Abstract

A case of chronic neutrophilic leukemia (CNL), a rare myeloproliferative syndrome associated with monoclonal gammopathy of uncertain significance (MGUS-Type IgGk), is reported. Karyotypic study, carried out on bone marrow, excluded Philadelphia-pos. chronic myeloid leukemia (CML) and showed Y loss (45 XO). Only a few cases of CNL with paraproteinemia have been reported, but no case of associated karyotypic abnormalities and paraproteinemia has so far been described.

Abstract

This paper provides an interpretation of the behaviour of retaining structures made of geosynthetic-reinforced earth, subjected to a severe seismic loading. It is seen that during strong ground motion the main source of energy dissipation derives from the transient activation of plastic mechanisms within the soil mass: these mechanisms can be global, local, or a combination of the two. Using numerical pseudo-static analyses and limit analysis methods it is shown that three retaining structures having a similar overall seismic resistance, expressed by their critical seismic coefficient, activate different - global, local, or combined - plastic mechanisms. The seismic performance of the different retaining structures is then evaluated through a series of dynamic analyses in which acceleration-time histories are imposed to the bottom boundary of the same numerical models used for the pseudo-static analyses. The results of the dynamic analyses are interpreted in the light of the plastic mechanisms evaluated with the pseudo-static procedure. They show that for the reinforced-earth structures there is always a local contribution to the dissipation of energy during strong motion, evidenced by the attainment of the available strength in different portions of the soil-reinforcement system, and that this energy dissipation has a substantial influence on the seismic performance of the system. 192. Fetal ovarian cysts: postnatal evolution and indications for surgical treatment.

Abstract

Fetal ovarian cysts are an increasingly common finding during routine prenatal ultrasonography (US). These lesions were occasionally diagnosed in the past, but only on the basis of a palpable abdominal mass or, rarely, because a twisted ovarian cyst caused an acute abdomen. Surgery is currently recommended for neonates after prenatal diagnosis, even in asymptomatic cases, when the cyst has a diameter larger than 5 cm or echographic findings suggest torsion or intracystic hemorrhage. In our 12 cases of suspected fetal ovarian cysts, wide variations in diameter were seen in 6 during pre- and postnatal follow-up. Two of these were not confirmed at birth; 7 were treated conservatively and excluding 1 perinatal death from multiple associated anomalies and 1 case lost to follow-up, the remaining cases showed uneventful, spontaneous regression of the cyst. Postnatal US signs suggesting necrosis secondary to torsion in a previously uncomplicated cyst and requiring surgery were seen in only 3 cases. Conservative treatment must be recommended in most prenatally diagnosed cysts; indications for surgery cannot be based only on dimensions. The incidence of complications has been overemphasized and the risk of associated malignancy is negligible in newborns, while possible size reduction and resorption can be expected in most cases. Nonoperative management may also be recommended when signs of torsion are seen before birth and spontaneous asymptomatic involution of the cystic ovary is documented in early postnatal life. Accurate clinical and US follow-up is mandatory in all cases. Surgery must be reserved for symptomatic cases or when an alteration of the cystic structure is documented by US in a previously uncomplicated cyst. [ABSTRACT FROM AUTHOR]

Abstract

Different variables influence the possibility that maternal viral infection may be transmitted to the fetus, although not all fetal infections result in fetal “illness” with consequent fetopathy. As concerns the fetus, prenatal diagnosis includes invasive techniques necessary for fetal tissue sampling. These techniques carry some risks. The fetal infectious risk, as determined by maternal clinico-serological profile and according to sonographic investigation, always should be weighed against the risks and benefits of invasive diagnostic procedures. The present study re-elaborates the criteria necessary for defining fetal risk as related to the maternal serological profile. In the 26 mothers with rubeola infection, the incidence of fetal mortality was 7.7%. Fetal prognosis worsens with the precocity of eruption. In these cases the esantema is the most reliable prognostic element as an indication to perform the invasive procedure. In the 15 patients with cytomegalovirus infection, no fetal or postnatal losses occurred. Morbility occurred in 13.3% of cases, and the two ill fetuses were classified in the same risk group. In this group of patients, the maternal serological profile is a significant predictor of fetal morbility.

