151. Reducing Post-Operative Alveolo-Pleural Fistula by Applying PGA-Sheets (Neoveil) after Major Lung Resection: A Historical Case-Control Study

Abstract: Alveolo-pleural fistula remains a serious post-operative complication in lung cancer patients after surgery, which is associated with prolonged hospital stay and higher healthcare costs. The aim of this study is to evaluate the efficacy of a polyglycol acid (PGA)-sheet known as Neoveil in preventing post-operative air-leak in cases of detected intra-operative air-leak after lung resection. Between 11/2021 and 7/2022, a total of 329 non-small cell lung cancer (NSCLC) patients were surgically treated in two institutions. Major lung resections were performed in 251 cases. Among them, 44 patients with significant intra-operative air-leak at surgery were treated by reinforcing staple lines with Neoveil (study group). On the other hand, a historical group (selected by propensity score matched analysis) consisting of 44 lung cancer patients with significant intra-operative air leak treated by methods other than the application of sealant patches were considered as the control group. The presence of prolonged air-leak (primary endpoint), pleural drainage duration, hospital stay, and post-operative complication rates were evaluated. The results showed that prolonged air-leak (>5 days after surgery) was not observed in study group, while this event occurred in four patients (9.1%) in the control group. Additionally, a substantial reduction (despite not statistically significant) in the chest tube removal was noted in the study group with respect to the control group (3.5 vs. 4.5, p = 0.189). In addition, a significant decrease in hospital stay (4 vs. 6 days, p = 0.045) and a reduction in post-operative complications (2 vs. 10, p = 0.015) were observed in the study group when compared with the control group. Therefore, in cases associated with intra-operative air-leak after major lung resection, Neoveil was considered a safer and more effective aerostatic tool and represents a viable option during surgical procedures.

155. Alpine plant communities differ in their seed germination requirements along a snowmelt gradient in the Caucasus

Abstract: Mesoscale heterogeneity of alpine landscapes generates snowmelt gradients resulting in a distinct vegetation zonation from almost snow-free fellfields to long-lasting snowbeds. Although the vegetative trait variation along such gradients has been intensively studied, little is known about whether and how seed germination is adapted to the variable snowpack duration. Here, we compare the seed germination niches of 18 characteristic plant species occurring in four distinct plant communities (alpine lichen heath—Festuca varia grassland—Geranium-Hedysarum meadow—snowbed) along an alpine snowmelt gradient in the North Caucasus (Russia). In a fully factorial experiment, we tested seed germination responses to temperature (10/2, 14/6, 18/10, 22/14, 26/18 and 30/22 °C) and water potential (0, − 0.2, − 0.4, − 0.6 and − 0.8 MPa) gradients, reflecting the full spectrum of seedling establishment conditions in the study system. Community-specific germination niches were identified by generalised mixed model with Bayesian estimation. Our study revealed that the species from the four focal communities significantly differed in their temperature requirements for germination, whereas soil moisture was found to be a limiting factor for seedling establishment along the entire gradient. The snowbed germination was constrained to comparatively high temperatures above 10 °C, whereas seed germination in alpine lichen heaths occurred under a wide range of experimental conditions. The seed germination patterns of species from Festuca varia grasslands and Geranium-Hedysarum meadows reflected their intermediate position along the snowmelt gradient. We conclude that seed germination niche is affected by the environmental filtering along the snowmelt gradient thus departing from the general alpine germination syndrome.

