A time-resolved analysis of the amplitude and phase of THz pulses propagating through three-dimensional photonic crystals is presented. Single-cycle pulses of THz radiation allow measurements over a wide frequency range, spanning more than an octave below, at and above the bandgap of strongly dispersive photonic crystals. Transmission data provide evidence for slow group velocities at the photonic band edges and for superluminal transmission at frequencies in the gap. In this paper we address many of the fundamental open questions regarding the glassy behavior of the magnetic/electronic phase segregated state in rare earth perovskites. In particular, magnetic relaxation experiments support that the collective effects (memory, ageing, etc.) are due to interparticle interactions, rather than the double-exchange vs. superexchange competition. A careful study of the non-linear susceptibility in the critical region is performed, and the critical exponents contrasted with those of conventional spin-glasses and concentrated quenched ferrofluids. We report the suppression of the magnetic phase transition in La1-xCaxMnO3 close to the localized-to-itinerant electronic transition, i.e. at x = 0.2 and x = 0.5. A new crossover temperature Tf can be defined for these compositions instead of TC. Unlike in common continuous magnetic phase transition the susceptibility does not diverge at Tf and a spontaneous magnetization cannot be defined below it. We propose that the proximity to the doping-induced metal-insulator transition introduces a random field which breaks up the electronic/magnetic homogeneity of the system and explains these effects., Comment: 9 pages, 3 figures

We are reporting the dielectric response of La1.5Sr0.5NiO4, a system that presents a charge-ordered state above room temperature and a rearrangement of its charge-order pattern in the temperature region 160-200 K. A careful analysis of the role of the electric contacts used, sample thickness and grain size on the experimental data allows us to determine that this material exhibits high values of intrinsic dielectric constant. Magnetic anisotropy has been measured for the ferromagnetic La0.7Sr0.3CoO3 perovskite from an analysis of the high-field part of the magnetization vs. field curves, i.e., the magnetic saturation regime. These measurements give a magnetic anistropy one order of magnitude higher than that of reference manganites. Surprisingly, the values of the magnetic anisotropy calculated in this way do not coincide with those estimated from measurements of coercive fields which are one order of magnitude smaller. It is proposed that the reason of this anomalous behaviour is a transition of the trivalent Co ions under the external magnetic field from a low-spin to an intermediate-spin state. Such a transition converts the Co3+ ions into Jahn-Teller ions having an only partially quenched orbital angular momentum, which enhances the intra-atomic spin-orbit coupling and magnetic anisotropy., Comment: Accepted of publication in Europhysics Letters, 11 pages, 5 figures

Ultrasonic attenuation and phase velocity measurements have been carried out in the ferromagnetic perovskites La_{2/3}Ca_{1/3}MnO_3 and La_{2/3}Sr_{1/3}MnO_3. Data show that the transition at the Curie temperature, T_C, changes from first- to second-order as Sr replaces Ca in the perovskite. The compound with first-order transition shows also another transition at a temperature T* > T_C. We interpret the temperature window T_C < T < T* as a region of coexistence of a phase separated regime of metallic and insulating regions, in the line of recent theoretical proposals., Comment: 4 pages, 2 figures

MnAs exhibits a first-order phase transition from a ferromagnetic, high-spin metal NiAs-type hexagonal phase to a paramagnetic, lower-spin insulator MnP-type orthorhombic phase at T_C = 313 K. Here, we report the results of neutron diffraction experiments showing that an external magnetic field, B, stabilizes the hexagonal metallic phase above T_C. The phase transformation is reversible and constitutes the first demonstration of a bond-breaking transition induced by a magnetic field. At 322 K the hexagonal structure is restored for B > 4 tesla. The field-induced phase transition is accompanied by an enhanced magnetoresistance of about 17 % at 310 K. We discuss the origin of this phenomenon, which appears to be similar to that of the colossal magnetoresistance response observed in some members of the manganese perovskite family., Comment: 4 Figures

