Lower limb regional anaesthesia is complex as it is a field where the research is growing at a rapid rate producing new blocks, new approaches to established blocks and new data is emerging regarding old techniques. This article explores the choice of general anaesthesia (GA) versus neuraxial anaesthesia (NA). It also explores the impact of peripheral nerve blocks on lower limb anaesthesia. We also then look at newer techniques of regional anaesthesia options for the lower limb. For hip surgery, newer options include: PEricapsular Nerve Group (PENG) block, the Anterior Block of Capsule (ABC) block, the Supra Inguinal Fascia Iliaca (SIFI) block and the Quadratus Lumborum (QL) block. For knee surgery options include: adductor canal block, Nerve to Vastus Medialis (NVM) block, genicular nerve blocks and the Infiltration between Popliteal Artery and Capsule of the Knee (IPACK) block. Current international optical science research focuses on the non-visual effects of lighting on human cognition, mood, and biological rhythms to enhance overall well-being. Nocturnal roadway lighting, in particular, has a substantial impact on drivers’ physiological and psychological states, influencing behavior and safety. This study investigates the non-visual effects of correlated color temperature (CCT: 3000K vs. 4000K vs. 5000K) and illuminance levels (20 lx vs. 30 lx) of urban motor vehicle road lighting on driver alertness during various driving tasks. Conducted between 19:00 and 20:30, the experiments utilized a human-vehicle-light simulation platform. EEG (β waves), reaction time, and subjective evaluations using the Karolinska Sleepiness Scale (KSS) were measured. The results indicated that the interaction between CCT and illuminance, as well as between CCT and task type, significantly influenced driver alertness. However, no significant effect of CCT and illuminance on reaction time was observed. The findings suggest that higher illuminance (30 lx) combined with medium CCT (4000K) effectively reduces reaction time. This investigation enriches related research, provides valuable reference for future studies, and enhances understanding of the mechanisms of lighting’s influence on driver alertness. Moreover, the findings have significant implications for optimizing the design of urban road lighting.

The Poverty Alleviation Relocation (PAR) policy is widely regarded as an effective approach for breaking the cycle of ecological vulnerability and poverty. However, quantitative research on the ecological restoration effectiveness of PAR lacks sufficient experimental data support. This study focuses on the karst region and employs analysis methods such as volume-derived biomass and correlation analysis to evaluate the impact of PAR on carbon storage in forest ecosystems using on-site experimental data. The objective is to enhance and broaden the research framework for assessing PAR’s ecological restoration effectiveness. The findings reveal that, compared to the pre-PAR implementation period in 2015, the study area experienced an 8.16% increase in forest land area and a 6.57% increase in carbon storage after six years of PAR implementation in 2021. Following PAR implementation, carbon storage in the stone desertification area surged by 14.31%, indicating a significant correlation between PAR households and carbon storage variables. In the karst area, carbon storage increased by 4.34%, exhibiting a significant correlation between the two variables. Conversely, in the non-karst area, carbon storage rose by 5.01%, but no significant correlation was observed between the variables. Furthermore, post-PAR implementation, there is a discernible trend of stronger carbon storage enhancement with increasing distance from the relocated PAR households.

The paper is an extension to earlier PIV work published in Progress in Aerospace Science Vol. 27, pp. 237-265. 1990. DPIV (Digital Particle Image Velocimetry) measurements have been made at transonic speeds using a new method of both digitally capturing and visualising high speed flow-fields. It has provided a quantitative whole field image of the transonic flow. The area of interest for this test was of the wing/pylon/nacelle/inboard gully region of a 5.7% scale model wing and engine combination. The measurements show velocity and flow angle changes in this region, to a measurement accuracy of 4%, made at an optical stand off distance of 0.5 m.

