152. Altered resting-state EEG source functional connectivity in schizophrenia: the effect of illness duration

Despite the increasing body of evidence supporting the hypothesis of schizophrenia as a disconnection syndrome, studies of resting-state EEG Source Functional Connectivity (EEG-SFC) in people affected by schizophrenia are sparse. The aim of the present study was to investigate resting-state EEG-SFC in 77 stable, medicated patients with schizophrenia (SCZ) compared to 78 healthy volunteers (HV). In order to study the effect of illness duration, SCZ were divided in those with a short duration of disease (SDD; n = 25) and those with a long duration of disease (LDD; n = 52). Resting-state EEG recordings in eyes closed condition were analyzed and lagged phase synchronization (LPS) indices were calculated for each ROI pair in the source-space EEG data. In delta and theta bands, SCZ had greater EEG-SFC than HV; a higher theta band connectivity in frontal regions was observed in LDD compared with SDD. In the alpha band, SCZ showed lower frontal EEG-SFC compared with HV whereas no differences were found between LDD and SDD. In the beta1 band, SCZ had greater EEG-SFC compared with HVs and in the beta2 band, LDD presented lower frontal and parieto-temporal EEG-SFC compared with HV. In the gamma band, SDD had greater connectivity values compared with LDD and HV. This study suggests that resting state brain network connectivity is abnormally organized in schizophrenia, with different patterns for the different EEG frequency components and that EEG can be a powerful tool to further elucidate the complexity of such disordered connectivity., Version of Record

153. Use of Vitex agnus-castusin migrainous women with premenstrual syndrome: an open-label clinical observation

- Abstract
Premenstrual syndrome (PMS) affects most women during their reproductive life. Headache is regarded as a typical symptom of PMS and, close to menses, migrainous women could experience their worst migraine attacks. Vitex agnus-castus(VAC) is a phytopharmaceutical compound, considered worldwide to be a valid tool to treat PMS. Aim of this study is to explore if headache is ameliorate in migrainous women treated with VAC for PMS by an open-label clinical observation. Migrainous women with PMS were enrolled in the study and advised to assume a treatment with VAC (40 mg/day) for PMS for a 3-month period. Effects both on PMS and headache were assessed. Out of 107 women, 100 completed the 3-month treatment for PMS. Out of them, 66 women reported a dramatic reduction of PMS symptoms, 26 a mild reduction, and 8 no effect. Concerning migraine, 42 % of patients experienced a reduction higher than 50 % in frequency of monthly attacks, and 57 % of patients experienced a reduction higher than 50 % in monthly days with headache. No patients reported remarkable side effects. 154. Unified modeling of the mammalian and fish proton-dependent oligopeptide transporter PepT1

- Abstract
The partial and complete cycle of the intestinal pH-dependent oligopeptide transporter PepT1 from three species (seabass, zebrafish and rabbit) were studied using an electrophysiological approach and a biophysical analysis, in order to identify similarities and differences. On the whole the presteady state currents of the fish transporters were similar to each other, while presenting some quantitative differences with respect to rabbit PepT1: this last form showed slower decaying currents and the charge vs. potential (Q/V) and time constant vs. potential (τ/V) curves shifted to more positive potentials. All isoforms were similarly affected by external pH, showing acidity-induced slowing of the transients and positive shifts in the Q/V and τ/V curves. Analysis of the pH dependence of the unidirectional rates of the intramembrane charge movement suggested that external protonation of the protein limits the speed of this process in both directions. The complete cycle of the transporter was studied using the neutral dipeptide Gly-Gln. Michaelis-Menten analysis confirmed that in all species the apparent affinity for the substrate is significantly increased by acidity, while the maximal transport current is not strongly affected. Simulations using a kinetic model incorporating the new findings show good agreement with experimental data for all three species both with respect to the presteady-state and transport currents.

