Resumo Fundamento Algumas síndromes têm características específicas e facilmente reconhecíveis, enquanto outras podem ser mais complexas de se identificar e podem apresentar diferentes manifestações fenotípicas, por exemplo. Um diagnóstico etiológico é importante para entender a natureza da doença, para estabelecer o prognóstico e para começar o tratamento, permitindo a inclusão de pacientes na sociedade e reduzindo o custo financeiro dessas doenças. Objetivo A proposta inicial deste estudo foi a triagem citogenética para detectar a síndrome de deleção 22q11.2 (SD22q11.2) em recém-nascidos e crianças com doença cardíaca congênita utilizando a técnica da amplificação multiplex de sondas dependente de ligação (MLPA). Assim, por meio da pesquisa, outras mudanças genômicas foram identificadas nesses pacientes cardíacos. Nosso objetivo se estendeu a investigar essas outras mudanças citogenéticas. Métodos Investigamos 118 recém-nascidos com doenças cardíacas congênitas nascidos consecutivamente durante um ano, utilizando a técnica da MLPA. Resultados A técnica da MLPA permitiu a detecção da SD22q11.2 em 10/118 pacientes (8,5%). Outras alterações genômicas foram identificadas em 6/118 pacientes (5%): 1p36 del, 8p23 del (2 casos), 7q dup, 12 dup e 8q24 dup. Conclusão Este estudo ressalta a relevância da detecção de alterações genômicas que estão presentes em recém-nascidos e crianças com doenças cardíacas congênitas por meio de ferramentas citogenômicas. Abstract Background Some syndromes have specific and easily recognizable features, while others may be more complex to identify and may present different phenotypic manifestations, for example. An etiological diagnosis is important to understand the nature of the disease, to establish the prognosis and to start the treatment, allowing the inclusion of patients in society and reducing the financial cost of such diseases. Objective The initial proposal of this study was cytogenetic screening for the detection of the 22q11.2 deletion syndrome in consecutive newborns and infants with congenital heart disease using the multiplex ligation-dependent probe amplification (MLPA) technique. Therefore, throughout our research, other genomic alterations were identified in these cardiac patients. Thus, our objective was extended to investigate these other cytogenetic alterations. Methods We investigated 118 neonates with congenital heart diseases born consecutively during one year using the MLPA technique. Results The MLPA technique allowed the detection of 22q11.2DS in 10/118 patients (8.5%). Other genomic alterations were also identified in 6/118 patients (5%): 1p36 del, 8p23 del (2 cases), 7q dup, 12 dup and 8q24 dup. Conclusion This study highlights the relevance of detecting genomic alterations that are present in newborns and infants with congenital cardiac diseases using cytogenomic tools.

Background Increased utilization of telemedicine has created a need for supplemental pain medicine education, especially for the virtual physical assessment of the pain patient. Traditional clinical training utilizes manual and tactile approaches to the physical examination. Telemedicine limits this approach and thus alternative adaptations are necessary to acquire information needed for sound clinical judgement and development of a treatment plan. Clinical assessment of pain is often challenging given the myriad of underlying etiologies contributing to the sensory experience. The COVID-19 pandemic has led to a dramatic increase in the use of virtual and telemedicine visits, further complicating the ease of assessing patients in pain. The increased reliance on telemedicine visits requires clinicians to develop skills to obtain objective information from afar. While eliciting a comprehensive history and medication assessment are performed in a standard fashion via telemedicine, a virtual targeted physical examination is a new endeavor in our current times. In order to appropriately diagnose and treat patients not directly in front of you, a pivot in education adaptations are necessary. Objective To summarize best care practices in the telemedicine physical exam while presenting an algorithmic approach towards virtual assessment for the pain practitioner. Design Review of the literature and expert multidisciplinary panel opinion. Setting Nationally recognized academic tertiary care centers. Subjects Multidisciplinary academic experts in pain medicine. Methods Expert consensus opinion from the literature review. Results An algorithm for the virtual physical exam for pain physicians was created using literature review and multidisciplinary expert opinion. Conclusions The authors here present simple, comprehensive algorithms for physical exam evaluations for the pain physician stemming from a review of the literature.

