Abstract

The extent to which stress generation occurs in bipolar spectrum disorders (BSD) is not well understood. The present study examined whether 75 BSD participants experienced elevated rates of behaviordependent life events, as compared with 38 normal control participants. Within the BSD group, we also examined whether depressive or hypomanic symptoms prospectively predicted increases in various types of negative and positive life events. Results indicated that BSD participants experienced overall increases in behavior-dependent events over the follow-up, as compared with normal controls. At the symptom level, the event generation process occurred in more specific event domains. Results suggest that the stress generation theory of unipolar depression can be extended to BSD and that the type of generated events may be polarity-specific. Abstract

Bipolar disorders and substance use disorders (SUDs) show high co-occurrence. One explanation for this co-occurrence may be common personality vulnerabilities involved in both. The authors tested whether high behavioral approach system (BAS) sensitivity and impulsiveness are shared personality vulnerabilities in bipolar spectrum disorders and substance use problems and their co-occurrence in a longitudinal study of 132 individuals on the bipolar spectrum and 153 control participants. At Time 1, participants completed the Behavioral Inhibition System/BAS Scales and the Impulsive Nonconformity Scale. Substance use problems were assessed via the Michigan Alcoholism Screening Test and the Drug Abuse Screening Test at 4-month intervals for 1 year. Participants with bipolar disorder had higher rates of lifetime SUDs and substance use problems during the follow-up, relative to control participants. In line with hypotheses, higher BAS sensitivity and impulsiveness predicted bipolar status and increased substance use problems prospectively. BAS total, BAS Fun Seeking, and impulsiveness mediated the association between bipolar spectrum status and prospective substance use problems, with impulsiveness as the most important mediator. High BAS sensitivity and impulsiveness may represent shared personality vulnerabilities for both disorders and may partially account for their co-occurrence. Abstract

The social zeitgeber theory emphasizes the importance that social rhythm regularity may play in promoting internal synchronization of circadian rhythms in individuals with or at risk for bipolar spectrum disorders. This study examined the relationship of lifestyle regularity, affective symptomatology, and sleep in 71 individuals exhibiting cyclothymic mood and behavior patterns. Participants were randomly assigned to either an experimental group in which they were encouraged to regulate their daily routines or to a control group. Participants in the experimental group were able to successfully regulate their daily schedules. Although relationships between regularity and severity of depressive symptoms, across-day variances in mood and behavior, and sleep duration were identified during baseline, increased lifestyle regularity did not differentially result in changes in these variables. Abstract

INTRODUCTION: Depressive episodes are often prevalent among patients with bipolar disorder, but little is known regarding the differential patterns of development over time. We aimed to determine and characterize trajectories of depressive symptoms among adults with bipolar disorder during 6 months of systematic treatment.METHOD: The pragmatic clinical trial, Bipolar Clinical Health Outcomes Initiative in Comparative Effectiveness (CHOICE) randomized 482 outpatients with bipolar disorder to lithium or quetiapine. Depressive symptoms were rated at up to 9 visits using the Montgomery-Asberg Depression Rating Scale (MADRS). Growth Mixture Modelling was utilized to identify trajectories and multinomial regression analysis estimated associations with potential predictors.RESULTS: Four distinct trajectories of depressive symptoms were identified. The Responding class (60.3%) with a rapid reduction and subsequent low level; the Partial-responding class (18.4%) with an initial reduction followed by an increase during the remaining weeks; the Fluctuating class (11.6%) with a fluctuation in depressive symptoms; the Non-responding class (9.7%) with sustained moderate-severe depressive symptoms. Bipolar type I predicted membership of the Non-responding class and randomization to quetiapine predicted membership of either the Responding or the Non-responding class.CONCLUSION: Approximately 30% experienced a partial or fluctuating course and almost 10% had a chronic course with moderate-severe depression during 6 months. Patients diagnosed with bipolar type 1 had higher risk of being categorized into a class with a worse outcome. While no differences in average overall outcomes occurred between the lithium and quetiapine groups, trajectory analysis revealed that the lithium group had more variable courses. Abstract

Introduction Veterans involved in the justice system are an understudied population warranting attention given their higher risk of mental health concerns and psychosocial challenges. Thus, there is a need for programs to support the psychological health of incarcerated veterans. The present study sought to determine the acceptability and effectiveness of a mind-body stress reduction program for incarcerated veterans. Materials and Methods Twenty-four incarcerated veterans (100% male; Mage  = 44.87 (SD   = 13.84)) completed a 6 week mind-body stress reduction course, or Resilient Warrior. Participants completed validated, self-report assessments of mood, functioning, mindfulness, and coping at pre- and post-program as well as having post-program qualitative feedback surveys. Results Results showed improvement in depressive symptom severity, self-efficacy, and resilience over the study duration (P  < .05), but not stress reactivity, mindfulness, sleep disturbance, or perceived stress. The intervention was reported as helpful by 96% of participants, with 86% of participants stating they would recommend the course to others. Conclusions The Resilient Warrior program is acceptable and well-tolerated for incarcerated veterans as well as may improve aspects of psychological health. Future studies could examine how stress reductions program might improve recidivism rates and quality of life after the release of incarcerated veterans. [ABSTRACT FROM AUTHOR]

