Abstract

The fully energy-damped yields from the $^{35}$Cl+$^{12}$C reaction have been systematically investigated using particle-particle coincidence techniques at a $^{35}$Cl bombarding energy of $\sim$ 8 MeV/nucleon. The fragment-fragment correlation data show that the majority of events arises from a binary-decay process with rather large numbers of secondary light-charged particles emitted from the two excited exit fragments. No evidence is observed for ternary break-up events. The binary-process results of the present measurement, along with those of earlier, inclusive experimental data obtained at several lower bombarding energies are compared with predictions of two different kinds of statistical model calculations. These calculations are performed using the transition-state formalism and the Extended Hauser-Feshbach method and are based on the available phase space at the saddle point and scission point of the compound nucleus, respectively. The methods give comparable predictions and are both in good agreement with the experimental results thus confirming the fusion-fission origin of the fully-damped yields. The similarity of the predictions for the two models supports the claim that the scission point configuration is very close to that of the saddle point for the light $^{47}$V mass-asymmetry-dependent fission barriers needed in the transition-state calculation., Comment: 20 pages, revtex, figures available upon request, to appear in Phys. Abstract

The music composition portfolio consists of three works presented in three chapters. The first chapter titled “Casuarina-salvaged Dreaming” includes a work for eight performers plus electronics, lasting for approximately 10 minutes. This composition was premiered by the graduate students of UC San Diego’s Department of Music, and conducted by Steve Schick. The work is presented in a semi-flexible time-banned system whereby the conductor can assert some agency in the duration of the composition. The electronics utilise sound sources that are localised within the Southwest of Australia, and these samples are drawn randomly from different categorised pools with differing durations, and cast across the 4 channels of the concert hall. Then, the ensemble reacts, replicates, and maneuvers around these electronics.The second chapter consists of a 6-minute composition for a small chamber ensemble entitled “a moment exposed”. Through this work I focus on creating a extensive introduction, a dialogue between the clarinet and violin. The two instruments paired together, performing in a sort of chaotic unity where the two instruments interact and replicate each other's gestures in a raucous opening — acting together, but vocal in insisting on their own instrumental identity. Often, the clarinet sits above the violin’s register, with the violin utilising more of its lower register — the 4th string is also tuned lower to ground the violin further. Out of the pair’s final explosion emerges the rest of the ensemble. The two low strings are in their upper register, and the piano vamps soft chords — acting as a cloud which sits underneath the interjections of the clarinet and violin.Chapter three is entitled “Star Picc” for amplified piccolo, vocoder, tape, and a big drum. This work features live processing of the piccolo using pitch shifter. The shifter has 6 independent lines, one for each channel. The speakers are separated with the left channel next to the performer, and the right channel behind the audience — becoming more present in the latter half of the work. The piece explores contemporary performance practice, contrasted against kitsch robotic sounds in the fixed media and electronic pitch bending in its processing.

168. Coronary Plaque Characteristics Associated With Major Adverse Cardiovascular Events in Atherosclerotic Patients and Lesions: A Systematic Review and Meta-Analysis.

Abstract

The clinical value of high-risk coronary plaque characteristics (CPCs) to inform intensified medical therapy or revascularization of non–flow-limiting lesions remains uncertain. The authors performed a systematic review and meta-analysis to study the prognostic impact of CPCs on patient-level and lesion-level major cardiovascular adverse events (MACE). Thirty studies (21 retrospective, 9 prospective) with 30,369 patients evaluating the association of CPCs with MACE were included. CPCs included high plaque burden, low minimal lumen area, thin cap fibroatheroma, high lipid core burden index, low-attenuation plaque, spotty calcification, napkin ring sign, and positive remodeling. CPCs were evaluated with the use of intracoronary modalities in 9 studies (optical coherence tomography in 4 studies, intravascular ultrasound imaging in 3 studies, and near-infrared spectroscopy intravascular ultrasound imaging in 2 studies) and by means of coronary computed tomographic angiography in 21 studies. CPCs significantly predicted patient-level and lesion-level MACE in both unadjusted and adjusted analyses. For most CPCs, accuracy for MACE was modest to good at the patient level and moderate to good at the lesion level. Plaques with more than 1 CPC had the highest accuracy for lesion-level MACE (AUC: 0.87). Because the prevalence of CPCs among plaques was low, estimated positive predictive values for lesion-level MACE were modest. Results were mostly consistent across imaging modalities and clinical presentations, and in studies with prevailing hard outcomes. Abstract

