Recent events have highlighted the importance of implant design for survival and wear-related complications following metal-on-metal hip resurfacing arthroplasty. The mid-term survival of the most widely used implant, the Birmingham Hip Resurfacing (BHR), has been described by its designers. The aim of this study was to report the ten-year survival and patient-reported functional outcome of the BHR from an independent centre. In this cohort of 554 patients (646 BHRs) with a mean age of 51.9 years (16.5 to 81.5) followed for a mean of eight years (1 to 12), the survival and patient-reported functional outcome depended on gender and the size of the implant. In female hips (n = 267) the ten-year survival was 74% (95% confidence interval (CI) 83 to 91), the ten-year revision rate for pseudotumour was 7%, the mean Oxford hip score (OHS) was 43 (SD 8) and the mean UCLA activity score was 6.4 (SD 2). In male hips (n = 379) the ten-year survival was 95% (95% CI 92.0 to 97.4), the ten-year revision rate for pseudotumour was 1.7%, the mean OHS was 45 (SD 6) and the mean UCLA score was 7.6 (SD 2). In the most demanding subgroup, comprising male patients aged < 50 years treated for primary osteoarthritis, the survival was 99% (95% CI 97 to 100). This study supports the ongoing use of resurfacing in young active men, who are a subgroup of patients who tend to have problems with conventional THR. In contrast, the results in women have been poor and we do not recommend metal-on-metal resurfacing in women. Continuous follow-up is recommended because of the increasing incidence of pseudotumour with the passage of time.

Background: Femoroacetabular impingement (FAI) can be a source of hip pain in young adults. Repetitive kicking associated with youth soccer may lead to morphologic changes of the proximal femur that predispose a person to the development of FAI., Hypothesis: Young adults who participated in high-level soccer competition as youths are more likely to demonstrate radiographic changes consistent with FAI and to have increased alpha angles as compared with controls., Study Design: Cross-sectional study; Level of evidence, 3., Methods: Pelvic radiographs (anteroposterior and frog-lateral) were obtained on 50 individuals who participated in high-level soccer during skeletal immaturity and 50 controls who did not participate in high-level soccer. There were 25 men and 25 women in each group. All subjects were between 18 and 30 years of age, had a body mass index of less than 30, and had not sought or received treatment for hip disorders. Radiographs were analyzed independently for the presence of FAI, and alpha angles were measured. Hips with alpha angles that measured greater than or equal to 55° were deemed to have cam deformity., Results: Fifteen of the 25 male subjects had evidence of cam deformity, compared with 14 male controls. Nine of the 25 female subjects had evidence of cam deformity, compared with 8 female controls. Neither of these differences was statistically significant. There was a significantly higher prevalence of cam deformity in men as compared with women (29 vs 17, P = .016)., Conclusion: Participation in high-level soccer during skeletal immaturity is not associated with a higher risk of development of cam deformity in the young adult years. There is a high prevalence of cam deformity in the young adult population. Males demonstrate a higher prevalence of cam deformity than do females.

We studied 100 hip joints in Indian patients to measure femoral head sphericity, head-neck offset and alpha angle. Our study indicates that the mean values of the above measurements are far below the "danger" level for the onset of femoro-acetabular impingement. It remains to be seen whether these findings explain the low incidence of primary hip osteoarthritis in the Indian population.