Abstract: Research in global change ecology relies heavily on global climatic grids derived from estimates of air temperature in open areas at around 2 m above the ground. These climatic grids do not reflect conditions below vegetation canopies and near the ground surface, where critical ecosystem functions occur and most terrestrial species reside. Here, we provide global maps of soil temperature and bioclimatic variables at a 1-km2 resolution for 0–5 and 5–15 cm soil depth. These maps were created by calculating the difference (i.e. offset) between in situ soil temperature measurements, based on time series from over 1200 1-km2 pixels (summarized from 8519 unique temperature sensors) across all the world's major terrestrial biomes, and coarse-grained air temperature estimates from ERA5-Land (an atmospheric reanalysis by the European Centre for Medium-Range Weather Forecasts). We show that mean annual soil temperature differs markedly from the corresponding gridded air temperature, by up to 10°C (mean = 3.0 ± 2.1°C), with substantial variation across biomes and seasons. Over the year, soils in cold and/or dry biomes are substantially warmer (+3.6 ± 2.3°C) than gridded air temperature, whereas soils in warm and humid environments are on average slightly cooler (−0.7 ± 2.3°C). The observed substantial and biome-specific offsets emphasize that the projected impacts of climate and climate change on near-surface biodiversity and ecosystem functioning are inaccurately assessed when air rather than soil temperature is used, especially in cold environments. The global soil-related bioclimatic variables provided here are an important step forward for any application in ecology and related disciplines. Nevertheless, we highlight the need to fill remaining geographic gaps by collecting more in situ measurements of microclimate conditions to further enhance the spatiotemporal resolution of global soil temperature products for ecological applications.

Abstract: Research in global change ecology relies heavily on global climatic grids derived from estimates of air temperature in open areas at around 2 m above the ground. These climatic grids do not reflect conditions below vegetation canopies and near the ground surface, where critical ecosystem functions occur and most terrestrial species reside. Here, we provide global maps of soil temperature and bioclimatic variables at a 1-km2 resolution for 0-5 and 5-15 cm soil depth. These maps were created by calculating the difference (i.e. offset) between in situ soil temperature measurements, based on time series from over 1200 1-km2 pixels (summarized from 8519 unique temperature sensors) across all the world's major terrestrial biomes, and coarse-grained air temperature estimates from ERA5-Land (an atmospheric reanalysis by the European Centre for Medium-Range Weather Forecasts). We show that mean annual soil temperature differs markedly from the corresponding gridded air temperature, by up to 10°C (mean = 3.0 ± 2.1°C), with substantial variation across biomes and seasons. Over the year, soils in cold and/or dry biomes are substantially warmer (+3.6 ± 2.3°C) than gridded air temperature, whereas soils in warm and humid environments are on average slightly cooler (-0.7 ± 2.3°C). The observed substantial and biome-specific offsets emphasize that the projected impacts of climate and climate change on near-surface biodiversity and ecosystem functioning are inaccurately assessed when air rather than soil temperature is used, especially in cold environments. The global soil-related bioclimatic variables provided here are an important step forward for any application in ecology and related disciplines. Nevertheless, we highlight the need to fill remaining geographic gaps by collecting more in situ measurements of microclimate conditions to further enhance the spatiotemporal resolution of global soil temperature products for ecological applications.

Abstract: Resurrecting extinct species is a fascinating and challenging idea for scientists and the general public. Whereas some theoretical progress has been made for animals, the resurrection of extinct plants (de-extinction sensu lato) is a relatively recently discussed topic. In this context, the term ‘de-extinction’ is used sensu lato to refer to the resurrection of ‘extinct in the wild’ species from seeds or tissues preserved in herbaria, as we acknowledge the current impossibility of knowing a priori whether a herbarium seed is alive and can germinate. In plants, this could be achieved by germinating or in vitro tissue-culturing old diaspores such as seeds or spores available in herbarium specimens. This paper reports the first list of plant de-extinction candidates based on the actual availability of seeds in herbarium specimens of globally extinct plants. We reviewed globally extinct seed plants using online resources and additional literature on national red lists, resulting in a list of 361 extinct taxa. We then proposed a method of prioritizing candidates for seed-plant de-extinction from diaspores found in herbarium specimens and complemented this with a phylogenetic approach to identify species that may maximize evolutionarily distinct features. Finally, combining data on seed storage behaviour and longevity, as well as specimen age in the novel ‘best de-extinction candidate’ score (DEXSCO), we identified 556 herbarium specimens belonging to 161 extinct species with available seeds. We expect that this list of de-extinction candidates and the novel approach to rank them will boost research efforts towards the first-ever plant de-extinction.