155. Seven to Twelve Year Results with Versys ET Cementless Stem. A Retrospective Study of 225 Cases

- Abstract
A retrospective review was conducted to evaluate the mid-term results (8.5 years follow-up) of the Versys ET stem (Zimmer, Warsaw, Indiana, USA). From 1995 to 2000, 225 total hip arthroplasties (THA) were performed using this device. All patients were evaluated clinically by the Harris Hip Score, and radiographically. The Hip Score increased from 54 preoperatively to 97 points postoperatively. The stem displayed a varus alignment between 5° and 10° in 17 cases without any clinical consequences (HHS 96.8). We recorded one case of septic loosening and one case of aseptic loosening. The stability of the stem was radiographically evaluated according to Engh's criteria, confirming bone ongrowth in all other cases.

156. Treatment of multiple wounds of aplasia cutis congenita on the lower limb: a case report

- Abstract
Introduction:Aplasia cutis congenita (ACC) is a rare congenital disease, characterised by absence of skin, that can affect different parts of the body. ACC is more frequent on the scalp, but can involve limbs, with partial-or total-thickness loss of skin, with a functional impairment of the joint affected. There is no clear indication for surgical or conservative treatment, therefore, it would be helpful to find scientific support for the optimal treatment strategies.Clinical presentation:We present a case of a newborn female, with six wounds on the inferior limbs, treated with conservative therapy. To prevent infection and promote healing, the defects were kept moist and covered with non-adherent/antimicrobical dressings. Complete wound healing occurred in eight weeks. The duration of follow-up was three years.Results:In this kind of ACC with superficial partial-thickness wounds of the lower limbs, conservative treatment was successful. The same conservative treatment can be proposed for larger or deeper wounds of lower limbs with no involvement of underlying structures, with delayed scar excision after the complete growth of the patient. ACC scar excision could require complex tissue rearrangement, tissue expansion, or skin grafting.Conclusion:In the evaluation for treatment of ACC of inferior limbs, even in cases of total-thickness skin loss, even on joints, the conservative approach could be considered as the first choice.Declaration of interest:None of the authors has a financial interest in any of the products, devices, or drugs mentioned in this manuscript.

L’obiettivo qualità rappresenta sicuramente uno dei cardini attorno cui ruota la produzione ortofrutticola moderna, che si mostra sempre più incline a produrre secondo elevati standard qualitativi. Alle caratteristiche qualitative “tradizionali” oggi, se ne affiancano altre, legate a nuove esigenze da parte dei consumatori che sono sempre più interessati ad una dieta basata sul consumo di ortofrutta senza residui di pesticidi e con un alto valore nutrizionale. Nell’ambito di un possibile programma di qualificazione delle produzioni ortofrutticole, risulta quindi fondamentale uno studio dei determinanti della qualità secondo un approccio euristico, idoneo ad individuare il contributo dei singoli fattori nel determinare la qualità del prodotto. Le conoscenze genetiche applicate a diverse tecniche di costituzione varietale rappresentano sicuramente la base per la creazione di variabilità utile al miglioramento della qualità senza però limitare la produttività colturale. Sotto il profilo agronomico, le opportunità offerte da una gestione sostenibile dei fattori della produzione, orientata verso il miglioramento di uno o più parametri di qualità, sono senz’altro numerose e riconducibili, ora al miglioramento della interazione pianta-ambiente, ora ad un più mirato input dei mezzi di produzione, ora alla induzione di condizioni di stress di natura diversa. Anche le tecniche del post-raccolta risultano ora sempre più importanti per garantire che la qualità ottenuta in campo sia garantita fino al consumatore. In considerazione di tale articolazione e facendo riferimento ad alcune colture modello si cercherà di delineare un quadro volto ad evidenziare, con riferimento ai principali determinanti, le potenzialità in tema di qualificazione delle corrispondenti produzioni.