Tl$_{2}$Mn$_{2}$O$_{7}$ pyrochlores present colossal magnetoresistance (CMR) around the long range ferromagnetic ordering temperature (T$_{C}$). The character of this magnetic phase transition has been determined to be first order, by purely magnetic methods, in contrast to the second order character previously reported by Zhao et al. (Phys. Rev. Lett. 83, 219 (1999)). The highest CMR effect, as in Tl$_{1.8}$Cd$_{0.2}$Mn$_{2}$O$_{7}$, corresponds to a stronger first order character. This character implies a second type of magnetic interaction, besides the direct superexchange between the Mn$^{4+}$ ions, as well as a phase coexistence. A model is proposed, with a complete Hamiltonian (including superexchange and an indirect interaction), which reproduce the observed phenomenology., Comment: 6 pages. Figures included

- Abstract
Yeasts are a diverse group of fungal microorganisms that are widely used to produce fermented foods and beverages. In Mexico, open fermentations are used to obtain spirits from agave plants. Despite the prevalence of this traditional practice throughout the country, yeasts have only been isolated and studied from a limited number of distilleries. To systematically describe the diversity of yeast species from open agave fermentations, here we generate the YMX‐1.0 culture collection by isolating 4524 strains from 68 sites with diverse climatic, geographical, and biological contexts. We used MALDI‐TOF mass spectrometry for taxonomic classification and validated a subset of the strains by ITS and D1/D2 sequencing, which also revealed two potential novel species of Saccharomycetales. Overall, the composition of yeast communities was weakly associated with local variables and types of climate, yet a core set of six species was consistently isolated from most producing regions. To explore the intraspecific variation of the yeasts from agave fermentations, we sequenced the genomes of four isolates of the nonconventional yeast Kazachstania humilis. The genomes of these four strains were substantially distinct from a European isolate of the same species, suggesting that they may belong to different populations. Our work contributes to the understanding and conservation of an open fermentation system of great cultural and economic importance, providing a valuable resource to study the biology and genetic diversity of microorganisms living at the interface of natural and human‐associated environments. La existencia de una diversidad de factores que incide en las actividades productiva generada en México causa impactos en el crecimiento empresarial. El objetivo de la investigación fue analizar el uso de la deuda pública como herramienta financiera utilizada por los gobiernos para propiciar mejores condiciones en infraestructura, espacios y/o servicios públicos que coadyuven en fomentar la productividad y bienestar de las regiones, permitiendo conocer cómo la deuda pública impacta el desarrollo y la competitividad regional, particularmente en el estado de Tamaulipas, México. Se trata de una investigación descriptiva con enfoque cuantitativo, de alcance correlacional utilizando la técnica multivariante de mínimos cuadrados ordinarios De los resultados obtenidos, se resalta la existencia de una deuda subnacional delineada bajo una muestra de tendencia positiva en la adquisición de deuda a largo plazo a nivel estatal, contrario a la baja que se presenta desde lo municipal; sin embargo, la determinación de mayor inversión en espacios públicos que muestra un ratio de deuda pública con un incremento que propician competitividad de algunos sectores. Se concluye que existe una relación positiva significativa entre la deuda pública y la inversión, adicionalmente se evidencia mejoras en las condiciones para un mayor desarrollo social y económico.

- Abstract
B-cell acute lymphoblastic leukemia (B-ALL) is the commonest childhood cancer. High hyperdiploidy (HHD) identifies the most frequent cytogenetic subgroup in childhood B-ALL. Although hyperdiploidy represents an important prognostic factor in childhood B-ALL, the specific chromosome gains with prognostic value in HHD-B-ALL remain controversial, and the current knowledge about the hierarchy of chromosome gains, clonal heterogeneity and chromosomal instability in HHD-B-ALL remains very limited. We applied automated sequential-iFISH coupled with single-cell computational modeling to identify the specific chromosomal gains of the eight typically gained chromosomes in a large cohort of 72 primary diagnostic (DX, n = 62) and matched relapse (REL, n = 10) samples from HHD-B-ALL patients with either favorable or unfavorable clinical outcome in order to characterize the clonal heterogeneity, specific chromosome gains and clonal evolution. Our data show a high degree of clonal heterogeneity and a hierarchical order of chromosome gains in DX samples of HHD-B-ALL. The rates of specific chromosome gains and clonal heterogeneity found in DX samples differ between HHD-B-ALL patients with favorable or unfavorable clinical outcome. In fact, our comprehensive analyses at DX using a computationally defined risk predictor revealed low levels of trisomies +18+10 and low levels of clonal heterogeneity as robust relapse risk factors in minimal residual disease (MRD)-negative childhood HHD-B-ALL patients: relapse-free survival beyond 5 years: 22.1% versus 87.9%, P < 0.0001 and 33.3% versus 80%, P < 0.0001, respectively. Moreover, longitudinal analysis of matched DX-REL HHD-B-ALL samples revealed distinct patterns of clonal evolution at relapse. Our study offers a reliable prognostic sub-stratification of pediatric MRD-negative HHD-B-ALL patients.

