Abstract

Background and Aim Post-polypectomy bleeding (PPB) is the most common complication of endoscopic procedures. To reduce the risk of thromboembolic incidents, Japanese guidelines for gastroenterological endoscopy were revised to indicate that antithrombotic agents were not to be discontinued for endoscopic treatment. However, carrying out endoscopic procedures under antithrombotic medication potentially increases the incidence of hemorrhagic complications. The present study investigated the impact of the revised guidelines on the frequency of complications after colonoscopic procedures. Methods The surveillance period comprised the year before the initiation of the new guidelines (2012), which served as a control period, and 2 years after initiation of the new guidelines (2013 and 2014). During the control period, 3955 cases were examined colonoscopically and 1601 lesions were treated endoscopically. During the 2-year period under the new guidelines, 8749 colonoscopies and 3768 endoscopic treatments were carried out. Changes in treatment methods and rates of complications were compared. Results PPB rate was not significantly different before and after the revision (0.87% vs 1.01%). With the new guidelines, PPB rates in antithrombotic non-users and users were 0.60% and 3.13%, respectively (OR 5.11, P = 0.000). Multivariable analysis showed that the risks for PPB were as follows: heparin bridging therapy (OR 6.34, P = 0.0002); low-dose aspirin (LDA) continuation (OR 5.30, P = 0.0079); and lesion size (OR 1.06, P < 0.0001). Conclusion The present study showed that the overall PPB rate under the new guidelines was not significantly higher when compared with the previous data obtained before the new guidelines were introduced. Abstract

Emery–Dreifuss muscular dystrophy (EDMD), caused by mutations in genes encoding nuclear envelope proteins, is clinically characterized by muscular dystrophy, early joint contracture, and life-threatening cardiac abnormalities. To elucidate the pathophysiological mechanisms underlying striated muscle involvement in EDMD, we previously established a murine model with mutations in Emdand Lmna(Emd−/−/LmnaH222P/H222P; EH), and reported exacerbated skeletal muscle phenotypes and no notable cardiac phenotypes at 12 weeks of age. We predicted that lack of emerin in LmnaH222P/H222Pmice causes an earlier onset and more pronounced cardiac dysfunction at later stages. In this study, cardiac abnormalities of EDMD mice were compared at 18 and 30 weeks of age. Contrary to our expectations, physiological and histological analyses indicated that emerin deficiency causes no prominent differences of cardiac involvement in LmnaH222P/H222Pmice. These results suggest that emerin does not contribute to cardiomyopathy progression in LmnaH222P/H222Pmice.

Abstract

Snowpack samples were collected from a snow pit sequence (6 m in depth) at the Murodo-Daira site near the summit of Mt. Tateyama, central Japan, an outflow region of Asian dusts. The snow samples were analyzed for a homologous series of low molecular weight normal (C1-C10) and branched (iC4-iC6) monocarboxylic acids as well as aromatic (benzoic) and hydroxy (glycolic and lactic) acids, together with major inorganic ions and dissolved organic carbon (DOC). The molecular distributions of organic acids were characterized by a predominance of acetic (range 7.8-76.4 ng/g-snow, av. 34.8 ng/g) or formic acid (2.6-48.1 ng/g, 27.7 ng/g), followed by propionic acid (0.6-5.2 ng/g, 2.8 ng/g). Concentrations of normal organic acids generally decreased with an increase in carbon chain length, although nonanoic acid (C9) showed a maximum in the range of C5-C10. Higher concentrations were found in the snowpack samples containing dust layer. Benzoic acid (0.18-4.1 ng/g, 1.4 ng/g) showed positive correlation with nitrate (r = 0.70), sulfate (0.67), Na+ (0.78), Ca2+ (0.86) and Mg+ (0.75), suggesting that this aromatic acid is involved with anthropogenic sources and Asian dusts. Higher concentrations of Ca2+ and SO4^[2-] were found in the dusty snow samples. We found a weak positive correlation (r = 0.43) between formic acid and Ca2+, suggesting that gaseous formic acid may react with Asian dusts in the atmosphere during long-range transport. However, acetic acid did not show any positive correlations with major inorganic ions. Hydroxyacids (0.03-5.7 ng/g, 1.5 ng/g) were more abundant in the granular and dusty snow. Total monocarboxylic acids (16-130 ng/g, 74 ng/g) were found to account for 1-6% of DOC (270-1500 ng/g, 630 ng/g) in the snow samples.

We describe the synthesis of 4,4′-oxyneolignan, the proposed structure for naturally occurring apteniol D. The diphenyl ether moiety in 4,4′-oxyneolignan was formed via classical Ullmann ether synthesis using excess copper powder inN,N-dimethylacetamide. Abstract

Compound-specific radiocarbon analysis of individual fatty acids was conducted for a semi-urban aerosol sample using a preparative capillary gas chromatography (PCGC) system and an accelerator mass spectrometry (AMS). The aerosol fatty acids (C16 to C34) showed a typical bimodal distribution with two maxima at C16 and C26. Their carbon isotopic compositions ranged from -30.3 ‰ (C26) to -23.3 ‰ (C18), suggesting that they are derived from terrestrial higher plants and marine sources. Radiocarbon measurements of C16-C22 fatty acids showed modern 14C ages, suggesting that the acids are emitted from living higher plants and possibly from marine organisms. On the other hand, C24-C26 fatty acids that are characteristic to terrestrial plant waxes showed 14C ages of 5860 (±200) years. This suggests that some portion of higher molecular weight fatty acids have been stored in geochemical reservoirs such as soils at least for hundreds to thousands of years and then emitted to the atmosphere. These aged fatty acids could be in part transported long distances from the Asian continent through the atmosphere.