158. “Conus” Uncemented Stem in Developmental Hip Dysplasia

- Abstract
The purpose of this study was to evaluate 115 “Conus” uncemented stems at an average follow-up of 6 years (2–9 years). Seventeen patients were lost at follow-up for a final number of 98 stems in 75 patients. The clinical outcome of 71 hips (72%) was graded excellent, 25 (25%) were good, one (2%) fair and one (2%) poor. The average Harris Hip Score increased from 49.35 before operation to 96.30 at the time of the last follow-up.None of the stems implanted required a revision for aseptic or septic loosening. Cortical hypertrophy or radiolucencies were never seen in any of the stems evaluated. Heterotopic ossification was seen in one hip (1%). Modification of the calcar was observed in 29 hips (30%) and they consisted in a slight rounding off of the proximal medial edge of the cut femoral neck.These results encourage the use of this versatile stem which showed a good primary and secondary stability, with a good load distribution along the femur.

160. Radiographic evaluation of the hip rotation centre after total joint replacement in hip dysplasia

- Abstract
A retrospective study was conducted to correlate the clinical result of joint replacement in dysplastic hips with the obtained modifications of the rotation centre.Sixty primary total hip arthroplasties were performed on 46 patients suffering from hip dysplasia at our institution. The radiographic evaluation of the hip rotation centre was performed using the method proposed by Pierchon. The clinical assessment was by Harris Hip Score. Our results revealed that it was possible to modify the position of the pathological rotational centre with a statistically significant difference (p<0.05) for both coordinates. The general clinical evaluation relates well to the radiological evaluation as the average HHS increased to 92.683 (p<0.05). The data from our study are in accordance with the most recent literature and confirm the importance of a suitable biomechanical position of the hip rotation centre in a distal and medial sense.

161. Autologous Chondrocyte Implantation Using a Bilayer Collagen Membrane: A Preliminary Report

- Abstract
Purpose. To present preliminary clinical experience with Matrix-induced autologous chondrocyte implantation, a new tissue-engineering technique for treatment of deep cartilage defects, in which autologous chondrocytes are seeded on a tridimensional scaffold provided by a bilayer type I–III collagen membrane.Methods. From December 1999 to January 2001, 13 patients underwent implantation procedure for deep cartilage defects. Age of patients ranged from 18 to 49 years (mean age, 35 years). The mean defect size was 3.5 cm2(range, 2.0–4.5 cm2). Clinical and functional evaluation were performed using various score systems for the ankle and the knee, and magnetic resonance imaging was performed at 6 and 12 months postoperatively. Membrane structure and cellular population were investigated by light microscopy, scanning electron microscopy, and electrophoresis before implantation.Results. The mean follow-up was 6.5 months (range, 2–15 months). No complications were observed in the postoperative period. The 6 patients with a minimum follow-up of 6 months showed an improvement in clinical and functional status after surgery. Magnetic resonance images showed the presence of hyaline-like cartilage at the site of implantation; there was evidence of chondroblasts and type II collagen inside the seeded membrane.Conclusion. Matrix-induced autologous chondrocyte implantation offers several advantages with respect to the traditional cultured cell procedure. These include technical simplicity, short operating time, minimal invasiveness, and easier access to difficult sites. It appears to be a reliable method for the repair of deep cartilage defects.

163. Tomato fruit quality in relation to water and carbon fluxes

- Abstract
This review is about the involvement of the water and carbon fluxes in the formation of tomato fruit quality. This approach has provided new insights that help in understanding and controlling some of the major variables of fruit quality. Variations in the concentration of dry matter and sugars have been coupled with the conditions of climate and root medium: these concentrations increase under high radiation, high vapour pressure deficit (VPD) or high nutrient concentration. Dry matter and sugar concentrations would not be affected by the availability of assimilates (CO
$_2$ concentration or fruit load). Such increases in dry matter or sugar concentration could be associated with variations in the balance between water and assimilate influx to the fruit i.e. between the fluxes of the phloem and xylem saps and of fruit transpiration. Similarly, the observed relation between the occurrence of blossom-end rot (BER) and conditions of osmotic or water stress can be explained by variations of Ca transport to the fruit by the xylem influx. Fruit cracking could be related to high variations in fruit growth and water influx under changing conditions of temperature or VPD. In contrast, acids and products of the secondary metabolism that are synthesised during the maturation stages could not be linked to the water and carbon fluxes between the plant and the fruit.- Published