- Abstract
Animal models currently used to test the efficacy and safety of cell therapies, mainly murine models, have limitations as molecular, cellular, and physiological mechanisms are often inherently different between species, especially in the brain. Therefore, for clinical translation of cell-based medicinal products, the development of alternative models based on human neural cells may be crucial. We have developed an in vitro model of transplantation into human brain organoids to study the potential of neural stem cells as cell therapeutics and compared these data with standard xenograft studies in the brain of immunodeficient NOD.Cg-Prkdcscid Il2rgtm1Wjl/SzJ (NSG) mice. Neural stem cells showed similar differentiation and proliferation potentials in both human brain organoids and mouse brains. Our results suggest that brain organoids can be informative in the evaluation of cell therapies, helping to reduce the number of animals used for regulatory studies.

- Abstract
Objective: The medical residency educational program aims to train competent specialists, so you must use a wide variety of affordable and easily accessible resources to ensure a quality education. The purpose of the manuscript is to convey the modifications that have been implemented in the educational model of our Institution to mitigate the educational delay that the pandemic brought to medical education. Material and method: In the situation of a pandemic, it is difficult to maintain the education of medical-surgical residences while preserving the health and safety of residents, teachers and patients. In this context, innovative and reproducible solutions are proposed to achieve this objective. Results: The solutions that are proposed are included in a virtual classroom model through platforms that manage learning, videoconferences, surgical videos and clinical cases with a problem-solving approach. Conclusions: A combined educational model is proposed, in which the use of innovative technological resources must be exploited to the maximum to provide quality material in the theoretical part and, on the other hand, use surgical simulation as a method to acquire skill and motor skills in the practice., Objetivo: El programa educativo de residencias médicas tiene como objetivo la formación de especialistas competentes, por lo que debe utilizar una amplia variedad de recursos que sean rentables y de fácil acceso para asegurar una educación de calidad. El objetivo del manuscrito es transmitir las modificaciones que se han implementado en el modelo educativo de nuestra Institución para mitigar el retraso formativo que la pandemia trajo a la educación médica. Material y método: Ante la situación de una pandemia, resulta complicado mantener la educación de las residencias médico-quirúrgicas conservando la salud y la seguridad de los residentes, profesores y pacientes. En este contexto se proponen soluciones innovadoras y reproducibles para lograr este objetivo. Resultados: Las soluciones que se plantean se engloban en un modelo de aula virtual por medio de plataformas que gestionan el aprendizaje, videoconferencias, videos quirúrgicos y casos clínicos con un enfoque de resolución de problemas. Conclusiones: Se propone un modelo educativo combinado, en el que se debe explotar al máximo el uso de recursos tecnológicos innovadores para proveer material de calidad en la parte teórica y por otro lado utilizar la simulación quirúrgica como un método para adquirir destreza y motricidad en la práctica.