196. Fetal ureteral dilatation: evolution and current problems in management

Abstract

The management of 35 dilated ureters in 26 newborns admitted on the basis of a prenatal diagnosis is reviewed. Cases were divided according to the etiology of the ureteral dilatation after complete investigation (intravenous pyelogram, micturating cystogram, Tc-DTPA diuretic renogram). Primary non-obstructive/non-refluxing dilatations were managed conservatively, as were most of those secondary to bladder outlet obstruction after valve fulguration. Most of the cases with primary obstruction, reflux, or a duplicated system were treated with a primary or staged procedure. The authors believe that a certain number of neonatal dilated ureters discovered at prenatal ultrasonography can be successfully treated in a conservative manner after their etiology has been clearly demonstrated by accurate investigation. Attention must be paid in these very young patients, particularly if they are asymptomatic, to avoid overtreatment.

197. A comparative evaluation of spin-on-glass cure by FTIR technique

Abstract

Abstract: Planarization techniques requires a deeper understanding of spin-on-glass (SOG) material behaviour. Besides chemical properties, the cure processes are becoming more and more relevant to achieve a positive impact over microelectronic devices manufacturing flow. In this paper we present a comparative evaluation of different thermal and plasma SOG treatments as detectable by FTIR technique. As far as the vacuum cure is concerned, we found 425‡ C at 300 mTorr a suitable process window. In order to investigate the plasma cure effect on SOG materials we used N2 plasma connected to either a radio frequency or microwave power generator. We found these plasmas not effective in SOG curing. The O2 plasma, in a barrel type reactor connected to a radio frequency generator, resulted in both an effective material shrinkage by an ashing reaction, and C elimination by an oxydation reaction. These results, together with the non cumulative effects of plasma and the vacuum cure, represent the more relevant conclusions of our investigation. Thus, the simple vacuum cure process has been found the most suitable for microelectronics manufacturing applications.

Background. Efficacy of Thalidomide-dexamethasone (TD) as induction therapy in preparation for autologous stem cell transplantation (ASCT) in multiple myeloma provided the basis for the design of the phase II “Bologna 2002”study incorporating TD into double autotransplantation as up-front therapy for younger patients (pts) with newly diagnosed disease. Aim. We performed a molecular study aimed at identifying a gene expression profile (GEP) signature predictive of attainment of at least near complete response (nCR) to TD and subsequent autotransplantation. Methods. For this purpose, we analyzed bone marrow samples obtained at diagnosis from 112 pts who received TD before double ASCT. The differential gene expression of CD138+ enriched plasma cells was evaluated by means of expression microarray using the Affymetrix platform. Abstract

Background: During recent years, interest on Sleep Disordered Breathing (SDB) in pediatric age has increased, due to the impact on quality of life, psycho-physical attitude and other serious morbidities if undiagnosed and untreated. Methods: Italian Pediatric Respiratory Diseases Society (SIMRI) SDB-Working Group carried out an exploratory survey in Italy, from January to December 2016, to assess the diagnostic and therapeutic pathways, perception and relevance of SDB in Italian Hospitals. Results: A questionnaire was sent to 180 Pediatric Units (PUs) distributed throughout the Italy; 102 Pediatric Units (PUs; 56.6%) answered and among them 57% dealt with SDB, and 94% recognized SDB as a major problem. Instrumental tests performed by the PUs were saturimetry (66%), nocturnal polygraphy with complete cardio-respiratory monitoring (46%) and full polysomnography (23%). In addition, hospital pediatricians reported that 54% of parents were unaware of the SDB and 84% did not know their complications. In the Northern Italy, the diagnosis was frequently performed with instrumental tools and the treatment was often surgical. In the Southern Italy the diagnosis was clinical, and the treatment was usually with drugs. Conclusions: The results of our study showed a heterogeneity in the diagnosis and treatment of SDB throughout Italy. Parents know little about SDB and their complications.