164. Preliminary Results of a Survey of the Use of Antimicrobial Agents as Prophylaxis in Orthopedic Surgery

AbstractAn epidemiological survey of the use of antimicrobial prophylaxis in Italian hospitals was carried out under the auspices of the Journal of Chemotherapy. Out of 500 Italian orthopedic centers queried, 225 agreed to participate in this study. A total of 136,321 surgical procedures were reported in the 166 centers reporting complete answers on type of surgery. They comprised hip and knee prosthesis (13.9%), spine surgery (4%), hip endoprosthesis (5.2%), osteosynthesis (26.9%), arthroscopy (24.4%), and others (25.5%). Perioperative antimicrobial prophylaxis was used in 75% of operations (ranging from 57.1% to 99.4% in arthroscopy and joint prosthesis, respectively). Short term (<24 h) antimicrobial prophylaxis was performed in 38.4% of the 206 centers answering this question correctly. 61.1% of centers employed single agent prophylaxis and 70.8% of these prescriptions were betalactam antibiotics. Bacteriological analysis revealed Gram-positive isolates in 73.3% of cases. Methicillin resistance was present in 45% of 915 tested strains. Out of 4221 patients with high risk of infectious complications (joint prosthesis surgery) given antimicrobial prophylaxis in 46 centers, the percentage of surgical wound infections was overall 2.1%, while that of non-surgical wound infections was 3.6%. The total infection rate was decreased by about half in association with long-term (>24 h) as compared to short-term (<24 h) antibiotic treatment (3.7% vs7.6%, respectively), and with use of antibiotic drug combinations vssingle antibiotic drugs (3.9 vs6.6%, respectively). The incidence of surgical-site infection was not decreased by extending the chemoprophylaxis for more than the first 24 h after surgery, while it was reduced from 2.5 to 1.4% by use of combination antibiotic therapy.

165. High vapour pressure deficit influences growth, transpiration and quality of tomato fruits

- Abstract
Plants were grown in two glasshouse compartments under two vapour pressure deficit (vpd) levels: low vpd was obtained by increasing air humidity with a fogging system, and high vpd was obtained during sunny hours in a greenhouse where air humidity was not controlled. The mean value, of the six driest hours of the day concerning the growing period of the fruits considered, was 1.6 kPa under low vpd and 2.2 kPa under high vpd conditions. Over the whole experimental period, the difference in mean hourly temperature never exceeded 0.8°C. The oldest leaves were removed to different extents in different plots to analyse whether a different leaf/fruit ratio could modify the effect of high vpd.

Summary: Traumatic injuries of male external genitalia are rare and not usually life threatening; however, they can have psychological repercussions. The reconstructive management of these lesions is challenging and articulated. We report the case of a 38-year-old farmer suffering from a degloving wound on the external genitalia. The first reconstructive step used to treat the wound was the incorporation of a dermal regeneration template (Integra

168. Radiographic Evaluation of the “Conus” Uncemented Stem

- Abstract
The purpose of this study was to evaluate 54 “Conus” uncemented stems both clinically and radiologically. The radiological evaluation was carried out to verify the stability of this rounded cross-section stem in terms of bone ingrowth and axial subsidence. Clinically, 39 hips (72%) were rated excellent, 13 (24%) were rated good, 2 (4%) were rated fair or poor. The average Harris Hip Score increased from 53 before operation to 93 at the time of the last follow-up (average FU 22.5 months). None was reoperated. Radiologically, 13 stems (24%) were judged to have subsided using a method described by Loudon and Charnley (6). Calcar modification, observed in 22 hips (41%), consisted of a slight rounding off of the proximal medial edge at the cut femoral neck. These results encourage the use of this versatile stem which revealed good primary and secondary stability, with good load distribution along the femur.