Abstract: Research in global change ecology relies heavily on global climatic grids derived from estimates of air temperature in open areas at around 2 m above the ground. These climatic grids do not reflect conditions below vegetation canopies and near the ground surface, where critical ecosystem functions occur and most terrestrial species reside. Here, we provide global maps of soil temperature and bioclimatic variables at a 1-km² resolution for 0‐5 and 5‐15 cm soil depth. These maps were created by calculating the difference (i.e. offset) between in situ soil temperature measurements, based on time series from over 1200 1‐km² pixels (summarized from 8519 unique temperature sensors) across all the world’s major terrestrial biomes, and coarse-grained air temperature estimates from ERA5-Land (an atmospheric reanalysis by the European Centre for Medium-Range Weather Forecasts). We show that mean annual soil temperature differs markedly from the corresponding gridded air temperature, by up to 10° degrees C (mean = 3.0 +/‐ 2.1° degrees C), with substantial variation across biomes and seasons. Over the year, soils in cold and/or dry biomes are substantially warmer (+3.6 +/‐2.3° degrees C) than gridded air temperature, whereas soils in warm and humid environments are on average slightly cooler (‐0.7 +/‐ 2.3° degrees C). The observed substantial and biome-specific offsets emphasize that the projected impacts of climate and climate change on near-surface biodiversity and ecosystem functioning are inaccurately assessed when air rather than soil temperature is used, especially in cold environments. The global soil-related bioclimatic variables provided here are an important step forward for any application in ecology and related disciplines. Abstract: Resurrecting extinct species is a fascinating and challenging idea for scientists and the general public. Whereas some theoretical progress has been made for animals, the resurrection of extinct plants (de-extinction sensu lato) is a relatively recently discussed topic. In this context, the term ‘de-extinction’ is used sensu lato to refer to the resurrection of ‘extinct in the wild’ species from seeds or tissues preserved in herbaria, as we acknowledge the current impossibility of knowing a priori whether a herbarium seed is alive and can germinate. In plants, this could be achieved by germinating or in vitro tissue-culturing old diaspores such as seeds or spores available in herbarium specimens. This paper reports the first list of plant de-extinction candidates based on the actual availability of seeds in herbarium specimens of globally extinct plants. We reviewed globally extinct seed plants using online resources and additional literature on national red lists, resulting in a list of 361 extinct taxa. We then proposed a method of prioritizing candidates for seed-plant de-extinction from diaspores found in herbarium specimens and complemented this with a phylogenetic approach to identify species that may maximize evolutionarily distinct features. Finally, combining data on seed storage behaviour and longevity, as well as specimen age in the novel ‘best de-extinction candidate’ score (DEXSCO), we identified 556 herbarium specimens belonging to 161 extinct species with available seeds. We expect that this list of de-extinction candidates and the novel approach to rank them will boost research efforts towards the first-ever plant de-extinction.

161. Observational, Multicenter Study on the Efficacy, Tolerability, and Safety of Nintedanib in Patients with Idiopathic Pulmonary Fibrosis Older than 80 Years

Abstract: Background: Idiopathic pulmonary fibrosis (IPF) primarily affects old patients. Old age is a predictor of mortality. Nintedanib, the only antifibrotic drug approved in Italy for patients aged >80 years, can slow the progression of IPF by reducing the rate of decline in forced vital capacity (FVC) and the risk of exacerbations. Objectives: The primary aim of the study was to compare the decline of FVC after 12 months of nintedanib in patients aged >80 years versus younger patients. Differences related to other functional data, safety, tolerability, hospitalizations, exacerbations, and mortality were evaluated. Methods: An observational, retrospective, multicenter study was carried out in Italy. Results: 159 (122 [76.7%] males) patients were recruited: 106 (66.7%) aged ≤80 years and 53 (33.3%) aged >80 years. FVC decline after 12 months of therapy was not significantly different (−45 mL [−170; 75] vs. −20 mL [−138; 110] mL; p: 0.51). No differences were found for other functional data. Diarrhea was the most frequent adverse event (AE). Rate and type of any AEs, permanent/temporary dose reduction, or drug discontinuation were not significantly different between patients aged ≤80 vs. >80 years. Furthermore, acute exacerbations, hospitalization, and mortality were not significantly different. Conclusions: Nintedanib is effective and safe in patients with IPF aged >80 years, and no significant differences were found when clinical outcomes were compared with those of younger patients. Thus, older age should not be a barrier for the early prescription of antifibrotic treatment in IPF patients.

