- Abstract
The aim of this study is to elucidate the health vulnerabilities experienced by women deprived of their liberty. This is a qualitative and descriptive-exploratory study, carried out with 13 women deprived of their liberty. The information was obtained through semi-structured interviews and the data submitted to content analysis. The results show that the participants experience numerous health vulnerabilities with an emphasis on gynecological care, which, added to other conditions, potentiate or trigger diseases. It was also identified that access to health services in the prison unit is weakened, causing care to be carried out in the usual health institutions in the municipality. It is observed that the penitentiary system presents several weaknesses in the fulfillment of the proposals of the National Health Plan in the Penitentiary System, which aggravates the maintenance of women's health, disregarding their specific needs and aggravating their vulnerabilities. Introducción. El cáncer de mama es una neoplasia grave que se origina en los tejidos mamarios y cuyo tratamiento demanda quimioterapia, con los consecuentes cambios en la calidad de vida. Objetivo. Determinar los factores de riesgo asociados con la calidad de vida relacionada con la salud en mujeres con cáncer de mama sometidas a quimioterapia en Cali, Colombia. Materiales y métodos. Se hizo un estudio observacional y transversal con componente analítico en una muestra de 80 mujeres, utilizando los cuestionarios QLQ-C30 para cáncer y QLQ-BR23 para cáncer de mama, así como información sociodemográfica y clínica. Se hizo un análisis de regresión logística para determinar los factores asociados con las razones de momios (odd ratios, OR) ajustadas y un intervalo de confianza (IC) de 95 %; la calidad de vida se clasificó mediante el cuestionario QLQ-BR23. Resultados. Los síntomas más relevantes fueron fatiga, insomnio y pérdida de cabello. Se redujeron la funcionalidad física, las sensaciones de placer y la actividad sexual. Los factores asociados con la baja calidad de vida fueron los síntomas mamarios (OR ajustada=5,5; IC95% 1,2-24,8; p=0,038), los efectos secundarios del tratamiento sistémico (OR ajustada=7,3; IC95% 2,6-22,1; p=0,012), un menor placer sexual (OR ajustada=1,8; IC95% 1,2-11,8; p=0,027) y la reducción de expectativas para el futuro (OR ajustada=4,2; IC95% 1,1-17,8; p=0,045). Conclusiones. En las mujeres con cáncer de mama sometidas a quimioterapia, la calidad de vida se vio afectada principalmente por los efectos secundarios del tratamiento, en tanto que los signos y los síntomas más relevantes fueron la pérdida del cabello, el insomnio y la fatiga, además de los síntomas mamarios, la menor funcionalidad física y la menor sensación de placer sexual. Se recomienda la implementación de estrategias de intervención destinadas a mejorar la calidad de vida, y el cuidado físico y emocional de las pacientes.

The main health interventions for health promotion and disease prevention that should be performed in women in the Peruvian health system are described. A review of normative technical documents and the recommendations of the main organizations for worldwide prevention was carried out. The prevention activities included physical activity, healthy eating, tobacco counseling, immunizations; In addition, the main screening for women, such as depression, violence, cardiovascular risk, cervical cytology, mammography, colon cancer, are detailed; and within the spectrum of quaternary prevention, interventions that have not shown evidence of benefit to women are detailed. The health interventions that are offered from the Peruvian health system for women, being merely focused on reproductive aspects, lose the conception of integrality that should prevail for the maintenance of health. In that sense, it is proposed to develop strategies that not only have evidence, but also know how to respond to the needs of women in the Peruvian context.

