168. Substance use disorders in schizophrenia: Prevalence, etiology, biomarkers, and treatment

Abstract

Substance use disorders (SUDs) are common among individuals with schizophrenia and negatively affect the clinical course of the illness. Despite the high prevalence and significant impact on outcomes, the etiology of SUDs in individuals with schizophrenia remains poorly understood. A better understanding of the etiology, shared mechanisms, temporal evolution, and complex interplay between pathophysiologies will provide the foundation for rational development of targeted therapeutics. We review the literature on the prevalence, etiology, biomarkers, and treatment for individuals with comorbid SUDs and schizophrenia. Substance use is associated with increased risk of subsequent development of schizophrenia. However, the directionality of the relationship between substance use and schizophrenia remains unclear. While several SUDs are associated with increased psychotic symptoms, others are correlated with less severe psychopathology. Individuals with schizophrenia who have more severe symptoms are more likely to have a SUD. There is evidence for shared genetic risk for SUDs and schizophrenia. Comorbid SUDs and schizophrenia share abnormalities in the mesolimbic dopamine system, affecting drug reward, sensitization, and craving. Neuroimaging studies have identified molecular, structural, and functional pathophysiology in co-occurring schizophrenia and SUDs. Neuroinflammation may also represent a shared pathophysiology between SUDs and schizophrenia. While treatments for SUDs exist, they are generally less effective and underutilized for individuals with schizophrenia. Future research is essential to understand the underlying etiology of SUDs in schizophrenia. We propose that neuromodulatory experiments can establish this etiology and identify therapeutic targets. Longitudinal studies across multiple modalities (i.e. neuroimaging, serum, genetics) may help to identify risk factors and biomarkers.

Abstract

Depression in the geriatric population is an important public health concern.  Research has highlighted the prevalence of depression in the geriatric population to range from 28 to 33% (Hu et al., 2022; Zenebe et al., 2021). Late-life depression (LLD) increases the risk of comorbid chronic illness and mortality. Despite the high treatment-resistant rates of antidepressants, there have been few studies which evaluate approaches to treatment resistant depression (TRD) in older adults. Approximately 1/3 of geriatric patients are resistant to treatment (Medeiros da Frota Ribeiro & Riva-Posse, 2017). The use of intravenous ketamine to treat TRD has been shown to be effective in general adult populations, without the focus being specifically on geriatric populations. For example, a recent study by Dai et al. (2022) showed that adults who had repeated intravenous ketamine infusions had an average decrease of 50% on their self-reported depression. The current study examines the response that geriatric patients with TRD have to intravenous ketamine treatment in comparison to non-geriatric patients.

Accurate treatment mandates as detailed history, often with collaboration with the family, and thorough medical evaluation for other co-morbid illnesses.

185. Neuromodulation Treatments for Geriatric Mood and Cognitive Disorders

Abstract

There is increasing evidence for the efficacy of neuromodulation in the treatment of resistant mood disorders and emerging data supporting the use of neuromodulation in cognitive disorders. A significant minority of depressed elders do not respond to pharmacotherapy and/or psychotherapy. This has led clinicians to recommend the increasing use of electroconvulsive therapy (ECT) in the treatment of medication-resistant or life-threatening geriatric depression. Multiple studies have supported the safety and efficacy of ECT in the elderly, yet ECT is associated with side effects including cardiovascular and cognitive side effects. Neuromodulation therapies have the potential for providing effective treatment for treatment-resistant older adults with reduced side effects and this review will outline the risks and benefits of neuromodulation treatment in geriatric psychiatry. There is also emerging evidence of the efficacy of neuromodulation devices in the treatment of cognitive disorders. Pharmacotherapy has been largely ineffective in changing the course of neurodegenerative diseases causing dementia and other treatments are clearly needed. This review will outline the available evidence for neuromodulation in the treatment of mood and cognitive disorders in the elderly.

Abstract

A major medical problem for patients undergoing electroconvulsive therapy (ECT) is the occurrence of postictal agitation (PIA). This phenomenon is associated with confusion and disorientation that can have severe clinical implications for the safety of the patient and health care professionals. Many different pharmacological strategies have been used to prevent PIA. We present data on 40 patients who suffered from PIA after a course of ECT and evaluate the prophylactic use of orally disintegrating olanzapine in the prevention of PIA in subsequent ECT treatments.