Abstract: Background: The role of endoscopic ultrasound with bronchoscope fine-needle aspiration (EUS-B-FNA) in the diagnosis of suspected malignant pulmonary lesions adjacent to the esophagus has been poorly investigated. The aim of the present study was to assess the accuracy of EUS-B-FNA for the diagnosis and molecular profiling of paraesophageal pulmonary lesions, as well as its predictors of success. Materials and Methods: Patients who underwent EUS-B-FNA for the diagnosis of paraesophageal lesions were consecutively enrolled in four Italian centers. Demographic, clinical, procedural, pathological, and molecular characteristics of the malignant samples were collected. The primary outcome was the diagnostic accuracy for pulmonary malignancies. Secondary outcomes were diagnostic yield and predictors of success for diagnosis and molecular profiling. Results: 107 adult patients (60 [56.1%] males; median (interquartile range) age: 69 [60–70] years) were enrolled. The diagnostic accuracy of EUS-B-FNA was 95.3% in the overall cohort and 95.2% in the 99 patients with a final diagnosis of malignancy. Neither clinical nor procedural variables significantly affected the diagnostic accuracy, whereas rapid on-site evaluation (ROSE), performed by pathologists or trained pulmonologists, was a strong predictor for a complete molecular profiling (OR [95% CI]: 12.9 [1.2–137.4]; p value: 0.03). Conclusion: EUS-B-FNA is a safe and accurate method for the diagnosis of paraesophageal pulmonary lesions. The presence of ROSE is relevant for a complete molecular profiling in this selected cohort of patients with advanced lung cancer.

Abstract: Research in global change ecology relies heavily on global climatic grids derived from estimates of air temperature in open areas at around 2 m above the ground. These climatic grids do not reflect conditions below vegetation canopies and near the ground surface, where critical ecosystem functions occur and most terrestrial species reside. Here, we provide global maps of soil temperature and bioclimatic variables at a 1-km2 resolution for 0-5 and 5-15 cm soil depth. These maps were created by calculating the difference (i.e. offset) between in situ soil temperature measurements, based on time series from over 1200 1-km2 pixels (summarized from 8519 unique temperature sensors) across all the world's major terrestrial biomes, and coarse-grained air temperature estimates from ERA5-Land (an atmospheric reanalysis by the European Centre for Medium-Range Weather Forecasts). We show that mean annual soil temperature differs markedly from the corresponding gridded air temperature, by up to 10°C (mean = 3.0 ± 2.1°C), with substantial variation across biomes and seasons. Over the year, soils in cold and/or dry biomes are substantially warmer (+3.6 ± 2.3°C) than gridded air temperature, whereas soils in warm and humid environments are on average slightly cooler (-0.7 ± 2.3°C). The observed substantial and biome-specific offsets emphasize that the projected impacts of climate and climate change on near-surface biodiversity and ecosystem functioning are inaccurately assessed when air rather than soil temperature is used, especially in cold environments. The global soil-related bioclimatic variables provided here are an important step forward for any application in ecology and related disciplines. Nevertheless, we highlight the need to fill remaining geographic gaps by collecting more in situ measurements of microclimate conditions to further enhance the spatiotemporal resolution of global soil temperature products for ecological applications.

