Depressive disorders represent the largest proportion of mental illnesses, and by 2030, they are expected to be the first cause of disability-adjusted life years [1]. The COVID-19 pandemic exacerbated prevalence and burden of depression and increased the occurrence of depressive symptoms in general population [2]. The urgency of implementing mental health services to address new barriers to care persuaded clinicians to use telemedicine to follow patients and stay in touch with them, and to explore digital therapeutics (DTx) as potential tools for clinical intervention [2]. The combination of antidepressants and psychotherapy is widely recommended for depression by international guidelines [3] but is less frequently applied in real-world practice. Commonly used treatments are pharmacological, but while being effective, some aspects such as adherence to the drug regimen, residual symptoms, resistance, lack of information, and stigma may hinder successful treatment. In case of less severe depression, standalone psychological therapies should be the first-line treatment option [3], but access to trained psychotherapists remains inequitable. DTx are evidence-based therapies driven by software programs to treat or complement treatment of a specific disease. DTx are classified as Medical Devices, and given their therapeutic purpose, they need to be validated through randomized controlled clinical trials, as for drug-based therapies. In the last 10 years, studies of digital interventions have proliferated; these studies demonstrate that digital interventions increase remission rates and lower the severity of depressive symptoms compared with waitlist, treatment as usual, and attention control conditions [4]. Despite the efficacy demonstrated in clinical trials, many of these tools never reach real-life patients; thus, it might be necessary to implement DTx in the public health system to expand access to valid treatment options. In this framework, DTx represent a good opportunity to help people with depression receive optimal psychotherapeutic care [5].

Anderson et al. demonstrate a method for seeding and engrafting primary porcine hepatocytes into a bioengineered liver (BEL) scaffold previously reendothelialized with HUVECs. This study constitutes an important advance in the effort to generate transplantable livers of a clinically relevant size for humans.

This study explores how providing assisted dying services affects the psychological distress of practitioners. It investigates the influence of professional norms that endorse such services within their field. Study 1 included veterinarians (N = 137, 75.2% female, Mage = 43.1 years, SDage = 12.7 years), and Study 2 health practitioner students (N = 386, 71.0% female, Mage = 21.0 years, SDage = 14.4 years). In both studies, participants indicated their degree of psychological distress following exposure to scenarios depicting assisted dying services that were relevant to their respective situations. In Study 1, we found that higher willingness to perform animal euthanasia was associated with lower distress, as were supportive norms. In Study 2, a negative association between a greater willingness to perform euthanasia and lower psychological distress occurred only when the provision of such services was supported by professional norms. Context Over the past decade, the production and consumption of organic food (OF) have received increasing interest. Scientific studies have shown better quality of organic fruit and vegetables (FV) in terms of nutrients and pesticide contents, but it appears difficult to conclude if there are potentially greater health benefits of these products compared with conventional food (CF). Objective To determine whether the current scientific literature demonstrates that a diet rich in organic FV is healthier than 1 based on conventional produce. Methods A systematic search was conducted using the PubMed and Web of Science databases for articles published between January 2003 and December 2022. Articles were analyzed uniformly by 2 reviewer, using a specific template summary sheet, and scored from 1 to 5. The level of evidence and the quality of studies in humans were assessed using the Jadad score and the French National Authority for Health method. Results A total of 12 human studies were included. Studies often reported contradictory or even opposite results, with methodological limitations. Only 6 of the 12 studies found significant associations between OF and the health outcomes evaluated. Conclusion The current data do not enable a firm conclusion about a greater health benefit for a diet rich in FV based on products grown organically compared with conventional farming. There is a paucity of available data and considerable heterogeneity in study designs (participants, exposures, durations, health outcomes, and residual confounding factors). Human-pet relations are imbued with power imbalances, with many pets depending on humans for food and water, shelter, health care, and sheer survival. A majority of people report loving their pets and consider them to be integral family members; however, the care provided to pets varies widely and can be, in some cases, suboptimal. Yet, building more equal relations between humans and their pets could provide benefits to both parties. To achieve this increased equality and mutuality, the current paper proposes theory-based solutions. Specifically, and building on established social psychological theories, namely theories of intergroup relations and of human motivation, the current paper identifies both social and relational factors which, if socially and individually promoted, could trigger more equal and possibly mutually beneficial relationships with pets. We provide concrete examples illustrating how these factors can be maximized and promoted.