Studying spin-momentum correlations in hadronic collisions offers a glimpse into a three-dimensional picture of proton structure. The transverse single-spin asymmetry for midrapidity isolated direct photons in p ↑ + p collisions at s = 200 GeV is measured with the PHENIX detector at the Relativistic Heavy Ion Collider (RHIC). Because direct photons in particular are produced from the hard scattering and do not interact via the strong force, this measurement is a clean probe of initial-state spin-momentum correlations inside the proton and is in particular sensitive to gluon interference effects within the proton. This is the first time direct photons have been used as a probe of spin-momentum correlations at RHIC. The uncertainties on the results are a 50-fold improvement with respect to those of the one prior measurement for the same observable, from the Fermilab E704 experiment. These results constrain gluon spin-momentum correlations in transversely polarized protons.

In the current living environment, health insurance has become an essential necessity, especially with the relentless pace of medical inflation. Unfortunately, a significant portion of the population remains without adequate coverage. This study focuses on Millennials in Malaysia, a population of around 11 million with the financial capability to afford health insurance. Apparently, millennials are born between 1981 and 1996 (ages 23 to 38 in 2019). Despite this, the actual insured population in Malaysia is only at 41%, signifying a lack of insurance protection. The key factors influencing health insurance purchase behavior among Millennials are examined in this research, including financial literacy, insurance premium affordability, and insurer brand equity. The research objectives in this study serve as the guiding pillars that shape the purpose of the investigation. The core focus of this research revolves around understanding the health insurance purchase behavior of Malaysia's Millennials. The objectives are designed to delve into the relationship between various key factors and this behavior. This research offers novel insights into the health insurance purchase behavior of Millennials in Malaysia. It highlights the financial literacy of individuals as a key driver in insurance decisions, emphasizing the need for financial education and literacy programs. Furthermore, the study underlines the importance of making insurance premiums more affordable to encourage participation. Finally, it reinforces the value of insurer brand equity in attracting Millennials to health insurance, prompting insurance providers to focus on building and maintaining their brand reputation. The measurements are fully corrected for detector effects, and reported differentially in both the jet transverse momentum and jet radius parameter. We compare the measurements to established leading-order Monte Carlo event generators and find that STAR-tuned pythia-6 reproduces the data, while LHC tunes of pythia-8 and herwig-7 do not agree with the data, providing further constraints on parameter tuning. Finally, we observe that softdrop grooming, for which the contribution of wide-angle nonperturbative radiation is suppressed, shifts the jet mass distributions into closer agreement with the partonic jet mass as determined by both pythia-8 and a next-to-leading-logarithmic accuracy perturbative QCD calculation. These measurements complement recent LHC measurements in a different kinematic region, as well as establish a baseline for future jet mass measurements in heavy-ion collisions at RHIC.

The Cn and κn are presented as a function of collision energy, centrality and kinematic acceptance in rapidity, y, and transverse momentum, pT. The data were taken during the first phase of the Beam Energy Scan (BES) program (2010-2017) at the BNL Relativistic Heavy Ion Collider (RHIC) facility. Two-particle azimuthal correlations are measured with the ALICE apparatus in pp collisions at $\sqrt{s} = 13$ TeV to explore strangeness- and multiplicity-related effects in the fragmentation of jets and the transition regime between bulk and hard production, probed with the condition that a strange meson (K$^{0}_{\rm S}$) or baryon ($\Lambda$) with transverse momentum $p_{\rm T} > 3$ GeV/c is produced. Azimuthal correlations between kaons or $\Lambda$ hyperons with other hadrons are presented at midrapidity for a broad range of the trigger ($3 < p_{\rm T}^{\rm trigg} < 20$ GeV/$c$) and associated particle $p_{\rm T}$ (1 GeV/$c$ $< p_{\rm T}^{\rm assoc} < p_{\rm T}^{\rm trigg}$), for minimum-bias events and as a function of the event multiplicity. The near- and away-side peak yields are compared for the case of either K$^{0}_{\rm S}$ or $\Lambda$($\overline{\Lambda}$) being the trigger particle with that of inclusive hadrons (a sample dominated by pions). Chemotherapy-induced neuropathy has limited treatment options, and conventional medications used to treat neuropathic pain often do not provide adequate pain relief for patients with a history of cancer. Neuromodulation such as dorsal root ganglion stimulation remains a treatment that has been studied for chronic painful conditions such as low back pain, pelvic pain, complex regional pain syndrome, and phantom limb pain. Dorsal root ganglion stimulation has been presented for treatment of chemotherapy-induced neuropathy, but with limited duration of follow-up. We present a case of pain resolution after placement of a dorsal root ganglion stimulation for persistent chemotherapy-induced neuropathy. Our patient developed burning pain and allodynia in both feet 3 mos into her chemotherapy regimen, with worsened symptoms after cessation of chemotherapy. After failure of conservative pharmacotherapies, a 7-day dorsal root ganglion stimulation trial was implanted, resulting in 100% pain relief. A dorsal root ganglion stimulation was then implanted permanently, and our patient reported continued resolution of symptoms at evaluation 3 yrs after placement. To the author's knowledge, this is the first case of sustained relief with dorsal root ganglion stimulation placement for chemotherapy-induced neuropathy and presents a treatment option that warrants further investigation.

