The article presents a case study of a 70-year-old patient, from consanguineous nonaffected parents, was followed up for a congenital transgrediens PPK with classical clinical features of MDM. Topics discussed include diagnosis of metastatic melanoma of the left inguinal lymph nodes; cutaneous malignancies found on palmoplantar keratoderma sites; and diagnosis of amelanotic melanoma arising in an area of SLURP-1 mutated Mal de Meleda.

Lichen planus (LP) is an inflammatory skin condition characterized by the presence of pruritic, polygonal, purple flat-topped papules and plaques typically symmetrically distributed. The occurrence of these lesions could be attributed to the isomorphic response of koebner occuring regularly in LP A 38-year-old woman developed pruritic purplish papules spread on a brown lichenoid patch over the left chest area 3 months after radiation therapy for an invasive carcinoma of the left breast. These lesions were confined to the radiation therapy site. Almost any type of irritant can provoke the isomorphic response including traumatisms, friction, infection and ultraviolet light. Background: If the severity of dermatitis artefacta (DA) is accepted by most authors, few published studies have sought to clarify its etiology and impact. It is in this context that this work aimed to compare Life Events (LE) and quality of life (QoL) scores in patients with DA, in their siblings and in control patients with other chronic dermatological diseases. Methods: This is a descriptive and comparative cross-sectional study carried out in the dermatology department of Farhat Hached hospital in Sousse, Tunisia. Thirty female patients diagnosed with DA according to DSM-IV criteria were retrospectively recruited. For each patient with DA, one of her sisters, the closest in age, was enrolled in the siblings group. The control group consisted of thirty female patients with other chronic dermatological diseases, matched with DA patients for age and duration of disease progression. Assessment was based on Paykel inventory for LE and on SF-36 for QoL. Results: Compared to both control groups, DA patients reported more LE with more objective negative impact of these events and had a lower score and more often impaired mean global score of QoL. Conclusions: LE would have a role in the pathogenesis of DA which seems to alter the QoL of patients. These results could help to improve the understanding of DA and incite clinicians to focus not only on the diagnosis and treatment of DA but also on the impact of this disease. Purpureocillium lilacinum is a saprophytic fungus found in soil and decaying organic matter, but has been reported as an emerging pathogen in immunocompromised patients and following surgical procedures. Infections caused by this mold are often difficult to treat because of its intrinsic resistance to conventional antifungal agents and variable susceptibility to novel triazoles. In immunocompetent subjects, infections caused by P. lilacinum are unusual and mainly involve the skin. We describe herein a case of cutaneous hyalohyphomycosis due to this fungus in an immunocompetent girl without any predisposing risk factors and review the previously reported cases in immunocompetent hosts. PR commonly presents on the trunk as oval scaly papules and plaques arranged along skin tension lines in a "Christmas tree" pattern.[1] In its typical form, PR is easily recognizable clinically. The strictly unilateral involvement is rarely seen in PR.[2] In our patient, the diagnosis of PR was made based on the acute onset of lesions, their distibution along skin tension lines, and the presence of a herald patch. Even though histological findings are not specific in PR, lesional skin biopsy may be mandatory in atypical forms so that important differential diagnoses, such as drug eruption, secondary syphilis, or psoriasis, are not missed. Introduction: X-linked recessive ichthyosis (XLI) is a genodermatosis, caused by a deficiency of the steroid sulphatase enzyme encoded by the STS gene (OMIM # 300,747). Adopted XLI molecular diagnosis approaches differ from one laboratory to another depending on available technical facilities. Our work aims to figure out a sound diagnostic strategy for XLI. Patients and methods: We collected 8 patients with XLI, all males, from 3 unrelated Tunisian families from central Tunisia. Genetic diagnosis was conducted through a large panel of genetic techniques including: Sanger Sequencing, haplotype analysis of STR markers, MLPA analysis, FISH and array CGH. Results: Direct Sanger sequencing of the STS gene showed the same deletion of 13 base pairs within the exon 4 in all patients resulting in a premature stop codon. However, all patients' mothers were not carriers of this variant and no common haplotype flanking STS gene was shared between affected patients. Sequence alignment with reference human genome revealed an unprocessed pseudogene of the STS gene located on the Y chromosome, on which the 13 bp deletion was actually located. STS MLPA analysis identified a deletion of the entire STS gene on X chromosome for all affected patients. This deletion was confirmed by FISH and delineated by array CGH. Conclusion: All our patients shared a deletion of the entire STS gene revealed by MLPA, confirmed by FISH and improved by array CGH. In conclusion, even if we cannot identify a direct pathological link between vaccines and the onset of PF, there is a clear temporal relation between these two events and it is noteworthy that SARS-CoV-2 vaccination could induce pemphigus in susceptible people. Pemphigus Foliaceus (PF) is a rare autoimmune disorder of