Twenty-one primary bone tumours of the hand in children from 8 paediatric hospitals are reported. Osteochondromas and enchondromas were not included. Our material consisted of 16 patients with common tumours (3 Ewing's sarcoma, 5 aneurysmal bone cyst, 6 osteoid osteoma and 2 epidermoid cyst) and 5 patients with uncommon tumours (osteoma, simple bone cyst, haemangiopericytoma, capillary angiomatous tumour and benign ossifying fibroma or osteoblastoma). The X-ray diagnosis of the common tumours should have high concordance with histology, whereas that of uncommon tumours is much more difficult and uncertain. The characteristic features of Ewing's sarcoma are stressed as all our children with this tumour had a delayed diagnosis and a fatal outcome. Differential diagnosis with other short tubular bone lesions of the hand - specifically osteomyelitis - is discussed and the possibilities of microscopic diagnosis are stressed.

D.A., a 46 year old male, was involved in a motor vehicle accident in October, 1986, during which he reported a hyperflexion injury of the cervical spine. The patient immediately began complaining of persistent neck pain, and after several weeks left arm pain and paraesthesia of the 4th and 5th fingers of the left hand appeared, and he consulted a doctor. The first x-rays taken in January 1987 were considered negative, although in retrospect, an initial alteration of the body of C7 was quite evident (Fig. 1). Because of persistent pain, a second set was taken in February 87, which showed a crushed and deformed vertebral body at C7 (Fig. 2). A minerva cast was applied for 4 months, but despite this a definite lower limb spastic paralysis appeared. CT scan, performed on May 87 showed no extrinsic medullary compression, but the body of C7 looked like an empty bony shell interrupted anteriorly and posteriorly. 4 months had elapsed since the crushed vertebra was diagnosed. The patient was admitted to our Clinic in June 1987. Initial physical examination revealed obvious signs of medullary and radicular compression. Myelography was performed and showed complete interruption of the dye column at the inferior border of CI. That night the neurologic picture suddenly worsened and the patient became quadriplegic. An emergency operation was performed with decompressive somatectomy of C7 by an anterior approach, and arthrodesis of C6-T1 using a tricortical cancellous graft taken from the iliac crest [1].

Muscle lesions induced by ischaemia have been investigated in depth from the morphological standpoint, though the metabolic changes thus caused are still the subjects of discussion. Measurements were therefore made of enzyme activities in rat calf muscle, and their changes following incomplete ischaemia. Three enzymes were studied: CPK (typical of muscle metabolism), G-6-PDH (pentose cycle, indicator of the extent of repair processes), and uptake of a labelled amino acid (L-leucine, on the part of tissue proteins. An initial increase in activity and uptake was followed by recovery functions, due to installation of a collateral circulation. This appeared to be complete by the 5th day.