Introducción: El cateterismo venoso es esencial para los procedimientos anestésicos y quirúrgicos, cuyas características son compatibles con la estabilidad, el volumen de flujo en periodos cortos de tiempo y el grueso calibre para evitar el trauma vascular. Objetivo: Identificar las representaciones sociales de las mujeres sobre la punción venosa con fines anestésicos y quirúrgicos a la luz de los estresores de Neuman. Materiales y métodos: Estudio cualitativo con enfoque estructural en las representaciones sociales realizado en una unidad de cirugía en Brasil. Se realizó una muestra de selección completa (n=180) con delimitación temporal compuesta por mujeres (edad ≥ 18 años) sometidas a venopunción durante procedimientos anestésicos y quirúrgicos. Se utilizó la técnica de evocación lexical libre con el término inductivo “toma de vena para anestesia y cirugía” y se realizó un análisis prototípico mediante el software EVOC 2003. Asimismo, se cumplieron los requisitos ético-legales. Resultados: Las mujeres tenían entre 20 y 39 años (63%) con 10 y 13 años de escolaridad (56.1%). En el posible elemento nuclear, se encuentran sentimientos (“dolor” y “miedo”) en oposición a la zona de contraste “(in)certidumbre-habilidad profesional” y “calma-tranquilidad”, con elementos objetivos sobreactivados asignados en la primera periferia que representan estresores intrapersonales, interpersonales y extrapersonales. Discusión: El dolor, como objeto de representación, remitió a la función justificadora cuando se les acercó la aguja, lo que reafirma la aparición del miedo, el nerviosismo y la ansiedad (estresores). Conclusiones: En las representaciones sociales se identificó al dolor, la medicación, la ansiedad y el nerviosismo como factores estresantes, los cuales necesitan ser controlados terapéuticamente mediante relaciones interpersonales de confianza entre el profesional y el usuario, además del desempeño en la punción, para lo que es necesario promover la educación continua de los profesionales encargados de esta tarea. Objective: Breastfeeding is a practice that provides many benefits to the mother-child binomial and needs to be increasingly oriented, especially during pregnancy. This study aimed to understand how health education proposals can contribute to the aggregation of new knowledge and positive attitudes of pregnant women towards breastfeeding. Methods: This is the conclusion of a nursing undergraduate course at the Federal University of Campina Grande, carried out in 2019, based on the action research method and analyzed through the methodological process of the Collective Subject Discourse. Results: After a situational diagnosis, conducted through a semistructured interview, three meetings were held with the pregnant women, where the feelings linked to the desire to breastfeed were discussed, fears expressed by pregnant women about breastfeeding and the rights of breastfeeding women, respectively. Conclusion: The educational actions allowed the construction of knowledge about breastfeeding through the sharing of knowledge and exchange of experiences among pregnant women, in addition, it was possible to reverse the deficit of knowledge that was perceived in the evaluation phase, making them holders and multipliers of knowledge in their collective. Abortion is one of the main causes of maternal mortality, because health inequalities and the nonexistence of public health policies that protect women and allowing them to perform abortion on adequate sanitary conditions. The induced and unsafe abortion is a product of several social determinants, like gender, sociodemographic, ethnic-racial and socioeconomic factors, with direct results on women's health, like comorbidities and mortality. This theoretical reflection aims to understand the abortion-related Social Determinants of Health and the existing social protection policies in Brazil. The impact of the unsafe abortion as a public health problem and the maintenance of punitive laws leads to underreporting and lack of the real set of the abortion problem and that makes the creation of effective public policies unfeasible. The researches about abortion from Social Determinants of Health perspective are fundamental to identify what influences this phenomenon on order to create, implement and evaluate equanimous and effective health policies. Objetivo: Describir las acciones que realiza el Instituto Nacional Materno Perinatal, respecto al acceso a los servicios de salud para la atención de la mujer para aborto terapéutico. Material y Métodos: Estudio, descriptivo, transversal. Se revisaron todos los expedientes de solicitudes de aborto terapéutico y que fueron atendidos en el Instituto Nacional Materno Perinatal durante los periodos de enero a diciembre del 2019 y 2020. Resultados. Se atendieron 117 y 59 solicitudes para aborto terapéutico durante los periodos 2019 y 2020 respectivamente. La edad no es impedimento para solicitar el procedimiento de interrupción terapéutica voluntaria del embarazo, solo en los casos de menores de edad, se acompaña la firma de los padres o apoderados de la menor. Además, la institución cumple con informar al Ministerio Público para garantizar las medidas legales de tutela jurisdiccional en defensa de la libertad sexual de la menor. Todas las solicitudes, han seguido el procedimiento administrativo para su evaluación en junta médica, en los cuales se determina la aceptación o la negación del procedimiento. En 2019 y 2020 fueron aprobadas en junta médica 105 y 55 solicitudes respectivamente; y denegadas 12 y 4 casos respectivamente. Cumplieron con los criterios de las guías nacionales del MINSA en 90 y 46 solicitudes durante los años 2019 y 2020 respectivamente; quedando en alcance del protocolo médico institucional en 27 y 13 casos respectivamente. Cuentan con informe de salud mental aceptada en junta médica en 105 y 55 casos durante los periodos 2019 y 2020 respectivamente. Conclusión: Todas las solicitudes de interrupción terapéutica voluntaria del embarazo, presentadas por las gestantes, han seguido el procedimiento administrativo para su evaluación mediante junta médica, sin embargo, en algunos expedientes no se han encontrado copia del informe de salud mental.

