Deploying an everyday life approach, this article focuses on the kindergarten meal, defined as a space in which humans, materials and discursive elements interact. The article identifies and discusses two co-existing perspectives on the everyday meals that emphasise children as future beings and here-and-now beings. Through the concepts of smoothing and striation the paper discusses how these perspectives produce different mechanisms of regulation and agency, and position the eaters differently. The paper emphasises kindergarten mealtime as an ambiguous space that does not offer simple discussions about good and bad meal situations. The paper thereby adds to the existing literature within children’s geographies which emphasise interactional, relational and material aspects of children’s lives. It does so by revisiting some concepts, striation and smoothing, that has been used to explore children’s spaces and child–adult relations, but argue that these concepts describe ambivalent and complex processes in children’s and adult’s everyday lives. Background: Studies in taxane and/or anthracycline refractory metastatic breast cancer (mBC) patients have shown approximately 30% response rates to irinotecan. Hence, a significant number of patients will experience irinotecan-induced side effects without obtaining any benefit. The aim of this study was to lay the groundwork for development of predictive biomarkers for irinotecan treatment in BC. Methods: We established BC cell lines with acquired or de novo resistance to SN-38, by exposing the human BC cell lines MCF-7 and MDA-MB-231 to either stepwise increasing concentrations over 6 months or an initial high dose of SN-38 (the active metabolite of irinotecan), respectively. The resistant cell lines were analyzed for cross-resistance to other anti-cancer drugs, global gene expression, growth rates, TOP1 and TOP2A gene copy numbers and protein expression, and inhibition of the breast cancer resistance protein (ABCG2/BCRP) drug efflux pump. Results: We found that the resistant cell lines showed 7-100 fold increased resistance to SN-38 but remained sensitive to docetaxel and the non-camptothecin Top1 inhibitor LMP400. The resistant cell lines were characterized by Top1 down-regulation, changed isoelectric points of Top1 and reduced growth rates. The gene and protein expression of ABCG2/BCRP was up-regulated in the resistant sub-lines and functional assays revealed BCRP as a key mediator of SN-38 resistance. Conclusions: Based on our preclinical results, we suggest analyzing the predictive value of the BCRP in breast cancer patients scheduled for irinotecan treatment. [Display omitted] Recent work has shown that label-efficient few-shot learning through self-supervision can achieve promising medical image segmentation results. However, few-shot segmentation models typically rely on prototype representations of the semantic classes, resulting in a loss of local information that can degrade performance. This is particularly problematic for the typically large and highly heterogeneous background class in medical image segmentation problems. Previous works have attempted to address this issue by learning additional prototypes for each class, but since the prototypes are based on a limited number of slices, we argue that this ad-hoc solution is insufficient to capture the background properties. Motivated by this, and the observation that the foreground class (e.g., one organ) is relatively homogeneous, we propose a novel anomaly detection-inspired approach to few-shot medical image segmentation in which we refrain from modeling the background explicitly. Instead, we rely solely on a single foreground prototype to compute anomaly scores for all query pixels. The segmentation is then performed by thresholding these anomaly scores using a learned threshold. Assisted by a novel self-supervision task that exploits the 3D structure of medical images through supervoxels, our proposed anomaly detection-inspired few-shot medical image segmentation model outperforms previous state-of-the-art approaches on two representative MRI datasets for the tasks of abdominal organ segmentation and cardiac segmentation. We sequenced the D21S11 locus in 77 individuals from Danish paternity cases using 454 FLX next generation sequencing (NGS) technology. All samples were also typed with the AmpFlSTR® Profiler Plus® or the AmpFlSTR® Identifiler® PCR Amplification kits as part of paternity investigations. In 18 of the confirmed trios, a genetic inconsistency was observed between one of the parents and the child at the D21S11 locus. NGS of the D21S11 locus revealed which allele had mutated from which parent to the child in 13 of these trios. All characterized mutations could be explained by single-step mutations in the longest sub-repeat of D21S11. A total of 53 of the 77 sequenced samples originated from unrelated individuals. Twenty different D21S11 alleles were detected by NGS in these individuals whereas only 13 different alleles were observed with fragment analysis. Several alleles had the same lengths but different sequences, e.g. four and three different alleles were detected by NGS with lengths determined by CE corresponding to allele 30 and allele 31, respectively. Sustainable agricultural education is a fundamental base for the sustainable development of the agri-food systems. Sustainable education should provide the necessary skills to the practical world. This paper is focused on skills and competences required by the agri-food stakeholders for their current work and for future sustainable development around the world. An online survey was disseminated for data collection, and quantitative and qualitative analyses were employed to understand the impact of the demographic factors on the variety of skills mentioned by the different stakeholders. As a result, according to all of the stakeholders, the