High-frequency (8 to 20 kHz) hearing sensitivity was compared in thirty-six, 20 to 29-year-old military veterans with histories of steady-state or impulsive noise exposure. Threshold shifts were prominent for the steady-state noise subjects from 13 to 20 kHz. Mean thresholds from 8 through 12 kHz were maximally 20 dB poorer than a sample of young adult normals. Audiometric configurations for this group were generally smooth and symmetrical above 8000 Hz. For the impulsive noise group, substantial shifts in sensitivity were seen from 2 to 20 kHz and the high-frequency audiometric configurations were often jagged and/or asymmetrical. The variability of subjects in this group was greater than that seen in the steady-state noise exposed sample. Several case studies are presented to illustrate these characteristics. Measurement of auditory sensitivity from 8 to 20 kHz extends the mapping of basal cochlear function, providing information which often is not predictable from conventional audiometric measurement. This paper introduces the Continuity Moderate Resolution Imaging Spectroradiometer (MODIS)-Visible Infrared Imaging Radiometer Suite (VIIRS) Cloud Mask (MVCM), a cloud detection algorithm designed to facilitate continuity in cloud detection between the MODIS (Moderate Resolution Imaging Spectroradiometer) on the Aqua and Terra platforms and the series of VIIRS (Visible Infrared Imaging Radiometer Suite) instruments, beginning with the Soumi National Polar-orbiting Partnership (SNPP) spacecraft. It is based on the MODIS cloud mask that has been operating since 2000 with the launch of the Terra spacecraft (MOD35) and continuing in 2002 with Aqua (MYD35). The MVCM makes use of fourteen spectral bands that are common to both MODIS and VIIRS so as to create consistent cloud detection between the two instruments and across the years 2000–2020 and beyond. Through comparison data sets, including collocated Aqua MODIS and Cloud-Aerosol LIdar with Orthogonal Polarization (CALIOP) from the A-Train, this study was designed to assign statistical consistency benchmarks between the MYD35 and MVCM cloud masks. It is shown that the MVCM produces consistent cloud detection results between Aqua MODIS, SNPP VIIRS, and NOAA-20 VIIRS and that the quality is comparable to the standard Aqua MODIS cloud mask. Globally, comparisons with collocated CALIOP lidar show combined clear and cloudy sky hit rates of 88.2%, 87.5%, 86.8%, and 86.8% for MYD35, MVCM Aqua MODIS, MVCM SNPP VIIRS, and MVCM NOAA-20 VIIRS, respectively, for June through until August, 2018. For the same months and in the same order for 60S–60N, hit rates are 90.7%, 90.5%, 90.1%, and 90.3%. From the time series constructed from gridded daily means of 60S–60N cloud fractions, we found that the mean day-to-day cloud fraction differences/standard deviations in percent to be 0.68/0.55, 0.94/0.64, −0.20/0.50, and 0.44/0.82 for MVCM Aqua MODIS-MVCM SNPP VIIRS day and night, and MVCM NOAA-20 VIIRS-MVCM SNPP VIIRS day and night, respectively. It is seen that the MODIS and VIIRS 1.38 µm cirrus detection bands perform similarly but with MODIS detecting slightly more clouds in the middle to high levels of the troposphere and the VIIRS detecting more in the upper troposphere above 16 km. In the Arctic, MVCM Aqua MODIS and SNPP VIIRS reported cloud fraction differences of 0–3% during the mid-summer season and −3–4% during the mid-winter. [ABSTRACT FROM AUTHOR]

Introduction: Delirium is an acute and fluctuating change in attention and cognition that increases the risk of functional decline, institutionalisation and death in hospitalised patients. After delirium, patients have a significantly higher risk of readmission to hospital. Our aim was to investigate factors associated with hospital readmission in people with delirium. Methods: We carried out an observational retrospective cohort study using linked mental health care and hospitalisation records from South London. Logistic regression models were used to predict the odds of 30‐day readmission and Cox proportional hazard models to calculate readmission risks when not restricting follow‐up time. Results: Of 2814 patients (mean age 78.9 years SD ±11.8) discharged from hospital after an episode of delirium, 823 (29.3%) were readmitted within 30 days. Depressed mood (odds ratio (OR) 1.34 (95% confidence interval (CI) 1.08–1.66)), moderate‐to‐severe physical health problems (OR 1.67 (95% CI 1.18–2.2.36)) and a history of serious circulatory disease (OR 1.29 (95% CI 1.07–1.55)) were associated with higher odds of hospital readmission, whereas a diagnosis of delirium superimposed on dementia (OR 0.67 (95% CI 0.53–0.84)) and problematic alcohol/substance (OR 0.54 (95% CI 0.33–0.89)) use were associated with lower odds. Cox proportionate hazard models showed similar results. Conclusion: Almost one‐third of patients with delirium were readmitted within a short period of time, a more detailed understanding of the underlying risk factors could help prevent readmissions. Our findings indicate that the aetiology (as alcohol‐related delirium), the recognition that delirium occurred in the context of dementia, as well as potentially modifiable factors, as depressed mood affect readmission risk, and should be assessed in clinical settings. This paper summarizes the Collection-6 refinements in the Moderate Resolution Imaging Spectroradiometer (MODIS) operational cloud-top properties algorithm. The focus is on calibration improvements and on cloud macrophysical properties including cloud-top pressure-temperature-height and cloud thermodynamic phase. The cloud phase is based solely on infrared band measurements. In addition, new parameters will be provided in Collection 6, including cloud-top height and a flag for clouds near the tropopause. The cloud parameters are improved primarily through 1) improved knowledge of the spectral response functions for the MODIS 15- μm carbon dioxide bands gleaned from comparison of coincident MODIS and Atmospheric Infrared Sounder (AIRS) radiance measurements and 2) continual comparison of global MODIS and Cloud-Aerosol Lidar and Infrared Pathfinder Satellite Observations ( CALIPSO) instantaneous cloud products throughout the course of algorithm refinement. Whereas the cloud-top macrophysical parameters were provided through Collection 5 solely at 5-km spatial resolution, these parameters will be available additionally at 1-km spatial resolution in Collection 6.
