164. Genomic and transcriptomic profile in malignant melanoma patients.

Abstract

Malignant melanoma continues to remain one of the most aggressive malignancies worldwide and, despite significant advances in immunotherapy and targeted strategies, the mortality rate is increasing. The aim of our study was to understand the mechanisms involved in the development of malignant melanoma in patients with primary tumors locally advanced with pathological status pIIIb and pIVb, using expression profiling of miRNAs through microarray analysis, mutational status and frequency among patients. NGS cancer panels and the usual IHC staining performed in the clinic were used. MiR-205, miR-122, miR-192, miR125b, miR-21-5p, miR-210, miR-29c, miR-221, miR-143-3p, let-7c and let-7b are a few relevant miRNAs identified in our study to be involved in targeting major genes in malignant melanoma. Moreover, our data demonstrated that S-100 remains the most suitable marker for melanocytic lesions among IHC markers, and that HMB-45, Melan-A and MITF exhibited a sensitive specificity, together with S100. Our results revealed a specific profile between males and females in both molecular analyses. Furthermore, we demonstrated that the actual biomarkers used for immunohistochemistry staining are not specific enough to offer a complete overview of the tumor status. 198. Miniplatforms for Screening Biological Samples for KRAS and Four Mismatch Repair Proteins as New Tools for Fast Screening for Gastric and Colon Cancers

Abstract

Two miniplatforms based on stochastic microsensors designed using Nitrogen (9.3%) and Boron (2.4%) - dopped graphene (NB-DG) modified with frutafit HD and frutafit TEX were designed and validated for the assay of MLH1, MSH2, MSH6, PMS2, and of KRAS in whole blood, urine, saliva, and tumoral tissues. The sensitivities recorded using the miniplatform based on frutafit TEX were higher (MLH1:1.07 × 104, MSH2: 5.31; MSH6: 1.58 × 103; KRAS: 1.36 × 10−2s−1μg−1ml) than those recorded when frutafit HD was used. A lower value of the limit of determination (0.32 fg ml−1) was recorded for the frutafit HD based miniplatform when used for the assay of MLH1, while the lowest value of the limit of determination for the assay of KRAS (2.2 fg ml−1) was recorded when the frutafit TEX was used in the design of the miniplatform. The % recoveries of MLH1, MSH2, MSH6, PMS2, and of KRAS in whole blood, urine, saliva, and tumoral tissues were higher than 99.00 with RSD (%) values lower than 0.08%. more...

199. Proposal of a Dukes-Mac-Like Staging System for Gastric Cancer

Abstract

The aim of this study was to present an epidemiological update regarding the classical prognostic parameters of gastric cancer (GC) in 3 countries from Eastern Europe and to suggest a modification of the pTNM staging system. In 333 consecutive cases which were diagnosed between 2003 and 2012 in 3 departments of pathology from Romania, Hungary, and Poland, the following parameters were analyzed: age and gender of patients, tumor localization, macroscopic and microscopic aspects including the degree of discohesivity, depth of tumor infiltration, and pTNM stage. From all of the studied parameters, the following proved to have independent prognostic value, indicating a lower survival rate: presence of distant metastases (p=0.001), lymph node positivity (p=0.0009), depth of tumor infiltration (p=0.04), age over 50 (p=0.02), proximally located tumors (p=0.03), and ulceroinfiltrative or diffusely infiltrative macroscopic aspect (p=0.0002). The pT2N1-3 staged cases showed a worse prognosis compared with the pT3N0 ones (p=0.02). Regardless of depth of invasion, the lymph node status remains the strongest indicator of the survival rate in GC. The pTN staging system should be adapted and a Dukes-MAC-like staging system should include the following groups: stage A1—T1N0, stage A2—T1N1-3, stage B1—T2N0, stage B2—T2N1-3, stage C1—T3N0, stage C2—T3N1-3, and stage D—T4N0-3. 200. Histopathological Evaluation and Expression of the Pluripotent Mesenchymal Stem Cell-Like Markers Cd105 and Cd44 in the Synovial Membrane of Patients with Primary versus Secondary Hip Osteoarthritis

Abstract

To present the morphological changes of classic primary versus rapidly progressive and secondary hip osteoarthritis (HO) and to examine the expression of two pluripotent mesenchymal stem cell-like markers in the synovial membrane. A prospective observational study was conducted in 57 consecutive cases of radiologically confirmed HO in which total hip arthroplasty was performed. Based on the radiological and clinicopathological features, the cases were divided into three categories: classic primary HO (group A; n=16), rapidly destructive HO (group B; n=24), and HO secondary to avascular osteonecrosis of the femoral head (group C; n=17). Immunostains were performed using the markers CD44 and CD105. The cases from group A were mainly characterized by a marked perivascular inflammatory infiltrate and simple synovial hyperplasia. In group B, the papillary type of synovial hyperplasia was found and presence of chondromatosis, ossification, and ectopic follicles with germinal centers in the subsynovial layer was characteristic, whereas marked calcification and/or ossification were seen in group C. Focal expression of the CD105 and CD44 was noted in the hyperplastic synovial cells and subsynovial layer in cases from group A, whereas synovial cells from group B were diffusely positive for both CD44 and CD105. In secondary HO, CD44 marked the inflammatory cells. Mobilization of the CD44/CD105 positive synovial cells seems to play a role in the genesis of HO. The number of the pluripotent mesenchymal stem cell-like cells derived from the hyperplastic synovial cells might be related to the severity of possible immune-mediated rapidly destructive HO. more...