skills of navigating in a changing world, networking and strategic development are relevant for their current work, and technical skills were mentioned as relevant for future sustainable development. Abstract: In this study, 10 young steers received no beta agonist (C), 50 animals all received zilpaterol hydrochloride (Z), with 1 group receiving Z while the other 4 groups received Z and vitamin D3 at the following levels (IU/animal /day) and durations before slaughter: 7 million for 3days (3D7M) or 6days (6D7M), 7 million for 6days with 7days no supplementation (6D7M7N) and 1 million for 9days (9D1M). LD samples were vacuum-aged 14days post mortem, and repacked in high-oxygen modified atmosphere (70% O2/30% CO2) for a further 7days. Parameters included Warner Bratzler shear force (WBSF), myofibril fragment length, thiobarbituric acid reactive substances, free thiol levels, muscle fat, and instrumental colour parameters. When allowing for a conversion period of vitamin D3 to its active metabolites, supplementing the zilpaterol treated steers with a high dosage of vitamin D3 (6D7M7N) resulted in improved colour stability, higher stability towards protein oxidation and lower stability towards lipid oxidation. The neural cell adhesion molecule (NCAM) plays a critical role in development and plasticity of the nervous system and is involved in the mechanisms of learning and memory. Here, we show that intracerebroventricular administration of the FG loop (FGL), a amino acid peptide corresponding to the binding site of NCAM for the fibroblast growth factor receptor 1 (FGFR1), immediately after training rats in fear conditioning or water maze learning, induced a long-lasting improvement of memory. In primary cultures of hippocampal neurons, FGL enhanced the presynaptic function through activation of FGFR1 and promoted synapse formation. These results provide the first evidence for a memory-facilitating effect resulting from a treatment that mimics NCAM function. They suggest that increased efficacy of synaptic transmission and formation of new synapses probably mediate the cognition-enhancing properties displayed by the peptide. [ABSTRACT FROM AUTHOR]

Whole blood is most often the matrix of choice for postmortem analysis but it is not always available. In these cases, muscle tissue can be used as an alternative matrix. Therefore, an ultra-high-performance liquid chromatography-tandem mass spectrometry method for the quantification of 29 drugs and metabolites of toxicological interest in postmortem muscle tissue was developed and validated. Additionally, a validation of whole blood was carried out to compare the results from the two matrices. Solid-phase extraction was performed by an automated robotic system to minimize manual labour and risk of human errors, and increase robustness, sample throughput and sample traceability. The method was validated in terms of selectivity, matrix effect, extraction recovery, process efficiency, measuring range, lower limit of quantification, carry-over, stability, precision and accuracy. To correct for any inter-individual variability in matrix effects on analyte accuracy and precision, deuterated analogues of each analyte were used as internal standards. The lower limit of quantification in both blood and muscle homogenate ranged between 0.002 and 0.005 mg/kg, while the upper limit of quantification spanned from 0.20 to 1.0 mg/kg. Corrected with the 4-fold dilution factor, the corresponding concentrations in muscle tissue were 0.008-0.02 mg/kg at the lower limit of quantification and 0.80-4.0 mg/kg at the upper limit of quantification. The method showed acceptable precision and accuracy, with precision below 12% and accuracies ranging from 87% to 115% at up to 6 levels for all analytes in both matrices. In addition, comparison between calibration standards in spiked muscle homogenate and spiked blood showed that analyte concentrations in muscle samples could be quantified by using spiked blood samples as calibration standards with acceptable precision and accuracy when using deuterated analogues as internal standards. The investigation of matrix effects showed no great difference between blood and homogenates of non-decomposed and decomposed muscle tissue for most analytes. In the samples where high ion suppression or enhancement was observed, the results were corrected by the internal standards. Statistical comparison of quality control samples in blood and muscle tissue showed no obvious differences, and therefore muscle tissue was included in the routine method for analysis of blood samples and used in autopsy cases where no blood was available. By adding a semi-automated homogenization step before the remaining automated sample preparation, muscle tissue samples were easily incorporated into the workflow of the existing routine method. This study investigated the efficacy of a 10-session nature-based therapeutic intervention for people with post-concussion symptoms. The intervention involved physical and vestibular exercises, sensory training, relaxation, and psychoeducation, all of which were integrated with the natural environment in a forest therapy garden. This study was designed with a passive control period followed by the intervention (n = 30). The Mental Fatigue Scale (MFS) was the primary outcome measure. The secondary outcome measures were the Warwick–Edinburg Mental Wellbeing Scale and the short version of the Quality of Life after Brain Injury. A Likert scale was used to examine the mental strain of the sessions themselves. The MFS (primary outcome) exhibited a significant decrease with a medium-sized effect from before to after the intervention. The secondary outcomes exhibited significant increases from the beginning to the end of the intervention. All outcomes were sustained at follow-up ten weeks later. No significant difference was found from the control period. This