Two cases of decerebrate posturing that resolved following the administration of dextrose are reported. The literature on hypoglycemia-induced neurologic deficits is reviewed and an explanation for the quick remission of the decerebrate posturing is postulated.

The majority of patients with esophageal carcinoma present with dysphagia. The selection criteria and the staging of patients for potential curative resection as well as endoscopic therapeutic options for palliation of this symptom are discussed. The West Virginia University experience has included 14 cases treated with surgical resection and 20 cases treated with laser photoablative therapy. Concurrent radiation and/or chemotherapy have been used in many of the patients.

Foveomacular vitelliform degeneration, adult type (FVDAT) is a disciform, macular degeneration that is usually encountered in the fifth to seventh decade of life. It appears ophthalmoscopically similar to the vitelliform or "egg-yolk" stage of Best's disease (vitelliform degeneration), but shows no inheritance pattern. Electrooculographic (EOG) testing reveals a normal or only slightly reduced Arden ratio (light peak/dark trough). The prognosis is optimistic, as most patients retain reading vision throughout life. This case report describes an atypical case in which the signs and symptoms of FVDAT are not exactly those regularly seen. However, it does closely resemble the disease sufficiently to warrant this diagnosis.

The experiences from these initial efforts have been favorable without exception. Educational activities for both staff and residents have steadily increased, not merely in number but in quality of resources. A tremendous number of man hours have been productively uttilized which otherwise would have been wasted in travel. The opportunity for immediate medical and psychologic care from a variety of facilities is now available. Indeed, it is felt that the Interact system in conjunction with the Windsor Correctional Facility has demonstrated a model which can be replicated and expanded upon elsewhere.
