Abstract

The discovery of X-rays at the end of the nineteenth century is one of the most important discoveries that changed the human being life in all fields and applications. Since that date, X-ray systems are developed rapidly, and still new applications are emerging in medical, agricultural, and industrial fields. This chapter provides sufficient historical background for X-ray production and image acquisition techniques. It covers also fluoroscopic equipment and radiation dosimetry and protection techniques in detail. Image-guided intervention is here to stay. Thus, suitable radiation protection measures are required to minimize the risk to patients and staff to its minimal level without affecting the outcome of the procedures.

127. Acetone vapor-sensing properties of chitosan-polyethylene glycol using surface plasmon resonance technique

Abstract

To non-invasively monitor and screen for diabetes in patients, there is need to detect low concentration of acetone vapor in the range from 1.8 ppm to 5 ppm, which is the concentration range of acetone vapor in diabetic patients. This work presents an investigation for the utilization of chitosan-polyethylene glycol (PEG)-based surface plasmon resonance (SPR) sensor in the detection of trace concentration acetone vapor in the range of breath acetone in diabetic subjects. The structure, morphology, and elemental composition of the chitosan-PEG sensing layer were characterized using FTIR, UV-VIS, FESEM, EDX, AFM, and XPS methods. Response testing was conducted using low concentration of acetone vapor in the range of 0.5 ppm to 5 ppm using SPR technique. All the measurements were conducted at room temperature and 50 mL/min gas flow rate. The sensor showed good sensitivity, linearity, repeatability, reversibility, stability, and high affinity toward acetone vapor. The sensor also showed better selectivity to acetone compared to methanol, ethanol, and propanol vapors. More importantly, the lowest detection limit (LOD) of about 0.96 ppb confirmed the applicability of the sensor for the non-invasive monitoring and screening of diabetes.

Abstract

• Filters reduce the noise at the expense of visual image quality. • Digital interpretation of images prevents misinterpretation of the images due to blurring of the images. • Dose reduction without compromising the diagnostic. Medical images are usually affected by biological and physical artifacts or noise, which reduces image quality and hence poses difficulties in visual analysis, interpretation and thus requires higher doses and increased radiographs repetition rate. This study aims at assessing image quality during CT abdomen and brain examinations using filtering techniques as well as estimating the radiogenic risk associated with CT abdomen and brain examinations. The data were collected from the Radiology Department at Royal Care International (RCI) Hospital, Khartoum, Sudan. The study included 100 abdominal CT images and 100 brain CT images selected from adult patients. Filters applied are namely: Mean filter, Gaussian filter, Median filter and Minimum filter. In this study, image quality after denoising is measured based on the Mean Squared Error (MSE), Peak Signal-to-Noise Ratio (PSNR), and the Structural Similarity Index Metric (SSIM). The results show that the images quality parameters become higher after applications of filters. Median filter showed improved image quality as interpreted by the measured parameters: PSNR and SSIM, and it is thus considered as a better filter for removing the noise from all other applied filters. The noise removed by the different filters applied to the CT images resulted in enhancing high quality images thereby effectively revealing the important details of the images without increasing the patients' risks from higher doses. Filtering and image reconstruction techniques not only reduce the dose and thus the radiation risks, but also enhances high quality imaging which allows better diagnosis. [ABSTRACT FROM AUTHOR]