Femoroacetabular cup impingement (FACI), defined as the presence of a bony spur or indentation at the femoral neck corresponding to the abutment site of metallic cup, was observed in 9 (11%) of the 84 hips with contemporary resurfacing arthroplasty of the hip. All FACIs occurred in men after a mean of 14 months (range, 8-24 months) postoperatively. Five patients had persistent groin pain if the hip was moved into flexion, abduction, and external rotation. One patient sustained a late-onset fracture through the femoral neck already weakened by postoperative change of osteonecrosis. Mean postoperative Harris hip score in the FACI group was poorer than that in the non-FACI group (P = .003). Multiple logistic regression analysis showed a significant association of FACI with a low acetabular cup inclination (odds ratio, 1.42; 95% confidence interval, 1.01-1.99; P = .046) and a high cup uncoverage ratio (odds ratio, 1.36; 95% confidence interval, 1.01-1.84; P = .045)., (Copyright © 2012 Elsevier Inc. Objective: To study the asphericity of the femoral head in femoroacetabular impingement using the radiological indices alpha angle and triangular index, and correlation with risk factors., Methods: The study was conducted retrospectively from January 2008 to June 2010 on 50 consecutive patients with suspected cam type femoroacetabular impingement of the hip who reported to the orthopaedics outpatients department of the Postgraduate Institute of Medical Education and Research. Ten controls were also used in the study. Radiographs of the affected hip were taken and then the alpha angle and triangular index were measured and correlated with various risk factors., Results: In the anteroposterior view the alpha angle range was from 55° to 106° for the cases and from 54° to 70° for the controls. In the lateral view the range was from 62° to 104° for the cases and from 54° to 62° for the controls. The mean alpha angle in the anteroposterior view was measured as 75°± 10° in the cases and 61°± 6° in the controls, and the mean in the lateral view was 74°± 8° in the cases and 58°± 6° for the controls. The triangular index range in the radiographs was from 1 to 7 in the cases and from 0 to 1 for the controls. The mean triangular index in the radiographs was 2.9 ± 1.2 for the cases and 0.2 ± 0.4 for the controls., Conclusion: There was no significant correlation between the age and gender of the patient and the femoroacetabular impingement. We performed a retrospective examination of the anteroposterior pelvic CT scout views of 419 randomly selected patients between April 2004 and August 2009 in order to determine the prevalence of cam-type femoroacetabular deformity in the asymptomatic population. The CT scans had all been undertaken for conditions unrelated to disorders of the hip. The frequency of cam-type femoroacetabular deformity was assessed by measuring the α-angle of each hip on the anteroposterior images. The α-angles were classified according to the Copenhagen Osteoarthritis Study. Among 215 male hips (108 patients) the mean α-angle was 59.12° (37.75° to 103.50°). Of these, a total of 30 hips (13.95%) were defined as pathological, 32 (14.88%) as borderline and 153 (71.16%) as normal. Among 540 female hips (272 patients) the mean α-angle was 45.47° (34.75° to 87.00°), with 30 hips (5.56%) defined as pathological, 33 (6.11%) as borderline and 477 (88.33%) as normal. It appears that the cam-type femoroacetabular deformity is not rare among the asymptomatic population. These anatomical abnormalities, as determined by an increased α-angle, appear to be twice as frequent in men as in women. Although an association between osteoarthritis and femoroacetabular impingement is believed to exist, a long-term epidemiological study is needed to determine the natural history of these anatomical abnormalities.

Background: Implant-related impingement has been reported following metal-on-metal hip resurfacing, and reactive osseous patterns associated with implant-bone impingement have been identified. The purpose of this study was to determine the prevalence and clinical implications of radiographic signs of femoral neck-acetabular cup impingement following metal-on-metal hip resurfacing., Methods: Serial anteroposterior and lateral radiographs made five to 12.9 years postoperatively were available for ninety-one of the first 100 metal-on-metal hip resurfacing procedures (in eighty-nine patients) performed by the senior author. These radiographs were reviewed by a single independent observer, who was blinded to the clinical results. Radiographic signs of impingement were assessed and were correlated with clinical outcomes., Results: Twenty hips (in eighteen patients) had at least one of two reactive osseous signs: a solitary exostosis (six hips, 7%) and an erosive "divot-type" deformity (twenty hips, 22%). Each radiographic sign occurred predominantly at the superior aspect of the femoral neck just distal to the femoral component. None of the patients with such an impingement sign reported any symptoms or discomfort during examination of the range of hip motion. These patients had a greater mean postoperative University of California Los Angeles activity score and a greater mean range of hip motion than the patients without an impingement sign. Based on the numbers available, there was no association between component size, abduction angle and anteversion angle of the socket, femoral stem-femoral shaft angle, or femoral component-femoral neck ratio and the occurrence of repetitive impingement signs on radiographs., Conclusions: The reactive osseous features identified in this study should facilitate the radiographic assessment of impingement in other patients following hip resurfacing arthroplasty. Longer-term follow-up is needed to determine whether radiographic signs of impingement are of prognostic consequence.

Femoroacetabular impingement is the abutment between the proximal femur and the rim of the acetabulum. It is a common cause of labral injury that has been identified as an early cause of hip osteoarthritis. The diagnosis of femoroacetabular impingement of the hip is currently well defined in orthopedic surgery but should attract the attention of physicians in other disciplines. Conversely, much less is known about the etiology and natural history of femoroacetabular impingement.The goal of this study was to assess the number of articles published on femoroacetabular impingement over 11 years in orthopedic vs nonorthopedic medical journals, and to evaluate the quality of available evidence. PubMed and OvidSP databases were searched for articles on femoroacetabular impingement published from 1999 to 2009. Articles were characterized by publication type and journal type per year. Regression analysis was used to determine the effect of publication year on number of publications of each type. The search yielded 206 publications on femoroacetabular impingement during the evaluation period. Seventy-two percent were published in orthopedic journals. Overall, the number of publications increased exponentially with time. There was an increase in clinical trials over the course of the study period. However, studies with high-quality evidence were scarce. The increase in data from orthopedic and nonorthopedic disciplines is welcome. Nevertheless, high-quality evidence on femoroacetabular impingement is lacking. We believe the current trend toward evidence-based orthopedic surgery will impact future research on this relatively new disorder., (Copyright 2011, SLACK Incorporated.)