164. Towards a More Efficient In and Ex Situ Conservation of Sri Lankan Wild Rice Species.

Abstract: Five species of wild Oryza (O. nivara, O. rufipogon, O. eichengeri, O. rhizomatis and O. granulata), including the endemic species O. rhizomatis, have been recorded in Sri Lanka. These species are facing continuous decline in their populations due to natural and anthropogenic processes, with habitat loss being the main threat. This study aimed to provide information on the distribution, the current status of ex situ and in situ conservation, and to identify high-priority species and sites of wild rice in Sri Lanka, in order to improve the effectiveness of conservation efforts. Occurrence records of Sri Lankan wild rice species were collected from literature, gene banks, and field surveys. The distribution of these species was mapped, and areas with high species richness were identified. A gap analysis was conducted to determine the high-priority areas and species for ex situ and in situ conservation. It was found that about 23% of the wild rice populations in Sri Lanka were within protected areas, and by expanding these protected areas by 1 km, an additional 22% of the populations located on the border of these areas could be effectively conserved. Our analysis also revealed that 62% of Sri Lankan wild rice populations were not represented in gene banks. The species-rich areas were found to be in only two districts (Polonnaruwa and Monaragala), and less than 50% of these areas were within protected areas. Based on these findings, O. rhizomatis, O. eichengeri, and O. rufipogon were identified as high-priority species for in situ conservation. Ex situ collections were also deemed necessary for O. granulata and O. rhizomatis to ensure diversity representation in gene banks. [ABSTRACT FROM AUTHOR]

Abstract: The alarming current and predicted species extinction rates have galvanized conservationists in their efforts to avoid future biodiversity losses, but for species extinct in the wild, few options exist. We posed the questions, can these species be restored, and, if so, what role can ex situ plant collections (i.e., botanic gardens, germplasm banks, herbaria) play in the recovery of plant genetic diversity? We reviewed the relevant literature to assess the feasibility of recovering lost plant genetic diversity with using ex situ material and the probability of survival of subsequent translocations. Thirteen attempts to recover species extinct in the wild were found, most of which used material preserved in botanic gardens (12) and seed banks (2). One case of a locally extirpated population was recovered from herbarium material. Eight (60%) of these cases were successful or partially successful translocations of the focal species or population; the other 5 failed or it was too early to determine the outcome. Limiting factors of the use of ex situ source material for the restoration of plant genetic diversity in the wild include the scarcity of source material, low viability and reduced longevity of the material, low genetic variation, lack of evolution (especially for material stored in germplasm banks and herbaria), and socioeconomic factors. However, modern collecting practices present opportunities for plant conservation, such as improved collecting protocols and improved cultivation and storage conditions. Our findings suggest that all types of ex situ collections may contribute effectively to plant species conservation if their use is informed by a thorough understanding of the aforementioned problems. We conclude that the recovery of plant species currently classified as extinct in the wild is not 100% successful, and the possibility of successful reintroduction should not be used to justify insufficient in situ conservation.Colecciones Ex Situ y su Potencial para la Restauración de Plantas Extintas Resumen Las alarmantes tasas de extinción actuales y pronosticadas han incitado a los conservacionistas a esforzarse para evitar las futuras pérdidas de biodiversidad, pero para las especies que ya se encuentran extintas en vida silvestre existen pocas opciones. Nos preguntamos si estas especies pueden ser restauradas, y de ser así, qué papel pueden desempeñar las colecciones ex situ de plantas (es decir, jardines botánicos, bancos de germoplasma, herbarios) en la recuperación de la diversidad genética de las plantas. Revisamos la literatura relevante para evaluar la factibilidad de la recuperación de la diversidad genética perdida y la probabilidad de supervivencia subsecuente de las reubicaciones. Encontramos 13 intentos por recuperar especies extintas en vida silvestre, la mayoría de los cuales usó material preservado en jardines botánicos (12) y en bancos de semillas (2). También hubo un caso de una población eliminada localmente que fue recuperada con material de un herbario. Abstract: Tracheal diverticulum is a rare benign entity. Tracheobronchomegaly (TBM), also known as Mounier-Kuhn syndrome, is a rare disorder characterized by marked dilation of the trachea and main bronchi, associated with thinning or atrophy of the elastic tissue. Because of the weakened trachea and increased intraluminal pressure related to chronic cough, some patients may develop mucosal herniation leading to tracheal diverticulosis. We report the case of a patient with TBM with a huge tracheal diverticulum, diagnosed by bronchoscopy and computed tomography with three-dimensional reconstruction. To our knowledge this is the largest tracheal diameter described in a patient affected by this syndrome.

