151. The Influence of a Lilly Endowment Grant to Recruit and Retain Part-Time Faculty in a Community College System

Abstract

This qualitative study examines the influence of a grant from the Lilly Endowment to recruit and retain intellectual capital of part-time faculty in a community college system. Through the use of grant funds, the college used in this study developed and implemented nine college-wide initiatives. This study examined adjunct faculty members' awareness of the grant and the nine initiatives, as well as examined the influence the grant had on their experience. Qualitative data on the lived experiences of adjunct faculty were collected and analyzed. Based upon the analysis of data, five themes emerged in this study: (a) limited awareness of the grant; (b) limited knowledge of the nine initiatives developed and implemented under the grant; (c) lack of formal communication regarding the grant and its initiatives; (d) the adjunct faculty experience was significantly influenced by orientation to the position; and (e) mentoring had a significant influence on their experience. The findings of this study resulted in implications for institutions of higher education, as well as generated recommendations for future practice and research. 152. Noncontingent Reinforcement Is an Empirically Supported Treatment for Problem Behavior Exhibited by Individuals with Developmental Disabilities

Abstract

Noncontingent reinforcement (NCR) is a function-based treatment for problem behavior that has produced robust effects across a variety of response topographies and reinforcement functions among individuals with developmental disabilities. Several narrative reviews have adequately described this literature. The purpose of the present article was to quantitatively analyze and classify the empirical support for NCR using the criteria developed by The Task Force on the Promotion and Dissemination of Psychological Procedures ["Task Force Promoting Dissemination of Psychological Procedures." (1995). "Training in and dissemination of empirically-validated psychological treatments: Report and recommendations." "Clinical Psychology," 48, 3-23]. Of the 59 studies identified for analysis, 24 met the criteria to be included in treatment classification. Fixed-time reinforcer delivery (plus extinction and schedule thinning) was classified as "well established," while fixed-time reinforcer delivery (plus extinction) and variable-time reinforcer delivery (plus extinction) were deemed "probably efficacious." 153. The Effects of Person versus Performance Praise on Children's Motivation: Gender and Age as Moderating Factors

Abstract

Two studies were conducted to determine how gender and age moderate the long-term and post-failure motivational consequences of person versus performance praise. In Study 1, fourth- and fifth-grade students (n = 93) engaged in a puzzle task while receiving either no praise, person praise, product praise, or process praise. Following a subsequent failure experience, behavioural measures indicated that product and process praise enhanced motivation and person praise dampened motivation for girls, but that there were few effects of praise on subsequent motivation for boys. In Study 2, a parallel procedure with preschool children (n = 76) showed that person, product, and process praise all enhanced motivation, relative to neutral feedback, for both girls and boys. 155. Checklists to Narratives in Special Education

Abstract

Learning stories, a form of narrative assessment, were used by teams surrounding two New Zealand children with high and complex needs. Learning stories are currently used by New Zealand early childhood teachers and highlight strengths- and interest-based learning in natural settings. This contrasts with specialists' assessments, which highlight decontextualised developmental skills. Team members including parents, teachers, support workers, health and education professionals received professional development on the use of learning story assessment and shared their stories at individual planning meetings. Goals for the children were collaboratively developed, drawing upon the different perspectives team members brought to the interpretation of learning stories. The project highlighted the empowerment of parents and educators and the strengthening of collaborative relationships. The approach accommodated the many perspectives held by individuals within the teams and successfully harmonised the skills- and strength-based assessment models. Challenges to the adoption of narrative methodologies in inclusive early intervention contexts were identified.

156. Intrinsic and Extrinsic Motivational Orientations in the Classroom: Age Differences and Academic Correlates

Abstract

Age differences in intrinsic and extrinsic motivation and the relationships of each to academic outcomes were examined in an ethnically diverse sample of 797 3rd-grade through 8th-grade children. Using independent measures, the authors found intrinsic and extrinsic motivation to be only moderately correlated, suggesting that they may be largely orthogonal dimensions of motivation in school. Consistent with previous research, intrinsic motivation showed a significant linear decrease from 3rd grade through 8th grade and proved positively correlated with children's grades and standardized test scores at all grade levels. Extrinsic motivation showed few differences across grade levels and proved negatively correlated with academic outcomes. Surprisingly few differences based on children's sex or ethnicity were found. Causes and consequences of the disturbingly low levels of motivation for older, relative to younger, children are discussed.