This study explores the dynamics of the interaction between the engagement of a preschool child with autism spectrum disorder and the participation of adults, notably during the child’s transition from a segregated to inclusive setting. Nine classroom sessions were filmed over an 8-month period with a focus on two types of activities: free play and adult-led gross motor activities. Our results showed that the interactions evolved differently over time for the two activities. During gross motor activities, the active engagement of the child associated with the passive participation of adults, which increased in the segregated setting, continued to develop in the inclusive setting leading to the emergence of active engagement with peers at the end of the school year. During free-play, the child engagement progressed in the segregated setting. Though initially in a state of passive observation, the child became independently active, either with or without the guidance of adults. The transition from the segregated setting to the inclusive setting without adult participation leads to a momentary drop in the child’s active engagement before the reemergence of independent active engagement. The results of this study question the methods used and the resources invested in preschools to favor the inclusion of young children with autism. They highlight the importance of adults’ participation during the transition between segregated and inclusive settings. In addition, they encourage adults to accept the temporary regression in child engagement in order to attain desirable outcomes such as independent engagement at a later time.

Acute hepatic failure is associated with high morbidity and mortality for which the only definitive therapy is liver transplantation. Some fraction of those who undergo emergency transplantation have been shown to recover native liver function when transplanted with an auxiliary hepatic graft that leaves part of the native liver intact. Thus, transplantation could have been averted with the development and use of some form of hepatic support. The costs of developing and testing liver support systems could be dramatically reduced by the availability of a reliable large animal model of hepatic failure with a large therapeutic window that allows the assessment of efficacy and timing of intervention. Non-lethal forms of hepatic injury were examined in combination with liver-directed radiation in non-human primates (NHPs) to develop a model of acute hepatic failure that mimics the human condition. Porcine hepatocyte transplantation was then tested as a potential therapy for acute hepatic failure. After liver-directed radiation therapy, delivery of a non-lethal hepatic ischemia-reperfusion injury reliably and rapidly generated liver failure providing conditions that can enable pre-clinical testing of liver support or replacement therapies. Unfortunately, in preliminary studies, low hepatocyte engraftment and over-immune suppression interfered with the ability to assess the efficacy of transplanted porcine hepatocytes in the model. A model of acute liver failure in NHPs was created that recapitulates the pathophysiology and pathology of the clinical condition, does so with reasonably predictable kinetics, and results in 100% mortality. The model allowed preliminary testing of xenogeneic hepatocyte transplantation as a potential therapy.

Critical race theory (CRT) in education has been used to expose and analyze racism in K-12 schooling and higher education. However, the theory has been underutilized as an inventory lens applied to school leadership practice. Our paper takes on this inquiry by highlighting the work done by an administrative leadership team at a majority racially diverse middle school in the Mountain western region of the U.S. Through an examination of the practice of racism as whiteness as property through teacher expectations, classroom instruction and teacher-student and parent interactions and by implementing changes in areas of student discipline, and color-blind teacher perceptions, the leadership team developed racial equity pathways which served as an important implementation of CRT leadership.