Abstract Objective A rapid and accurate forecast for the early prognosis of ICH patients is challenging. This study investigated whether heart rate variability (HRV) and skin sympathetic nerve activity (SKNA) could prognosticate poor neurological outcomes in ICH patients. Methods Between November 2020 and November 2021, we studied 92 spontaneous ICH patients in the First Affiliated Hospital of Nanjing Medical University. Glasgow Outcome Scale (GOS) score at 2 weeks after the ICH was used to categorize patients into good and poor outcome groups. The modified Rankin Scale (mRS) assessed patients' ability to live independently for 1 year. We utilized a portable high‐frequency electrocardiogram (ECG) recording system to record the HRV and SKNA information in ICH patients and control participants. Results 77 patients were eligible for the prediction of neurological outcome and were allocated into the good (n = 22) or poor (n = 55) outcome groups based on the GOS grade. In univariate logistic regression analysis, significant variables that could differentiate the outcomes were age, hypertension, tracheal intubation, Glasgow Coma Scale (GCS) score, existing intraventricular hemorrhage, white blood cells, neutrophil, lnVLF, lnTP, and aSKNA. Variables in the best fit multivariable logistic regression model were age, hypertension, GCS score, neutrophils, and aSKNA. The GCS score was the only independent risk factor for poor outcomes. At 30 days and 1 year of follow‐up, patients with lower aSKNA had poor outcomes. Interpretation ICH patients had reduced aSKNA, which could be a prognostic indicator. A lower aSKNA suggested a worse prognosis. The present data indicate that ECG signals could be helpful for prognosticating ICH patients.

Abstract. Background:. Patellofemoral joint (PFJ) degeneration has traditionally been regarded as a contraindication to unicompartmental knee arthroplasty (UKA). More recently, some researchers have proposed that PFJ degeneration can be ignored in medial UKA, and others have proposed that this change should be reviewed in PFJ degenerative facets and severity. This study aimed to systematically evaluate the effect of PFJ degeneration on patient-reported outcome measures (PROMs) and revision rates after medial UKA. Methods:. Electronic databases (PubMed, Embase, Web of Science, etc.) were searched for studies assessing the influence of PFJ degeneration on medial UKA. A random-effects meta-analysis was conducted for the Oxford knee score (OKS), Knee society score (KSS), and revision rates and stratified by PFJ degenerative facets (medial/lateral/trochlear/unspecified), severe PFJ degeneration (bone exposed), and bearing type (mobile/fixed). Heterogeneity was assessed by the Cochran Q test statistic and chi-squared tests with the I-squared statistic. Results:. A total of 34 articles with 7007 knees (2267 with PFJ degeneration) were included (5762 mobile-bearing and 1145 fixed-bearing and 100 unspecified). Abstract The systemic RNAi-defective (SID) transmembrane family member 2 (SIDT2) is a putative nucleic acid channel or transporter that plays essential roles in nucleic acid transport and lipid metabolism. Here, we report the cryo-electron microscopy (EM) structures of human SIDT2, which forms a tightly packed dimer with extensive interactions mediated by two previously uncharacterized extracellular/luminal β-strand-rich domains and the unique transmembrane domain (TMD). The TMD of each SIDT2 protomer contains eleven transmembrane helices (TMs), and no discernible nucleic acid conduction pathway has been identified within the TMD, suggesting that it may act as a transporter. Intriguingly, TM3-6 and TM9-11 form a large cavity with a putative catalytic zinc atom coordinated by three conserved histidine residues and one aspartate residue lying approximately 6 Å from the extracellular/luminal surface of the membrane. Notably, SIDT2 can hydrolyze C18 ceramide into sphingosine and fatty acid with a slow rate. The information presented advances the understanding of the structure-function relationships in the SID1 family proteins.

Abstract State governance in corporations controls the economy and employment to develop prosperity in China. Does privatization achieve both objectives? Privatization theory views that giving up state control benefits firms economically but is quite silent on the value of employment benefits. However, market mix theory (Sappington and Stiglitz, J Policy Anal Manag 6(4):567–582, 1987; Stiglitz, Whither Socialism? MIT Press, Cambridge, MA, and London, England, 1994) views that state control of firms to provide employment is valuable. It further implies that corporate performance and employment objectives can co-exist well together. We formalize Market Mix Theory to reveal its testable implications on dual objectives and performance. We test the notion that Chinese corporations pursue two objectives: maximizing economic value and employment, unlike the singular objective of value maximization in Western firms. As well, we test the benefit of partial privatization. First, we test the market mix theory by studying the relationship between state ownership and employment and financial performance as a combined objective. We show that mixed firms have the highest performance in wealth maximization and employment compared to private and state-controlled firms. Second, we examine Chinese SOEs’ Employment with a large sample of 2536 public firms using panel regressions. We find that state ownership and firm employment have a positive relationship. State ownership is positively related to employment stability. Moreover, performance has a negative relationship with state ownership–employment. This result supports our over-employment hypothesis, meaning that economic performance is sacrificed to provide employment. Lastly, we show executive pay for SOE managers is positively related to employment. The employment objective is real in determining state control in Chinese corporations without conflicting to maximize shareholder wealth. Rather, the mixed form of ownership is optimal and vitally contributes overall to the stability and prosperity of China. Our formalization of market mix theory better reflects the reality of motivation, performance and benefits of partial privatization as exemplified in Chinese state-owned enterprises.