Abstract

When unperturbed, somatic stem cells are poised to affect immediate tissue restoration upon trauma. Yet, little is known regarding the mechanistic basis controlling initial and homeostatic ‘scaling’ of stem cell pool sizes relative to their target tissues for effective regeneration. Here, we show that TEAD1-expressing skeletal muscle of transgenic mice features a dramatic hyperplasia of muscle stem cells (i.e. satellite cells, SCs) but surprisingly without affecting muscle tissue size. Super-numeral SCs attain a ‘normal’ quiescent state, accelerate regeneration, and maintain regenerative capacity over several injury-induced regeneration bouts. In dystrophic muscle, the TEAD1 transgene also ameliorated the pathology. We further demonstrate that hyperplastic SCs accumulate non-cell-autonomously via signal(s) from the TEAD1-expressing myofiber, suggesting that myofiber-specific TEAD1 overexpression activates a physiological signaling pathway(s) that determines initial and homeostatic SC pool size. We propose that TEAD1 and its downstream effectors are medically relevant targets for enhancing muscle regeneration and ameliorating muscle pathology.

Abstract

Against the background of the pandemic caused by infection with the SARS-CoV-2, the German Society for Pneumology and Respiratory Medicine (DGP e.V.), in cooperation with other associations, has designated a team of experts in order to answer the currently pressing questions about therapy strategies in dealing with COVID-19 patients suffering from acute respiratory insufficiency (ARI).The position paper is based on the current knowledge that is evolving daily. Many of the published and cited studies require further review, also because many of them did not undergo standard review processes.Therefore, this position paper is also subject to a continuous review process and will be further developed in cooperation with the other professional societies.This position paper is structured into the following five topics:1. Pathophysiology of acute respiratory insufficiency in patients without immunity infected with SARS-CoV-22. Temporal course and prognosis of acute respiratory insufficiency during the course of the disease3. Oxygen insufflation, high-flow oxygen, non-invasive ventilation and invasive ventilation with special consideration of infectious aerosol formation4. Non-invasive ventilation in ARI5. Supply continuum for the treatment of ARIKey points have been highlighted as core statements and significant observations. Regarding the pathophysiological aspects of acute respiratory insufficiency (ARI), the pulmonary infection with SARS-CoV-2 COVID-19 runs through three phases: early infection, pulmonary manifestation and severe hyperinflammatory phase.There are differences between advanced COVID-19-induced lung damage and those changes seen in Acute Respiratory Distress Syndromes (ARDS) as defined by the Berlin criteria. In a pathophysiologically plausible - but currently not yet histopathologically substantiated - model, two types (L-type and H-type) are distinguished, which correspond to an early and late phase. This distinction can be taken into consideration in the differential instrumentation in the therapy of ARI.The assessment of the extent of ARI should be carried out by an arterial or capillary blood gas analysis under room air conditions and must include the calculation of the oxygen supply (measured from the variables of oxygen saturation, the Hb value, the corrected values of the Hüfner number and the cardiac output). In principle, aerosols can cause transmission of infectious viral particles. Open systems or leakage systems (so-called vented masks) can prevent the release of respirable particles. Procedures in which the invasive ventilation system must be opened, and endotracheal intubation must be carried out are associated with an increased risk of infection.The protection of personnel with personal protective equipment should have very high priority because fear of contagion must not be a primary reason for intubation. If the specifications for protective equipment (eye protection, FFP2 or FFP-3 mask, gown) are adhered to, inhalation therapy, nasal high-flow (NHF) therapy, CPAP therapy or NIV can be carried out according to the current state of knowledge without increased risk of infection to the staff. A significant proportion of patients with respiratory failure presents with relevant hypoxemia, often also caused by a high inspiratory oxygen fraction (FiO2) including NHF, and this hypoxemia cannot be not completely corrected. In this situation, CPAP/NIV therapy can be administered under use of a mouth and nose mask or a respiratory helmet as therapy escalation, as long as the criteria for endotracheal intubation are not fulfilled.In acute hypoxemic respiratory insufficiency, NIV should be performed in an intensive care unit or in a comparable unit by personnel with appropriate expertise. Under CPAP/NIV, a patient can deteriorate rapidly. For this reason, continuous monitoring with readiness to carry out intubation must be ensured at all times. If CPAP/NIV leads to further progression of ARI, intubation and subsequent invasive ventilation should be carried out without delay if no DNI order is in place.In the case of patients in whom invasive ventilation, after exhausting all guideline-based measures, is not sufficient, extracorporeal membrane oxygenation procedure (ECMO) should be considered to ensure sufficient oxygen supply and to remove COVor dem Hintergrund der Pandemie durch Infektionen mit dem SARS-CoV-2 hat die Deutsche Gesellschaft für Pneumologie und Beatmungsmedizin (DGP e.V.) federführend in Kooperation mit weiteren Verbänden ein Expertenteam benannt, um die zur Zeit drängenden Fragen zu Therapiestrategien im Umgang mit COVID-19-Patienten, die an akut respiratorischer Insuffizienz (ARI) leiden, zu beantworten. Das Positionspapier basiert auf dem momentanen aktuellen Wissen, das sich täglich weiterentwickelt. Viele der publizierten und zitierten Studien bedürfen weiterer Überprüfungen, auch weil viele kein übliches Review-Verfahren durchlaufen haben Daher unterliegt auch dieses Positionspapier einer ständigen Überprüfung und wird in Zusammenarbeit in der Zusammenarbeit mit den anderen Fachgesellschaften weiterentwickelt.Dieses Positionspapier wurde in die folgenden fünf Themenfelder gegliedert:1. Prozeduren, bei denen das invasive Beatmungssystem geöffnet werden muss, sowie die endotracheale Intubation sind mit einem erhöhten Infektionsrisiko verbunden.Der Schutz des Personals durch persönliche Schutzausrüstung soll sehr hohe Priorität haben, weil die Angst vor Ansteckung kein primärer Intubationsgrund sein darf. Bei Einhaltung der Vorgaben zu Schutzausrüstung (Augenschutz, FFP2- bzw. Abstract

