169. Patient misidentification in the neonatal intensive care unit: quantification of risk

Abstract

OBJECTIVE. To quantify the potential for misidentification among NICU patients resulting from similarities in patient names or hospital medical record numbers (MRNs). METHODS. A listing of all patients who received care in 1 NICU during 1 calendar year was obtained from the unit's electronic medical record system. A patient day was considered at risk for misidentification when the index patient shared a surname, similar-sounding surname, or similar MRN with another patient who was cared for in the NICU on that day. RESULTS. During the 1-year study period, 12 186 days of patient care were provided to 1260 patients. The unit's average daily census was 33.4; the maximum census was 48. Not a single day was free of risk for patient misidentification. The mean number of patients who were at risk on any given day was 17 (range: 5-35), representing just over 50% of the average daily census. During the entire calendar year, the risk ranged from 20.6% to a high of 72.9% of the average daily census. The most common causes of misidentification risk were similar-appearing MRNs (44% of patient days). Identical surnames were present in 34% of patient days, and similar-sounding names were present in 9.7% of days. Twins and triplets contributed one third of patient days in the NICU. After these multiple births were excluded from analysis, 26.3% of patient days remained at risk for misidentification. Among singletons, the contribution to misidentification risk of similar-sounding surnames was relatively unchanged (9.1% of patient days), whereas that of similar MRNs and identical surnames decreased (17.6% and 1.0%, respectively). CONCLUSIONS. NICU patients are frequently at risk for misidentification errors as a result of similarities in standard identifiers. This risk persists even after exclusion of multiple births and is substantially higher than has been reported in other hospitalized populations. Abstract

Objective. Technological and scientific advances have progressively decreased neonatal morbidity and mortality. Less attention has been given to meeting the psychosocial needs of the infant and family than on meeting the infant's physical needs. Parents' participation in making decisions and caring for their child has often been limited. Environments designed for efficient technological care may not be optimal for nurturing the growth and development of sick neonates or their families. Eleven centers collaborating on quality improvement tried to make the care of families better by focusing on understanding and improving family-centered care. Methods. Through internal process analysis, review of the evidence, collaborative learning, and benchmarking site visits to centers of excellence in family-centered care, a list of potentially better practices was developed. Choice of which practices to implement and methods of implementation were center specific. Improvement goals were in 3 areas: parent-reported outcomes, staff beliefs and practices, and clinical outcomes in length of stay and feeding practices. Measurement tools for the first 2 areas were developed and pilots were conducted. Results. Length of stay and feeding outcomes were not different before the collaboration (1998) and at the formal end of the collaboration (2000). Conclusion. Prospective parent-reported outcomes are being collected, and the staff beliefs and practices questionnaire will be repeated in all centers to determine the impact of the project in those areas. Abstract

The article reports on the development a polymerase chain reaction (PCR) assay specific for Mannheimia haemolytica (M.h.) to determine its presence in the pneumonic lungs of bighorn sheep during the 2009-10 outbreaks in three states. M. h. was detected in 77% of archived pneumonic lung tissues that were negative by culture. Collective results show that culture-based methods are unreliable for detecting M. h., and that leukotoxin-positive M.h. was a predominant agent in the outbreaks.

Abstract

The article presents research on patient monitoring in which medical personnel were tested for their responses to sounds issued to indicate deviations for desired heart beat and oxygen saturation levels in a patient. Personnel were tested for their ability to discriminate between sounds, to infer the task implied by each sound and to evaluate the risk to the patient indicated by the sounds. It was found that the performance of personnel was improved by the system.