study indicates that the described nature-based intervention is a feasible treatment for reducing prolonged post-concussion symptoms. Tumor segmentation is a crucial but difficult task in treatment planning and follow-up of cancerous patients. The challenge of automating the tumor segmentation has recently received a lot of attention, but the potential of utilizing hybrid positron emission tomography (PET)/magnetic resonance imaging (MRI), a novel and promising imaging modality in oncology, is still under-explored. Recent approaches have either relied on manual user input and/or performed the segmentation patient-by-patient, whereas a fully unsupervised segmentation framework that exploits the available information from all patients is still lacking. We present an unsupervised across-patients supervoxel-based clustering framework for lung tumor segmentation in hybrid PET/MRI. The method consists of two steps: First, each patient is represented by a set of PET/MRI supervoxel-features. Then the data points from all patients are transformed and clustered on a population level into tumor and non-tumor supervoxels. The proposed framework is tested on the scans of 18 non-small cell lung cancer patients with a total of 19 tumors and evaluated with respect to manual delineations provided by clinicians. Experiments study the performance of several commonly used clustering algorithms within the framework and provide analysis of (i) the effect of tumor size, (ii) the segmentation errors, (iii) the benefit of across-patient clustering, and (iv) the noise robustness. The proposed framework detected 15 out of 19 tumors in an unsupervised manner. Moreover, performance increased considerably by segmenting across patients, with the mean dice score increasing from 0. 169 ± 0. 295 (patient-by-patient) to 0. 470 ± 0. 308 (across-patients). Results demonstrate that both spectral clustering and Manhattan hierarchical clustering have the potential to segment tumors in PET/MRI with a low number of missed tumors and a low number of false-positives, but that spectral clustering seems to be more robust to noise. Background: PROMs can help healthcare professionals gain an improved understanding of patients' physical burdens, functional levels, and (health-related) quality of life throughout disease and medical treatment. The aim of this study was to investigate the barriers and potential opportunities PROMs may present in a haematological outpatient clinic from three different perspectives: patients, nurses and haematologists. Methods: The present study synthesizes three previously published studies that separately explored the experiences of patients, nurses and haematologists when implementing PROMs. The studies were all guided by the qualitative methodology Interpretive Description, including a focused ethnographic approach, to develop implications for future practice. Results: The overall themes that emerged from the analysis were "Structural similarities influence the adoption of PROMs" and "Different perspectives on the potential of PROMs." Conclusion: Across the different user groups in the haematological outpatient clinic, the use of PROMs was thwarted due to an unquestioned commitment to biomedical knowledge and the system's rationality and norms: PROM data was not used in patient consultations. Nurses and haematologists expressed different preferences related to potential future PROMs and different objectives for PROMs in clinical practice. From the different perspectives of the patients, nurses and haematologists, PROMs were not compatible with clinical practice. Further research is recommended to develop PROMs validated for use in haematological outpatient clinics. Driving cessation, often due to health-related changes, can be a particularly challenging and troublesome transition in older adulthood that can lead to social isolation. While policy makers have long recognized the potential impact of an aging population on Canada's health care and national pension plans, the transportation needs of older adults have received relatively less attention. For older adults residing in small towns and rural areas who rely, more often than not, on the personal automobile there is usually limited or no access to public transportation. For policy makers to respond effectively to the transportation needs of an aging population, particularly those living in rural areas, the first step is to understand the travel behaviours of older adults living in such areas, particularly as they approach the transition when they will cease to drive. This paper reports on qualitative research exploring the driving-related challenges and needs faced by older adults in small towns and rural areas near Hamilton, Ontario, Canada. Results revealed four major themes: lack of transportation options in rural areas, changes in driving behaviours, the lack of planning for driving cessation, and the social isolation that comes from the loss (or potential loss) of one's driver's license. In forensic investigations, such as drug-facilitated crimes, reference values are useful for interpretation of hair results. The aim of this study was to establish levels of zopiclone and two main metabolites, N-desmethylzopiclone and zopiclone N-oxide, in hair after the administration of a single dose of zopiclone, as very limited data are published. A controlled study was performed, where 16 volunteers consumed either 5 or 10mg zopiclone. Hair was sampled prior to consumption and 14, 30, 60, and 120 days after intake. The deposition of drug in hair segments of all sampling time points was followed in small hair segments of 5-mm, using a validated ultra-high performance liquid chromatography-tandem mass spectrometry method. In all participants, hair segments corresponding to the time of intake were positive for zopiclone, but also with lower concentrations in the neighbouring segments. The