Abstract

Capacitive parasitic couplings in power electronics systems are gaining increased attention due to the emergence of the wide bandgap devices, with the main challenges being faster switching speeds and increased operating voltage capabilities causing increased voltage slew rate (dv/dt). The high dv/dt induces capacitive displacement currents through the channel of the power semiconductor devices, effectively reducing the switching speed, resulting in an increase in the Volt-Ampere integral, incurring surplus switching energy dissipation. This paper highlights that the parasitic power module gate high side capacitance should be analyzed in parallel to the high-side gate-drain capacitance and thus contributing to the “equivalent” high-side Miller capacitance, slowing down the switching speed of the high-side device compared to the low-side device in the half-bridge power module. Effectively this causes an imbalance in switching energy dissipation between high-side and low-side devices. A combined physics based and behavioural based digital twin simulation is used to evaluate high-side and low-side switching energy dissipation through double pulse testing. Simulations identify a significant increase in turn-on energy on the high-side device compared to the low side device, due to the equivalent high-side Miller capacitance increase. Possible reasons for why such significant deviation in switching energy dissipation between the high-side and low-side devices is unprecedented, are shared from the authors perspectives.

163. Treatment of intractable infected giant skin ulcer on a leg by continuous intra-arterial infusion of antibiotics, using a tunneled catheter technique without a port and final skin graft, avoiding recommended amputation of the leg

Abstract

The purpose of this paper is to report the usefulness of continuous intra-arterial infusion of antibiotics for thetreatment of intractable skin infection, using a tunneled catheter technique without a port. An elderly woman presented with pain in the left leg with a chronically infected intractable giant skin ulcer, forwhich amputation was recommended by former doctors to avoid the occurrence of skin cancer, because she hadmultiple（ five times in 14 years） skin grafts and the skin ulcer continuing for so many years. The patient requestedan alternative and interventional radiological treatment was selected. The pus in the ulcer yielded numerous Pseudomonas aeruginosa and Proteus vulgaris. She was treated by continuous intra-arterial infusion of an antibiotic, Sulbenicillin, using a tunneled catheter technique without a port for 18 days. The organisms in the ulcer were completely eradicated, and the ulcer gradually healed with decreased size. When the size of the ulcer became minimal a skin graft completed the treatment, avoiding amputation. No local or subcutaneous infection relating to the procedure was noted during or after the treatment. In a follow-up two years after the treatment, she was found to be living well without any complications.

186. Role of polyunsaturated fatty acids in Japanese patients with coronary spastic angina.

Abstract

n-3 polyunsaturated fatty acids (PUFAs) reduce the risk of ischemic heart disease. However, there are few reports of a relationship between n-3 PUFAs and coronary spastic angina (CSA). This study aimed to assess the age-dependent role of serum levels of fatty acid in patients with CSA. We enrolled 406 patients who underwent ergonovine tolerance test (ETT) during coronary angiography for evaluation of CSA. All ETT-positive subjects were diagnosed as having CSA. We categorized the patients by age and results of ETT as follows: (1) young (age ≤ 65 years) CSA-positive (n = 32), (2) young CSA-negative (n = 134), (3) elderly (age > 66 years) CSA-positive (n = 36), and (4) elderly CSA-negative (n = 204) groups. We evaluated the serum levels of eicosapentaenoic acid (EPA), docosahexaenoic acid (DHA), arachidonic acid, and dihomo-gamma-linolenic acid. In the young groups, the serum levels of EPA (64.3 ± 37.7 μg/mL vs. 49.4 ± 28.8 μg/mL, p = 0.015) and DHA (135.7 ± 47.6 μg/mL vs. 117.4 ± 37.6 μg/mL, p = 0.020) were significantly higher in the CSA-positive group than in the CSA-negative group, respectively. However, this was not the case with elderly groups. In the multivariate analysis in young groups, the serum levels of EPA (p = 0.028) and DHA (p = 0.049) were independently associated with the presence of CSA, respectively. Our results suggested that the higher serum levels of EPA and/or DHA might be involved in the pathophysiology of CSA in the young population but not in the elderly population. Methods The study included 224 pregnant women during their first, second, or third trimesters of pregnancy who completed the PPAQ-C on their first visit and wore a uniaxial accelerometer (Lifecorder; Suzuken Co. Ltd) for 7 days. One week after the first visit, we collected the data from the uniaxial accelerometer records, and the women were asked to complete the PPAQ-C again. Results We used intraclass correlation coefficients to determine the reliability of the PPAQ-C. The intraclass correlation coefficients were 0.77 for total activity (light and above), 0.76 for sedentary activity, 0.75 for light activity, 0.59 for moderate activity, and 0.28 for vigorous activity. The intraclass correlation coefficients were 0.74 for "household and caregiving", 0.75 for "occupational" activities, and 0.34 for "sports/exercise". Validity between the PPAQ-C and accelerometer data was determined by Spearman correlation coefficients. Although there were no significant correlations for moderate activity (r = 0.19, P > 0.05) or vigorous activity (r = 0.15, P > 0.05), there were significant correlations for total activity [light and above; r = 0.35, P < 0.01)] and for light activity (r = 0.33, P < 0.01). Conclusions for Practice The PPAQ-C is reliable and moderately accurate for measuring physical activity in pregnant Chinese women.