Objective: To systematize good prenatal care practices in a referral center for pregnant teenagers in Campinas, São Paulo, Brazil, from the perspective of healthcare professionals. Material and methods: Qualitative study conducted between June and July 2017 using semi-structured interviews and a focus group with 10 healthcare professionals providing care to pregnant teenagers in a public hospital in the region of Campinas, São Paulo, Brazil. Data were examined using the thematic content analysis technique, which gave rise to new analytical categories. Triangulation of the results was used for validation. The theoretical and practical applicability of the main findings was identified. Results: The study looked into three categories arising from the discourse of the professionals and the evidence found in the literature: 1) comprehensive analysis of pregnant teenagers; 2) professional experiences related to the care of pregnant teenagers reflecting service quality; and 3) strengths and improvement opportunities potentially replicable as a care model. Good practices in prenatal care are those which prioritize comprehensive care for pregnant teenagers, are coordinated by a multidisciplinary team and result in a multidimensional approach, valuing the complexity of motherhood in the teenage years. Conclusion: It is advisable to develop education strategies, strengthen group care and involve the teenager's support network as part of sexual and reproductive health services in order to promote a positive experience during prenatal care when the process of becoming a mother takes place during adolescence. Objective: To reveal the meaning of breast cancer. Materials and method: A narrative research study that selected, using the snowball technique, 11 women (data saturation) diagnosed with breast cancer, on disease remission, and living in the region of Florianópolis (Brazil). Semi-structured interviews were conducted between August and December 2018, and they were subjected to the Collective Subject Discourse technique. Four central ideas emerged from the analysis of the narratives; this article presents the central ideal called "Meaning of the breast cancer diagnosis", which has eight sense categories. Results: The central idea covers the following categories: "Suffering when facing the diagnosis", "Fear of the disease and of death", "Professional inability for bad news", "Initiatives to streamline treatment", "Concern and affection with the family members as a driving force to face cancer", "Difficulties with the family members", "Support from the family members, from the professionals and/or from the support group", and "Faith and spirituality". Conclusions: The discourse reveals that the meaning of the disease diagnosis is linked to the need for treatment, to the support provided by faith, family members, the health team, and support groups, and to the difficulties faced with family members and professionals. Introducción: la muerte materna es un indicador que permite medir el desarrollo social de un país, y tiene un gran impacto a nivel familiar y social. Objetivo: el presente estudio caracteriza las muertes maternas ocurridas en la provincia de Chimborazo desde el 2013 al 2017, con la finalidad de que los resultados contribuyan al diseño de estrategias y actividades que permitan mejorar este indicador negativo. Metodología: se partió de la información entregada por el proceso de Vigilancia de la Salud Pública de la Coordinación Zonal de Salud 3, así como de los datos del Instituto Nacional de Estadísticas y Censos (INEC) sobre nacidos vivos en los años de estudio. Se consideró sólo las muertes maternas directas. Resultados: las principales causas de muerte materna se deben a hemorragia, trastornos hipertensivos del embarazo y sepsis. Se presentan con más frecuencia en instituciones de salud, población indígena y rural, relacionadas con la primera y tercera demora. Conclusiones: realizar capacitaciones permanentes de los profesionales de los establecimientos, así como en la comunidad para que se reconozcan oportunamente los signos de peligro.

