However, dietary habits are difficult to modify and most education programs are ineffective.Design: A phenomenological qualitative study was conducted. Intentional sampling was performed in ten older persons with KO who had lost weight and improved their health-related quality of life after participating in a health education program. A thematic content analysis was conducted following the stages proposed by Miles and Huberman.Findings: Participants understood obesity as a risk factor for health problems and stigma. They believed that the cause of obesity was multifactorial and criticized health professionals for labeling them as “obese” and for assigning a moral value to slimness and diet. The factors identified as contributing to the effectiveness of the program were a tolerant attitude among health professionals, group education that encouraged motivation, quantitative dietary recommendations, and a meaningful learning model based on social learning theories.Conclusion: Dietary self-management without prohibitions helped participants to make changes in the quantity and timing of some food intake and to lose weight without sacrificing some foods that were deeply rooted in their culture and preferences. Dietary education programs should focus on health-related quality of life and include scientific knowledge but should also consider affective factors and the problems perceived as priorities by patients. The objective of this study was to assess the effectiveness of an education program and telephone call follow-up at improving the health related quality of life (HRQL) of patients with chronic obstructive pulmonary disease (COPD). Method: Experimental, controlled, randomized, single blind study, masked data analysis. Duration of 2 years and 3 months. Patients hospitalised for exacerbation. The effectiveness was evaluated by calculating the absolute and relative change (%) of the St. George questionnaire scores (total and by dimensions) before and after the intervention program. Calculation of the effect of the group variable on the absolute and relative changes of the variables, Multiple Analysis of Variance (MANOVA). Results: Completed study of 116 patients. Greater effects on their HRQL reported at admission (48.3 +/- SD 20.0 years). Patients in the intervention group improved significantly in their total SGRQ scores (-6.83) in absolute and relative terms and more significantly in their activity dimension (-16.05). Conclusions: The education program was effective at improving global HRQL, especially the activity dimension, in exacerbated COPD patients. Practice implications: This research contributes to clarifying the benefits and contents of education programs for patients with COPD; hospital admission is the suitable moment to contact these patients. Objective: to evaluate the efficacy of quality of life questionnaires St. George Respiratory Questionnaire and Chronic Obstructive Pulmonary Disease Assessment Test in patients with chronic obstructive pulmonary disease based on correlation and agreement analyses, and identify the most effective tool to assess their quality of life. Method: cross-sectional cohort study with patients hospitalized in a Spanish hospital for exacerbation of chronic obstructive pulmonary disease. Health-related quality of life was assessed with both questionnaires. The correlation and the agreement between the questionnaires were analyzed, as well as the internal consistency. Associations were established between the clinical variables and the results of the questionnaire. Results: one hundred and fifty-six patients participated in the study. The scales had a correlation and agreement between them and high internal consistency. A higher sensitivity of the Chronic Obstructive Pulmonary Disease Assessment Test was observed for the presence of cough and expectoration. Conclusion: the questionnaires have similar reliability and validity to measure the quality of life in patients with acute chronic obstructive pulmonary disease, and the Chronic Obstructive Pulmonary Disease Assessment Test is more sensitive to detect cough and expectoration and requires a shorter time to be completed.

Background/Objectives The goal of this article is to present and demonstrate the applicability of an original method to assess the economic and health impacts of compliance with food-based recommendations. The method takes account of consumers’ preferences and the associated adoption cost in the assessment of various recommendations. Subjects/Methods We combine an economic model of diet choice with an epidemiological model to compute the health impacts of dietary changes. To demonstrate the use of the method, we analyse the impacts of a 5% variation in the consumption of seven food groups taken separately: a 5% increase in consumption of fruits and vegetables (F&V;) and milk products; and a 5% decrease in consumption of red meat, all meats, salty/sweet products, ready meals and butter/cream/cheese. Results A recommendation, when adopted by consumers, generates important changes in the whole diet due to substitutions and complementarities among foods. All simulated recommendations have a positive impact on health. The F&V; recommendation has the largest impact on the number of DALYs averted, but the highest adoption cost for consumers, especially for low-income consumers. Alone, the change in energy intake explains from 71% to 98% of the DALYs averted induced by a recommendation. Conclusions Small increases in recommended foods have the potential of generating relatively significant health gains. Preference-driven substitutions among foods have a major effect on simulated health outcomes and should be included in the assessment of dietary recommendations, together with the adoption cost borne by consumers.