The authors' "metatools" are a collection of tools for generic programming. This includes generating Java sources from mathematically well-founded specifications, as well as the creation of strictly typed document object models for XML encoded texts. In this context, almost every computer-internal structure is treated as a "model", and every computation is a kind of model transformation. This concept differs significantly from "classical model transformation" executed by specialized tools and languages. Therefore it seemed promising to the organizers of the TTC 2011, as well as to the authors, to apply metatools to one of the challenges, namely to the "compiler optimization task". This is a report on the resulting experiences.

162. Transfer of Veno-venous Extracorporeal Membrane Oxygenation Patients With COVID-19 Associated Acute Respiratory Distress Syndrome

Abstract

Interhospital transport of acute respiratory distress syndrome (ARDS) patients bears transport-associated risks. It is unknown how interhospital extracorporeal membrane oxygenation (ECMO) transfer of COVID-19 patients by mobile ECMO units affects ARDS mortality. We compared the outcome of 94 COVID-19 patients cannulated in primary care hospitals and retrieved by mobile ECMO-teams to that of 84 patients cannulated at five German ECMO centers. Patients were recruited from March 2020 to November 2021. Twenty-six transports were airborne, 68 were land-based. Age, sex, body-mass-index, Simplified Acute Physiology Score (SAPS) II, days invasively ventilated, and P/F-Ratio before ECMO initiation were similar in both groups. Counting only regional transports (≤250 km), mean transport distance was 139.5 km ± 17.7 km for helicopter (duration 52.5 ± 10.6 minutes) and 69.8 km ± 44.1 km for ambulance or mobile intensive care unit (duration 57.6 ± 29.4 minutes). Overall time of vvECMO support (20.4 ± 15.2 ECMO days for transported patients vs.21.0 ± 20.5 for control, p= 0.83) and days invasively ventilated (27.9 ± 18.1 days vs.32.6 ± 25.1 days, p= 0.16) were similar. Overall mortality did not differ between transported patients and controls (57/94 [61%] vs.51/83 [61%], p= 0.43). COVID-19 patients cannulated and retrieved by mobile ECMO-teams have no excess risk compared with patients receiving vvECMO at experienced ECMO centers. Patients with COVID-19-associated ARDS, limited comorbidities, and no contraindication for ECMO should be referred early to local ECMO centers.

