusion Eyeglasses for filtering UV light, sunglasses and special safety glasses, such as welding helmets and wearing headgear protect against UV light exposure to the anterior segment of the eye and the associated diseases.

140. Ophthalmic emergencies: training via interactive key feature cases for medical students

Abstract

Background. Autonomous diagnosis and assessment of medical emergencies are important skills to acquire for medical students. Ophthalmology features certain specialty-specific red flag signs and symptoms, which pose a challenge for educators in ophthalmology. To support medical students in identifying those red flags we developed and implemented interactive cases for our e-learning platform. Material and methods. A total of seven interactive cases with key feature problems regarding potentially dangerous signs and symptoms, such as painless loss of vision or red eye were developed. Medical students were guided through a case and performed formative assessments. The interactive cases were created with e-learning authoring software and were available on the learning management system presence of the department of ophthalmology. They were mandatory for medical students in the ophthalmology course. Students evaluated the cases after the course. Results. The interactive cases were rated on average at 1.51 +/- 0.68 (mean +/- standard deviation; n = 163) on a grade scale (1 = best, 6 = worst). On a Likert scale they were perceived as helpful for individual learning at 1.60 +/- 0.81 (1 = very helpful, 7 = not helpful at all; n = 164). The information provided on the cases and selection of scenarios was positively evaluated. Conclusion. To support students in identifying and managing ophthalmic emergencies in the context of limited time in tightly packed curricula, interactive key feature cases can be part of corresponding e-learning resources. An integration of such cases was evaluated as desirable.

Abstract

Background/Aims: This study aimed to analyze the effects of perinatal history on tear film properties and lid geometry in adults born preterm. Methods: The Gutenberg Prematurity Eye Study (GPES) is a German prospective examination of adults born preterm and term aged 18 to 52 years with Keratograph® 5M and Schirmer test I. Main outcome measures were first non-invasive tear film break-up time (F-NITBUT), bulbar redness (BR), Schirmer test, and nasal palpebral angle measurement. The associations with gestational age (GA), birth weight (BW), and BW percentile, retinopathy of prematurity (ROP), ROP treatment, and other perinatal factors were evaluated using regression analyses. Results: 489 eyes of 255 preterm and 277 eyes of 139 full-term individuals (aged 28.6 +/− 8.8 years, 220 females) were included. Of these, 33 participants (56 eyes) had a history of spontaneously regressed ROP and 9 participants (16 eyes) had a history of ROP treatment. After adjustment for age and sex, lower F-NITBUT (<20 s) was associated with ROP treatment (OR = 4.42; p = 0.025). Lower GA correlated with increased bulbar redness (B = −0.02; p = 0.011) and increased length of wetting in the Schirmer test (B = −0.69; p = 0.003). Furthermore, low GA was associated with narrowing of the nasal palpebral angle (B = 0.22; p = 0.011) adjusted for age and sex, but not when considering ROP in the multivariable model. Conclusion: Our analyses indicate that perinatal history affects ocular surface properties, tear production and lid geometry in adults born term and preterm. This might indicate that affected persons have a predisposition to diseases of the corneal surface such as the dry eye disease.

Abstract

Prematurity, prenatal growth restriction, and retinopathy of prematurity (ROP) are associated with altered ocular geometry, such as a steeper corneal shape in childhood, but it is unclear whether perinatal history affects corneal thickness development, so this study investigated whether corneal thickness in adulthood is affected by perinatal history. The Gutenberg Prematurity Eye Study (GPES) is a retrospective cohort study with a prospective ophthalmologic examination in Germany. The corneal thickness was measured by Scheimpflug imaging (Pentacam HR, Oculus Optikgeräte GmbH, Wetzlar, Germany), and the relationship between perinatal parameters respective birth weight percentile and corneal thickness at different locations was assessed using uni- and multivariable linear regression models. Covariates included age, sex, mean corneal radius, white-to-white distance, gestational age, birth weight percentile, ROP occurrence, and treatment. The main outcome measures were corneal thickness at the apex, the pupil center, and the corneal periphery. The corneal thickness was measured in 390 participants (754 eyes, mean age 29.7+/-8.7 years, 224 females). In multivariable analyses, a lower birth weight percentile was associated with a lower corneal thickness at the apex (B = 0.20, p = 0.003) and the pupil (B = 0.19, p = 0.007). These effects diminished towards the corneal periphery and were not observed beyond the 4-mm diameter circle around the thinnest corneal position. Neither gestational age, ROP occurrence, or ROP treatment affected the corneal thickness. A lower birth weight percentile in subjects born preterm as a proxy for restricted fetal growth is associated with corneal thickness thinning in adults aged 18 to 52 years, indicating that corneal thickness development, particularly in the corneal center, may originate in the fetal stage.
