“Hospitals” as a name for a journal might appear simply as an umbrella term for healthcare-relevant research. It is certainly an inclusive and ambitious choice of title. We aspire for Hospitals to be a melting pot, and a home for all researchers interested in improving and innovating hospital policies, governance, and services, and empowering patients. From patient safety to hospital governance, fromsurgical simulation to employeemanagement, from autopsies to implementation of artificial intelligence, from hospital virtopsy to assessment of medical devices, and from medical malpractice to risk management, papers on biomedical, medico-legal, economic, and legal aspects of hospital functions will be warmly welcomed. We acknowledge that some may be skeptical of a journal with such a broad scope. We are confident that this effort will be of great value to many fields, and we will work diligently to make Hospitals a meaningful and sound reflection of current issues and innovations in hospital care and management.

The NFV-SDN Doctoral Symposium is committed to fostering collaboration amongst PhD students and experts from all communities researching and working in the areas of Network Function Virtualization (NFV) and Software Defined Networks (SDN). It offers a unique opportunity for PhD students to present their latest research results, discuss new research ideas, and to gather valuable expert feedback on their work from experienced researchers from both industry and academia. Artificial intelligence needs big data to develop reliable predictions. Therefore, storing and processing health data is essential for the new diagnostic and decisional technologies but, at the same time, represents a risk for privacy protection. This scoping review is aimed at underlying the medico-legal and ethical implications of the main artificial intelligence applications to healthcare, also focusing on the issues of the COVID-19 era. Starting from a summary of the United States (US) and European Union (EU) regulatory frameworks, the current medico-legal and ethical challenges are discussed in general terms before focusing on the specific issues regarding informed consent, medical malpractice/cognitive biases, automation and interconnectedness of medical devices, diagnostic algorithms and telemedicine. We aim at underlying that education of physicians on the management of this (new) kind of clinical risks can enhance compliance with regulations and avoid legal risks for the healthcare professionals and institutions.

The titin gene (TTN) is associated with several diseases, including inherited arrhythmias. Most of these diagnoses are attributed to rare TTN variants encoding truncated forms, but missense variants represent a diagnostic challenge for clinical genetics. The proper interpretation of genetic data is critical for translation into the clinical setting. Notably, many TTN variants were classified before 2015, when the American College of Medical Genetics and Genomics (ACMG) published recommendations to accurately classify genetic variants. Our aim was to perform an exhaustive reanalysis of rare missense TTN variants that were classified before 2015, and that have ambiguous roles in inherited arrhythmogenic syndromes. Rare missense TTN variants classified before 2015 were updated following the ACMG recommendations and according to all the currently available data. Our cohort included 193 individuals definitively diagnosed with an inherited arrhythmogenic syndrome before 2015. Our analysis resulted in the reclassification of 36.8% of the missense variants from unknown to benign/likely benign. Of all the remaining variants, currently classified as of unknown significance, 38.3% showed a potential, but not confirmed, deleterious role. Most of these rare missense TTN variants with a suspected deleterious role were identified in patients diagnosed with hypertrophic cardiomyopathy. More than 35% of the rare missense TTN variants previously classified as ambiguous were reclassified as not deleterious, mainly because of improved population frequencies. Despite being inconclusive, almost 40% of the variants showed a potentially deleterious role in inherited arrhythmogenic syndromes. Our results highlight the importance of the periodical reclassification of rare missense TTN variants to improve genetic diagnoses and help increase the accuracy of personalized medicine.

We examined 29 autopsy cases (investigated between October 2020 and February 2021) whose postmortem swabs tested positive for SARS-CoV-2. Twenty-two of 29 cases died while hospitalized (H), while the remaining 7 cases were not hospitalized (NH). Since we included only cases in which the time since death was known (excluding unwitnessed NH deaths), the interval between death and postmortem swab(s) was registered, with a mean NH value of 5.50 days and a mean H value of 3.98 days. The mean age of NH was 65 years, while H were older (mean age: 73 years). Twenty-eight nasopharyngeal and 27 lungs postmortem swabs were obtained and real-time reverse transcriptase‒polymerase chain reaction assay for total and replicative SARS-CoV-2 RNA and mRNA detection was performed. Although the mean death-postmortem swabs interval was higher in NH than in H, the mean viral load of NH was higher than that of H (2.53 × 1011 copies/mL vs 9.31 × 108 copies/mL). In 13/29 cases (6 NH and 7 H), indicators of active replication were found. The relationship between the presence of replicative mRNA and death without hospitalization and that between the minimum cycle threshold value of SARS-CoV-2 RNA and the cycle threshold value of replicative SARS-CoV-2 mRNA were found to be statistically significant (with respective P values of 0.013 and 0.000). Therefore, especially in NH, full compliance with guidelines on biological safety in the autopsy room is essential, and no autopsy can be performed on infected cases in a structure that does not meet the established safety criteria.

Background: Neurosurgical diseases have a devastating impact on society. It is estimated that approximately 14 million essential neurosurgical cases develop worldwide annually, of which more than 80% arise in low- and middle-income countries. Neurosurgical cadaveric dissection remains largely unexploited as a learning tool for the training of surgeons in developing countries, often because of the assumed high costs. Methods: The minimum requirements to establish a neurosurgical cadaver laboratory are the availability of minimally equipped environment to perform dissection, respecting safety requirements, fitting surgical instruments, anatomic samples, and materials to be used for preservation and preparation of anatomical specimens. Moving from these basic foundations, we established our Neurosurgical Dissection Laboratory at Fondazione Policlinico Universitario A. Gemelli IRCCS in Rome, Italy. The laboratory is located at the Institute of Public Health Section of Legal Medicine of University. Results: After reviewing relevant literature and discussing our experience, we provide advice for setting up a neurosurgical dissection cadaver laboratory with specific focus on suitable location identification, surgical equipment procurement, fresh cadaver and frozen specimen acquisition, and preparation and description of a step-up strategy to progressively enrich the laboratory. Conclusions: Our study demonstrates the feasibility of establishing a neurosurgical cadaver dissection laboratory for training and research purposes even in presence of limited resources. The introduction of cost-effective guidelines and targeted funding could represent an added value to target the unmet neurosurgical disease need by promoting development of local neurosurgical expertise with the aim of providing health coverage for the treatment of common neurosurgical pathologies in developing countries.