The inguinal hernia is the most common type of hernia in the world and is one of the most common surgeries performed by the general surgeon worldwide. This type of hernias is 8-10 times more common in men than in women. The Lichtenstein technique is the open tension-free technique that continues to be the most performed and recommended by guidelines, this is due to the fact that it is a tension-free technique that has shown a low recurrence rate, which is reproducible , low cost, minimally invasive and has a lower learning curve than laparoscopic approaches. This article describes the important aspects in the technique and updates on the same and how we manage to adapt this to our clinical practice in a public institution., La hernia inguinal es el tipo de hernia más común en el mundo y es una de las cirugías más frecuentemente realizadas por el cirujano general a nivel mundial. Este tipo de hernias es 8-10 veces más común en hombres que en mujeres. La técnica de Lichtenstein es la técnica abierta sin tensión más utilizada y recomendada por las guías internacionales, esto debido a que es una técnica sin tensión, con baja tasa de recurrencia, bajo costo, mínimamente invasiva, reproducible y con una curva de aprendizaje menor que los abordajes laparoscópicos. - Abstract
Objective: To know the particularities of surgical patients in Intensive Care Units (ICU). Surgical patients present specific alterations derived from serious underlying pathologies and the surgical trauma typical of surgery. Studying the most relevant data of these patients is necessary to provide them with the best available management. Material and Methods: A bibliographic search was performed in PUBMED and Google Scholar for articles related to patients with surgical pathologies admitted to the ICU. The data were grouped by the authors into the following topics: demographics, the proportion of surgical patients, specialties of origin, comorbidities and risk factors, diagnoses, complications in the ICU, and mortality. Results: The age of surgical patients admitted to an ICU is older than those who were not admitted. Admissions for surgical pathologies represent 30% of patients in an ICU. Most of these patients are admitted from the operating room after elective surgery in the specialty of general surgery. The main reasons for admission to an ICU are post-surgical observation and the presence of cardiovascular disorders. The most frequent diagnoses are peritonitis, trauma, and intestinal occlusion. The mortality of surgical patients in this area is comparable to that of non-surgical patients. Conclusions: There are identifiable risk factors in this group of patients. Differences were observed between the results of patients with surgical pathologies compared to those who did not present them. There is a large area of opportunity for resear ch and implementation of preventive measures to improve the prognosis of this group of patients., Objetivo: Conocer las particularidades de los pacientes quirúrgicos en Unidades de Cuidados Intensivos (UCI). Los pacientes quirúrgicos presentan alteraciones específicas derivadas de patologías graves subyacentes y del trauma quirúrgico propio de la cirugía. Estudiar los datos más relevantes de estos pacientes es necesario para brindarles el mejor manejo disponible. Material y Métodos: Se realizó una búsqueda bibliográfica en PUBMED y Google Scholar de artículos relacionados a pacientes con patologías quirúrgicas ingresados en UCI. Los datos fueron agrupados por los autores en los siguientes puntos: demografía, proporción de pacientes quirúrgicos, especialidades de procedencia, comorbilidades y factores de riesgo, diagnósticos, complicaciones en UCI, y mortalidad. Resultados: La edad de los pacientes quirúrgicos ingresados a una UCI es mayor que los que no ingresaron. Las admisiones por patologías quirúrgicas representan el 30% de los pacientes en una UCI. La mayoría de estos pacientes ingresa desde quirófano posterior a una cirugía electiva por la especialidad de cirugía general. Los principales motivos de admisión a una UCI son la observación postquirúrgica y presencia de alteraciones cardiovasculares. Los diagnósticos más frecuentes son peritonitis, trauma y oclusión intestinal. La mortalidad de los pacientes quirúrgicos en esta área es equiparable a la de los pacientes no quirúrgicos. Conclusiones: Existen factores de riesgo identificables en este grupo de pacientes. Se observaron diferencias entre los resultados de pacientes con patologías quirúrgicas comparado con quienes no las presentaron. Existe una gran área de oportunidad para la investigación e implementación de medidas preventivas para mejorar el pronóstico de este grupo de pacientes.