163. Generalized Effects of Modeled Self-Reinforcement Training. Final Report.

Abstract

Four experiments focused on ways children could be trained to imitate others in imposing on themselves higher performance standards in game situations. The study also attempted to determine whether this internal achievement motivation behavior would be transferred by the children to situations such as learning in a classroom. The subjects were 122 male and 119 female elementary school children in grades two through six. The experiments involved children observing a peer model exhibiting high or low standards of self-reward in a novel game after which the subjects played the game, or subjects observing peer models choosing either difficult or easy goals in a novel athletic game after which the subjects played the same game, and, finally, subjects being given training in self-monitoring and simple goal-setting. Substantial persistence of the effects of exposure to the models and generalization of these effects to a new game was demonstrated. Subjects who had seen a model prefer more difficult goals had, themselves, chosen more difficult goals. Exposure to self-monitoring training produced an interest in achievement, although the goal-setting procedures had no effect on either study behavior or achievement. The study suggests that even relatively brief systematic attempts to affect children's goal-setting and self-monitoring behavior have significant beneficial effects, and that investigation into the adaptation of such techniques to educational contexts seems highly worthy of further pursuit.(Author/MS)

164. The Effects of Fantasy Context on Children's Learning and Motivation.

Abstract

Examined were the effects on children's learning and motivation of the embedding of instructional materials in various fantasy contexts. Students in the third grade worked with graphics commands from the LOGO programming language under one of three conditions. In a control condition, students were given a series of abstract problems characteristic of traditional instructional methods. In two fantasy conditions, identical problems were presented within a fantasy problem-solving context. In one group, children were given their choice of three alternative fantasy contexts; in the other, children were assigned fantasies yoked to the choices of the other group. Students were tested on their knowledge of LOGO and several underlying geometric concepts before, immediately after, and 2 weeks after the experimental sessions. Both fantasy groups showed significantly greater knowledge of LOGO, but not of underlying concepts, than their no-fantasy counterparts. There were no differences between the two fantasy groups. Unexpectedly, girls outperformed boys on both tests. Implications regarding the use of motivational embellishments to enhance interest and promote learning are discussed. (Author/RH)

167. Extrinsic Rewards and Intrinsic Motivation in Children. Final Report.

Abstract

Three experiments were conducted to examine the effects of providing extrinsic rewards for engaging in an activity on children's subsequent intrinsic interest in that activity. In each study, preschool children were asked to engage in an activity of initial intrinsic interest in individual experimental sessions. The children agreed to engage in this target activity under three different reward conditions. After these sessions, unobtrusive measures of the children's subsequent intrinsic interest were obtained during a series of free-play periods. In each of the studies, results indicated that asking children to engage in an activity of initial interest as a means to some ulterior end proved a consistently effective method for undermining these children's intrinsic interest in that activity. In addition, the results indicated that close adult surveillance also produced a similar decrement in subsequent intrinsic interest. Suggestions to maintain children's intrinsic motivation included: 1) systems of extrinsic reward systems should be employed only when nesessary to elicit the desired behavior pattern; and 2) when necessary, such programs should attempt to employ the least powerful rewards when required to produce the desired behavior change. 177. Incidence and predictors of cardiac arrhythmias in patients with COVID-19 induced ARDS.

Abstract

Recent studies suggest cardiac involvement with an increased incidence of arrhythmias in the setting of coronavirus disease 2019 (COVID-19). The aim of this study was to evaluate the risk of potentially lethal arrhythmias and atrial fibrillation in patients with COVID-19-induced acute respiratory distress syndrome (ARDS) and to elicit possible predictors of arrhythmia occurrence. A total of 107 patients (82 male, mean age 60 ± 12 years, median body mass index 28 kg/m2) treated for COVID-19-induced ARDS in a large tertiary university hospital intensive care unit between March 2020 and February 2021 were retrospectively analyzed. Eighty-four patients (79%) had at least moderate ARDS, 88 patients (83%) were mechanically ventilated, 35 patients (33%) received vvECMO. Forty-three patients (40%) died during their hospital stay. Twelve patients (11%) showed potentially lethal arrhythmias (six ventricular tachycardia, six significant bradycardia). Atrial fibrillation occurred in 27 patients (25%). In a multivariate logistic regression analysis, duration of hospitalization was associated with the occurrence of potentially lethal arrhythmias (p = 0.006). There was no association between possible predictive factors and the occurrence of atrial fibrillation. Invasive ventilation, antipsychotics, and the QT c interval were independently associated with acute in-hospital mortality, but this was not arrhythmia-driven as there was no association between the occurrence of arrhythmias and mortality. In this relatively young population with COVID-19-induced ARDS, the incidence of potentially lethal arrhythmias was low. While overall mortality was high in these severely affected patients, cardiac involvement and arrhythmia occurrence was not a significant driver of mortality. Abstract