Abstract

Objectives: Lithium and quetiapine are known to be effective treatments for bipolar disorder. However, little information is available to inform prediction of response to these medications. Machine-learning methods can identify predictors of response by examining variables simultaneously. Further evaluation of models on a test sample can estimate how well these models would generalize to other samples. Methods: Data (N=482) were drawn from a randomized clinical trial of outpatients with bipolar-I or II disorder who received adjunctive personalized treatment plus either lithium or quetiapine. Elastic Net Regularization (ENR) was used to generate models for lithium and quetiapine; these models were evaluated on a test set. Results: Predictions from the lithium model explained 17.4% of the variance in actual observed scores of patients who received lithium in the test set, while predictions from the quetiapine model explained 32.1% of the variance of patients that received quetiapine. Of the baseline variables selected, those with the largest parameter estimates were: severity of mania; attention-deficit/hyperactivity disorder (ADHD) comorbidity; non-suicidal self-injurious behavior; employment; and comorbidity with each of two anxiety disorders (social phobia/society anxiety and agoraphobia). Predictive accuracy of the ENR model outperformed the simple and basic theoretical models.Conclusion: ENR is an effective approach for building optimal and generalizable models. Variables identified through this methodology can inform future research on predictors of response to lithium and quetiapine, as well as future modeling efforts of treatment choice in BD.

Background: Adjunctive antidepressants are frequently used for bipolar depression but their clinical efficacy has been studied in few trials and little is known about how co‐occurring manic symptoms affect treatment response. Methods: Bipolar Clinical Health Outcomes Initiative in Comparative Effectiveness (N = 482) and Lithium Treatment Moderate‐Dose Use Study (N = 281) were similar comparative effectiveness trials on outpatients with bipolar disorder comparing four different randomized treatment arms with adjunctive personalized guideline‐based treatment for 24 weeks. Adjunctive antidepressant treatment could be used if clinically indicated and was assessed at every study visit. Adjusted mixed effects linear regression analyses compared users of antidepressants to nonusers overall and in different subcohorts. Results: Of the 763 patients, 282 (37.0%) used antidepressant drugs during the study. Antidepressant users had less improvement compared to nonusers on the Clinical Global Impression Scale for Bipolar Disorder and on measures of depression. This was particularly true among patients with co‐occurring manic symptoms. Exclusion of individuals begun on antidepressants late in the study (potentially due to overall worse response) resulted in no differences between users and nonusers. We found no differences in treatment effects on mania scales. Conclusions: In this large cohort of outpatients with bipolar disorder, clinically indicated and guideline‐based adjunctive antidepressant treatment was not associated with lower depressive symptoms or higher mania symptoms. Abstract

Objective: Current treatment guidelines for bipolar disorder (BD) recommend several medications as equally effective first-line mood stabilizers. However, knowledge of moderators in bipolar treatments can determine subsets of patients that would benefit from certain medications. Method: Data (N=482) were drawn from a randomized clinical trial (RCT) of outpatients with bipolar-I or II disorder that received either lithium + adjunctive personalized treatment (APT) or quetiapine+APT. The primary outcome measure was the Clinical Global Impressions Scale-Bipolar Version (CGI-BP) measured at week-24. Variable selection using machine-learning techniques identified predictors of differential response to lithium and quetiapine response, and the Personalized Advantage Index (PAI) was used to identify, for each patient, which treatment was expected to lead to a superior response.Results: Seven pre-treatment variables moderated treatment response in the full sample (leisure, non-suicidal self-injurious behavior, increased appetite, social phobia/social anxiety disorder, mania, attention deficit/hyperactivity disorder, and irritability). Across repeated 10-fold cross-validation, patients who received their PAI-indicated treatment exhibited superior clinical response relative to those that did not receive their indicated treatment (d=0.23; t=2.51, p=0.02). Evidence of the importance of the recommendation strength was found in the larger observed advantage (d=0.32; t=2.70, p=0.01) amongst the subset of patients (60%) with the largest PAIs.Conclusions: Results suggest that a patient’s characteristics can be used to identify whether lithium or quetiapine are to be preferred in the treatment of bipolar disorder. This PAI model should be tested prospectively before implementation can be considered. These findings demonstrate the potential for precision medicine to improve individual outcomes.
