124. Mean Rank, Equipercentile, and Regression Mapping of World Health Organization Quality of Life Brief (WHOQOL-BREF) to EuroQoL 5 Dimensions 5 Levels (EQ-5D-5L) Utilities

Abstract

BACKGROUND: Existing methods to link preference-based and profile-based health-related quality of life (HRQoL) questionnaires have their limitations. Hence, we developed a new mapping method (the mean rank method, MRM) and applied it to map the World Health Organization Quality of Life Questionnaire (WHOQOL-BREF) to the EuroQoL 5 Dimensions 5 Levels (EQ-5D-5L). We then compared the new MRM with current methods; i.e., regression-mapped (OLS method) and equipercentile method (EPM). METHODS: Singapore residents, aged ≥21 y, were recruited from the general population and 2 outpatient clinics in acute care hospitals. Performance of the MRM was evaluated using both simulation and split-sample validation ( n = 658 in training and n = 657 in validation samples). Using the training sample, we derived 3 sets of mapped EQ-5D-5L utilities based on MRM, OLS method and EPM. Using simulation and the validation sample, we compared the performance of the mapping methods in terms of distribution parameters, mean utility by strata, association with health covariates, and prediction errors at the individual level, among others. RESULTS: The WHOQOL-BREF Physical Health domain is the only domain significantly associated with EQ-5D-5L utilities. Simulation showed that MRM more accurately reproduced the variance and percentiles of the distribution of the observed utilities than did the OLS method or EPM. OLS method tended to underestimate the mean utility of good health states, overestimate the mean utility of poor health states, and underestimate the association with covariates. An analysis of validation sample gave similar results. CONCLUSION: In scenarios similar to the mapping of WHOQOL-BREF to the EQ-5D-5L, the MRM outperformed the OLS method and EPM in important-though not all-aspects. The simplicity and reproducibility of the MRM makes it an attractive alternative to current methods. Abstract

Revised subscales of the Children's Eating Behavior Questionnaire (CEBQ) have been proposed to be more appropriate for assessing appetitive traits in Singaporean 3 year-olds, but the CEBQ has not yet been validated in older children in this population. The current study aimed to validate the CEBQ at ages 5 (n = 653) and 6 (n = 449) in the ethnically diverse GUSTO cohort. Confirmatory factor analysis (CFA) examined whether the established eight-factor model of the CEBQ was supported in this sample. Overall, the CFA showed a poor model fit at both ages 5 and 6. At both ages 5 and 6, an exploratory factor analysis revealed a six-factor structure: food fussiness, enjoyment of food, slowness in eating, emotional undereating, emotional overeating and desire to drink. Cronbach's alpha estimates ranged from 0.70 to 0.85 for all subscales. Criterion validity was tested by correlating subscales with the weight status of 6 years of age. At age 5 and 6, lower scores of slowness of eating while higher scores of enjoyment of food was associated with child overweight. At age 6, higher scores of desire to drink was also associated child overweight. In conclusion, a revised six factor-structure of the CEBQ at ages 5 and 6 were more appropriate for examining appetitive traits in this sample.

Aim: To evaluate the relative importance of CYP2C19 genotype-guided treatment attributes to patients. Patients & methods: A discrete choice experiment questionnaire was administered to 63 patients with acute coronary syndrome. Attributes examined in the discrete choice experiment questionnaire were: cost of genetic testing (S$50, S$100, S$200); cost of antiplatelet medication (S$100, S$500, S$1000); heart attack or stroke risk (5 in 100, 15 in 100, 25 in 100); bleeding risk (5 in 100, 15 in 100, 25 in 100); doctor's recommendation (yes, neutral). Mixed logit model was used for analysis. Results & conclusion: All attributes were important in patients' decision-making. Most displayed strong preference for doctor's recommendation and reduced heart attack or stroke risk. Genotyping was chosen by 63.5% of the patients. [ABSTRACT FROM AUTHOR]

Abstract

Data analytics is playing an important role in health care because of the potential actionable insights that can be derived from individual-level medical records in the electronic health records (EHRs). This paper explores the utilization of EHR data for predictive analytics at an academic health system in Singapore to facilitate patient stratification for intensive case management among individuals with type 2 diabetes mellitus (T2DM). Though a multidisciplinary team approach, we developed a risk score for high health care utilizers with EHRs. A backward stepwise variable selection model building approach was performed to develop a risk score using the multiple logistic regression model and Akaike Information Criterion where the variables from 2010 was used to predict the top 10% health care spenders in 2011. The list of predictors in the risk score included sociodemographic, biochemistry, comorbidity and healthcare utilization variables. The Area under the Curve (AUC) of the risk score was 0.708, which was higher than having total cost in 2010 as the only predictor (AUC = 0.658). The absence of biochemistry measurements could either be a proxy of no regular follow-up for managing T2DM condition if their regular measurements were part of the clinical practice for T2DM, or be a proxy of a favorable perception of patient's medical condition otherwise. A close collaboration across multiple disciplines is important to ensure a holistic interpretation of a risk score.

Abstract

Electronic health records (EHRs) offer an alternative rich source of data for research. However, additional steps are required before utilizing them for research purposes. One of the important steps is the assessment of data quality. In this paper, we illustrate an objective and quantitative framework for assessing data quality of temporal data elements and identifying sources contributing to the variation in data quality. The identified sources of variation in our application can be attributed to the implementation of the EHR system. A close collaboration between analytics and domain experts is pertinent to ensure a holistic assessment of data quality. The approach proposed in this paper demonstrates the potential to advance the assessment of data quality from descriptive to diagnostic analytics in a clinical setting by utilizing data mining techniques on features generated from EHRs in a data rich era.