Since February 2020, the Italian National Healthcare System had to mitigate the possibility of Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) transmission to vulnerable patients. Healthcare professionals rapidly reviewed their workflow to maintain a safe and high standard treatment, but weak scientific evidences and organizational limits resulted in the adoption of heterogeneous measures. Adherence to screening protocols and follow-up programs pregnant women and oncological patients has not been always guaranteed: this scenario could evolve in an enormous number of medico-legal actions. This context, showing the weakness of the Italian law No. 24/2017, imposes an urgent reorganization of the legal framework to homogenize the judgements to “protect” healthcare professionals involved in this epochal emergency.

OBJECTIVE: To develop a deep learning-based decision tree for the primary care setting, to stratify adult patients with confirmed and unconfirmed coronavirus disease 2019 (COVID-19), and to predict the need for hospitalization or home monitoring.PATIENTS AND METHODS: We performed a retrospective cohort study on data from patients admitted to a COVID hospital in Rome, Italy, between 5 March 2020 and 5 June 2020. A confirmed case was defined as a patient with a positive nasopharyngeal RT-PCR test result, while an unconfirmed case had negative results on repeated swabs. Patients' medical history and clinical, laboratory and radiological findings were collected, and the dataset was used to train a predictive model for COVID-19 severity.RESULTS: Data of 198 patients were included in the study. Twenty-eight (14.14%) had mild disease, 62 (31.31%) had moderate disease, 64 (32.32%) had severe disease, and 44 (22.22%) had critical disease. The G(2) value assessed the contribution of each collected value to decision tree building. On this basis, SpO2 (%) with a cut point at 92 was chosen for the optimal first split. Therefore, the decision tree was built using values maximizing G(2) and LogWorth. After the tree was built, the correspondence between inputs and outcomes was validated.CONCLUSIONS: We developed a machine learning-based tool that is easy to understand and apply. It provides good discrimination in stratifying confirmed and unconfirmed COVID-19 patients with different prognoses in every context. Our tool might allow general practitioners visiting patients at home to decide whether the patient needs to be hospitalized.

Insanity defense is sometimes invoked in criminal cases, and its demonstration is usually based on a multifactorial contribution of behavioural, clinical, and neurological elements. Neuro-radiological evidence of structural alterations in cerebral areas that involve decision‐making and moral reasoning is often accepted as a useful tool in these evaluations. On the other hand, the genetic predisposition to anti‐social behavior is still controversial. In this paper, we describe two cases of violent crimes committed by young carriers of genetic variants associated with personality disorder; both the defendants claimed to be insane at the time of the crime. We discuss these cases and review the scientific literature regarding the relationship between legal incapaci-ty/predisposition to criminal behavior and genetic mutations. In conclusion, despite some genetic variants being able to influence several cognitive processes (like moral judgement and impulse control), there is currently no evidence that carriers of these mutations are, per se, incapable of intentionally committing crimes.

Nei processi penali per l’accertamento delle eventuali responsabilità del personale sanitario in caso di morte che colpisce soggetti affetti da SARS-CoV-2 si discute del rilievo da annettere all’esame autoptico al fine di appurare la causa della morte, soprattutto quando l’autopsia non sia stata svolta all’epoca della morte, e il suo svolgimento comporti l’estumulazione della salma. La causa della morte rimane un elemento imprescindibile da appurare se si vuole chiarire l’efficacia salvifica del comportamento alternativo doveroso, e quindi la regola cautelare che l’esercente la professione sanitaria avrebbe dovuto osservare per prevenire il contagio che ha determinato il decesso. L’articolo s’incarica, inoltre, di individuare rimedi diversi dall’autopsia là dove questa non risulti utile o possibile, e sottolinea come il ricorso all’intelligenza artificiale possa aiutare a potenziare l’interpretazione delle immagini radiologiche ante- o postmortem.

Genetic interpretation of rare variants associated with arrhythmogenic cardiomyopathy (ACM) is essential due to their diagnostic implications. New data may relabel previous variant classifications, but how often reanalysis is necessary remains undefined. Five years ago, 39 rare ACM-related variants were identified in patients with features of cardiomyopathy. These variants were classified following the American College of Medical Genetics and Genomics’ guidelines. In the present study, we reevaluated these rare variants including novel available data. All cases carried one rare variant classified as being of ambiguous significance (82.05%) or likely pathogenic (17.95%) in 2016. In our comprehensive reanalysis, the classification of 30.77% of these variants changed, mainly due to updated global frequencies. As in 2016, nowadays most variants were classified as having an uncertain role (64.1%), but the proportion of variants with an uncertain role was significantly decreased (17.95%). The percentage of rare variants classified as potentially del-eterious increased from 17.95% to 23.07%. Moreover, 83.33% of reclassified variants gained cer-tainty. We propose that periodic genetic reanalysis of all rare variants associated with arrhythmo-genic cardiomyopathy should be undertaken at least once every five years. Defining the roles of rare variants may help clinicians obtain a definite diagnosis.