141. Radiation safety and shielding evaluation of newly installed medical LINAC facility in Bangladesh

Abstract

Medical linear accelerator (Linac) is widely used for radiotherapy purposes which is one of the modalities for radiation therapy. When a Linac is being installed, an initial survey is required during the first beam and a complete shielding survey must be carried out after the Linac has been operational and calibration is completed. Based on the detailed survey report, the facility license is issued. Worldwide several organizations provide various recommendations regarding shielding design and radiation safety issues. The purpose of this paper is to show how to assess or validate the radiation safety and shielding status of the newly installed Linac facility in Bangladesh. In this study, one of the Linac facilities in Dhaka city has been chosen which was installed in 2021. The dose rate meter RadEYE PRD and tele survey meter TELE-STTC are used for x-ray/gamma-ray dose measurement and REM 12/4 count rate meter for neutron measurement. A maximum photon dose of 5.56 μSvh−1was observed at a gantry angle of 900along the center front of the door, while a maximum neutron dose of 1 μSvh−1was found at three different locations. The obtained results are in line with the recommendations by controlling organizations. This study found that the Linac can be operated safely for both conventional and IMRT modes, and the same shielding procedure can be followed all over the country for the same Linac setting.

Abstract

About 70% of all collective doses from medical tests come from computed tomography (CT), the primary public radiation exposure source. Patients received higher doses during contrast-enhanced CT procedures (Angiography) than routine CT procedures. The current research aims to quantify the patients' radiation doses, estimate the cancer risk probability, and establish institutional diagnostic reference level (IDRL) during lower extremity angiography. One hundred eleven patients (61.3% male and 38.7% female) underwent contrast-enhanced CT procedures. A 160-Slice CT scanner was used for image acquisition for all patient populations. IDRL values were quantified based on the 3rd quartile value of the CT dose descriptors. All procedures were carried out at Al-Moaallim Medical City, Khartoum, Sudan. A computer software (CT-Expo, ImPACT) was used to extrapolate the effective dosage per extremity CTA examination was estimated. The mean patient age (years) was 56.1 ± 18 (18–82). The patient's radiation doses was quantified using volume CTDI and dose length product (DLP) to 7.3 ± 2 (2.9–16.8) (mGy) and 3712 ± 126 (279–8374) (mGy.cm), respectively. The average effective doses were 22 mSv (range: 1.7–50) per lower CT angiography for the lower extremity. The proposed IDRL for the VCTA procedure was 3562 mGy cm and 17 mGy for DLP and CTDIvol, respectively. The proposed IDRL is higher than the previously published studies; therefore, patient dose optimization is recommended. • Radiation doses were determined for patients undergoing VCTA procedure. • A 160-Slice CT scanner with iterative reconstruction techniques was used for examining the patients. • The average effective doses were 22 mSv (range: 1.7–50) per VCTA. • The proposed LDRL for the VCTA procedure was 3562 mGy cm and 17 mGy for DLP and CTDIvol, respectively. Abstract

Background There is an exponential growth of public concern worldwide regarding the exposure to electromagnetic fields (EMF) generated by wind turbines. The high concern of the general population enhances the fact that EMF exposure remains a crucial issue that demands specific actions and reliable data to evaluate possible health hazards. Materials and Methods EMF exposure measurements were conducted in two wind farm (W/F) areas in Greece. The magnetic field strength concentrated in the proximity of 22 wind turbines (20 type of Gamesa Eolica Siemens of total power of 17 MW and 2 type of E53 Enercon, 800 KW each). Measurements were conducted using a Narda-NBM-550 basic unit (Narda Safety Test Solutions Company) using EHP-50F probe. The EMF exposure measurements were processed applying the weighted peak method (WPM), according to the 2013/35/EU Directive. WPM provides correction of the measured value of pulse signals, taking into account the phases of the various frequency components. The data were classified under two scenarios of EMF exposure: 'high wind' and 'low wind' conditions. Results The variation of the magnetic field strength was calculated between 1.13 and 1.42% (WPM). Measurements indicate that the intensity of the emitted magnetic field at the base of the wind turbines, in both high and low wind conditions, was relatively low (mean = 0.146 μT), while decreasing rapidly with increasing distance and reaching background values 6 m from the base. Conclusion The results of the present study indicate that EMF levels are similar or even lower compared to those in urban areas and well below the national and international safety limits. 147. An open-source software for calculating 1D gamma index in radiation therapy.

