Abstract

Anti-phospholipid antibody syndrome (APS) has a broad spectrum of thrombotic and nonthrombotic clinical manifestations. The diagnosis requires a set of clinical criteria of thrombosis along with persistently positive anti-phospholipid antibody tests. In this report, we are presenting a case of APS, who is a 38-year-old male, presented with complains of seizures and found to have stroke, which on further investigation revealed to have been caused possibly from a left atrial mass. Therefore, high index of suspicion is required for the diagnosis of APS in young patients, who present with various neurological and cardiovascular manifestations, mostly secondary to thrombosis.

Abstract

We use the notions of left- and right-complete quasi-b-metric spaces and partial ordered sets to obtain a couple of common fixed-point results for strictly weakly isotone increasing mappings and relatively weakly increasing mappings, which satisfy a pair of almost generalized contractive conditions. To illustrate our results, throughout the paper, we give several relevant examples. Further, we use our results to establish sufficient conditions for existence and uniqueness of solution of a system of nonlinear matrix equations and a pair of fractional differential equations. Finally, we provide a nontrivial example to validate the sufficient conditions for nonlinear matrix equations with numerical approximations.

167. Individualized Homeopathic Medicines in the Management of Symptomatic Adenotonsillar Hypertrophy in Children: A Prospective Observational Study.

Abstract

Background Globally, adenotonsillar hypertrophy (ATH) is one of the most prevalent upper respiratory tract disorders of children, with associated troublesome symptoms such as sleep apnea and cognitive disturbances. In this study, we evaluated the potential role of individualized homeopathic medicines in the management of symptomatic ATH in children. Methods A multicenter prospective observational study was conducted at five institutes under the Central Council for Research in Homoeopathy, India. Primary and secondary outcomes (symptom score for adenoids, other symptoms of ATH, Mallampati score, tonsillar size, Sleep-Related Breathing Disorder of the Paediatric Sleep Questionnaire [SRBD-PSQ]) were assessed through standardized questionnaires at baseline and at 3, 6, 9 and 12 months. Radiological investigations for assessing the adenoid/nasopharyngeal (A/N) ratio were carried out at baseline, 6 and 12 months. All analyses were carried out using an intention-to-treat approach. Results A total of 340 children were screened and 202 children suffering from ATH were enrolled and followed up monthly for 12 months. Each patient received individualized homeopathic treatment based on the totality of symptoms. Statistically significant reductions in adenoid symptom score, Mallampati score (including tonsillar size), SRBD-PSQ sleep quality assessment and A/N ratio were found over time up to 12 months (p < 0.001). Homeopathic medicines frequently indicated were Calcarea carbonicum, Phosphorus, Silicea, Sulphur, Calcarea phosphoricum, Pulsatilla, Lycopodium and Tuberculinum. No serious adverse events were recorded during the study period. Conclusion This study suggests that homeopathic medicines may play a beneficial role in the management of symptomatic ATH in children. 175. Influence of skimmed coconut milk starter on the fermentation rate and biochemical quality attributes of virgin coconut oil

Abstract

Virgin coconut oil (VCO) is renowned for its numerous health benefits and widespread culinary applications. Fermentation is one of the simplest cold press methods used for the production of VCO. However, the longer fermentation time affects the scale ability and VCO quality. To enhance its production process and improve overall product quality, this study investigates the impact of using aerobic fermented skimmed coconut milk starter (SCMS) at different concentrations of the culture (5–15%) to maximize the yield and quality and minimize the cost, time, and energy during the production of virgin coconut oil (VCO) in fermentation process. It was observed that 24-h-old SCMS at 10% concentration recorded a maximum yield (19.2%) in the 19-h fermentation process. Individual and interactive effects of culture type (24-h and 48-h fermented SCMS) and its concentration significantly affect yield; however, its effects on physical (moisture content, viscosity, specific gravity, and refractive index) and chemical profile (free fatty acid, peroxide value, acid value, saponification value, iodine value, and total phenolic content) of the resultant VCO remain insignificant. The physicochemical properties of VCO prepared to utilize the SCMS-induced fermentation method confirmed the standards stipulated by the Food Safety and Standards Authority of India (FSSAI) and Asian and Pacific Coconut Community (APCC). SCMS is the cheapest alternative that could be exploited to enhance the fermentation rate during the production of VCO.

Abstract

Freshwater ecosystem degradation in the Turag River basin is increasing due to rising human settlement, reduced water flow during the dry season, new large land development projects, and toxic emissions caused by brick kilns. The population of river dolphins was estimated through surveys carried out from December 2012 to November 2013. Surveys were conducted on a fortnightly basis. The dolphins were counted within an 18.4 km stretch (known to be the most polluted stretch) of the 75 km long river. A total of 62 dolphin sightings were recorded, both as individual sightings and as groups. The highest monthly count was in August. The best-high-low estimates of 9-11-7 individuals were recorded in August 2013 in river ecology. Seasonal changes in spatial distribution were observed. The survey revealed that rising floodwater stimulated an increase in the dolphin population in the study area from July to October. The maximum encounter rate was also 0.49 sightings km−1 for the month of August. The sightings and encounter rates of dolphins were lower from December to July (dry and premonsoon period) likely due to reduced water flow and adverse water quality of this river.