Soft tissue defects of the thumb with exposure of tendons, periosteum, or neurovascular bundles require a complex reconstruction aimed at restoring both skin coverage and the essential function of pinching and manipulation. When large defects are involved, a free tissue transfer is indicated. The proximal ulnar perforator flap represents an interesting solution without the drawbacks of other more widespread free flaps. This report describes the case of a patient with a squamous cell carcinoma of the thumb that required circumferential resection of the soft tissue of the distal phalanx and the interphalangeal joint. The exposed structures were covered with pliable and texture-matching skin harvested from the proximal volar forearm and based on the proximal ulnar perforator. After 12-month follow-up, the patient was satisfied with the aesthetic outcome. Donor-site morbidity was minimal and no functional impairment in daily-life activities was reported. Even though a skilled microsurgical technique is required for the dissection of the perforator, its constant vascular anatomy and the low risk of damage to the main neurovascular bundle should make this flap reliable for the majority of hand surgeons. Summary Background Both tissue expanders and implants are commonly used during breast reconstructions. These devices are the preferred choice of many plastic surgeons around the world during breast reconstruction due to their technical ease of use, low comorbidity and safety. However, several issues such as the integrity of the chest wall during and after tissue expansion remain unclear. Here we present a longitudinal study that shows deformities of the chest wall caused by the use of tissue expanders. Patients and methods A prospective longitudinal study of the chest wall in 36 patients who underwent immediate two-stage expander-to-implant reconstruction from 2010 to 2013 was conducted to evaluate the possible causes of chest wall deformity. Computed tomography (CT) scans of the chest walls were taken before the second-stage reconstruction and after 1 year. Chest wall deformities (graded from I, no deformities, to V, costal fracture) were evaluated with CT scans. Results This study examined 36 breast reconstructions. Chest wall deformities were observed by a CT scan before the second-stage reconstruction. There were eight patients with grade I scores, 14 with grade II, 10 with grade III and four with grade IV. No cases of costal fracture (grade V) were observed. At the 1-year follow-up after the TE/implant change, 22 patients had the same degree of chest wall deformity. Ten patients showed an improvement and four a higher-grade deformity. Conclusion Chest wall deformities commonly occur after tissue expander/implant reconstruction. Abstract: We report the use of the double-pulley technique for arthroscopic fixation of the bony PASTA (partial articular surface tendon avulsion) lesion. Arthroscopic examination documented a 15-mm-long and 8-mm-wide comminuted bony avulsion with 2 main fragments. Two double-loaded suture anchors were placed with a transtendinous technique at the anterior and posterior edges of the lesion respecting the tendon insertion to the avulsed fragment. The medial sutures were retrieved through the intact supraspinatus tendon medially to the fracture. The sutures were initially coupled in a double-pulley configuration generating 2 sutures oriented from anterior to posterior; then a simple suture for each anchor oriented from medial to lateral was obtained. At the end of the procedure, the adequacy of reduction and stability of the fragments were confirmed. At 2 months from surgery, radiographic healing of the fracture was noted and integrity of the supraspinatus tendon insertion to the footprint was confirmed by arthro–magnetic resonance imaging, with full recovery of daily activities and complete active range of motion confirmed at 6 and 12 months. The double-pulley technique allows optimal reduction of bony fragments and reconstruction of normal footprint anatomy even in comminuted fractures. Moreover, it creates a waterproof reduction of the fragments, protecting the fracture site from synovial fluid. [Copyright &y& Elsevier]

Objective: The objective of the study was to compare scar quality associated with different types of wound closure methods after cesarean section (CS). Study Design: Patients were randomized to have skin closure following CS with either staples or 3 different types of subcuticular sutures. Scar quality was evaluated 2 and 6 months postoperatively. The Vancouver Scar Scale, the Patient and Observer Scar Assessment Scale (POSAS), and a visual analog scale were used as scar assessment tools. Results: Of the 180 patients who were recruited, 123 successfully completed the study. No difference in both subjective and objective scar rating was detected across groups at either 2 months or 6 months. In the overall study population, objective scores correlated with patient rating, and correlation was strongest between the observer and patient components of the POSAS (r = 0.48). Conclusion: In women undergoing CS, stapled wounds and those closed with subcuticular sutures result in equivalent cosmetic appearance of the scar. [Copyright &y& Elsevier]

This study investigated at medium term follow-up the clinical outcomes and any progression of knee osteoarthritis in a population of patients that underwent arthroscopic placement of a collagen meniscus implant. Thirty-four patients underwent arthroscopic placement of a collagen meniscus implant for a symptomatic deficiency of medial meniscal tissue. Follow-up evaluation included Lysholm II score and Tegner activity scores and MR arthrography of the knee at 2 and 5 years after surgery. Plain radiographs were also obtained at 5 years. Six patients were excluded. In eight cases arthroscopic second look evaluation was performed. Lysholm and Tegner activity scores at 2 and 5 years after surgery improved significantly compared to the preoperative score. These patients showed good to excellent clinical results after 5 years from a CMI placement. The chondral surfaces of the medial compartment had not degenerated further since placement of the CMI. MR signal had continued to mature between 2 and 5 years after implant, progressively decreasing signal intensity but in any case comparable to the low signal of a normal meniscus. In most of cases the CMI-new tissue complex had a slight reduction in size, compared to a normal medial meniscus, but the new tissue had no apparent negative effects.

