103. Monitoring the use of disease-modifying drugs in multiple sclerosis

Abstract

Background: Current treatment of multiple sclerosis (MS) with disease-modifying drugs (DMDs) includes interferon- (IFN-), glatiramer acetate (GA), and immunosuppressive drugs (e.g., mitoxantrone-MIT, azathioprine-AZA, cyclophosphamide-CY). Knowledge on effectiveness in clinical practice of each DMD, optimal pattern of use (especially, when to start drug therapy) require further investigation. Objectives: To identify criteria for choice of DMDs in MS and to evaluate their comparative effectiveness in clinical practice. Methods: A database of DMDs prescribed to MS pts of an Italian region (4,000,000 inhabitants) provided the following information: patient history, disease characteristics (MS course, disability degree in terms of EDSS, relapses, MRI results), drug therapies (drugs, doses, switches, side effects). Data from May 2006 to December 2007 were collected and the relationship between prescribed drugs and diagnostic parameters was analysed by non-adjusted odds ratio (OR, CI 95%). Results: Data on 934 pts (71% females) were collected. IFN- was prescribed in 73% of cases, followed by GA (12%), by AZA (8%) and by MIT (6%). Other immunosuppressive agents were prescribed only in 1% of cases. IFN-1a represented the most frequent drug choice (67%) in the relapsing remitting pts, whereas in the progressive forms IFN-1b (27%), MIT (25%), and AZA (20%) were significantly preferred to IFN-1a once a week. In pts with high disability (EDSS 3), DMDs with higher frequency of administration (3 times a week, each other day, daily) were significantly preferred to IFN-1a once a week. Conclusions: IFN-1a and GA were preferred in pts with stable and moderate MS, whereas IFN-1b and immunosuppressive agents were especially used in those experienced worsening of the disease. This tool may allow to obtain more solid information on MS therapies and to produce evidence-based recommendations.

Abstract

Introduzione. La pratica clinica attuale per il trattamento della sclerosi multipla, in termini di terapia con farmaci disease-modifying, prevede il ricorso all’interferone-β o a glatiramer, e in seconda istanza a trattamenti immunosoppressivi (es. azatioprina, mitoxantrone). I primi hanno una sufficiente documentazione in questo ambito, ma presentano l’inconveniente dell’alto costo. I secondi sono di uso ampiamente consolidato per altre situazioni che richiedono immunosoppressione, ma ancora in corso di indagine nella sclerosi multipla. Per identificare elementi aggiuntivi riguardo i criteri alla base della scelta dei farmaci e sull’efficacia comparativa dei vari trattamenti, i Centri per la sclerosi multipla in Emilia- Romagna collaborano tra loro in un progetto di monitoraggio delle terapie farmacologiche prescritte. Metodi. Da maggio 2006 è attivo un database regionale in cui, per ogni paziente in trattamento, vengono inserite dal medico specialista le informazioni relative a: dati anagrafici del paziente, storia della malattia, criteri diagnostici della malattia, trattamento scelto, ricadute, modifica del trattamento, reazioni avverse. I dati pervenuti al settembre 2006, sono stati raccolti in un’analisi descrittiva e analizzati statisticamente. Risultati. Sono stati raccolti i dati relativi a 138 pazienti (75% femmine, 48% con meno di 40 anni). Il decorso relapsing remitting è quello maggiormente presente (77%). La scelta del trattamento è ricaduta nell’85% dei casi sull’interferone-β, nell’8% sul glatiramer e nel 6% sul mitoxantrone; 1 solo soggetto ha ricevuto azatioprina. L’interferone-β1a ha rappresentato la scelta più frequente (74%) nel decorso relapsing remitting, mentre nelle forme progressive è prevalso il ricorso a interferone-β1b (42%), seguito da interferone-β1a e mitoxantrone (21%, ciascuno). L’interferone-β1a è stato usato a tutti i gradi di disabilità, e maggiormente nei casi con bassi punteggi EDSS (46%, EDSS tra 1 e 1,5), mentre il β1b ha trovato maggior impiego nei casi di disabilità più gravi e nei pazienti con un elevato numero di riaccensioni a 12 mesi dalla diagnosi. Abstract

Background and purpose Receiving clear, complete and up-to-date information and having a satisfying relationship with the health professional ( HP) are of primary importance for MS patients. Healthcare organization plays a key role in promoting an effective relationship and communication between patients and HPs. The present study aims to explore which care organization and service characteristics provided by Italian MS centres best predict patients' satisfaction with healthcare. Methods Eighty-one centres and 707 patients (502 women, mean age 40.5 years, SD 10.2; mean education 12.2 years, SD 3.6; time since diagnosis 5.9 years, SD 1.5) were included in the analysis. The care organization and service provided by each centre were evaluated in comparison with the National Institute for Health and Care Excellence ( NICE) guidelines on management of MS. Patients' satisfaction with care was measured using the patient self-assessed questionnaire 'Comunicazione medico-paziente nella Sclerosi Multipla, revised' section 2 ( COSM-R section 2). Results The clinical characteristics of patients significantly affected their satisfaction. A multivariate regression model showed that higher patients' satisfaction ( COSM-R score) was inversely associated with hospital size (number of patients under care) ( β = −0.21, 95% confidence interval −0.35; −0.07) and directly associated with psychological interventions ( β = 2.44, 95% confidence interval 0.29; 4.59). Conclusions Multiple sclerosis patients from larger hospitals are less satisfied with the information received and the relationship with HPs. Building an individualized relationship between patients and HPs and tailoring the communication of information improve patients' satisfaction. Such a goal is probably less likely to be accomplished in larger centres with many incoming patients. Abstract

In a multicenter cross-sectional study, the authors assessed pain in patients with multiple sclerosis (MS) using a symptom-oriented approach. Out of 2,077 questionnaires, we used 1,672 for data analysis. Pain and frequencies included trigeminal neuralgia 2%, Lhermitte's sign 9%, dysesthetic pain 18.1%, back pain 16.4%, and painful tonic spasms 11%. Comparison between different groups showed significant differences for age, Expanded Disability Status Scale, disease duration, and disease course, but not for sex. This study underlines the relevance of pain in the clinical history of MS.

