Abstract

The authors report the case of a patient suffering from collagen colitis in whom the administration of omeprazole achieved the rapid and total abolition of the clinical signs and a significant reduction of the collagen band (measurements determined over 10 cryptic spaces using a graduated micrometric ocular microscope). Although the outcome of this disorder is variable, with the possibility of spontaneous recovery or, more frequently, of remission, the authors consider, in their conclusions, that omeprazole may have a beneficial effect in this disorder even though the mechanism for this has not been established.

Abstract

A semi-automated commercial system (ID 32 E, bioMérieux) for 24-hour identification of Enterobacteriaceae and other gram-negative fermentative and nonfermentative bacteria encountered in diagnostic microbiology was evaluated. Overall, the system correctly identified 506 (91.5 %) of the 553 strains tested, 94 (17.0 %) strains requiring additional tests for complete identification. Six (1.1 %) strains were misidentified and 33 (6.0 %) strains were not identified. Eight (1.4 %) strains were not present in the database and were misidentified or not identified. The system is a suitable alternative to existing systems for the identification of Enterobacteriaceae and other gram-negative bacteria frequently encountered in clinical samples. Abstract

ABSTRACTThe epidemiology of methicillin-resistant Staphylococcus aureus(MRSA) infections has changed worldwide. From being strictly nosocomial, MRSA is now frequently found as a community-associated (CA) pathogen. Denmark has been a low-prevalence country for MRSA since the mid-1970s but has in recent years experienced an increasing number of CA-MRSA cases. The aim of this study was to describe the emergence of CA-MRSA infections in Denmark. All Danish MRSA specimens and corresponding clinical data from 1999 to 2006 were investigated. Isolates were analyzed by antibiotic resistance and molecular typing and were assigned to clonal complexes (CC). Clinical data were extracted from discharge summaries and general practitioners' notes, from which assessments of community association were made for all infected cases. CA-MRSA cases constituted 29.4% of all MRSA infections (n= 1,790) and an increasing proportion of the annual numbers of MRSA infections during the study period. CA-MRSA was associated with a young age, skin and soft tissue infections, and non-Danish origin. Transmission between household members was frequently reported. Molecular typing showed >60 circulating clones, where 89.4% of the isolates belonged to five CC (CC80, CC8, CC30, CC5, and CC22), 81.2% carried staphylococcal cassette chromosome mecIV, and 163/244 (69.4%) were positive for Panton-Valentine leukocidin. Clinical and microbiological characteristics indicated that import of MRSA occurs frequently. Resistance to =3 antibiotic classes was observed for 48.8% of the isolates. The emergence of CA-MRSA in Denmark was caused by diverse strains, both well-known and new CA-MRSA strains. The results suggest multiple introductions of MRSA as an important source for CA-MRSA infections in Denmark.