- Abstract
Understanding what controls global leaf type variation in trees is crucial for comprehending their role in terrestrial ecosystems, including carbon, water and nutrient dynamics. Yet our understanding of the factors influencing forest leaf types remains incomplete, leaving us uncertain about the global proportions of needle-leaved, broadleaved, evergreen and deciduous trees. To address these gaps, we conducted a global, ground-sourced assessment of forest leaf-type variation by integrating forest inventory data with comprehensive leaf form (broadleaf vs needle-leaf) and habit (evergreen vs deciduous) records. We found that global variation in leaf habit is primarily driven by isothermality and soil characteristics, while leaf form is predominantly driven by temperature. Given these relationships, we estimate that 38% of global tree individuals are needle-leaved evergreen, 29% are broadleaved evergreen, 27% are broadleaved deciduous and 5% are needle-leaved deciduous. The aboveground biomass distribution among these tree types is approximately 21% (126.4 Gt), 54% (335.7 Gt), 22% (136.2 Gt) and 3% (18.7 Gt), respectively. We further project that, depending on future emissions pathways, 17-34% of forested areas will experience climate conditions by the end of the century that currently support a different forest type, highlighting the intensification of climatic stress on existing forests. By quantifying the distribution of tree leaf types and their corresponding biomass, and identifying regions where climate change will exert greatest pressure on current leaf types, our results can help improve predictions of future terrestrial ecosystem functioning and carbon cycling.

- Abstract
Forests are a substantial terrestrial carbon sink, but anthropogenic changes in land use and climate have considerably reduced the scale of this system1. Remote-sensing estimates to quantify carbon losses from global forests2–5 are characterized by considerable uncertainty and we lack a comprehensive ground-sourced evaluation to benchmark these estimates. Here we combine several ground-sourced6 and satellite-derived approaches2,7,8 to evaluate the scale of the global forest carbon potential outside agricultural and urban lands. Despite regional variation, the predictions demonstrated remarkable consistency at a global scale, with only a 12% difference between the ground-sourced and satellite-derived estimates. At present, global forest carbon storage is markedly under the natural potential, with a total deficit of 226 Gt (model range = 151–363 Gt) in areas with low human footprint. Most (61%, 139 Gt C) of this potential is in areas with existing forests, in which ecosystem protection can allow forests to recover to maturity. The remaining 39% (87 Gt C) of potential lies in regions in which forests have been removed or fragmented. Although forests cannot be a substitute for emissions reductions, our results support the idea2,3,9 that the conservation, restoration and sustainable management of diverse forests offer valuable contributions to meeting global climate and biodiversity targets.

- Abstract
[Abstract] The present study investigated citizens’ self-reported pro-environmental behaviors by adopting a survey and bootstrapping via structural equation modeling for five different personality traits. Adopting one traditional psycho-social theory, this study investigates how motivations, values, intentions, norms, and behaviors are connected by using different constructs from the value-belief-norm theory extended with external influences and three different pro-environmental behaviors, namely, waste preventer, green consumer, and avoider, and how the connections change between different personality traits from the Big Five personality test, namely, openness, extroversion, neuroticism, agreeableness, and consciousness. According to this study, personal conscience may inspire environmentally beneficial behaviors like green purchasing and waste reduction when appealing to the emotions of the general public. As a result, strengthening the population’s subjective norms requires interventions that concentrate on communication tactics to raise knowledge of penalties and obligations for communal advantages.