Abstract: Knowledge of germination and propagation requirements are crucial for the use of plant genetic resources. Here, we compare different treatments to improve the germination of Bunias erucago (Cruciferae), a Mediterranean vegetable herb of growing commercial interest. Its cultivation is limited by the poor germination shown by the seeds which are enclosed in indehiscent silicles. To improve the germination, we investigated four treatments: (1) pre-soaking silicles in water for 48 hours; (2) scarification of silicles with sandpaper; (3) seed extraction; and (4) seed exposure to gibberellic acid (GA3). The same treatments (except the GA3 treatment) were also used to investigate seedling emergence under glasshouse conditions. In the absence of treatments and/or soaking in water, there was very low final germination (< 10%), while scarification and seed extraction increased the germination to ∼40% and > 90%, respectively. Scarification was the most effective treatment in a horticultural context, since the brittleness of seeds makes their extraction too delicate and time-consuming. The seedling emergence results confirmed those of seed germination in the laboratory, underlining the effectiveness of the treatments for plant cultivation. Seed germination performance varied among wild populations, underlining the importance of provenance when using wild plants as new crops.

Abstract: Tuberculosis (TB) is an important clinical and public health issue worldwide. Despite improved treatment success rates following the introduction of antibiotics in daily clinical practice, the expected decline in incidence has been hampered by HIV epidemics and multi- and extensively drug-resistant TB. During the pre-antibiotic era, TB therapies were mainly based on improving hygiene conditions, strengthening the immune system, and targeting the rest of the affected lungs with invasive techniques. Detailed knowledge of old non-pharmacological therapies might support physicians and researchers in the identification of new solutions for difficult-to-treat patients. We performed a narrative literature review on the main old therapeutic options prescribed for patients with TB. The main recommendations and contraindications of sanatorium therapies (i.e., bed rest, fresh air, sunlight) and pulmonary collapse techniques are reviewed, evaluating their physiological basis and their impact on patient outcomes. We report studies describing new interventional pulmonary and surgical techniques and assess new perspectives based on old medical and surgical treatments, whose potential implementation could help complicated patients. Abstract: A 35-year-old woman presented to the emergency department complaining of right chest pain, tachycardia, and low-grade fever. Arterial blood gas analyses revealed hypoxemia and hypocapnia, and blood tests showed only a mild elevation of C-reactive protein (CRP). Chest ultrasonography revealed small right pleural effusion. Computed tomography pulmonary angiography (CTPA) confirmed the right pleural effusion and no significant lung parenchymal involvement. Thromboembolism was ruled out. The patient received oxygen support and empirical therapy, including broad-spectrum antibiotics and systemic glucocorticoids. Blood tests showed positivity for anti-Mycoplasma pneumoniae immunoglobulin M (IgM). The patient experienced remarkable clinical improvement and was discharged with the diagnosis of parapneumonic pleural effusion. One month later, the patient had fever recurrence, accompanied by shivering and chest pain. Chest radiography and ultrasonography showed recurring small pleural effusion on the right side. Blood tests showed a high leukocyte count and elevation of inflammatory indexes. The QuantiFERON tuberculosis (TB) test was positive. The patient was admitted to the infectious disease unit, where gastric aspirate and bronchoscopy with bronchoalveolar lavage were performed. However, Mycobacterium tuberculosis was not found. Ultrasound-guided thoracentesis allowed for obtaining a very small amount (45 mL) of exudative pleural fluid. Polymerase chain reaction (PCR) for detection of the ribosomal ribonucleic acid (rRNA) of Mycobacterium tuberculosis complex was positive. Pleural tuberculosis was then diagnosed, and antituberculosis treatment was started. This chapter addresses tuberculous pleural involvement, indications for diagnostic thoracentesis, and the role of chest ultrasonography in the context of pleural diseases and transthoracic procedures.