Purpose To reduce radiation doses to the tongue, a patient-specific semi-customized tongue displacement device (SCTDD) was developed using a 3D printer (3DP) for head and neck (H&N) radiation therapy (RT). Dosimetric characteristics of the SCTDD were compared with those of a standard mouthpiece (SMP). Materials and methods The SCTDD consists of three parts: a mouthpiece, connector with an immobilization mask, and tongue displacer, which can displace the tongue to the contralateral side of the planning target volume. Semi-customization was enabled by changing the thickness and length of the SCTDD. The instrument was printed using a 3DP with a biocompatible material. With the SCTDD and SMP, two sets of planning computed tomography (CT) and tomotherapy plans were obtained for seven H&N cancer patients. Dosimetric and geometric characteristics were compared. Results Using the SCTDD, the tongue was effectively displaced from the planning target volume without significant tongue volume change compared to the SMP. The median tongue dose was significantly reduced (29.6 Gy vs. 34.3 Gy). The volumes of the tongue receiving a dose of 15 Gy, 30 Gy, 35 Gy, 45 Gy, and 60 Gy were significantly lower than using the SMP. Conclusion The SCTDD significantly decreased the radiation dose to the tongue compared to the SMP, which may potentially reduce RT-related tongue toxicity.

We present the first measurement of the proton–Ω correlation function in heavy-ion collisions for the central (0–40%) and peripheral (40–80%) Au + Au collisions at s NN = 200 GeV by the STAR experiment at the Relativistic Heavy-Ion Collider (RHIC). Predictions for the ratio of peripheral collisions to central collisions for the proton–Ω correlation function are sensitive to the presence of a nucleon–Ω bound state. These predictions are based on the proton–Ω interaction extracted from ( 2 + 1 ) -flavor lattice QCD calculations at the physical point. The measured ratio of the proton–Ω correlation function between the peripheral (small system) and central (large system) collisions is less than unity for relative momentum smaller than 40 MeV/c. Comparison of our measured correlation ratio with theoretical calculation slightly favors a proton–Ω bound system with a binding energy of ∼ 27 MeV.

Recently, the academic and non-academic staff turnover rate at a local university in Selangor, Malaysia, has increased, and the cause for resignation is unclear. This study investigates the employee perception of human resource management practices and their turnover intention. A thorough literature review suggests that performance appraisal, working conditions, equal employment opportunity, training & development, and rewards are the turnover determinants in the Malaysian context. This study considered the equity theory of motivation model, known as Adam's equity theory of motivation. The primary data was collected through a survey questionnaire involving 140 academic and non-academic staff. The 5-point Likert scale was used in constructing the items of the survey questionnaire. The majority of the respondents are females between the age of 26 and 35. They are Malays married, have an educational qualification of a bachelor's degree, earn between the range of RM 3,500 and RM4,000, and work as lecturers and executives. The Cronbach's Alpha for turnover intention, performance appraisal, working conditions, training and development, and rewards is 0.600, 0.677, 0.774, 0.805, 0.680, and 0.772. Pearson correlation shows that turnover intention is significantly negatively correlated with performance appraisal(r=-0.632), working condition(r=-0,404), equal employment opportunity-0.454) training & development (-0.189), and rewards (-0.102). The multiple regression model’s R-square value is 35.8%. The hypotheses test results reveal a significant relationship between all the independent and dependent variables. This study supported the motivation theory model where better performance appraisal, working conditions, training and development opportunities, and a better reward system is important to reduce employees' intention to turnover.