ObjectivesTo compare dual antiplatelet therapy (DAPT) de-escalation with five alternative DAPT strategies in patients with acute coronary syndrome (ACS) undergoing percutaneous coronary intervention (PCI).DesignWe conducted a systematic review and network meta-analysis (NMA). Parallel-arm randomised controlled trials (RCTs) comparing DAPT strategies were included and arms of interest were compared via NMA. Partial ranking of each identified arm and for each investigated endpoint was also performed.Setting and participantsAdult patients with ACS (≥18 years) undergoing PCI with indications for DAPT.Search methodsA comprehensive search covered several databases (PubMed, Embase, Cochrane Central, MEDLINE, Conference Proceeding Citation Index-Science) from inception to 15 October 2023. Medical subject headings and keywords related to ACS, PCI and DAPT interventions were used. Reference lists of included studies were screened. Clinical trials registers were searched for ongoing or unpublished trials.InterventionsSix strategies were assessed: T1 arm: acetylsalicylic acid (ASA) and prasugrel for 12 months; T2 arm: ASA and low-dose prasugrel for 12 months; T3 arm: ASA and ticagrelor for 12 months; T4 arm: DAPT de-escalation (ASA+P2Y12 inhibitor for 1–3 months, then single antiplatelet therapy with potent P2Y12 inhibitor or DAPT with clopidogrel); T5 arm: ASA and clopidogrel for 12 months; T6 arm: ASA and clopidogrel for 3–6 months.Main outcome measuresPrimary outcome: Cardiovascular mortality. Secondary outcomes: bleeding events (all, major, minor), stent thrombosis (ST), stroke, myocardial infarction (MI), all-cause mortality, major adverse cardiovascular events (MACE).Results23 RCTs (75 064 patients with ACS) were included. No differences in cardiovascular mortality, all-cause death, recurrent MI or MACE were found when the six strategies were compared, although with different levels of certainty of evidence. ASA and clopidogrel for 12 or 3–6 months may result in a large increase of ST risk versus ASA plus full-dose prasugrel (OR 2.00, 95% CI 1.14 to 3.12, and OR 3.42, 95% CI 1.33 to 7.26, respectively; low certainty evidence for both comparisons). DAPT de-escalation probably results in a reduced risk of all bleedings compared with ASA plus full-dose 12-month prasugrel (OR 0.49, 95% CI 0.26 to 0.81, moderate-certainty evidence) and ASA plus 12-month ticagrelor (OR 0.52, 95% CI 0.33 to 0.75), while it may not increase the risk of ST. ASA plus 12-month clopidogrel may reduce all bleedings versus ASA plus full-dose 12-month prasugrel (OR 0.66, 95% CI 0.42 to 0.94, low certainty) and ASA plus 12-month ticagrelor (OR 0.70, 95% CI 0.52 to 0.89).ConclusionsDAPT de-escalation and ASA-clopidogrel regimens may reduce bleeding events compared with 12 months ASA and potent P2Y12 inhibitors. 3–6 months or 12-month aspirin-clopidogrel may increase ST risk compared with 12-month aspirin plus potent P2Y12 inhibitors, while DAPT de-escalation probably does not.

Abstract

Background: Coronavirus disease 2019 (COVID-19) has led to over 1 million deaths worldwide and has been associated with cardiac complications including cardiac arrhythmias. The incidence and pathophysiology of these manifestations remain elusive. In this worldwide survey of patients hospitalized with COVID-19 who developed cardiac arrhythmias, we describe clinical characteristics associated with various arrhythmias, as well as global differences in modulations of routine electrophysiology practice during the pandemic. Methods: We conducted a retrospective analysis of patients hospitalized with COVID-19 infection worldwide with and without incident cardiac arrhythmias. Patients with documented atrial fibrillation, atrial flutter, supraventricular tachycardia, nonsustained or sustained ventricular tachycardia, ventricular fibrillation, atrioventricular block, or marked sinus bradycardia (heart rate<40 bpm) were classified as having arrhythmia. Deidentified data was provided by each institution and analyzed. Results: Data were collected for 4526 patients across 4 continents and 12 countries, 827 of whom had an arrhythmia. Cardiac comorbidities were common in patients with arrhythmia: 69% had hypertension, 42% diabetes, 30% had heart failure, and 24% had coronary artery disease. Most had no prior history of arrhythmia. Of those who did develop an arrhythmia, the majority (81.8%) developed atrial arrhythmias, 20.7% developed ventricular arrhythmias, and 22.6% had bradyarrhythmia. Regional differences suggested a lower incidence of atrial fibrillation in Asia compared with other continents (34% versus 63%). Most patients in North America and Europe received hydroxychloroquine, although the frequency of hydroxychloroquine therapy was constant across arrhythmia types. Abstract

Patients with cardiac implantable electronic devices have usually been scheduled for routine in-hospital visits. In addition, they are now monitored remotely. The remote monitoring of cardiac implantable electronic devices is a valuable tool to screen and triage patients at very high risk of deterioration. The continuous expansion of remote monitoring in real-world settings brought a substantial increase of published evidence on the topic. Therefore, this review aims to summarize challenges and knowledge gaps in the field. Challenges that were identified as issues to be solved comprise warranty of data security and accessibility, integration with clinical repositories, patient selection and persistence, and resource availability. Future improvements of telemedicine will need to face these significant residual challenges.

Abstract

We report the first case of new technique of replacement of a Micra TPS, due to battery depletion. A 38-year-old patient was admitted due to battery depletion of a TPS, after 44 months of regular pacemaker functioning. After routine implantation of a new TPS, we use a snare loop inserted in the delivery system to capture the old TPS. We believe this approach a good option not to abandon the depleted device, to avoid possible electrical interference or space occupation in right ventricle. This new approach allows to change the strategy during procedure and does not increase significantly the procedure costs.

Abstract

A 67-year-old man with history of heart failure developed dyspnea. In this report, we describe an increase in his device-detected respiratory rate. Monitoring respiratory rate is recommended for evaluating acute cardiac decompensation, but such an algorithm could also be used to diagnose episodes of pneumonia caused by severe acute respiratory syndrome-coronavirus-2 infection. Abstract

Primary or revision irreparable rotator cuff tears remain a challenge. Clear algorithms do not exist. Several joint-preserving options are available, but no technique has been definitely proven to be better than another. Although superior capsule reconstruction has been shown to be effective in restoring motion, lower trapezius transfer can provide strong external rotation and abduction moment. The aim of the present article was to describe an easy and reliable technique to combine both options in 1 surgery, aiming to maximize the functional outcome by getting motion and strength back.