- Abstract
Background: Nirmatrelvir/ritonavir treatment decreases the hospitalisation rate in immunocompetent patients with COVID-19, but data on efficacy in patients with haematological malignancy are scarce. Here, we describe the outcome of nirmatrelvir/ritonavir treatment in a large cohort of the latter patients. Methods: This is a retrospective cohort study from the multicentre EPICOVIDEHA registry (NCT04733729) on patients with haematological malignancy, who were diagnosed with COVID-19 between January and September 2022. Patients receiving nirmatrelvir/ritonavir were compared to those who did not. A logistic regression was run to determine factors associated with nirmatrelvir/ritonavir administration in our sample. Mortality between treatment groups was assessed with Kaplan–Meier survival plots after matching all the patients with a propensity score. Additionally, a Cox regression was modelled to detect factors associated with mortality in patients receiving nirmatrelvir/ritonavir. Findings: A total of 1859 patients were analysed, 117 (6%) were treated with nirmatrelvir/ritonavir, 1742 (94%) were treated otherwise. Of 117 patients receiving nirmatrelvir/ritonavir, 80% had received ≥1 anti-SARS-CoV-2 vaccine dose before COVID-19 onset, 13% of which received a 2nd vaccine booster. 5% were admitted to ICU. Nirmatrelvir/ritonavir treatment was associated with the presence of extrapulmonary symptoms at COVID-19 onset, for example anosmia, fever, rhinitis, or sinusitis (aOR 2.509, 95%CI 1.448–4.347) and 2nd vaccine booster (aOR 3.624, 95%CI 1.619–8.109). Chronic pulmonary disease (aOR 0.261, 95%CI 0.093–0.732) and obesity (aOR 0.105, 95%CI 0.014–0.776) were not associated with nirmatrelvir/ritonavir use. After propensity score matching, day-30 mortality rate in patients treated with nirmatrelvir/ritonavir was 2%, significantly lower than in patients with SARS-CoV-2 directed treatment other than nirmatrelvir/ritonavir (11%, p = 0.036). - Abstract
1. Biodiversity is an important component of natural ecosystems, with higher species richness often correlating with an increase in ecosystem productivity. Yet, this relationship varies substantially across environments, typically becoming less pronounced at high levels of species richness. However, species richness alone cannot reflect all important properties of a community, including community evenness, which may mediate the relationship between biodiversity and productivity. If the evenness of a community correlates negatively with richness across forests globally, then a greater number of species may not always increase overall diversity and productivity of the system. Theoretical work and local empirical studies have shown that the effect of evenness on ecosystem functioning may be especially strong at high richness levels, yet the consistency of this remains untested at a global scale. 2. Here, we used a dataset of forests from across the globe, which includes composition, biomass accumulation and net primary productivity, to explore whether productivity correlates with community evenness and richness in a way that evenness appears to buffer the effect of richness. Specifically, we evaluated whether low levels of evenness in speciose communities correlate with the attenuation of the richness–productivity relationship. 3. We found that tree species richness and evenness are negatively correlated across forests globally, with highly speciose forests typically comprising a few dominant and many rare species. Furthermore, we found that the correlation between diversity and productivity changes with evenness: at low richness, uneven communities are more productive, while at high richness, even communities are more productive. 4. Synthesis. Collectively, these results demonstrate that evenness is an integral component of the relationship between biodiversity and productivity, and that the attenuating effect of richness on forest productivity might be partly explain