Femoroacetabular impingement (FAI) has been accepted as a potential cause of idiopathic hip pain and osteoarthritis. Few cases of FAI have been reported in the Chinese population. We reviewed radiographic features of patients with idiopathic hip pain to characterize FAI in the Chinese population, any alternative radiographic measures, and potential symptom association. All anteroposterior pelvic radiographs of outpatients (n = 1,128) between December 2007 and August 2008 were reviewed. The radiographic measurements, including anteroposterior alpha angle (AP-a), radial height/radius ratio (R1), and 1/2 radial height/radius ratio (R2) as well as crossover signs, were recorded on both hip joints of all patients. The differences in AP-a, R1, and R2 were compared between cam-type hips (including mixed FAI) and non-cam-type hips (including non-FAI). From the group of patients, 269 were eligible for further evaluation, including those exhibiting FAI at the site of complaint (n = 38) or without symptoms (n = 7). FAI was significantly associated with hip pain (P = 0.009). For painful hips, the frequency of FAI was higher in males than in females (P = 0.038). AP-a was significantly higher in cam-type hips than in non-cam-type hips for either gender (P < 0.001, P < 0.001). R1 was comparable between the 2 subgroups for either gender (P > 0.05, P > 0.05), whereas R2 differed significantly (P < 0.001, P < 0.001). The occurrence of FAI was underestimated in Chinese patients showing negative radiographic findings with idiopathic hip pain. In addition, R2 was used to differentiate cam-type FAI from non-cam-type FAI.

The purpose was to assess the significance of herniation pits in the femoral neck for radiographic diagnosis of femoroacetabular impingement (FAI). Eighty hips in 62 patients (bilateral in 18) with neutral pelvic orientation were enrolled. Herniation pits were diagnosed when they were located at the anterosuperior femoral neck, close to the physis, and with a diameter of >3 mm. The five radiographic signs of FAI were used: lateral center edge angle (LCE) >39°, acetabular index (AI) ≤0, extrusion index (EI) <25%, acetabular retroversion, and pistol-grip deformity. Patients with radiographs suggesting FAI were retrospectively correlated with their clinical symptoms. Positive radiographic signs were observed in 7 hips with LCE, 7 with AI, and 80 with EI criteria. Only 3 hips out of 80 (3.8%) showed all of the signs. The acetabular retroversion and pistol-grip deformity were seen in 12/80 and 3/80 hips, respectively. The total number of hips that met radiographic criteria for FAI, including pincer type and cam type, was 18 (23%). However, none of these hips were clinically diagnosed with FAI. All symptomatic hips (11/80) presented only with nonspecific pain, and 2 hips out of 11 showed radiographic signs of FAI. The low frequency of positive radiographic signs suggesting FAI with related symptoms among patients with herniation pits suggests that herniation pits have limited significance in the diagnosis of FAI. Therefore it can be concluded that an incidental finding of herniation pits does not necessarily imply a correlation with FAI.

Two hundred and ninety-two patients, aged between 16 and 50 years and presenting with mechanical hip pathology, were included in a prospective multicenter study. The descriptive study concerned the clinical examination and analysis of three X-ray views (AP pelvic, Lequesne false profile and lateral axial view). The series comprised 62% males, mean age 35 years, with 53% right side and 22% bilateral involvement. Initial trauma was reported in 19% of cases, and direct familial history of hip pathology in 20%. Seventy percent of the patients played sports, 30% were high-level athletes, and 17% played combat sports. The physical impingement sign was present in 18% to 65% of cases depending on the variant studied. On imaging (n=241), 62% of hips showed osteoarthritis, with 25% at the evolved stage. In the series, as a whole, there was a 35% rate of dysplasia, 63% of impingement and 5% of normal X-ray results. The radiologic impingement aspects were 58% cam-type, 19% pincer-type and 23% mixed. Twenty-two percent of dysplasia cases showed signs of associated impingement. Pain experienced exclusively in flexion/internal rotation/adduction on examination showed little sensitivity (20%) but considerable specificity (86%) for the main diagnosis of impingement. Recently the relevance of femoroacetabular impingement as a cause of hip and groin pain in sportsmen has been recognized. The entity often poses diagnostic and therapeutic problems to the sports physician. The patients go through an odyssey of different diagnostic and therapeutic modalities before the correct diagnosis is made and an adequate therapy is implemented. Not seldom, patients get even operated at another site which is thought to cause the problems. The present review analyzes the current literature concerning diagnostic standards and therapy of femoroacetabular impingement focussing on their relevance for sports medicine. It is aimed to help the sports physician to recognise this entity as a cause for groin and hip pain in the athlete and realise its importance for the short term performance of the athlete and its long term significance in terms of development of early hip osteoarthrosis., (© Georg Thieme Verlag KG Stuttgart · New York.)