Transverse momentum (pT) dependence of flow coefficients (v2, v3, and v4) is presented for minimum bias collisions and three different centrality intervals. Number of constituent quark scaling of the measured flow coefficients in U+U collisions is discussed. We also present the ratio of vn scaled by the participant eccentricity (ɛn2) to explore system size dependence and collectivity in U+U collisions. The magnitude of v2/ɛ2 is found to be smaller in U+U collisions than that in central Au+Au collisions contradicting naive eccentricity scaling. Furthermore, the ratios between various flow harmonics (v3/v23/2, v4/v24/2) are studied and compared with hydrodynamic and transport model calculations.

Organizations are adapting to new and demanding changes in the digital economy. One of the ways to respond to the challenges is through quality improvement. In a military context, organizations such as the Military Armed Forces of Malaysia (MAF) has deployed, among others, the 5S quality management tool. Many believe that the success of quality management depends largely on the "people" of the organization. This study explores employees' perception of 5S practices within MAF and seeks to find answers to the main question, "How would MAF improve employees' involvement in 5S practices?". Data was collected from MAF personnel through interviews to meet the research objectives. Results from the qualitative study reveal five perceived important elements that would drive employees' active participation in 5S. We discuss the implications for military managers and future directions of research.

The new inclusive jet data are sensitive to the gluon helicity distribution, Δg(x,Q2), for gluon momentum fractions in the range from x≃0.05 to x≃0.5, while the new dijet data provide further constraints on the x dependence of Δg(x,Q2). The results are in good agreement with previous measurements at s=200 GeV and with recent theoretical evaluations of prior world data. Our new results have better precision and thus strengthen the evidence that Δg(x,Q2) is positive for xg0.05.

Neutral pions are observed in the forward direction relative to the transversely polarized proton beam, in the pseudorapidity region 2.7lηl3.8. Results are presented for π0s observed in the STAR forward meson spectrometer electromagnetic calorimeter in narrow Feynman x (xF) and transverse momentum (pT) bins, spanning the range 0.172.5 GeV/c. It is further observed that the value of AN is significantly larger for events with a large-pT isolated π0 than for events with a nonisolated π0 accompanied by additional jetlike fragments. The nuclear dependence r(A) is similar for isolated and nonisolated π0 events.

Purpose/Objective(s) Radiation-induced sarcoma (RIS) is a rare secondary malignancy resulting from ionizing radiation-related treatment after a long latency period. Despite the unfavorable clinical outcomes, the genomic footprints of ionizing radiation in RIS development remain largely unknown. Hence, this study aimed to characterize RIS genomes and the genomic alterations in them. Materials/Methods We performed whole-genome sequencing (WGS) of 11 cases of RIS matched with normal genomes to identify somatic alterations potentially associated with RIS development. Results The mutation abundance of RIS genomes including one hypermutated genome was variable. Cancer-related genes might show different types of genomic alterations. For instance, NF1, NF2, NOTCH1, NOTCH2, PIK3CA, RB1, and TP53 showed singleton somatic mutations; MYC, CDKN2A, RB1, and NF1 showed recurrent copy number alterations; and NF2, ARID1B, and RAD51B showed recurrent structural variations (SVs). The effects of non-homologous end joining on short insertions-deletions and SVs were substantial in RIS genomes, compared with in spontaneous osteosarcoma genomes, representing the genomic hallmark of RIS genomes. In addition, frequent chromothripsis and predisposing germline variants in DNA damage-repair pathways were identified. Conclusion Taken together, WGS-scale characterization of RIS genomes may pave the way for advanced diagnostic and therapeutic strategies for RIS.

