Abstract

Treatment of chronic heart failure (CHF) is strongly focused on evidence-based medicine. However, large trials are often far away from the "real world" of geriatric patients and their messages are poorly transferable to the clinical management of CHF elderly patients. Precipitating factors and especially non-cardiac comorbility may decompensate CHF in the elderly. More importantly, drugs of first choice, such as Angiotensin Converting Enzyme inhibitors and β-blockers, are still underused and effective drugs on diastolic dysfunction are not available. Poor adherence to therapy, especially due to cognitive disorders and depression, worsens the management. Electrical therapy is indicated, but attention to the older age groups with reduced life expectancy has to be paid. Physical exercise, stem cells, gene therapy and new devices are encouraging, but definitive results are not available yet. Palliative care plays a key role in the end-stage of the disease. Follow-up of CHF elderly patient is very important but tele-medicine is the future. Finally, self-care management, caregiver training and multidimensional team represent the critical point of the treatment of CHF elderly patients.

Abstract

Background Chronic heart failure (CHF) is characterized by a high mortality in the elderly. Moreover, depression status is also related to poor prognosis in advancing age. Thus, we sought to determine whether depressive status predicts long-term mortality in subject with or without CHF. Methods and results Long-term mortality after 12-year follow-up in 125 elderly subjects with CHF and 1143 elderly subjects without CHF was studied. Depression was evaluated using the Geriatric Depression Scale (GDS), and all subjects were stratified in tertiles according to GDS score (0–10, 11–20 and 21–30). With increasing GDS score, long-term mortality at the end of follow-up increased from 43Æ4% to 72Æ0% in subjects without (P < 0Æ001) and from 52Æ6% to 83Æ9% in subjects with (P < 0Æ007) CHF. In multivariate analysis, GDS appeared to be predictive of long-term mortality in the absence (Hazard ratio = 1Æ01; confidence interval 95% 1Æ00–1Æ05; P = 0Æ04) and, even more, in the presence of CHF (Hazard ratio = 1Æ08; confidence interval 95% 1Æ01–1Æ15; P < 0Æ01). Conclusions Depression symptoms predict long-term mortality in elderly subjects without and, even more, with CHF. Thus, depression can be considered a strong predictor of death in CHF elderly subjects.

Abstract

Background and aim of the study: Decellularized xenogeneic pulmonary valves have been introduced for right ventricular outflow tract (RVOT) reconstruction in congenital heart disease. In the present study, the intermediate-term results from three institutions were analyzed. Methods: Between January 2006 and September 2008, a total of 61 patients (median age 7 years; range: 9 days to 50 years; median body weight 21 kg; range: 1.9-140 kg) underwent RVOT reconstruction with either the Matrix P® (n = 9) or Matrix P Plus® (n = 52) tissue-engineered conduit. Eighteen patients underwent surgery in infancy, and 31 patients had previously undergone one or more RVOT interventions or operations. Results: The valve sizes ranged from 11 to 27mm. Five patients died during the hospital stay or within three months, from non-valve-related causes; hence, the early mortality was 8.2%. No deaths occurred during the follow up period. Reoperation due to valve failure became necessary in four patients; three patients underwent RVOT interventions due to distal anastomotic stenosis, and six reinterventions were performed distal to the valve due to hypoplastic branch pulmonary arteries. Patients with valve implantation during infancy showed a composite freedom from valve-related reoperation, catheter intervention or valve dysfunction (defined as dPmax >40 mmHg) of 87% at one and three years postoperatively. Both, computed tomography and magnetic resonance imaging studies demonstrated normal structural features, with no evidence of calcification. Conclusion: The Matrix P/Matrix Plus conduit represents a viable alternative for RVOT reconstruction in patients with congenital heart disease. The intermediate-term performance of the conduits was favorable compared to that of other currently available implants. © Copyright by ICR Publishers 2011.

Abstract

Although modern ultrasound provides remarkable images and biophysical measures, the technology is expensive and the observations are only available over a short time. Longer term monitoring is achieved in a clinical setting using ultrasonic Doppler cardiotocography (CTG) but this has a number of limitations. Some pathologies and some anomalies of cardiac functioning are not detectable with CTG. Moreover, although frequent and/or long-term foetal heart rate (FHR) monitoring is recommended, mainly in high risk pregnancies, there is a lack of established evidence for safe ultrasound irradiation exposure to the foetus for extended periods (Ang et al., 2006). Finally, high quality ultrasound devices are too expensive and not approved for home care use. In fact, there is a remarkable mismatch between ability to examine a foetus in a clinical setting, and the almost complete absence of technology that permits longer term monitoring of a foetus at home. Therefore, in the last years, many efforts (Hany et al., 1989; Jimenez et al., 1999; Kovacs et al., 2000; Mittra et al., 2008; Moghavvemi et al., 2003; Nagal, 1986; Ruffo et al., 2010; Talbert et al., 1986; Varady et al., 2003) have been attempted by the scientific community to find a suitable alternative. The development of new electronic systems and sensors now offers the potential of effective monitoring of the foetus using foetal phonocardiography (FPCG) and foetal electrocardiography (FECG) with passive, fully non-invasive low cost digital recording systems that could be suitable for home monitoring. These advances provide the opportunity of extending the recordings of the current commonly used CTG from relative short to long term, and provide new previously unavailable measures of cardiac function. In this chapter, we present highlights of our research into non-invasive foetal monitoring. We introduce the use of FECG, FPCG and their combination in order to detect the foetal heart rate (FHR) and potential functional anomalies. We present signal processing methodologies, suitable for longer-term assessment, to detect heart beat events, such as first and second heart sounds and QRS waves, which provide reliable measures of heart rate, and offer the potential of new information about measurement of the systolic time intervals and foetus circulatory impedance.

Abstract

Polydnavirus-encoded IjB-like proteins are similar to insect and mammalian IjB, and an immunosuppressive function in the host cells has been inferred to these proteins. Here we show that the expression of one of these IjB-like viral genes, the TnBVank1, in the Drosophila germline affects the localization of gurken, bicoid, and oskar mRNAs whose gene products are relevant for proper embryonic patterning. The altered localization of these mRNAs is suggestive of general defects in the intracellular, microtubule-based, trafficking routes. Analysis of microtubule motor proteins components such as the dynein heavy chain and the kinesin heavy chain revealed defects in the polarized microtubule network. Interestingly, the TnBVANK1 viral protein is uniformly distributed over the entire oocyte cortex, and appears to be anchored to the microtubule ends. Our data open up a very interesting issue on novel function(s) played by the ank gene family by interfering with cytoskeleton organization.