A growing number of patients with inflammatory bowel diseases (IBD) experienced failure of targeted therapies (monoclonal antibodies and small molecules). Combined targeted therapy is increasingly used for patients with refractory IBD allowing multimodal control of inflammation and restoration of epithelial homeostasis. This review compiles evidences for using combined targeted therapies including data from retrospective cohort studies and early-phase randomized controlled trials. Background Both vedolizumab and ustekinumab can be considered for the treatment of ulcerative colitis [UC], but head-to-head trials are lacking. Aim We aimed to compare the effectiveness of vedolizumab and ustekinumab after anti-tumour necrosis factor [anti-TNF] failure in UC patients. Patients and Methods In this multicentre study, we included consecutive adult patients with UC, with partial Mayo score >2 and prior anti-TNF exposure, treated with vedolizumab or ustekinumab between January 2019 and August 2022. Comparisons were performed using propensity score analyses [inverse probability of treatment weighting]. Results Among a total of 293 patients included, 151 and 142 received vedolizumab and ustekinumab, respectively. After propensity score analysis, steroid-free clinical remission [SFCR] [partial Mayo score ≤2] was achieved at week 16 in 38.0% and 40.3% of patients treated with vedolizumab and ustekinumab, respectively (adjusted odds ratio [aOR] = 1.11, 95% confidence interval [0.39–3.13], p  = 0.85). Rates of SFCR in patients exposed to one, two, and three lines of biologics/small molecules among patients treated with vedolizumab and ustekinumab were respectively 53.3% vs 62.1% [ p  = 0.52], 44.4% vs 33.8% [ p  = 0.52], and 2.6% vs 19.1% [ p  = 0.027]. Endoscopic remission [SFCR and endoscopic Mayo score ≤1] and histological remission [SFCR, endoscopic remission, and Nancy histological index ≤1] at week 16 were achieved in respectively 5.3% vs 17.5% (aOR = 3.77 [1.25–11.36], p  = 0.018) and 2.1% vs 11.1% (aOR = 5.85 [1.47–23.30], p  = 0.012) in the vedolizumab and ustekinumab groups. No difference regarding the risk of drug discontinuation between the two groups (aHR = 1.03 [0.51–2.08], p  = 0.92) was observed. While no factor was identified for vedolizumab, primary failure to at least one biologic/small molecule (OR = 0.31 [0.11–0.82], p  = 0.018) was significantly associated with a decreased rate of SFCR among patients treated with ustekinumab. Conclusion While no difference in terms of short-term clinical remission was observed, ustekinumab appears to be more effective than vedolizumab in inducing endoscopic and histological remission at week 16 after failure of anti-TNFs in UC. The origin of modern crocodylians is rooted in the Cretaceous, but their evolutionary history is obscure because the relationships of outgroups and transitional forms are poorly resolved. Here, we describe a new form, Varanosuchus sakonnakhonensis gen. nov. sp. nov. from the Early Cretaceous of Thailand that fills an evolutionary gap between Paralligatoridae and Atoposauridae, two derived neosuchian lineages with previously unsettled phylogenetic relationships. Three individuals, including a complete skull and associated postcranial remains, allow for a detailed description and phylogenetic analysis. The new taxon is distinguished from all other crocodylomorphs by an association of features, including a narrow altirostral morphology, a dorsal part of the postorbital with an anterolaterally facing edge, a depression on the posterolateral surface of the maxilla, and fully pterygoid-bound choanae. A phylogenetic analysis confirms the monophyly and taxonomic content of Atoposauridae and Paralligatoridae, and we underline the difficulty in reaching a robust definition of Eusuchia. Furthermore, we put forward further arguments related to the putative terrestrial ecology with semi-aquatic affinities of atoposaurids based on their altirostral snout morphology and osteoderm ornamentation. Objective: To evaluate the feasibility of lung ultrasonography (LUS) performed by novice users' general practitioners (GPs) in diagnosing lower respiratory tract infections (LRTIs) in primary health care settings. Design: A prospective interventional multicenter study (December 2019–March 2020). Settings and subjects: Patients aged >3 months, suspected of having LRTI consulting in three different general practices (GPs) (rural, semirural and urban) in France. Main outcome measures: Feasibility of LUS by GPs was assessed by (1) the proportion of patients where LUS was not performed, (2) technical breakdowns, (3) interpretability of images by GPs, (4) examination duration and (5) patient perception and acceptability. Results: A total of 151 patients were recruited, and GPs performed LUS for 111 (73.5%) patients (LUS group). In 99.1% (n = 110) of cases, GPs indicated that they were able to interpret images. The median [IQR] exam duration was 4 [3–5] minutes. LRTI was diagnosed in 70.3% and 60% of patients in the LUS and no-LUS groups, respectively (p =.43). After LUS, GPs changed their diagnosis from 'other' to 'LRTI' in six cases (+5.4%, p <.001), prescribed antibiotics for five patients (+4.5%, p =.164) and complementary chest imaging for 10 patients (+9%, p <.001). Patient stress was reported in 1.8% of cases, 81.7% of patients declared that they better understood the diagnosis, and 82% of patients thought that the GP diagnosis was more reliable after LUS. Conclusions: LUS by GPs using handheld devices is a feasible diagnostic tool in primary health care for LRTI symptoms, demonstrating both effectiveness and positive patient reception. Abstract We demonstrate computational spectral-domain ghost imaging by encoding complementary Fourier patterns directly onto the spectrum of a superluminescent laser diode using a programmable spectral filter. Spectral encoding before the object enables uniform spectral illumination across the beam profile, removing the need for light collection optics and yielding increased signal-to-noise ratio. In addition, the use of complementary Fourier patterns allows reduction of deleterious of parasitic light effects. As a proof-of-concept, we measure the wavelength-dependent transmission of a Michelson interferometer and a wavelength-division multiplexer. Our results open new perspectives for remote broadband spectral measurements.

Long-life cycle pulses have significant food security potential, however, there is little evidence to explain why they are not more widely produced and consumed in sub-Saharan Africa (SSA). This study aimed at exploring existing knowledge to inform on future research priorities in mainstreaming superior species. As staple food along with cereals, pulses are an important source of nutrients whose intake is often inadequate in SSA, however, pulse consumption remain inadequate in SSA. Depending on the crop's life cycle, pulses have multiple functions that can support food systems and ecosystem resilience. Compared to short-life cycle pulses, long-life cycle pulses rank higher in multipurpose role. However, prior research has focused primarily on short-life cycle pulses due to rapid grain production. Long-life cycle pulses remain underutilized and neglected despite showing steady but modest yield increases and adaptation to environments, suggesting that they are better positioned to respond to the diverse needs of smallholder farmers in SSA. In the context of climate change, rain-fed agriculture, depleted agricultural soils, and lack of subsidized fertilizers, there is need to transform existing food systems toward sustainable food production and improved resilience. Increasing pulse agrobiodiversity by integrating long-life cycle pulses in existing farming systems could not only contribute in alleviating malnutrition, but also poverty and inequalities. In addition, representative and accurate data are needed based on the correct use of accepted scientific names for all data across the food system. This is a prerequisite for assessing pulse consumption adequacy and quantifying production and consumption trends.