the skin.1 In PF, the production of autoantibodies (Abs) against desmoglein (Dsg) 1 leads to a clinical phenotype characterized by fragile blisters that involve the skin and spare the mucous membranes.1 Several factors including genetic susceptibility, environment, malignant disorders, drugs intake and vaccines can provoke pemphigus.1 Pemphigus has been reported to be triggered or exacerbated by influenza hepatitis B rabies, tetanus and more recently, SARS-COV2 vaccines.1 Herein, we present two patients developing PF, without prior history of the diseases, following SARS-CoV-2 vaccination. In addition, it has been shown that, unlike CoronaVac vaccine, messenger RNA vaccination induces strong T helper-1 responses via the activation of Toll-like receptors.4 In patients vaccinated with BNT162b1, specific antibodies appear 14-21 days later. Lung carcinoma metastasize rarely to the skin. To our knowledge, 16 cases of lung cancer metastasis with clown nose‐like lesions have been reported. Here, we describe a new case. Psoriasis is a chronic inflammatory disease affecting the skin, nails, and joints. About 61% of psoriatic patients have nail involvement that can cause a significant social problem. Treating nail psoriasis is challenging but can improve the health outcomes and quality of life of patients. Treatment options available for nail psoriasis including topical therapy, intralesional injections, and systemic and biologic agents have various side effects and some benefits. Management is currently inconclusive. Intralesional injection of methotrexate in nail psoriasis was previously documented in few cases. We present a case of nail psoriasis successfully treated with low-dose intralesional methotrexate with no significant side effects in a 48-year-old psoriatic patient. Given the various side effects of conventional topical and systemic therapies limiting their use, we conclude that intralesional methotrexate injection seems to be a safe and effective treatment option for nail psoriasis. Pityriasis rosea is a common, acute, self limiting inflammatory skin disease. Pityriasis rosea‐like eruptions (PR‐LE) have been reported after drugs. The clinical presentation of PR‐LE can be distinguished from pityriasis rosea. We reporte a 41‐year‐old woman who developed PR‐LE 5 days after administration domperidone. The clinical presentation of Pityriasis rosea‐like eruptions (PR‐LE) can be distinguished from pityriasis rosea. Epidermolysis Bullosa (EB) is a group of genetic skin disorders characterized by extreme skin fragility and blistering. In North African countries, including Tunisia, complex genetic and phenotypic diversity is entangled with a scarcity of scientific research on EB. This lack of knowledge presents a distinct challenge in terms of diagnostic accuracy and patient care. Our study cohort includes 10 Tunisian patients with EB whose genetic profiles were investigated by exome sequencing. In silico analysis was conducted to determine the functional impact of three novel variants. We revealed ten genetic variants, including three novel ones within the COL7A1 and DST genes. The in silico analysis shed light on the potential structural and functional implications of these novel variants. By establishing the correlation between clinical features and genetic alterations, we have expanded the existing database of disease-causing variants associated with EB in Northern Africa. Our study fills a critical knowledge gap in the North African context, where the scarcity of clinical database and genetic testing in addition to the genetic diversity necessitates comprehensive research. Our findings have the potential to improve diagnosis and management strategies for EB patients in low and middle-income countries across the region, especially through the integration of exome sequencing and in silico analysis., (© 2024 Wiley Periodicals LLC.)

Introduction Alopecia areata (AA) is a common, chronic inflammatory, non-scarring form of hair loss affecting 0.1-0.2% of the population. It is a psychosomatic disease involving a T-cell-mediated immune reaction against hair follicle antigens during the anagen phase. Psychiatric morbidity in dermatological patients can significantly affect their quality of life and disease progression. Identifying and addressing these comorbidities in AA patients is crucial. Aim This study aimed to estimate the prevalence of depression and its associated factors among patients diagnosed with alopecia areata (AA). Methods The present study is a cross-sectional, descriptive, and analytical study conducted at Farhat Hached Hospital in Sousse, Tunisia, during the period from August to December 2019. Participants were recruited from the Dermatology Outpatient Department. The questionnaire covered socio-demographic characteristics, personal and family history, AA history, clinical presentation, disease severity (SALT score), nail involvement, and depression assessment using the Hamilton Depression Scale (validated Arabic version). Results A total of 60 AA patients were enrolled, with a mean age of 37.6 ± 12.9 years. Females exhibited a predominant representation, with a male to female ratio (M/F) of 0.76. According to the Hamilton Depression Scale (HAM-D), 31 cases (51.7%) were depressed, with a mean depression scale score of 11.33 (±6.57 SD). The presence of depression was significantly associated with younger age (p=0.018), the presence of the eyelashes/eyebrows form (p=0.035), nail involvement (p= 0.03), and a poor response to treatment (p=0.004). Conclusion Our research highlights the importance of providing psychological support to alopecia areata patients.