185. ¿Por qué 145 mujeres > de 40 años no han tenido hijos? Motivos médicos y psicosociales

Objetivo: En una serie de 145 mujeres > de 40 años que no han tenido hijos antes de esa edad, saber los motivos médicos y psicosociales; si han recurrido a técnicas de reproducción asistida (tra) o no, y saber si cuando eran adolescentes se imaginaban en el futuro como madres con hijos. Material y método: En un periodo de 4 meses, en la consulta de Ginecología del autor, tras finalizar la consulta por cualquier motivo, las que cumplían el requisito referido, tras dar su consentimiento informado, se entrevistaron de forma semiestructurada en profundidad, para obtener los datos, que se recogieron por escrito en hojas independientes numeradas. Se establecieron 2 grupos de pacientes: A, de 40-50 años, y B, > de 50 años. Resultados: En el grupo A, hubo más solteras (p< 0,01) y en el grupo B, hubo más casadas (p< 0,001), más viudas (p < 0,01) y más que no trabajaban (p< 0,01). En el grupo A hubo más motivos médicos (p< 0,01). Destacaron: los miomas (15,53%), la menopausia precoz (2,91%) y el factor varón (2,91%). En el grupo B, sobresalieron: la obstrucción tubárica (9,52%), el factor varón (7,14%) y el cáncer de mama (7,14 %). No hubo diferencias significativas en los motivos psicosociales. Los más importantes fueron globalmente: ausencia de relaciones sexuales (3,44%); la pareja no quería tener hijos (3,44%), y trastornos psiquiátricos (2,06%). Conclusiones: Los motivos más frecuentes para no tener hijos fueron globalmente: la decisión personal (40,68%) y la falta de pareja adecuada (37,24%). A favor de las tra estaban globalmente el 78,62% de las mujeres, y en contra el 15,86%. El 10,34% de ellas practicaron tra, sin resultado favorable. El 42,06% no quería ser madre, definitivamente.

El objetivo fue determinar variables socio-sanitarias asociadas al uso adecuado de la citología cervico-uterina en las mujeres del área rural-Cartagena. Estudio analítico de corte transversal. Participaron 638 mujeres de 25 años con vida sexual activa o entre los 25 a 69 años que iniciaron o no su vida sexual del área rural-Cartagena. Se realizó un muestreo por conglomerados conformado por 15 corregimientos de la ciudad de Cartagena. La información recolectada fue incluida en una hoja de cálculo de Microsoft Excel y analizada mediante el programa SPSS versión 20. Se encontró que un 82,1% (524) de las mujeres se realizan la citología, pero solo el 35% (223) cumple con el esquema 1-1-3, se refleja mayor cumplimiento en aquellas mujeres cuya EPS fue la responsable del pago de dicho servicio. Las mujeres del área del área rural de Cartagena se han realizado la citología cervico uterina en algún momento de su vida sin embargo más de la mitad no cumplen con el esquema 1-1-3, la variable socio-sanitaria que se encuentra asociada estadísticamente con un mayor cumplimiento de esquema, es el no tener que pagar por este servicio, lo que influye positivamente en un uso adecuado de la Citología.

Introducción. La masturbación es un tema que encierra múltiples mensajes, tanto culturales negativos como sexológicos positivos. Esta diversidad es una ventana para explorar la sexualidad femenina. Objetivo. Establecer la prevalencia de masturbación en estudiantes universitarias del área metropolitana de Bucaramanga, Colombia. Materiales y métodos. Estudio descriptivo transversal con cuestionario de autorreporte. Se realizó análisis descriptivo e inferencial con nivel de significancia de p35, de estratos socioeconómicos altos, no religiosas o católicas con poco o nulo nivel de religiosidad, que estudian, trabajan como independiente y se autoidentifican como bisexuales. Conclusiones. 1 de cada 3 mujeres que no refiere masturbarse reporta disfunción orgásmica; de esta forma, las actividades masturbatorias son un factor protector de la disfunción. Es importante realizar educación desde la autoexploración para que la mujer pueda conocerse y disfrutar plenamente de su sexualidad.

