153. Man vs machine: Performance of manual vs automated electrocardiogram analysis for predicting the chamber of origin of idiopathic ventricular arrhythmia

Abstract

BACKGROUND Radiofrequency catheter ablation of idiopathic ventricular arrhythmias (VAs) is performed to eliminate symptoms and to prevent or reverse arrhythmia-induced cardiomyopathy. Preprocedural prediction of the chamber of VA origin is critical for patient counseling, procedure planning, and guidance of invasive mapping. OBJECTIVE We aimed to assess the performance of manual expert versus automated 12-lead electrocardiogram (ECG) analysis in the prediction of VA origin. METHODS Patients with ablation of idiopathic VA and sustained success were included. The VA origin was defined as the site where ablation caused arrhythmia suppression. Standard baseline 12-lead ECGs with documentation of the VA were analyzed manually in a blinded fashion by three electrophysiologists and three electrophysiology (EP) fellows. In addition, the same standard 12-lead ECG was analyzed by an automated computer algorithm using a vectorcardiographic approach. RESULTS Thirty-eight patients (median age, 47 [interquartile range, 37-58]; 68% female) were enrolled. The VA originated from the right ventricle in 24 (63%) and the left ventricle in 14 (37%) patients. The electrophysiologists and EP fellows identified the VA chamber of origin with a similar accuracy of 73% and 72% (P = .72). The automated algorithm showed a higher accuracy of 89% (P = .03 compared with electrophysiologists and EP fellows). This resulted in a sensitivity of 95% and specificity of 86%. CONCLUSION While the manual ECG analysis of the standard 12-lead ECG by both electrophysiologists and EP fellows correctly identified the chamber of VA origin in around 75% of cases, an automated vectorcardiographic computer algorithm achieved an accuracy of 89% with clinically acceptable diagnostic parameters.

Abstract

Real-time detection of vulnerable atherosclerotic lesions, characterized by a high content of oxidized low-density lipoprotein (oxLDL)-laden macrophages or foam cells, remains an unmet clinical need. While fractional flow reserve (FFR)-guided revascularization in angiographically intermediate stenoses is utilized to assess hemodynamic significance, in vivo detection of oxLDL-rich plaques may provide a new paradigm for treating metabolically unstable lesions. Herein, we have demonstrated endoluminal mapping of lipid-laden lesions using 3-D electrical impedance spectroscopy-derived impedance tomography (EIT) in a pre-clinical swine model. We performed surgical banding of the right carotid arteries of Yucatan mini-pigs, followed by 16 weeks of high-fat diet, to promote the development of lipid-rich lesions. We implemented an intravascular sensor combining an FFR pressure transducer with a 6-point micro-electrode array for electrical impedance spectroscopy (EIS) measurements. 3-D EIT mapping was achieved using an EIS-based reconstruction algorithm. We demonstrated that EIT mapping corresponds to endoluminal histology for oxLDL-laden lesions. We further used computational models to theoretically predict and validate EIS measurements. Thus, our 3-D EIS-derived EIT provides in vivo detection of metabolically active plaques with the goal of guiding optimal intravascular intervention.

Abstract

Magnesium exhibits substantial property enhancement for a vast range of applications via alloying by rare earth elements (REs). The effect of different amounts of alloying elements (Zn and REs such as Gd, Y and Nd) on the microstructure, texture and mechanical properties of Mg has been investigated here by means of optical microscopy, electron backscattered diffraction, shear punch test and hardness test. Results indicate that the recrystallization process along the grain boundaries consists of dynamic, metadynamic and static recrystallization, while dynamic recrystallization as the dominant mechanism gives rise to bimodal microstructures in as-extruded samples with superior properties. The recrystallized area is narrow at low amounts of alloying elements, whereas addition of higher amounts of alloying elements promotes recrystallization, expands recrystallized areas and weakens texture intensity. Moreover, it is observed that there are some optimal REs ratios promoting a partially recrystallized grain structure which in turn provides higher shear strength, hardness values, and more enhanced ductility as well as a weaker texture., Peer Reviewed, Postprint (author's final draft)

Abstract

A study was conducted to evaluate the usefulness of bivalirudin as part of an early invasive strategy with optimal antiplatelet therapy in patients with acute coronary syndromes. It was found that in patients with unstable angina or myocardial infarction without ST-segment elevation who were undergoing early invasive management, the rates of ischemic events and bleeding were similar with the use of either bivalirudin or heparin when combined with the planned use of a glycoprotein IIb/IIIa inhibitor.

Abstract

The steady-state pharmacokinetics of lopinavir/ritonavir (LPV/r) and tenofovir during multiple-dose administration relative to administration of LPV/r and tenofovir disoproxil fumarate (TDF) is discussed. It was seen that higher tenofovir exposures did not result in any evidence of alterations in the safety profile for TDF and in the pharmacokinetic study.

Abstract

In this study, the Al-BN nanocomposite powders were prepared using two different methods of i) ultrasonic mixing, and ii) planetary ball milling. The Al-BN bulk nanocomposite samples were fabricated by hot extrusion. Morphology of powders through the preparation process and fracture surface were characterized using scanning electron microscopy and the microstructure of optimized bulk nanocomposite sample was investigated by a transmission electron microscopy and electron back scattered diffraction technique (EBSD). The mechanical properties of composite samples were examined by uniaxial tension test. With the use of planetary high energy ball milling and hot extrusion, the ultimate tensile strength of fabricated unmilled and milled pure Al samples reached to 104 MPa and 212 MPa, respectively. The ultimate tensile strength of extruded milled nanocomposite increased to 297, 330, 333MPa by adding 1, 2 and 4 wt.% BN, respectively. In the process of composite powders mixing and hot extrusion, the mechanical properties of samples were significantly decreased. By increasing the BN content within range of 0-6 wt.%, the tensile strength of mixing and extruded composite samples was not changed. On the other hand, the ductility is reduced from 24 % for pure Al to 5 % for Al-6 wt.% BN. [ABSTRACT FROM AUTHOR]
