Abstract

BACKGROUND AND OBJECTIVES: A collaborative study, involving 26 laboratories from 14 countries, was carried out in order to establish a World Health Organization (WHO) International Standard for human parvovirus B19 (B19) DNA nucleic acid amplification techniques (NAT). MATERIALS AND METHODS: Four samples: AA, BB (which were lyophilized), CC and DD (which were liquid preparations) were analysed using several different NAT assays. The mean B19 DNA content of each sample was determined for each laboratory using an end-point dilution method. RESULTS: There was good agreement between the overall mean 'equivalents'/ml obtained by the different assays. The mean log(10) 'equivalents'/ml were 5.76 for sample AA, 5.73 for sample BB, 5.82 for sample CC and 7.70 for sample DD. The differences in titre among samples AA, BB and CC were not statistically significant, but the titre of DD was significantly higher. CONCLUSIONS: Despite the range of NAT assays used in the study, it was possible to calculate the mean B19 DNA concentrations in the four preparations. Lyophilized preparation AA was established as the first International Standard for B19 DNA NAT assays and was assigned a concentration of 10(6) international units (IU)/ml.

Abstract

Chronic laxity of the subtalar joint derives from the functional significance of the joint with the talo-calcaneal interosseous ligament as the articular "pivot." The laxity of the subtalar joint is usually associated with traumatic or degenerative pathology of the interosseus ligament. The characteristic pain is present along the sinus tarsi and at times also more proximally along the posterior subtalar joint. Radiographic examination is not significant; however, with arthrography, the normal synovial recess of the posterior subtalar joint corresponding to the sinus tarsi is not visualized. With magnetic resonance imaging it has been possible to document structural alterations of ligament. Reconstruction of the interosseus talo-calcaneal ligament has a reasonable expectation of success. The author presents the results of this treatment in 47 operations (44 patients, 3 bilateral) between January 1982 and June 1992.

Abstract

One hundred and ten consecutive patients who had come to the Urogynaecological outpatients surgery of Desio Hospital with stress urinary incontinence were studied and treated with uro-rehabilitation at the Neuro-rehabilitation Division of Seregno Hospital between 1998 and 2000. The study only included the patients studied and treated in that three year period of research who suffered from stress urinary incontinence (genuine or mixed) with a 0/1 degree cystocele according to the Baden-Walker HWS classification with type I/II incontinence according to Blassas, with no suspect intrinsic clinical instrumental urethral defect and of whom all the subjective and objective parameters of evaluation of the gravity of the IUS adopted were available (micturition diary, VAS, Pad test according to the ICS, stress test according to Ferrari, I-S score, PC test) before and after treatment. All the patients voluntarily accepted to follow the combined therapeutic rehabilitation protocol at the Outpatients Surgery: One cycle of physiochinesitherapy associated to a three weekly cycle as outpatients of electro myographic biofeedback and FES with vaginal probe. The comparison between the initial and final numerical values studied after treatment show how the degree of the disorder, subjective and/or objective of the patients had significantly improved: the mean value of VAS passed from 6.8+/-2.1 to 3.5+/-2.6; the values of the PAD and stress tests went from 9.8±16.8 2.8±1.3 to 5.9±13.7 and 1.8±1.6 respectively an inverse trend being shown by the PC-test which rise from 1.1±1 to 2.2±0.9. In particular, 55% of the patients during their first visit mentioned a VAS equal to or above 7; while after the rehabilitation treatment in 51% of the population a VAS equal to or less than 3 was registered. At the last follow-up 35.5% remained unvaried, 25.4% improved and 39.1% completely recovered. Abstract

Following the recently established 'Binary-driven HyperNova' (BdHN) model, we here interpret GRB 970828 in terms of the four episodes typical of such a model. The 'Episode 1,' up to 40 s after the trigger time t, with a time varying thermal emission and a total energy of E = 2.60 × 10 erg, is interpreted as due to the onset of an hyper-critical accretion process onto a companion neutron star, triggered by the companion star, an FeCO core undergoing a SN explosion. The 'Episode 2,' observed up t + 90 s, is interpreted as a canonical gamma ray burst, with an energy of $$E_{tot}^{e^ + e^ - } = 1.60 \times 10^{53}$$ erg, a baryon load of B = 7× 10 and a bulk Lorentz factor at transparency of G = 142.5. From this Episode 2, we infer that the GRB exploded in an environment with a large average particle density 〈n〉 ≈103 particles/cm and dense clouds characterized by typical dimensions of (4-8) × 10 cm and δn/n ~ 10. The 'Episode 3' is identified from t +90 s all the way up to 10 s: despite the paucity of the early X-ray data, typical in the BATSE, pre-Swift era, we find extremely significant data points in the late X-ray afterglow emission of GRB 970828, which corresponds to the ones observed in all BdHNe sources. The 'Episode 4,' related to the Supernova emission, does not appear to be observable in this source, due to the presence of darkening from the large density of the GRB environment, also inferred from the analysis of the Episode 2. Abstract

Binary systems constituted by a neutron star and a massive star are not rare in the universe. The Induced Gravitational Gamma-ray Burst (IGC) paradigm interprets Gamma-ray bursts as the outcome of a neutron star that collapses into a black hole due to the accretion of the ejecta coming from its companion massive star that underwent a supernova event. GRB 130427A is one of the most luminous GRBs ever observed, of which isotropic energy exceeds 10 erg. And it is within one of the few GRBs obtained optical, X-ray and GeV spectra simultaneously for hundreds of seconds, which provides an unique opportunity so far to understand the multi-wavelength observation within the IGC paradigm, our data analysis found low Lorentz factor blackbody emission in the Episode 3 and its X-ray light curve overlaps typical IGC Golden Sample, which comply to the IGC mechanisms. We consider these findings as clues of GRB 130427A belonging to the IGC GRBs. We predicted on GCN the emergence of a supernova on May 2, 2013, which was later successfully detected on May 13, 2013.