Abstract Infection susceptibility, poor vaccination efficacy, age-related disease onset, and neoplasms are linked to innate and adaptive immune dysfunction that accompanies aging (known as immunosenescence). During aging, organisms tend to develop a characteristic inflammatory state that expresses high levels of pro-inflammatory markers, termed inflammaging. This chronic inflammation is a typical phenomenon linked to immunosenescence and it is considered the major risk factor for age-related diseases. Thymic involution, naïve/memory cell ratio imbalance, dysregulated metabolism, and epigenetic alterations are striking features of immunosenescence. Disturbed T-cell pools and chronic antigen stimulation mediate premature senescence of immune cells, and senescent immune cells develop a proinflammatory senescence-associated secretory phenotype that exacerbates inflammaging. Although the underlying molecular mechanisms remain to be addressed, it is well documented that senescent T cells and inflammaging might be major driving forces in immunosenescence. Potential counteractive measures will be discussed, including intervention of cellular senescence and metabolic-epigenetic axes to mitigate immunosenescence. In recent years, immunosenescence has attracted increasing attention for its role in tumor development. As a result of the limited participation of elderly patients, the impact of immunosenescence on cancer immunotherapy is unclear. Despite some surprising results from clinical trials and drugs, it is necessary to investigate the role of immunosenescence in cancer and other age-related diseases.

Abstract In recent years, 2D reconfigurable phototransistors (RPTs) have been applied in broadband convolutional processing, retinomorphic hardware devices, and non‐volatile memorizers. However, there has been a lack of investigation into all‐2D Schottky junctions used in RPT with polarity control behavior. Herein, a vertically stacked multilayered WS2/WTe2 Schottky RPT is reported. The semimetal characteristics of 1T’‐WTe2 is designed to form a built‐in electric field of 69 meV across the heterojunction and WS2 exhibits gate‐tunable characteristics. Therefore, reconfigurable rectifying behavior and self‐driven bidirectional photo response can be achieved. The phototransistor possesses a gate‐tunable rectification ratio ranging from 10−2 to 105, and the corresponding logic half‐wave rectifier shows excellent switchable rectifying states. Under 635 nm illumination, the responsivity can be adjusted from −1325 to 430 mA W−1 with reversed signs. Meanwhile, the maximum power conversion efficiency is 2.84%, and the specific detectivity is 1.47 × 1012 Jones. The device shows both negative and positive responsivity with linear gate dependence within a voltage window of 10 V. Impressively, nonvolatile photovoltaic performance can be demonstrated by reversing short‐circuit current and open‐circuit voltage by applying and releasing pulsed gate voltage. Finally, reconfigurable polarization behavior, single‐pixel imaging, and the optical logic circuit are applicable to the heterostructure.

In order to achieve global multiple seamless coverage, space-based internet usually adopts low Earth orbit (LEO) mega-constellation networks structure, which has the characteristics of high network topology dynamics, limited on-board computing and storage capacity, and uneven distribution of ground traffic. Such features may cause problems such as high transmission delay, network congestion and link interruption. Establishing a stable, efficient and balanced satellite communication link can effectively alleviate the performance of the transmission delay, load balancing, and network throughput. Taking advantage of the regularity of network topology, a pre-coded inter-satellite routing algorithm with load balancing is proposed, which includes 3 parts: (a) the routing sequence coding method and the concept of gateway satellite Service Region (GSSR) are proposed; (b) the initial routing sequence of GSSR is generated based on the maximum network flow method under the ideal situation of uniform satellite traffic distribution; (c) aiming at the uneven distribution of traffic, the Sinkhorn algorithm is used to improve the load balancing performance of inter-satellite links. Simulation results show that, for the Starlink Group-4 constellation, the proposed method can maintain a low transmission delay and improve the load balancing together with the network throughout performance with minimal hops and low time complexity.