Abstract

Introduction La clairance plasmatique de l’iohexol est une technique reconnue avec différentes procédures pour la mesure du débit de filtration glomérulaire (DFG). La clairance à prélèvement unique présente des avantages évidents dans le contexte africain. Dans cette étude, nous comparons les résultats de DFG obtenus à partir de prélèvements multiples (PM) et ceux obtenus à partir d’un prélèvement unique (PU). Patients/matériels et méthodes L’étude a concerné des patients référés pour une mesure précise du DFG. Pour tous les patients, 5 cm3 d’iohexol (OmnipaqueTM240 ; iohexol, 240 mg/mL, GE Healthcare BVBA, Belgique) ont été injectés. La mesure de l’iohexol plasmatique était réalisée classiquement par HPLC ou spectrométrie de masse. La clairance à PM consiste à calculer la pente de clairance de l’iohexol sur 4 points prélevés à 120, 180, 240 et 300 minutes et à appliquer la correction de Brochner–Mortensen. La clairance par PU a été calculée pour chaque patient à chaque temps par la méthode itérative décrite par Jacobsson. Nous avons étudié leur concordance à ± 10 % en utilisant la PM comme méthode de référence. Résultats Trois cent soixante-quatre adultes africains (RDC et Côte d’Ivoire) ont été inclus dans l’analyse (46 % femmes) : âge 40,0 ± 13,9 ans, poids 68 ± 15 kg/m2, taille 1,67 ± 0,09 m et DFG 96 ± 27 mL/min (valeurs de 4 à 183 mL/min). Seuls les résultats avec une décroissance linéaire (R2 ≥ 0,975) ont été considérés. Dans la population globale, les résultats de DFG avec PU à 120, 180, 240 et 300 minutes ont eu des concordances à ± 10 % respectives de 79, 93, 87 et 71 %. Comme chez le caucasien [1] , les concordances ont été plus faibles chez les sujets (n = 13) avec un DFG < 30 mL/min/1,73 m2. En excluant ces patients, les concordances à 120, 180, 240 et 300 minutes ont été respectivement de 82, 97, 90 et 73 %. Discussion Les résultats obtenus avec une clairance plasmatique d’iohexol à PU montrent une bonne concordance avec ceux obtenus avec PM chez le sujet africain. Comme chez le sujet caucasien, les concordances sont moins bonnes pour les DFG les plus bas. Dans notre population constituée en majorité de sujets sains, le temps à 180 minutes est le meilleur mais pour les patients avec un DFG plus bas, un PU plus tardif serait justifié. Conclusion Dans certaines circonstances notamment les études épidémiologiques, le calcul de la clairance plasmatique de l’iohexol par PU est une alternative acceptable aux PM. La modélisation mathématique proposée par Jacobsson pour le sujet européen paraît applicable à l’africain. [ABSTRACT FROM AUTHOR]

Abstract

This retrospective study was designed to investigate the therapeutic potential of phototherapeutic keratectomy (PTK) for the treatment of map-dot-fingerprint corneal dystrophy (MDFCD).PTK was performed with the Excimed UV 200 and with the Nidek EC5000 on 38 patients (55 eyes). Mean patient age was 51 years (range, 28-86 years). The mean follow-up period was 19 months (range, 8-54 months). The treatment goal for each patient was to improve vision (17 eyes), to heal recurrent corneal erosions (28 eyes) or both (10 eyes).In 13 of 17 eyes (76.47%) with reduction in visual acuity, best corrected visual acuity (BCVA) improved by two lines or more. In 36 eyes of 38 (94.7%) with recurrent corneal erosions, there was no recurrence during the follow-up period. No decreased BCVA was noted. No recurrence of corneal dystrophic changes was seen in the ablation zone at the final follow-up. The mean hyperopic shift caused by tissue ablation was +O.55+/-1.35 after 1 year.Excimer laser PTK is a safe and effective outpatient treatment and should be used as initial treatment for MDFCD.

Abstract

Outbreaks of Clostridium difficile infections (CDI) with increased severity, high relapse rate and significant mortality have been related to the emergence of a new, hypervirulent C. difficile strain in North America and Europe. This emerging strain is referred to as PCR ribotype 027 (Type 027). Since 2005, individual countries have developed surveillance studies about the spread of type 027.C. difficile Type 027 has been reported in 16 European countries. It has been responsible for outbreaks in Belgium, Germany, Finland, France, Ireland, Luxembourg, The Netherlands, Switzerland and the United Kingdom (England, Wales, Northern Ireland and Scotland). It has also been detected in Austria, Denmark, Sweden, Norway, Hungary, Poland and Spain. Three countries experienced imported patients with CDI due to Type 027 who acquired the infection abroad.The antimicrobial resistance pattern is changing, and outbreaks due to clindamycin-resistant ermB positive Type 027 strains have occurred in three European countries. Ongoing epidemiological surveillance of cases of CDI, with periodic characterisation of the strains involved, is required to detect clustering of cases in time and space and to monitor the emergence of new, highly virulent clones.