Abstract

This study was developed to create computer software for performing the gamma index comparison between measurement and Monte Carlo (MC) simulation for the percentage depth dose (PDD) and beam off-center ratio profile (OCR). The gamma software was built in the matrix laboratory (MATLAB) software environment. The developed software was compared with ScanDosematch and Bistromath software's gamma evaluation to assess its accuracy. A set of reference and evaluated dose distribution, which were obtained from measurement and MC simulation, was input to the software to calculate the 1D gamma index using different criteria (i.e. 3%/3 mm, 2%/3 mm, and 2%/2 mm). We compared the two results of gamma index at 3%/3 mm, 2%/3 mm, and 2%/2 mm criteria, one calculated by the proposed software and one manually. The comparison showed high agreement between the proposed software and theoretical calculation. Based on the results, we concluded that our developed software has high accuracy, compared to theoretical calculation. Abstract

Due to the extended localized fluoroscopy, many radiographic exposures, and multiple procedures that might result in tissue reaction, patients and personnel received a significant radiation dose during interventional cardiology (IR) procedures. This study aims to calculate the radiation risk and assess patient and staff effective doses during IC procedures. Thirty-two patients underwent a Cath lab treatment in total. Ten Cath lab personnel, including six nurses, two cardiologists, and two X-ray technologists. Optical stimulating-luminescent dosimeters (OSL) (Al2O3:C) calibrated for this purpose were used to monitor both occupational and ambient doses. Using an automated OSL reader, these badges were scanned. The Air Kerma (mGy) and Kerma Area Products (KAP, mGy.cm2) have a mean and standard deviation (SD) of 371 ± 132 and 26052, respectively. The average personal dose equivalent (mSv) and its range for cardiologists, nurses and X ray technologists were 1.11 ± 0.21 (0.96–1.26), 0.84 ± 0.11 (0.68–1.16), and 0.68 ± 0.014 (0.12–0.13), respectively. The current study findings showed that the annual effective dose for cardiologists, nurses, and X-ray technologists was lesser than the yearly occupational dose limit of 20 mSv recommended by national and international guidelines. The patients' doses are comparable with some previously published studies and below the tissue reaction limits. • Occupational and patient-effective doses were quantified during Cath lab procedures. • The staff dose was measured using calibrated optical stimulating-luminescent dosimeters (OSL) (Al2O3:C). • The annual effective dose for Cath lab personnel is within the defined dose limit. • The Cath lab personnel was well protected according to the current workload. Abstract

Introduction: The purpose of the present work was to evaluate the imaging characteristics of 89Zr-PET in comparison with those obtained using fluorine-18 Fluorodeoxyglucose (18FFDG) PET (a gold standard tracer in PET imaging) using a small-animal NanoScan PET/CT scanner. Methods: The system’s spatial resolution, sensitivity, uniformity, and image quality were measured on a Nano Scan small-animal PET/CT scanner according to the NEMA NU4-2008 protocols. For reconstruction images, we used 2D and 3D reconstruction algorithms. The reconstruction methods included filter back projection (FBP), the ordered subsets expectation maximization (OSEM) algorithm, and the 3D Tera-Tomo algorithm, which are developed for the NanoScan small-animal PET/CT scanner. Results: The results obtained showed a significant difference in the spatial resolution for 89Zr as compared to 22Na and 18F when using a 2D reconstruction algorithm. The spatial resolution values were much enhanced by using the 3D Tera-Tomo reconstruction for each isotope, the Full width at half maximum (FWHM) values was less than 1 for all isotopes at the center of the field of view (FOV). This difference in spatial resolution is dependent on the positron range, energy, and the reconstruction method. Conclusion: The long half-life of 89Zr makes it an ideal positron emitter for performing immuno- PET, which is matched with the biological half-life of intact mAbs.89Zr can also give several advantages over other long half-life positron emitters in relation to the overall imaging performance because of its relatively short positron range and simpler decay scheme. The values of 89Zr sensitivity that were obtained in the present study were less than those of previous studies.