Background: Trabecular Metal (TM) is a new highly porous material made of tantalum (Zimmer, Warsaw, Indiana, USA). Its three-dimensional structure is composed of a series of interconnected dodecahedron pores that are on average 550 μm in diameter. This size is considered optimal for bone ingrowth and is similar to trabecular bone. The elastic modulus of TM (3 GPa) is more similar to that of cancellous (0,1-1,5 GPa) or cortical (112-18 GPa) bone and is significantly less similar to that of Titanium (110 GPa) and Co-Cr alloys (220 GPa). These features enable bone apposition and remodeling. The purpose of the present study was to evaluate the histology of the bone-implant interface in a human specimen. Materials and Methods: A highly porous tantalum cup (Zimmer, Warsaw, Indiana, USA) was removed for recurrent dislocations three years after implantation. In order to obtain a slice of the cup, two cuts were made on the centre using an Exakt cutting machine. Then the slice was embedded in a Technovit resin and a Hematoxylin-eosin stain was used to study the bone tissue. Bone ingrowth was calculated using a method based on simple calculations of planar geometry. Results: The histological evaluation of the periprosthetic tissues revealed a typical chronic inflammation with few particles of polyethylene that were birefringent using polarized light. The quantitative evaluation of bone ingrowth revealed that more than 95% of voids were filled with bone. Discussion: In the literature, a lot of studies focused on tantalum were carried on animal model. Up to now little information is available about the histology of the bone-tantalum interface in a human artificial joint. We had an opportunity to remove a well integrated cup hence this study. The histology confirmed the strong relationship between the structure of this material and bone. The morphometric analysis revealed a high percentage of bone ingrowth.

Introduction Acetabular aseptic loosening is the main cause of failure in prosthetic hip surgery and the first cause of revision. Uncemented hemispherical cups are widely used and different biomaterials have been developed to obtain primary and secondary stability through osteointegration. Porous tantalum’s structure is similar to cancellous bone and presents with mechanical and biological characteristics aimed to enable rapid and complete osteointegration and improve the survival of the prosthesis. Materials and methods From May 2004 to June 2007 one hundred TMT modular acetabular components were implanted in 97 patients for primary THA. For this study we assessed 89 prostheses in 86 patients with 56 months (42–77 months) follow-up, both clinically and radiographically. Results At the last follow-up no implant appeared mobilized. Harris Hip Score changed from 52.75 points preoperatively to 95.72 postoperatively (p\0.0001). Oxford Hip Score improved from 37.16 to 14.78 points (p\0.0001). Gaps at the bone-prosthesis interface were documented at the immediate postoperative X-rays in 51 implants (57%), mostly in zone II. At the last radiographic assessment they appeared completely filled in 87% of the cases (mean time to fill 1 year), partially filled in 10%, leaving less than 1 mm gap, while in 2 cases they remain unchanged. Osteointegration signs according to Moore and McAuley were also considered: at least three signs were present in all cases indicating complete osteointegration of the implants. There was no evidence of radiolucent lines at last follow-up. One dislocation occurred in one case at 21 months and it was reduced in narcosis with no recurrence. Discussion Medium-term results of this cup are comparable with those reported in literature for porous tantalum monoblock components. The presence of the titanium layer in the metal-back of the modular component, that theoretically could affect the biomechanical characteristics of the cup, does not seem to affect host bone biological response when compared to the monoblock socket. Thanks to its mechanical characteristics and excellent osteoconduction, this biomaterial provides a good primary stability and excellent osteointegration of the component. Conclusions The results reported suggest that this cup represents at a medium-term follow-up a good implant for acetabular arthroplasty.