Abstract: A 58-year-old man presented with fever, dyspnea, anorexia and weight loss, weakness, and confusion. He was a current smoker (40 pack-year) and had a history of alcohol abuse. At admission, the patient had acute respiratory failure and low blood pressure. Blood tests revealed marked leukocytosis and high inflammation indices. Chest x-ray and ultrasound showed a right loculated pleural effusion. Thoracoscopy was considered dangerous because of the patient’s extreme fragility and hemodynamic instability. Thus a 20-French (Fr) chest tube was inserted, and dull yellow fluid was drained. The physical–chemical examination confirmed the purulent nature of the pleural effusion. The result of the microscopic examination for acid-fast bacilli (AFB) was negative. Aerobic and anaerobic cultures yielded no growth. The cytological analysis of the pleural fluid was negative for malignant tumor cells. Bedside ultrasonography demonstrated incomplete lung expansion and persistence of an extensive fibrin net with multiple pleural locules. Subsequently, the patient received intracavitary treatment with urokinase for 5 consecutive days, and progressive reduction of the loculations and pleural effusion was observed. This chapter discusses complicated pleural effusions, the role of intracavitary fibrinolytics, and the importance of the chemical–physical examination of pleural fluid.

182. Cloudy Pleural Effusion in a Heavy Smoker With Rheumatoid Arthritis

Abstract: A 60-year-old male Caucasian, who was a smoker, was referred to the pulmonology clinic because of dyspnea. He also had bilateral arthralgia of the hands and wrists. The patient had a history of rheumatoid arthritis, which was being treated with immunosuppressive drugs. Chest radiography revealed right-sided pleural effusion; thus ultrasound-guided right thoracentesis was performed, resulting in extraction of 1300 mL of cloudy nonmalodorous pleural fluid. Microbiological and cytological analyses were unremarkable, whereas the chemical–physical examination revealed pseudochylothorax. The triglyceride concentration was low, no chylomicrons were detected, and the cholesterol concentration was high. Subsequent chest computed tomography (CT) demonstrated thickened parietal pleura on the right side with a small area of residual pleural effusion, together with right lower lobe segmental atelectasis. The patient underwent medical thoracoscopy, which showed soft yellow peel over the basal parietal pleura. Multiple biopsy specimens were obtained. Microbiology results for the pleural fluid and parietal pleura were negative. Histological examination of biopsy specimens revealed infiltration by inflammatory cells, multinucleated giant cells, and very few mesothelial cells, in a background of amorphous material. No malignant cells or granulomas were detected. The findings were consistent with rheumatoid pleurisy. The patient was started on a new treatment with a selective Janus kinase inhibitor, which led to progressive reduction of rheumatoid arthritis symptoms. At the follow-up, chest ultrasonography and CT showed significant reduction in pleural effusion and lung atelectasis. This chapter provides a comprehensive description of pseudochylothorax and addresses related topics, such as pleural fluid pH measurement and moderate sedation during medical thoracoscopy.

Abstract: Background: The role of endoscopic ultrasound with bronchoscope fine-needle aspiration (EUS-B-FNA) in the diagnosis of suspected malignant pulmonary lesions adjacent to the esophagus has been poorly investigated. The aim of the present study was to assess the accuracy of EUS-B-FNA for the diagnosis and molecular profiling of paraesophageal pulmonary lesions, as well as its predictors of success. Materials and Methods: Patients who underwent EUS-B-FNA for the diagnosis of paraesophageal lesions were consecutively enrolled in four Italian centers. Demographic, clinical, procedural, pathological, and molecular characteristics of the malignant samples were collected. The primary outcome was the diagnostic accuracy for pulmonary malignancies. Secondary outcomes were diagnostic yield and predictors of success for diagnosis and molecular profiling. Results: 107 adult patients (60 [56.1%] males; median (interquartile range) age: 69 [60–70] years) were enrolled. The diagnostic accuracy of EUS-B-FNA was 95.3% in the overall cohort and 95.2% in the 99 patients with a final diagnosis of malignancy. Neither clinical nor procedural variables significantly affected the diagnostic accuracy, whereas rapid on-site evaluation (ROSE), performed by pathologists or trained pulmonologists, was a strong predictor for a complete molecular profiling (OR [95% CI]: 12.9 [1.2–137.4]; p value: 0.03). Conclusion: EUS-B-FNA is a safe and accurate method for the diagnosis of paraesophageal pulmonary lesions. The presence of ROSE is relevant for a complete molecular profiling in this selected cohort of patients with advanced lung cancer.