Sunflower is one of the four major oil crops in the world. 'Zaoaidatou' (ZADT), the main variety of oil sunflower in the northwest of China, has a short growth cycle, high yield, and high resistance to abiotic stress. However, the ability to tolerate adervesity is limited. Therefore, in this study, we used the retention line of backbone parent ZADT as material to establish its tissue culture and genetic transformation system for new variety cultivating to enhance resistance and yields by molecular breeding. The combination of 0.05 mg/L IAA and 2 mg/L KT in MS was more suitable for direct induction of adventitious buds with cotyledon nodes and the addition of 0.9 mg/L IBA to MS was for adventitious rooting. On this basis, an efficient Agrobacterium tumefaciens-mediated genetic transformation system for ZADT was developed by the screening of kanamycin and optimization of transformation conditions. The rate of positive seedlings reached 8.0%, as determined by polymerase chain reaction (PCR), under the condition of 45 mg/L kanamycin, bacterial density of OD600 0.8, infection time of 30 min, and co-cultivation of three days. These efficient regeneration and genetic transformation platforms are very useful for accelerating the molecular breeding process on sunflower.

The understanding of biodiversity-ecosystem functioning linkages under environment change has been advanced immensely by the development and application of the functional trait theory. Intertidal zones, characterized by significant environmental gradient differences, are severely threatened by sea-level rises caused by global warming. The functional trait distribution pattern for mangrove communities along the intertidal gradient and underlying mechanisms remain unclear. We investigated differences in the leaf functional traits of mangrove plant assemblages among the high, middle, and low intertidal zones, and their relationships to sediment physicochemical properties, in Dongzhaigang and Qinglan Harbor reserves, Hainan, China. In both reserves, most sediment physicochemical properties (pH; salinity; total potassium, carbon, nitrogen, and sulfur concentrations; organic carbon content; ammonium nitrogen and available phosphorus concentrations) differed significantly among gradient zones. The sediment pH, total phosphorus, and available potassium concentrations differed significantly between the dry and wet seasons. From the low to the high intertidal zone, the mangrove specific leaf area and leaf nitrogen and phosphorus concentrations increased significantly and the leaf thickness decreased significantly. The leaf tannin and lignin concentrations were significantly lower and the leaf phosphorus concentration, nitrogen concentration and hardness were significantly higher in the dry season than in the wet season. The sediment salinity, pH, and total sulfur and potassium contents were key factors affecting the distribution of mangrove leaf functional traits along the intertidal gradient. The changes in mangrove leaf functional traits reflect gradient-level responses to environmental stress and the sediment nutrient status. Our findings improve the understanding of the functional trait diversity and maintenance mechanisms of mangrove communities under environmental change and provide a new perspective for the prediction of how coastal mangroves will respond to future global warming.

Abstract O‐GlcNAcylation functions as a cellular nutrient and stress sensor and participates in almost all cellular processes. However, it remains unclear whether O‐GlcNAcylation plays a role in the establishment and maintenance of cell polarity, because mice lacking O‐GlcNAc transferase (OGT) are embryonically lethal. Here, a mild Ogt knockout mouse model is constructed and the important role of O‐GlcNAcylation in establishing and maintaining cell polarity is demonstrated. Ogt knockout leads to severe pulmonary fibrosis and dramatically promotes epithelial‐to‐mesenchymal transition. Mechanistic studies reveal that OGT interacts with pericentriolar material 1 (PCM1) and centrosomal protein 131 (CEP131), components of centriolar satellites required for anchoring microtubules to the centrosome. These data further show that O‐GlcNAcylation of PCM1 and CEP131 promotes their centrosomal localization through phase separation. Decrease in O‐GlcNAcylation prevents PCM1 and CEP131 from localizing to the centrosome, instead dispersing these proteins throughout the cell and impairing the microtubule‐centrosome interaction to disrupt centrosome positioning and cell polarity. These findings identify a previously unrecognized role for protein O‐GlcNAcylation in establishing and maintaining cell polarity with important implications for the pathogenesis of pulmonary fibrosis.