highest zopiclone concentrations were detected in samples collected 30 or 60 days after intake. For all sampling time points maximum values for the 5-mg dose ranged from 5.0-370pg/mg for zopiclone and 5.4 to 300pg/mg for N-desmethylzopiclone, where the maximum values for the 10-mg dose ranged from 17 to 590pg/mg for zopiclone and 25-410pg/mg for N-desmethylzopiclone for all sampling time points. No significant difference in concentrations was found between the two dosing groups for either zopiclone or N-desmethylzopiclone. Almost half of the participants showed lower levels 14 days after intake than in the later sampling time points. The metabolite to parent drug ratio of N-desmethylzopiclone to zopiclone varied from 0.6 to 3.4 (median=1.2) for the maximum levels of all sampling time points. N-desmethylzopiclone are suggested to serve as an additional marker to confirm the intake of zopiclone. Traces of zopiclone N-oxide were detected in hair from only eight participants. This study showed, that it was possible to follow zopiclone and N-desmethylzopiclone in hair for 4 months even though the drugs was divided into several segments in the latest collected hair samples, and no obvious wash-out effect between the sampling time points by e.g. personal hygiene could be discerned because the cumulated amount at each sampling time point was similar. We conclude that the analysis of short segments e.g. segments of 5-mm can help determine the time of a single intake of zopiclone and that obtaining a sample 1-2 months after a drug exposure provide the best conditions to detect and interpret the results. This article discusses the challenges of assessing the severity of specific skin lesions in hidradenitis suppurativa (HS) from the patient's perspective. The study involved interviews with adult patients with HS, who were asked to rate the severity of different types of lesions using a numeric rating scale and a fixed response scale. The results showed that patients rated abscesses as the most severe, followed by draining tunnels and inflammatory nodules. The study also identified four themes related to the patient experience of HS lesions. The findings suggest that the current severity weightings used in outcome measurement instruments do not align with the patient experience, and more research is needed to develop a measurement tool that better reflects both patient and physician perspectives. Introduction: Behavioural and psychological symptoms of dementia (BPSD) should only rarely and briefly be treated with antipsychotics. Despite recommendations to the contrary, the use of antipsychotics in nursing home residents with dementia is widespread and followed by serious adverse effects. Intervention studies on methods to reduce the use of antipsychotics in persons with dementia are few and needed. The aim of this protocol is to describe the rationale and content of the intervention DEprescribing and Care to reduce Antipsychotics in DEmentia (DECADE)–a hybrid effectiveness-implementation pilot study. Materials and methods: This is a protocol of a prospective hybrid effectiveness-implementation pilot study. The primary aim of DECADE is to reduce the use of antipsychotic drugs by 50% in 50% of nursing home residents with dementia while maintaining or improving BPSD. The intervention is implemented in six nursing homes including approximately 190 residents with dementia and consists of Academic Detailing, medication review, education of nursing home staff, and care plans. The evaluation of feasibility and potential effectiveness is an overall assessment of all clinical and process outcomes. Logistic regression analyses will be used to investigate factors characterizing situations with prescription of antipsychotics. BPSD is analysed with a before- and after design using self-controlled case series methods and the use of antipsychotics is analysed as interrupted time series. Human IVF embryos that are not used for fresh transfer are cryopreserved by vitrification for later embryo transfers. This study evaluates pre-vitrification and post-warming embryo characteristics that are suitable to predict the chance of clinical pregnancy in single vitrified blastocyst transfer (SVBT) cycles. In a multicenter observational trial (IMBOS trial), embryos were cultured in a time-lapse system before and after vitrification. Associations between clinical pregnancy, morphokinetic parameters, blastocyst collapse, KIDScore D5, pre-vitrification and post-warming Gardner scores, post-warming blastocyst size and re-expansion rates before SVBT were analyzed in 182 SVBTs which resulted in 89 clinical pregnancies. No association was found between clinical pregnancy after SVBT and the number of collapses or the maximal collapse size before vitrification. The multifactorial analysis of pre-vitrification Gardner scores showed a significant association with clinical pregnancy for trophectoderm grading but not for expansion/hatching status and inner cell mass grading. A significant association with clinical pregnancy was found for the time to reach a blastocyst after pronuclear fading (tB-tPNf), KIDScore D5 and post-warming size but not the rate of expansion or maximal expansion size. The selection of blastocysts for SVBT could benefit from using pre-vitrification parameters like tB-tPNf, trophectoderm grading and post-warming blastocyst size. [ABSTRACT FROM AUTHOR]

The article presents a study which highlighted the value of unbiased profiling of regulatory landscapes for the identification of key mechanisms underlying tissue maturation. Topics include mechanism underlying the transcriptional differences, implication of chromatin accessibility and 5'-C-phosphate-G-3' (CpG) methylation changes on transcriptional changes at enhancers and a subset of promoters, and association of different transcription factors (TFs) with stage-specific chromatin changes.