The share of the elderly population in France increases steadily, raising concerns that the aging households cannot maintain the pre-retirement level of consumption. We use panel data on food purchases to investigate whether households use the variation in the life-cycle availability of the two inputs of home production time and money, to sustain consumption. Our results indicate that food consumption peaks in early 40 s, drop afterwards in the late 50 s. However, it picks up at the early 60 s dramatically by as much as 40% compared to the late 50 s. This would indicate that food consumption remains uncompromised as households age. We investigate the effects of promoting simple climate-friendly diet recommendations in Denmark, Finland and France, with the objectives of identifying recommendations that lower greenhouse gas emissions, improve public health, and are cost-beneficial. The simulation approach combines a behavioural model of consumption adjustment to dietary constraints, a model of climate impact based on the life-cycle analysis of foods, and an epidemiological model calculating health outcomes. The five recommendations considered in the analysis focus on consumption of fruits and vegetables, red meat, all meat and all animal products, as well as the greenhouse gas emissions arising from the diet. The results show that trade-offs between climate and health objectives occur for some recommendations in all countries, and that substitutions may result in unintended effects. However, we identify some recommendations that would raise sustainability in its climate and health dimensions, while delivering value for money and increasing social welfare. In particular, promoting consumption of fruits and vegetables through campaigns of the five-a-day type is found to be cost-beneficial in all three countries. By contrast, targeting consumption of meat, consumption of all animal products, or the climate footprint of diets through social marketing campaigns is only found to be desirable in some country-specific contexts. Objective: To evaluate the level of participation of clinical nurses from Castellon where Universitat Jaumei nursing students do their clinical clerkship. To identify the variables that may influence clinical nurses' participation in students' clinical mentorship. Method: This observational, cross-sectional and descriptive study was conducted by applying the validated Involvement, Motivation, Satisfaction, Obstacles and Commitment (IMSOC) questionnaire. The variables collected were: age, work environment and previous training. The study was conducted between January and December 2014. Results: The sample included 117 nurses. The overall mean questionnaire score was 122.838 (standard deviation: +/- 18.692; interquartile range 95%: 119.415-126.26). Objective: To determine the association and predictive nature of certain socio-demographic, education, work and research variables on nurses' participation level in the clinical care of students. Design: A cross-sectional analytical study using a validated questionnaire between February and June 2014. Setting and Participants: A consecutive sample of 117 nurses who worked in public health centres in the province of Castellon (Spain) in 2014. The nurses who had never mentored students and the questionnaires that were < 50% complete were excluded. Methods: A descriptive analysis of the sample and an association analysis between variables were performed. The questionnaire and its dimensions were performed with a logistic regression and the maximum likelihood method, which used a complementary log-log link method. The concordance index was calculated using contingency tables. Results: The mean age was 42.56 years, and the overall mean questionnaire score was 122.84 (SD = 18.69; 95% CI: 119.415-126.26). Across the sample, 58.1% (n = 68) of the nurses obtained an Excellent score, followed by Adequate in 41% (n = 48). Overall, the predictive variables were age, mentoring of students in the last 5 years and previous training to mentor students. Conclusion: The main predictive variable for greater participation in the mentorship of students was previous training in mentoring. This study also reflected on other variables that could influence nurses' participation in student mentoring.

Objective To compare the agreement between two surgical checklists implanted in two hospitals in Spain and Argentina, using the international classification for patient safety as a framework. Method This was an expert opinion study carried out using an ad hoc questionnaire in electronic format, which included 7 of the 13 categories of the international classification for patient safety. Fifteen surgical security experts from each country participated in this study by classifying the items on the checklists into the selected ICPS categories. The data were analyzed with SPSS V20 software. Results There was a greater percentage of classifications in fields related to the prevention of critical events. The category “clinical processes and procedures” was mentioned most frequently in both lists. Conclusion The implementation of the surgical safety checklist is variable. Experts considered that the Argentinian list was clearer in every dimension.

