167. Adjuvant strategies for patients with T1b invasion after endoscopic submucosal dissection for esophageal squamous cell carcinoma

Abstract

Objective·To compare the prognostic effects of radical resection of esophageal cancer, concurrent chemoradiotherapy and simple follow-up observation on the prognosis of patients with T1b invasion of superficial esophageal squamous cell carcinoma after endoscopic submucosal dissection (ESD).Methods·From May 2016 to May 2021, the clinical data of 67 patients with esophageal squamous cell carcinoma who were pathologically confirmed as pT1b after ESD and treated in Shanghai Chest Hospital were retrospectively analyzed. According to the additional treatment after ESD, the patients were divided into additional surgery group (S group), chemoradio-therapy group (CRT group) and observation group (O group). χ2 test was used to compare the clinical baseline data and pathological information of the three groups of patients. The Kaplan-Meier survival curve and log-rank test were used to compare the disease free survival (DFS) and recurrence free survival (RFS) of the three groups of patients, and the Cox proportional hazards regression model was used on DFS and RFS by univariate and multivariate analysis.Results·Among all 67 patients, there were 23 cases in the S group, 19 cases in the CRT group, and 25 cases in the O group. There was no significant difference in age (P=0.080), gender (P=0.078), tumor length (P=0.485), tumor location (P=0.655), lesion circumferential ratio (P=0.310), histological grading (P=0.084), depth of tumor invasion (P=0.066) and lymphovascular invasion (P=0.279) among the three groups. During (42.6±16.7) months of follow-up, tumor recurrence was observed in 10 cases (14.9%), including 6 patients (60%) with local recurrence, 2 patients (20%) with regional lymph recurrence and 2 patients (20%) with distant metastasis. The median recurrence time of group S, group CRT, and group O was 40.1, 36.6, and 22.1 months, and the 3-year DFSs were 100%, 89.5%, and 74.5% (P-trend=0.040). Multivariate Cox analysis showed that additional esophagectomy was the key to improving independent protective factors of RFS (HR=0.097, 95%CI 0.010‒0.956, P=0.046).Conclusion·For patients with superficial esophageal squamous cell carcinoma confirmed as pT1b after ESD, additional surgery can significantly reduce the possibility of long-term recurrence.

168. A Pumping Method for Assessing Airtightness of Packs - Application to Heated Tobacco Products

Abstract

The airtightness of heated tobacco product (HTP) packs is a very important indicator for the product quality and is also of great importance during the conditioning process. A method for evaluation of the airtightness was developed based on the air pressure difference in a constant pumping configuration. The essential feature of this method is that the pressure difference between the inside and the outside of the HTP packs during the deflation process is used to characterize the sealing quality of HTP packs. The detailed setup, the principle as well as the determination procedure are described. The accuracy and the repeatability of the method were assessed, and the effect of airtightness on the conditioning process was also investigated. The developed method is proven to be reliable with a standard deviation less than 0.09 kPa and repeatability less than 0.30 kPa. In addition, it was found that, although the transmission of moisture between HTPs and atmosphere could not be entirely prevented by the packs, airtightness still plays a significant role during the conditioning process, especially if the airtightness was at a relatively low level (e.g., lower than 1.5 kPa under a pumping flow rate of 200 mL/min). The method provides a promising way to assess and monitor the sealing quality of HTP packs, and it is suggested that the airtightness of the pack should not be lower than 2 kPa under a pumping flow rate of 200 mL/min. 187. Dispatchable Region for Active Distribution Networks Using Approximate Second-Order Cone Relaxation

Abstract

Uncertainty in distributed renewable generation threatens the security of power distribution systems. The concept of the dispatchable region was developed to assess the ability of power systems to accommodate renewable generation at a given operating point. Although DC and linearized AC power flow equations are typically used to model dispatchable regions for transmission systems, these equations are rarely suitable for distribution networks. To achieve a suitable trade-off between accuracy and efficiency, this paper proposes a dispatchable region formulation for distribution networks using tight convex relaxation. Second-order cone relaxation is adopted to reformulate the AC power flow equations, which are then approximated by a polyhedron to improve tractability. Further, an efficient adaptive constraint generation algorithm is employed to construct the proposed dispatchable region. Case studies on distribution systems of various scales validate the computational efficiency and accuracy of the proposed method., Comment: 10 pages, 10 figures

188. Contention-based Grant-free Transmission with Extremely Sparse Orthogonal Pilot Scheme

Abstract

Due to the limited number of traditional orthogonal pilots, pilot collision will severely degrade the performance of contention-based grant-free transmission. To alleviate the pilot collision and exploit the spatial degree of freedom as much as possible, an extremely sparse orthogonal pilot scheme is proposed for uplink grant-free transmission. The proposed sparse pilot is used to perform active user detection and estimate the spatial channel. Then, inter-user interference suppression is performed by spatially combining the received data symbols using the estimated spatial channel. After that, the estimation and compensation of wireless channel and time/frequency offset are performed utilizing the geometric characteristics of combined data symbols. The task of pilot is much lightened, so that the extremely sparse orthogonal pilot can occupy minimized resources, and the number of orthogonal pilots can be increased significantly, which greatly reduces the probability of pilot collision.