The beam energy scan (BES) program at the BNL Relativistic Heavy Ion Collider (RHIC) was extended to energies below sNN=7.7 GeV in 2015 by successful implementation of the fixed-target mode of operation in the STAR (Solenoidal Tracker At RHIC) experiment. In this mode, ions circulate in one ring of the collider and interact with a stationary target at the entrance of the STAR time projection chamber. The first results for Au+Au collisions at sNN=4.5 GeV are presented, demonstrating good performance of all the relevant detector subsystems in fixed-target mode. Results presented here include directed and elliptic flow of identified hadrons, and radii from pion femtoscopy. The latter, together with recent HADES results, reveal a long-sought peak structure that may be caused by the system evolving through a first-order phase transition from quark-gluon plasma to the hadronic phase. Directed and elliptic flow for pions are presented for the first time at this beam energy. Pion and proton elliptic flow show behavior which hints at constituent quark scaling, and demonstrate that a definitive conclusion will be achievable using the full statistics of the ongoing second phase of BES (BES-II). In particular, BES-II to date has recorded fixed-target data sets with two orders of magnitude more events at each of nine energies between sNN=3.0 and 7.7 GeV.

The cross sections and their ratios are sensitive to quark and antiquark parton distribution functions. In particular, at leading order, the W cross section ratio is sensitive to the d¯/ū ratio. These measurements were taken at high Q2∼MW2,MZ2 and can serve as input into global analyses to provide constraints on the sea quark distributions. The results presented here combine three STAR datasets from 2011, 2012, and 2013, accumulating an integrated luminosity of 350 pb-1. We also assess the expected impact that our W+/W- cross section ratios will have on various quark distributions, and find sensitivity to the ū-d¯ and d¯/ū distributions.

Journal of high energy physics 10(10), 152 (2021). doi:10.1007/JHEP10(2021)152, Measurements of elliptic (v$_{2}$) and triangular (v$_{3}$) flow coefficients of ��$^{��}$, K$^{��}$, p+$ \overline{\mathrm{p}} $, $ {\mathrm{K}}_{\mathrm{S}}^0 $, and ��+$ \overline{\Lambda} $ obtained with the scalar product method in Xe-Xe collisions at $ \sqrt{s_{\mathrm{NN}}} $ = 5.44 TeV are presented. The results are obtained in the rapidity range |y| < 0.5 and reported as a function of transverse momentum, p$_{T}$, for several collision centrality classes. The flow coefficients exhibit a particle mass dependence for p$_{T}$< 3 GeV/c, while a grouping according to particle type (i.e., meson and baryon) is found at intermediate transverse momenta (3 < p$_{T}$< 8 GeV/c). The magnitude of the baryon v$_{2}$ is larger than that of mesons up to p$_{T}$ = 6 GeV/c. The centrality dependence of the shape evolution of the p$_{T}$-differential v$_{2}$ is studied for the various hadron species. The v$_{2}$ coefficients of ��$^{��}$, K$^{��}$, and p+$ \overline{\mathrm{p}} $ are reproduced by MUSIC hydrodynamic calculations coupled to a hadronic cascade model (UrQMD) for p$_{T}$< 1 GeV/c. A comparison with v$_{n}$ measurements in the corresponding centrality intervals in Pb-Pb collisions at $ \sqrt{s_{\mathrm{NN}}} $ = 5.02 TeV yields an enhanced v$_{2}$ in central collisions and diminished value in semicentral collisions.[graphic not available: see fulltext], Published by SISSA, [Trieste]

- Abstract
Cities in Malaysia are facing rapid urbanisation in the last decades and common issues faced by these cities include traffic congestion, environment pollution, security and deterioration of natural resources. Smart cities, in global context, are introduces as solutions to the urban issues. For Malaysia, smart cities are a new approach in urban management and development to make the cities more sustainable and liveable. This study analyses many of these challenges, in developing the smart city initiatives in the local context, offering a valuable insight of the relevant key literature, and a foundation for the development of smart city initiatives blueprint. The objectives of the study are to ascertain the determinants for developing of smart city initiatives and identify the level of importance for these initiatives from the local community perspectives. The study includes 20 items in the smart city initiatives such as public safety, public convenience, residents’ participation, communication and information, urban mobility (transportation), wastewater treatment, smart payment and finance, garbage collection services, neighbourhood maintenance / cleaning services, CCTVs monitoring system (traffic monitoring) and so forth. This is an online survey and a total of 533 valid responses are received from the questionnaires which are presented in three languages and were analysed using IBM Statistical Package for the Social Sciences (SPSS). The data are first analysed for reliability and validity and the determinants’ mean and variance are calculated for significant comparison purposes. The findings clearly indicate that the most important determinant in Miri’s Smart City initiatives is the garbage collection services which reflect the local community’s concerns for the quality of life and the public community services in the neighbourhood.