Objetivo. El Sistema Informático Perinatal (SIP) ha marcado un hito en el uso de información sistematizada en la Región de las Américas. Lo que se ha aprendido ha contribuido al desarrollo de un modelo basado en un conjunto mínimo de indicadores (CMI). El objetivo del estudio fue describir el proceso histórico y metodológico de desarrollo, implementación y escalamiento territorial de un CMI para monitorizar y evaluar políticas, programas y servicios de salud de la mujer y perinatal orientado a la gestión (SIP-GESTIÓN). Métodos. El estudio se llevó a cabo en dos etapas: 1) validación en cuatro fases de un CMI en una red de hospitales: a) construcción del modelo teórico de indicadores, b) implementación de la investigación operativa, c) selección final de indicadores, y d) definición de patrones de referencia, y 2) escalamiento territorial. Resultados. Se identificaron 17 modelos de indicadores. El modelo inicial incluyó 177 indicadores agrupados en siete dimensiones (contexto, hábitos, accesibilidad, uso de servicios, calidad de cuidados, impacto materno-fetal, e impacto materno-neonatal) que se redujeron a 21 tras tres rondas Delphi. El modelo final (SIP-GESTIÓN) incluyó 40 indicadores. Se analizaron 240 021 partos (79,1%) de un total de 303 559 atendidos en las 122 maternidades seleccionadas en 24 Jurisdicciones (100%) de Argentina. La información se presenta a nivel nacional y desagregada por región sanitaria, provincia y hospital. Conclusiones. Este modelo permitió alcanzar altos niveles de cobertura y calidad de la información y escalamiento territorial y es útil para la gestión, la investigación y la reorientación de programas y políticas.

Objective: To describe the health care experience of women with cytological abnormalities with negative biopsy for malignancy or invasive cervix cancer within two months after diagnosis, in La Guajira and Nariño departments, Colombia, between 2016 and 2017. Methodology: The qualitative component of a mixed methods study of convergent triangulation is presented, through ten interviews and three focus groups. Nine women with cancer and six women with cytological abnormalities between 43 and 65 years of age participated; six of them were indigenous, and fourteen are enrolled to the subsidized health regime. Results: Participants referred negative experiences in the health care system as they face numerous barriers for the diagnosis and access to treatment. Such as, delay in the assignment and authorization of medical appointments, tests, and procedures; the out-of-pocket expenses for transportation to capital cities where the health service is provided; difficulties to find family support outside their communities of origin, and language barriers among women from indigenous communities. Besides, effective communication problems with health personnel were also presented, which lead some participants to misunderstanding their diagnosis or treatment and feeling that they were not being heard. The experience of having cancer led to suffering, rejection from people in their social environment, and depersonalization. Conclusions: Barriers of various kinds for the timely access to treatment are exacerbated by baseline characteristics such as poverty and belonging to ethnic groups, turning survival into a daily struggle, which goes beyond the specific characteristics of the disease itself. Background: HIV/AIDS is affecting more and more women. Adherence to antiretroviral treatment (ART) is decisive for those infected to achieve quality of life. Purpose: The aim of the present study is to determine the degree of adherence displayed by a group of women with HIV/AIDS and to identify those factors and circumstances which might influence their adherence to ART. Method: A sequential mixed model research design was applied. The subjects studied consisted of 86 women diagnosed with HIV/AIDS and treated with ART. Findings and discussion: Qualitative data were collected from 18 of these women. 59.3% of participants were non-adherents. The determining factors for non-adherence were level of education, age, income, as well as personal and/or interpersonal factors such as socio-familial support, perceived stigma, self-stigma and factors related to disease and treatment. Nursing care should take a genderfocused approach towards those psychological and personal factors inherent in the adherence process. [ABSTRACT FROM AUTHOR]