BackgroundDisulfidptosis, a newly defined type of programmed cell death, has emerged as a significant regulatory process in the development and advancement of malignant tumors, such as lower-grade glioma (LGG). Nevertheless, the precise biological mechanisms behind disulfidptosis in LGG are yet to be revealed, considering the limited research conducted in this field.MethodsWe obtained LGG data from the TCGA and CGGA databases and performed comprehensive weighted co-expression network analysis, single-sample gene set enrichment analysis, and transcriptome differential expression analyses. We discovered nine genes associated with disulfidptosis by employing machine learning methods like Cox regression, LASSO regression, and SVM-RFE. These were later used to build a predictive model for patients with LGG. To confirm the expression level, functional role, and impact on disulfidptosis of ABI3, the pivotal gene of the model, validation experiments were carried out in vitro.ResultsThe developed prognostic model successfully categorized LGG patients into two distinct risk groups: high and low. There was a noticeable difference in the time the groups survived, which was statistically significant. The model’s predictive accuracy was substantiated through two independent external validation cohorts. Additional evaluations of the immune microenvironment and the potential for immunotherapy indicated that this risk classification could function as a practical roadmap for LGG treatment using immune-based therapies. Cellular experiments demonstrated that suppressing the crucial ABI3 gene in the predictive model significantly reduced the migratory and invasive abilities of both SHG44 and U251 cell lines while also triggering cytoskeletal retraction and increased cell pseudopodia.ConclusionThe research suggests that the prognostic pattern relying on genes linked to disulfidptosis can provide valuable insights into the clinical outcomes, tumor characteristics, and immune alterations in patients with LGG. This could pave the way for early interventions and suggests that ABI3 might be a potential therapeutic target for disulfidptosis.

BackgroundsDisulfidptosis, a newly discovered mechanism of programmed cell death, is believed to have a unique role in elucidating cancer progression and guiding cancer therapy strategies. However, no studies have yet explored this mechanism in glioma.MethodsWe downloaded data on glioma patients from online databases to address this gap. Subsequently, we identified disulfidptosis-related genes from published literature and verified the associated lncRNAs.ResultsThrough univariate, multivariate, and least absolute shrinkage and selection operator (LASSO) regression algorithms analyses, we identified 10 lncRNAs. These were then utilized to construct prognostic prediction models, culminating in a risk-scoring signature. Reliability and validity tests demonstrated that the model effectively discerns glioma patients’ prognosis outcomes. We also analyzed the relationship between the risk score and immune characteristics, and identified several drugs that may be effective for high-risk patients. In vitro experiments revealed that LINC02525 could enhances glioma cells’ migration and invasion capacities. Additionally, knocking down LINC02525 was observed to promote glioma cell disulfidptosis.ConclusionThis study delves into disulfidptosis-related lncRNAs in glioma, offering novel insights into glioma therapeutic strategies.

Introduction The dynamics of self-harm vary substantially around the world, yet it is severely under-researched outside of a small number of high-income ‘Western’ countries. South Asia is disproportionately impacted by suicide, yet we know less about non-suicidal self-injury (NSSI) in the region.Objective To review and summarise evidence on the prevalence, correlates and common methods of NSSI in South Asia.Methods We searched Medline, Embase and PsycINFO for the period 1 January 2000 to 31 August 2023, for peer-reviewed observational studies. A total of 11 studies from eight South Asian countries that reported prevalence and/or correlates of NSSI were included in this review. We assessed the quality of the studies using the Study Quality Assessment Tools for Observational Cohort and Cross-Sectional Studies. We used meta-regression to describe the sources of heterogeneity. Abstracted data were summarised using thematic synthesis.Results For non-clinical populations, the 12-month prevalence of NSSI ranged from 3.2% to 44.8%, and the lifetime prevalence ranged from 21% to 33%. For clinical populations, the 12-month prevalence of NSSI ranged from 5% to 16.4%, while the lifetime prevalence ranged from 2% to 27%. Male sex, unemployment, financial stress, history of suicidal behaviour and depression were associated with a higher risk of NSSI. Better access to counselling services, higher self-esteem and self-knowledge were associated with a lower risk of NSSI.Conclusion The burden of NSSI in South Asia appears to be high in both clinical and non-clinical populations. Further research, especially with general population samples, is needed to build evidence on the epidemiology, context and meaning of NSSI in South Asia to inform the design of context-specific interventions.PROSPERO registration number CRD42022342536.
