105. Intraoperative radiation therapy delivered prior to lumpectomy for early-stage breast cancer: a single institution study

Abstract

Objective: To evaluate the safety, cosmesis, and clinical outcome of intraoperative electron radiation therapy (IOERT) delivered prior to lumpectomy for early-stage breast cancer. Methods: From December 2008 to March 2012, 75 breast cancer patients (ages 34-66 years) were treated with IOERT during breast conservative surgery. IOERT was delivered using a mobile linear accelerator. Suitable energy and applicator size were chosen to ensure coverage of the tumor with anterior and posterior margins of 1 cm and lateral margins of 2 cm. Patients with sentinel node metastases or younger than 40 years received 8 Gy as boost followed by post-operative external beam radiation therapy of 50 Gy/25F; the others had 15 Gy, prescribed to the 90% isodose depth. Adjuvant treatment consisted of chemotherapy (55 patients), hormonal therapy (59 patients), or combined chemotherapy and hormonal therapy (41 patients). The safety, cosmesis, and short-term outcome were evaluated. Results: Median follow-up was 54 months (range: 30-66 months). Two (2.7%) patients developed post-surgical hematoma. Six (8.0%) patients developed mild breast fibrosis. Eight (10.7%) patients suffered from local pain. One (1.2%) patient experienced a post-operative infection. Sixteen (21.3%) patients developed Grade 1 pulmonary fibrosis. Forty-three (57.3%) patients had an excellent cosmetic result and 23 (30.7%) had a good cosmetic result. Three patients had an ipsilateral breast recurrence, with an actual 3-year local recurrence rate of 4.0%. One patient had an ipsilateral axillary recurrence, resulting in a 3-year regional recurrence rate of 1.3%. No distant metastases or deaths were observed. The 3-year disease free survival was 94.6%. Conclusions: Intraoperative electron radiation therapy delivered prior to lumpectomy is safe and feasible for selected patients with early-stage breast cancer. Early side effects, cosmesis and short-term efficacy are acceptable, but a longer follow-up is needed for evaluation of late side effects and long-term outcome.

Abstract

Based on the tree-ring growth characteristics of Erman's birch (Betula ermanii charm.) and the relationships between it and climatic factors at elevation of 1950m, the sensitivity of tree lines in Changbai Mountain to climatic factors was assessed. The results indicated tree line forest in Changbai Mountain had an obvious, sensitivity to climate factors. However, difference from other study sits is that the main climatic control factor on tree-ring growth was not current growth season temperatures, as might be expected, but previous winter and current March temperature. Although the precipitation in the region was quite abundant, the tree-ring growth was still significantly correlated with the precipitation during previous winter and current spring. Additionally, climatic factors which influenced the Erman's birch growth were not the yearly variables, but seasonal and monthly variables. Therefore the reported increase in yearly mean temperature and total yearly precipitation since 1980s was not responded by sustained increase in ring widths in recent decades.

Abstract

Two-dimensional vector vortex solitons in harmonic optical lattices are investigated. The stability properties of such solitons are closely connected to the lattice depth V0. For small V0, vector vortex solitons with the total zero-angular momentum are more stable than those with the total nonzero-angular momentum, while for large V0, this case is inversed. If V0 is large enough, both the types of such solitons are stable.

Abstract

Papillary renal cell carcinoma (PRCC) is traditionally classified into type 1 and type 2. Recently, an oncocytic variant of PRCC has been described. We report a series of 6 oncocytic renal papillary tumors (OPRCC) which tended to occur in older patients (mean, 56.8 years) with a male preference (male-to-female ratio is 5:1). All 6 patients are alive with no evidence of disease after initial resection, showing an indolent clinical behavior. Histologically, tumors exhibited predominant papillary structure with delicate fibrovascular cores. Papillae were lined by single layers of cells with large, deeply eosinophilic and finely granular cytoplasms and round regular nucleus. The phagocytosis of tumor cells was frequently and evidently seen in our cases that hemosiderin-laden tumor cells and foamy tumor cells were noticed in five and four cases respectively. All tumors were immunoreactive for racemase, vimentin, CD10, and MET and negative for CD117. While E-cadherin, EMA, and cytokeratin 7 exhibited variable immunopositivity. FISH analysis was performed in five of six cases and found heterogeneous results. Trisomy of chromosomes 7 was found in three cases and trisomy of chromosomes 17 in two cases. Loss of chromosome Y was noted in one of four tumors in male patients. MET gene status was also investigated by direct sequencing in all 6 cases and found no distinct mutation in any case. These results suggest that OPRCC shows distinct morphology, indolent clinical behavior, and similar immunohistochemical and cytogenetic features with PRCC, seems to be a variant in the PRCC group. Whether the strong expression of MET indicates a potential therapeutic target is still unknown and requires further investigation in clinical trials.

Abstract

The metallic Au-incorporated Au@AgPO heterostructure described in this study exhibited a highly productive photocatalytic activity in the generation process from hydroxyapatite to AgPO caused by the rapid release of hydroxyl groups which were converted into hydroxyl radicals by the combined effect of the LSPR by metallic Au nanoparticles and photoexcitation of AgPO crystals. A conceptually different approach was first demonstrated which we called 'instantaneous catalysis' (IC) to degrade rhodamine B, amido black 10b, and bromophenol blue dyes with an extra high degradation rate in about 5 s. It was estimated that the creative photocatalytic activity was due to the effective utilization of the hydroxyl radicals during the synthesis of Au@AgPO heterostructure. Abstract

Gallbladder carcinoma ( GBC) is an aggressive neoplasm, and the treatment options for advanced GBC are limited. Recently, long non-coding RNAs (lnc RNAs) have emerged as new gene regulators and prognostic markers in several cancers. In this study, we found that metastasis-associated lung adenocarcinoma transcript 1 ( MALAT1) expression was up-regulated in GBC tissues ( P < 0.05). Luciferase reporter assays and RNA pull down assays showed that MALAT1 is a target of miR-363-3p. Real-time quantitative PCR and Western blot analysis indicated that MALAT1 regulated Myeloid cell leukaemia-1 ( MCL-1) expression as a competing endogenous RNA (ce RNA) for miR-363-3p in GBC cells. Furthermore, MALAT1 silencing decreased GBC cell proliferation and the S phase cell population and induced apoptosis in vitro. In vivo, tumour volumes were significantly decreased in the MALAT1 silencing group compared with those in the control group. These data demonstrated that the MALAT1/miR-363-3p/ MCL-1 regulatory pathway controls the progression of GBC. Inhibition of MALAT1 expression may be to a novel therapeutic strategy for gallbladder cancer. [ABSTRACT FROM AUTHOR]