Abstract

It has been suggested that human herpesvirus 6 (HHV-6) might be involved in the pathogenesis of multiple sclerosis (MS). However, studies of the association between HHV-6 and MS are hindered by the difficulty in discriminating between latent and active infection. We undertook a study to determine whether HHV-6 establish a systemic active infection in the course of MS, and to investigate possible roles of HHV-7, a herpesvirus closely related to HHV-6. To discriminate between latent and active infection, we analysed viral transcription. The results indicate that both viruses are prevalent in PBMCs of MS patients as in healthy controls, and that viral sequences are maintained in a non-transcriptional state. These observations indicate that further studies should define the state of viral persistence in the central nervous system.

Abstract

OBJECTIVES—To estimate the incidence rate of Guillain-Barre syndrome variants in an unselected population and to describe their clinical features and prognosis. METHODS—A two year prospective multicentre study on the incidence and prognosis of Guillain-Barre syndrome was performed in Emilia-Romagna, northern Italy (3 909 512 inhabitants). A surveillance system was instituted within the study area, which comprised all the neurological departments, private and public general hospitals, and practising neurologists. The international classification of diseases (ICD) codes 357.XX (any peripheral neuropathy) of hospital discharges were also reviewed. RESULTS—Data were separately analysed for Miller Fisher syndrome and other Guillain-Barre syndrome variants. During the study period 18 patients with Guillain-Barre syndrome variants including seven with Miller Fisher syndrome were recruited; the incidence rates were 0.14/100 000/year (95% confidence interval (95% CI) 0.07-0.25) for Guillain-Barre syndrome variants (excluding Miller Fisher syndrome) and 0.09/100 000/year (95% CI 0.04-0.18) for Miller Fisher syndrome. Guillain-Barre syndrome variants alone (excluding Miller Fisher syndrome) accounted for 10.5% of total cases. Death and relapses were not found. Details of clinical, electrophysiological, and CSF findings of Guillain-Barre syndrome variants are provided. CONCLUSIONS—Guillain-Barre syndrome variants other than Miller Fisher syndrome, as obtained through a population based study, account for about 10% of total cases of Guillain-Barre syndrome and, as a whole, have a good prognosis. Their clinical features are heterogeneous; bifacial weakness (associated with other signs, mainly sensory disturbances) represents the most frequent finding.

Abstract

Background and Methods Multiple sclerosis often occurs in young women, and the effect of pregnancy on the disease is poorly understood. We studied 254 women with multiple sclerosis during 269 pregnancies in 12 European countries. The women were followed during their pregnancies and for up to 12 months after delivery to determine the rate of relapse per trimester and the score on the Kurtzke Expanded Disability Status Scale (range, 0 to 10, with higher scores indicating more severe disability). The relapse rate in each trimester was compared with the rate during the year before the pregnancy. The effects of epidural analgesia and breast-feeding on the frequency of relapse during the first three months post partum and the disability score at 12 months post partum were also determined. Abstract

Objective. Diagnosis of coeliac disease is based on the presence of villous atrophy which recovers following a gluten-free diet. The presence of circulating antiendomysial antibodies as well as their disappearance after a gluten-free diet supports the diagnosis. It has also been demonstrated that antiendomysial antibodies are detectable in supernatants of cultured intestinal biopsies from patients with coeliac disease. The objective of this study was to compare the histology and antiendomysial antibodies in culture supernatants of intestinal biopsies to validate the in vitro organ culture system as a future diagnostic tool for coeliac disease. Material and methods. Seventy-five antiendomysial serum-positive patients on a gluten-containing diet were evaluated. Patients underwent endoscopy with 5 biopsy fragments: 3 for histology, 1 cultured with and the other without gliadin-peptide activator. Antiendomysial antibodies were evaluated in all culture supernatants. Results. Sixty-eight patients had evidence of villous atrophy, while 73 out of 75 were positive to the organ culture system. The agreement rate between organ culture and histology results was 94%. Conclusions. As all the centres participating in the study obtained good agreement between organ culture and histology results, the new system could be considered a reliable tool for the diagnosis of coeliac disease. Nevertheless, it is possible to highlight cases with an organ culture-positive and -negative histology. This feature could be of considerable interest because, as the sensitivity of organ culture seems to be greater than the initial histology, the new system might be useful in uncertain cases where the risk of missing the diagnosis of coeliac disease is high. © 2006 Taylor & Francis.

Abstract

The objective of this study was to verify the adherence of Italian family physicians and neurologists to the Guidelines on Diagnosis of Dementia of the Italian Society of Neurology. A multicentre survey was carried out, in 72 neurological centres. The centres included at least 15 consecutive subjects suspected of having a dementia. The adherence of family physicians to the guidelines was poor. Neurologists performed a complete neuropsychological evaluation in a minority of the cases. Patients who had a decrease of Mini Mental Status Examination scores after six months higher than or equal to 4 were more represented among those patients for whom one or more recommendations were not respected. In Italy the adherence to the Guidelines on Diagnosis of Dementia and Alzheimer Validation studys Disease of the Italian Society of Neurology is very poor for family physicians (GPs) and satisfactory, albeit improvable, on the part of neurologists. Respect for the guidelines might improve the outcome of patients with dementia. © Springer-Verlag Italia 2004.