Background The Oxford Shoulder Score (OSS) is an English-language questionnaire specifically designed to evaluate patients affected by shoulder pain. Although this scoring system has been translated into other languages, an Italian version of it is still not available. The aim of the present study was to translate, culturally adapt, and validate the Italian version of the OSS. Materials and methods We recruited 140 patients with shoulder pain caused by degenerative or inflammatory state or disorder of the shoulder. Patients completed the following questionnaires: Italian OSS, University of California, Los Angeles (UCLA) Shoulder Rating Scale, Constant-Murley shoulder assessment, and the Medical Outcome Study Short-Form 36 Health Survey (MOS SF-36). Internal consistency was tested using Cronbach coefficient α. Reproducibility was assessed by asking 110 patients to complete another OSS 48 hours after the first. Correlation between the total results of both tests was determined by the Pearson correlation coefficient. Validity was assessed by calculating the Pearson correlation coefficient between the OSS and the UCLA, Constant-Murley, and SF-36 assessments. Results Cronbach α was 0.95. The Pearson correlation coefficient was r =0.97. With respect to validity, there was a significant correlation between the Italian OSS and the individual scores of UCLA, Constant-Murley, and SF-36. Discussion Psychometric properties of the Italian OSS compared well with those reported for the English OSS. As demonstrated by the high values of Cronbach α and Pearson correlation coefficients, in accordance with the English version of the OSS, the Italian version proved to be a reliable, valid, and reproducible measure of shoulder pain perception in Italian-speaking patients.

Reconstruction of the failed acetabular component in total hip arthroplasty (THA) can be challenging. Although there are multiple reconstructive options available, a cementless acetabular component inserted with screws has been shown to have good intermediate-term results and is the reconstructive method of choice for the majority of acetabular revisions This reconstruction is feasible provided at least 50\% of the implant contacts host bone. When such contact is not possible, and there is adequate medial and peripheral bone, techniques using alternative uncemented implants can be used for acetabular reconstruction. An uncemented cup can be placed at a "high hip center." Alternatively, the acetabular cavity can be progressively reamed to accommodate extra-large cups. Oblong cups, which take advantage of the oval-shaped cavity resulting from many failed acetabular components, can also be used. The success of these cementless techniques depends on the degree and location of bone loss. The correct indication to revision and the choice of the correct implant is the keystone for the success of this type of surgery and follows an accurate preoperative planning in order to understand the specific pathologic scenario. The aim of this paper is to review some technical options for the revision of the acetabular component also taking into account our personal experiences and series.

A retrospective review was conducted to evaluate the mid-term results (8.5 years follow-up) of the Versys ET stem (Zimmer, Warsaw, Indiana, USA). From 1995 to 2000, 225 total hip arthroplasties (THA) were performed using this device. All patients were evaluated clinically by the Harris Hip Score, and radiographically. The Hip Score increased from 54 preoperatively to 97 points postoperatively. The stem displayed a varus alignment between 5° and 10° in 17 cases without any clinical consequences (HHS 96.8). We recorded one case of septic loosening and one case of aseptic loosening. The stability of the stem was radiographically evaluated according to Engh's criteria, confirming bone ongrowth in all other cases.

200. Fracture of a polyethylene post in a 9-year-old posterior-stabilized knee prosthesis: light microscopy and SEM evaluation

- Abstract
Objective Aim of this study is to evaluate the surface of the TKA polyethylene liner, harvested after the breakage of the post nine years after the implant in a 63 years old female (BMI 39) after an hyperextension trauma. Methods During the revision we harvested sample of the periprosthetic tissue which was prepared for the light microscopy evaluation. The samples were stained using both haematoxylineosin and Von Kossa. The PE liner was prepared for the Scanning Electron Microscopy. Results The SEM evaluation revealed two different damage patterns considering the medial part and the lateral aspect of the sample. The medial part presented a fracture line laminated in front and smooth behind and with the tear lines with a medio-lateral and anterior posterior orientation. The lateral part presented a sharp fracture line that ends anteriorly with a laminated tear paralleled to the anterior edge of the polyethylene insert, and which implies that this area could be the terminal failure area of the fractured post. The medial part of the fracture edge appears to be smooth and with a different orientation of the fracture lines. Conclusions These features could be explained with a ‘‘two stage’’ rupture of the polyethylene post. This could have been caused by a non-optimal ligamentous balance that weakened the post, which was finally broken by a postero-anterior stress.