- Abstract
With an ever-increasing amount of (meta)genomic data being deposited in sequence databases, (meta)genome mining for natural product biosynthetic pathways occupies a critical role in the discovery of novel pharmaceutical drugs, crop protection agents and biomaterials. The genes that encode these pathways are often organised into biosynthetic gene clusters (BGCs). In 2015, we defined the Minimum Information about a Biosynthetic Gene cluster (MIBiG): a standardised data format that describes the minimally required information to uniquely characterise a BGC. We simultaneously constructed an accompanying online database of BGCs, which has since been widely used by the community as a reference dataset for BGCs and was expanded to 2021 entries in 2019 (MIBiG 2.0). Here, we describe MIBiG 3.0, a database update comprising large-scale validation and re-annotation of existing entries and 661 new entries. Particular attention was paid to the annotation of compound structures and biological activities, as well as protein domain selectivities. Together, these new features keep the database up-to-date, and will provide new opportunities for the scientific community to use its freely available data, e.g. for the training of new machine learning models to predict sequence-structure-function relationships for diverse natural products. - Abstract
Introduction: Molnupiravir and nirmatrelvir/ritonavir are antivirals used to prevent progression to severe SARS-CoV-2 infections and decrease hospitalisation and mortality rates. Nirmatrelvir/ritonavir was authorised in Europe in December 2021, whereas molnupiravir is not yet licensed in Europe as of February 2022. Molnupiravir may be an alternative to nirmatrelvir/ritonavir because it is associated with fewer drug-drug interactions and contraindications. A caveat for molnupiravir is the mode of action induces viral mutations. Mortality rate reduction with molnupiravir was less pronounced than that with nirmatrelvir/ritonavir in patients without haematological malignancy. Little is known about the comparative efficacy of the two drugs in patients with haematological malignancy at high-risk of severe COVID-19. Thus, molnupiravir and nirmatrelvir/ritonavir were compared in a cohort of patients with haematological malignancies. Methods: Clinical data from patients treated with molnupiravir or nirmatrelvir/ritonavir monotherapy for COVID-19 were retrieved from the EPICOVIDEHA registry. Patients treated with molnupiravir were matched by sex, age (±10 years), and severity of baseline haematological malignancy to controls treated with nirmatrelvir/ritonavir. Results: A total of 116 patients receiving molnupiravir for the clinical management of COVID-19 were matched to an equal number of controls receiving nirmatrelvir/ritonavir. In each of the groups, 68 (59%) patients were male; with a median age of 64 years (interquartile range [IQR] 53-74) for molnupiravir recipients and 64 years (IQR 54-73) for nirmatrelvir/ritonavir recipients; 56.9% (n=66) of the patients had controlled baseline haematological malignancy, 12.9% (n=15) had stable disease, and 30.2% (n=35) had active disease at COVID-19 onset in each group. - Abstract
Patients with previous CD19-directed chimeric antigen receptor (CAR) T-cell therapy have a prolonged vulnerability to viral infections. Coronavirus disease 2019 (COVID-19) has a great impact and has previously been shown to cause high mortality in this population. Until now, real-world data on the impact of vaccination and treatment on patients with COVID-19 after CD19-directed CAR T-cell therapy are lacking. Therefore, this multicenter, retrospective study was conducted with data from the EPICOVIDEHA survey. Sixty-four patients were identified. The overall mortality caused by COVID-19 was 31%. Patients infected with the Omicron variant had a significantly lower risk of death due to COVID-19 compared with patients infected with previous variants (7% vs 58% [P = .012]). Twenty-six patients were vaccinated at the time of the COVID-19 diagnosis. Two vaccinations showed a marked but unsignificant reduction in the risk of COVID-19–caused mortality (33.3% vs 14.2% [P = .379]). In addition, the course of the disease appears milder with less frequent intensive care unit admissions (39% vs 14% [P = .054]) and a shorter duration of hospitalization (7 vs 27.5 days [P = .022]). Of the available treatment options, only monoclonal antibodies seemed to be effective at reducing mortality from 32% to 0% (P = .036). We conclude that survival rates of CAR T-cell recipients with COVID-19 improved over time and that the combination of prior vaccination and monoclonal antibody treatment significantly reduces their risk of death. - Abstract
Objectives: Elderly patients with hematologic malignancies face the highest risk of severe COVID-19 outcomes. The infection's impact on different age groups remains unstudied in detail. Methods: We analyzed elderly patients (age groups: 65-70, 71-75, 76-80, and >80 years old) with hematologic malignancies included in the EPICOVIDEHA registry between January 2020 and July 2022. Univariable and multivariable Cox regression models were conducted to identify factors influencing death in COVID-19 patients with hematological malignancy. Results: The study included data from 3,603 elderly patients (aged 65 or older) with hematological malignancy, with a majority being male (58.1%) and a significant proportion having comorbidities. The patients were divided into four age groups, and the analysis assessed COVID-19 outcomes, vaccination status, and other variables in relation to age and pandemic waves. The 90-day survival rate for patients with COVID-19 was 71.2%, with significant differences between groups. The pandemic waves had varying impacts, with the first wave affecting patients over 80 years old, the second being more severe in 65-70, and the third being the least severe in all age groups. Factors contributing to 90-day mortality included age, comorbidities, lymphopenia, active malignancy, acute leukemia, less than three vaccine doses, severe COVID-19, and using only corticosteroids as treatment. Conclusion: These data underscore the heterogeneity of elderly hematological patients, highlight the different impacts of COVID-19 waves and the pivotal importance of vaccination, and may help in planning future healthcare efforts.