Introducción: Las tecnologías de la salud se refieren a todas aquellas herramientas que se utilizan para cuidar y educar a los demás. Frente a este concepto, esta revisión tiene como objetivo conocer las publicaciones científicas relacionadas con el desarrollo de las tecnologías para la salud de la mujer en Brasil. Materiales y métodos: Se realizó una revisión integrativa de tipo descriptivo mediante consulta en bases de datos como el Banco de Datos de Enfermería (BDENF), Literatura Latinoamericana y del Caribe en Ciencias de la Salud (LILACS) y Scientific Electronic Library Online (SciELO) entre 2009 y 2019. Resultados: A través de referencias cruzadas en las bases de datos mencionadas, se identificaron 46 estudios en el banco de datos BDENF, 280 en LILACS y 55 en SciELO, para un total de 381 estudios. Así mismo, se refinó la selección final a 23 estudios mediante la aplicación de filtros. Discusión: En el análisis de resultados se evidenció que el tema más referenciado fue el desarrollo de tecnologías blandas-duras. Al analizar el enfoque de estas tecnologías, se observó que uno de los temas más beneficiados del desarrollo de las tecnologías de la salud fue el de la educación en materia de salud de la mujer durante el embarazo y la lactancia. Conclusiones: Con esta revisión se demostró que es necesario seguir desarrollando, validando y aplicando todas las clases de tecnologías sanitarias, cuya calidad técnica y conceptual será analizada y verificada por profesionales que comprenden las necesidades reales del público objetivo. RESUMO Objetivo: refletir sobre a sacralização da amamentação e sua influência na sexualidade materna. Método: trata-se de estudo teórico-reflexivo sobre a experiência da amamentação, centrado nas concepções instituídas socialmente sobre o ato de amamentar e na vivência da sexualidade durante a maternidade. Resultados: a articulação do estudo com a realidade vivida por mulheres que amamentam possibilitou o reconhecimento da influência do paradigma vigente da sacralização da amamentação nos significados e escolhas da forma de agir das mulheres. As repercussões podem ser danosas tanto para a saúde materna quanto para a criança, por meio da vivência de sentimentos de culpa, vergonha ou até mesmo a decisão pelo desmame precoce. Conclusão: a amamentação é pautada no alicerce dos significados que envolve a divindade e a sacralização do ato de amamentar, anulando perspectivas da sexualidade e tendo como consequência uma visão distorcida e velada do que é vivido quando comparado com o que é desejado socialmente para esse período.

En este artículo, las autoras sintetizan los aspectos más destacados de esta ley y plantean los desafíos a tener en cuenta para su exitosa implementación.

Antecedentes: la pandemia COVID-19 tiene el potencial de impactar fuertemente en la agenda de la salud y los derechos sexuales y reproductivos desde la disminución de la calidad de los servicios de salud. Justificación: Uruguay se destaca por una situación privilegiada en lo referido a los derechos sexuales y reproductivos y por tal motivo es necesario, por un lado, sistematizar el impacto de la pandemia COVID-19 y, por otro, desarrollar medidas para prevenir dicho impacto sobre los servicios para mantener el más alto estándar de cuidados en lo relacionado a los derechos sexuales y reproductivos a pesar de la pandemia actual. Propósito: sistematizar el potencial impacto de la pandemia en los servicios de atención sanitaria de salud sexual y reproductiva y promover estrategias para detectar y prevenir dicho impacto. Metodología: se aborda el impacto de la pandemia COVID-19 sobre los derechos sexuales y reproductivos en dos dimensiones: la del modelo de retrasos en la atención de salud y la del análisis del impacto en cada derecho específico. Perspectivas: promover acciones de discriminación positiva desde los servicios de salud sexual y reproductiva a la vez que se realiza la vigilancia epidemiológica para facilitar el acoplamiento de las prestaciones en derechos sexuales y reproductivos a los cambios que se procesan en la respuesta sanitaria y social frente a la pandemia COVID-19.

Based on ethnographic work with undocumented immigrant women who developed depressive episodes due to various forms of violence, this article recovers the women’s experience of care in the clinic space of their host country. Through therapeutic itineraries of care, the article focuses on the case studies of two women of Mexican origin in New York City to describe, based on their experience, the communication and the encounter between the doctor and patient; the expectations of migrant women regarding mental health care services; and the explanatory models of depression of both the patients and the treating physicians. This analysis seeks to contribute to the knowledge regarding ways of treating the mental health problems of undocumented women in contexts of violence. [ABSTRACT FROM AUTHOR]

Objective: to evaluate the content validity, criterion-related validity, and internal consistency of contraceptive use assessment instrument. Method: an instrumental, cross-sectional study was carried out. An instrument was designed with 15 items, that was submitted to expert judgement and applied to 104 young women. Content validity was assessed with descriptive statistics, criterion-related validity with inferential statistics and internal consistency with Cronbach's Alpha. Results: the instrument had a content validity index per item of 1.0, per scale greater than .80 and for universal agreement of 1.0. Criterion-related validity did not show significant differences; Cronbach’s Alpha coefficients obtained to related items ranged between .70 and .79. Conclusion: the instrument items showed to be relevant and content valid; as for contraceptive use intention dimension the instrument items indicated an acceptable internal consistency.