Abstract: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic has changed bronchoscopy practices worldwide. Bronchoscopy is a high-risk aerosol-generating procedure with a potential for direct SARS-CoV-2 exposure and hospital-acquired infection. Current guidelines about personal protective equipment and environment considerations represent key competencies to minimize droplets dispersion and reduce the risk of transmission. Different measures should be put in field based on setting, patient's clinical characteristics, urgency and indications of bronchoscopy. The use of this technique in SARS-CoV-2 patients is reported primarily for removal of airway plugs and for obtaining microbiological culture samples. In mechanically ventilated patients with SARS-CoV-2, bronchoscopy is commonly used to manage complications such as hemoptysis, atelectasis or lung collapse when prone positioning, physiotherapy or recruitment maneuvers have failed. Further indications are represented by assistance during percutaneous tracheostomy. Continuous positive airway pressure, non-invasive ventilation support and high flow nasal cannula oxygen are frequently used in patient affected by Coronavirus Disease-2019 (COVID-19): management of patients' airways and ventilation strategies differs from bronchoscopy indications, patient's clinical status and in course or required ventilatory support. Sedation is usually administered by the pulmonologist (performing the bronchoscopy) or by the anesthetist depending on the complexity of the procedure and the level of sedation required. Finally, elective bronchoscopy for diagnostic indications during COVID-19 pandemic should be carried on respecting rigid standards which allow to minimize potential viral transmission, independently from patient's COVID-19 status. This narrative review aims to evaluate the indications, procedural measures and ventilatory strategies of bronchoscopy performed in different settings during COVID-19 pandemic.

199. Effects of Autumn and Spring Heat Waves on Seed Germination of High Mountain Plants.

Abstract: Alpine plants are considered to be particularly vulnerable to climate change and related extreme episodes, such as heat waves. Despite growing interest in the impact of heat waves on alpine plants, knowledge about their effects on regeneration is still fragmentary. Recruitment from seeds will be crucial for the successful migration and survival of these species and will play a key role in their future adaptation to climate change. In this study, we assessed the impacts of heat waves on the seed germination of 53 high mountain plants from the Northern Apennines (Italy). The seeds were exposed to laboratory simulations of three seasonal temperature treatments, derived from real data recorded at a meteorological station near the species growing site, which included two heat wave episodes that occurred both in spring 2003 and in autumn 2011. Moreover, to consider the effect of increasing drought conditions related to heat waves, seed germination was also investigated under four different water potentials. In the absence of heat waves, seed germination mainly occurred in spring, after seeds had experienced autumn and winter seasons. However, heat waves resulted in a significant increase of spring germination in c. 30% of the species and elicited autumn germination in 50%. When heat waves were coupled with drought, seed germination decreased in all species, but did not stop completely. Our results suggest that in the future, heat waves will affect the germination phenology of alpine plants, especially conditionally dormant and strictly cold-adapted chorotypes, by shifting the emergence time from spring to autumn and by increasing the proportion of emerged seedlings. The detrimental effects of heat waves on recruitment success is less likely to be due to the inhibition of seed germination per se, but rather due to seedling survival in seasons, and temperature and water conditions that they are not used to experiencing. Changes in the proportion and timing of emergence suggest that there may be major implications for future plant population size and structure.