Background: Writing a bachelor thesis (BT) is the last step to obtain a nursing degree. In order to perform an effective assessment of a nursing BT, certain reliable and valid tools are required. Objectives: To develop and validate a 3-rubric system (drafting process, dissertation, and viva) to assess final year nursing students' BT. Design: A multi-disciplinary study of content validity and psychometric properties. The study was carried out between December 2014 and July 2015. Settings: Nursing Degree at Universitat Jaume I. Spain. Participants: Eleven experts (9 nursing professors and 2 education professors from 6 different universities) took part in the development and content validity stages. Fifty-two theses presented during the 2014-2015 academic year were included by consecutive sampling of cases in order to study the psychometric properties. Methods: First, a group of experts was created to validate the content of the assessment system based on three rubrics (drafting process, dissertation, and viva). Subsequently, a reliability and validity study of the rubrics was carried out on the 52 theses presented during the 2014-2015 academic year. Results: The BT drafting process rubric has 8 criteria (S-CVI = 0.93; alpha= 0.837; ICC = 0.614); the dissertation rubric has 7 criteria (S-CVI = 0.9; alpha= 0.893; ICC = 0.74), and the viva rubric has 4 criteria (S-CVI = 0.86; alpha= 8.16; ICC = 0.895). Conclusion: A nursing BT assessment system based on three rubrics (drafting process, dissertation, and viva) has been validated. This system may be transferred to other nursing degrees or degrees from other academic areas. Introduction: Congenital malformations of the lung (CML) are rare but potentially dangerous congenital malformations. Their identification is important in order to define the most appropriate management. Materials and methods: We retrospectively reviewed data from 37 patients affected by CML treated in our Pediatric Surgery Unit in the last four years with minimally invasive surgery (MIS). Results: Prenatal diagnosis was possible in 26/37 patients. Surgery was performed in the first month of life in 3 symptomatic patients and between 6 and 12 months in the others. All patients underwent radiological evaluation prior to thoracoscopic surgery. Images collected were reconstructed using the VR render software. Discussion and conclusions: Volume rendering gives high anatomical resolution and it can be useful to guide the surgical procedure. Thoracoscopy should be the technique of choice because it is safe, effective and feasible. Furthermore it has the benefit of a minimal access technique and it can be easily performed in children.

En este artículo se plantea una forma novedosa de análisis y de presentación de los resultados generados por el estudio Global Entrepreneurship Monitor–GEM, denominada “La Tubería Empresarial”. En adición a su definición conceptual y metodológica se hace una aplicación concreta a los resultados obtenidos en cuatro países del Caribe, que participaron en los ciclos 2011 y 2012 del proyecto GEM Caribbean, apoyado por el IDRC de Canadá. La “Tubería Empresarial” resulta ser un mecanismo que analiza mejor el proceso empresarial, facilita la comprensión y presentación de los resultados, identifica las “fugas” empresariales y permite formular políticas específicas para cada etapa. This article presents a new way to analyze and to present GEM´s results. It´s called the “Entrepreneurial Pipeline”. In addition to the conceptual construction, the paper applies it to the analysis of the data obtained in four Caribbean countries in their 2011 and 2012 GEM national studies. They are members of the GEM Caribbean project supported by IDRC. This procedure analyze better the entrepreneurial process, makes easier the presentation and the understanding of the results, identifies the “leaks”, allows the formulation of specific policies for each stage.

In the tilapia species group, the major sex determining factors have been located on Linkage Group 1 (LG1), on a small chromosome (Chr) pair in Oreochromis niloticus, on LG3 (the largest Chr pair) in O. aureus or on both, depending on populations/strains. LG3 has all the traits of an old sex chromosome, whereas LG1 seems to be an emerging one. Taking advantage of its large size, LG3-Chr has been microdissected to search for sex-linked genes. It was isolated from metaphase spreads of XX-female and YY-male in O. niloticus and of ZZ-male in O. aureus. Using cDNA capture and direct cDNA selection procedures we isolated various transposons but a reduced number of genes. We therefore compared three different whole genomic amplification (WGA) methods (DOP-PCR, GenomePlex and GenomiPhi) using a pool of 30 microdissected chromosomes, to evaluate the best LG3-Chr representation. Loci from 5 microsatellites, 2 genes and 2 uncoded fragments located on LG3-Chr have been searched by PCR on the DNA obtained by the 3 methods. GenomePlex and GenomiPhi gave 60 % loci amplification. GenomePlex probe produced the best painting probe, entirely covering the two LG3-Chrs with weaker signals in the gene-rich pericentromeric region, in both species, confirming that this pair is essentially composed of conserved and specific repeated sequences. This will allow to trace its history within the tilapia group. (Texte intégral)