Esta pesquisa avalia o grau de satisfação dos idosos na participação em grupos de convivência de Santa Cruz do Sul-RS. A população foi composta por homens e mulheres maiores de 60 anos que frequentavam grupos de convivência para terceira idade. O estudo quanti/qualitativo utilizou-se de entrevista estruturada, gravada e transcrita para posterior análise. Dos 486 idosos entrevistados acerca da sua compreensão sobre satisfação com a experiência e vivência em grupo, 92,86% apontaram uma visão positiva no convívio em grupos. A pesquisa reflete que a convivência e os trabalhos em grupos melhoram a sensação de bemestar em relação à saúde e fortalecem a rede de relações sociais, sendo fatores que compõem a melhora na qualidade de vida do idoso.

Abstract

Este estudo pretendeu conhecer a percepção dos idosos frente a aspectos sociais, entender como a sociedade vê o idoso e como isto é refletido e apresentado pela mídia. A pesquisa, do tipo levantamento (descritiva), utilizou uma metodologia qualitativa, com entrevistas estruturadas em um instrumento de coleta de dados com questões abertas. Através das narrativas dos idosos entrevistados foram construídas categorias pela técnica da análise de conteúdo. Frente a estas narrativas, contatou-se que há percepções positivas e negativas acerca do que é veiculado na mídia sobre a temática do idoso e na visão da velhice pela sociedade em geral. Assim, concluiu-se que o tema do envelhecimento ainda carece de abordagens, pelas mídias, condizentes com as reais necessidades desta população que cresce visivelmente em nossa sociedade.

Abstract

Most studies demonstrate that MEC is the most common malignant neoplasm of the salivary glands, especially of the minor salivary glands. A 58-year-old female was referred for dental services due to discomfort on the left jugal mucosa after an aspirative puncture that was performed by another professional without a final diagnosis. Clinical presentation disclosed a firm nodule, approximately 0.5 cm in its larger diameter, resembling a fibrotic area from the prior intervention. An excisional biopsy was performed based on the initial clinical diagnosis. Microscopic evaluation disclosed a mucoepidermoid carcinoma (MEC) with focal compromised surgical boundaries. The patient was referred for further complementary treatment in an oncologic center. This case reinforces the importance of final and definitive diagnostic procedures in each lesion presented in the oral mucosa, careful clinical evaluation, adequate biopsy procedures, and microscopic evaluation of any material removed, even in apparently clinically innocuous lesions. Abstract

Against the background of the pandemic caused by infection with the SARS-CoV-2, the German Society for Pneumology and Respiratory Medicine (DGP e.V.), in cooperation with other associations, has designated a team of experts in order to answer the currently pressing questions about therapy strategies in dealing with COVID-19 patients suffering from acute respiratory insufficiency (ARI). The position paper is based on the current knowledge that is evolving daily. Many of the published and cited studies require further review, also because many of them did not undergo standard review processes.Therefore, this position paper is also subject to a continuous review process and will be further developed in cooperation with the other professional societies. This position paper is structured into the following five topics: 1. Pathophysiology of acute respiratory insufficiency in patients without immunity infected with SARS-CoV-2 2. Temporal course and prognosis of acute respiratory insufficiency during the course of the disease 3. Oxygen insufflation, high-flow oxygen, non-invasive ventilation and invasive ventilation with special consideration of infectious aerosol formation 4. Non-invasive ventilation in ARI 5. Supply continuum for the treatment of ARI Key points have been highlighted as core statements and significant observations. Regarding the pathophysiological aspects of acute respiratory insufficiency (ARI), the pulmonary infection with SARS-CoV-2 COVID-19 runs through three phases: early infection, pulmonary manifestation and severe hyperinflammatory phase.
