Abstract

e-ASTROGAM (‘enhanced ASTROGAM’) is a breakthrough Observatory space mission, with a detector composed by a Silicon tracker, a calorimeter, and an anticoincidence system, dedicated to the study of the non-thermal Universe in the photon energy range from 0.3 MeV to 3 GeV – the lower energy limit can be pushed to energies as low as 150 keV for the tracker, and to 30 keV for calorimetric detection. The mission is based on an advanced space-proven detector technology, with unprecedented sensitivity, angular and energy resolution, combined with polarimetric capability. Thanks to its performance in the MeV–GeV domain, substantially improving its predecessors, e-ASTROGAM will open a new window on the non-thermal Universe, making pioneering observations of the most powerful Galactic and extragalactic sources, elucidating the nature of their relativistic outflows and their effects on the surroundings. With a line sensitivity in the MeV energy range one to two orders of magnitude better than previous generation instruments, e-ASTROGAM will determine the origin of key isotopes fundamental for the understanding of supernova explosion and the chemical evolution of our Galaxy. The mission will provide unique data of significant interest to a broad astronomical community, complementary to powerful observatories such as LIGO-Virgo-GEO600-KAGRA, SKA, ALMA, E-ELT, TMT, LSST, JWST, Athena, CTA, IceCube, KM3NeT, and LISA.

154. Efficacy of Carvykti in CARTITUDE-4 Versus Other Conventional Treatment Regimens for Lenalidomide-Refractory Multiple Myeloma Patients Using Inverse Probability of Treatment Weighting

Abstract

Introduction:CARTITUDE-4 is a phase 3 randomized controlled trial (RCT) assessing the efficacy and safety of CARVYKTI (ciltacabtagene autoleucel; cilta-cel) versus pomalidomide, bortezomib and dexamethasone (PVd) or daratumumab, pomalidomide and dexamethasone (DPd) in patients with relapsed and refractory multiple myeloma (RRMM) who have received one to three prior line(s) of therapy that included an immunomodulatory agent (IMiD) and a proteasome inhibitor (PI), and who are refractory to lenalidomide. CARTITUDE-4 demonstrated superiority of cilta-cel over PVd and DPd on progression-free survival (PFS) and response rates (overall response rate [ORR], very good partial response [VGPR] or better, and complete response [CR] or better [CR or stringent CR]). Other available therapies for the treatment of this patient population where individual patient data was available for analysis include: daratumumab in combination with bortezomib and dexamethasone (DVd), bortezomib in combination with dexamethasone (Vd), carfilzomib in combination with daratumumab and dexamethasone (DKd) or dexamethasone alone (Kd), and pomalidomide in combination with dexamethasone (Pd). This analysis aims to compare efficacy outcomes in patients randomized to the cilta-cel arm (i.e., patients apheresed or infused with cilta-cel) in CARTITUDE-4 vs each of these regimens in the same target population.

Abstract

The application of bleaching agents before placement of resin-bonded fixed appliances significantly, but temporarily, reduces bond strength to tooth structure. Antioxidants have been studied as a means to remove residual oxygen that compromises bonding to bleached enamel. This in vitro study evaluated whether green tea (GT) could restore the shear bond strength between bonded orthodontic brackets and bleached enamel. Six experimental groups were compared: group 1, no bleaching plus bracket bonding (positive control); group 2, bleaching with 35% hydrogen peroxide (HP) plus bracket bonding (negative control); group 3, 35% HP plus 10% sodium ascorbate (SA) plus bracket bonding; group 4, 35% HP plus 10% GT plus bracket bonding; group 5, no bleaching plus 10% SA plus bracket bonding; group 6, no bleaching plus 10% GT plus bracket bonding. 164. In‐Flight Observation of Gamma Ray Glows by ILDAS

Abstract

An Airbus A340 aircraft flew over Northern Australia with the In‐Flight Lightning Damage Assessment System (ILDAS) installed onboard. A long‐duration gamma ray emission was detected. The most intense emission was observed at 12 km altitude and lasted for 20 s. Its intensity was 20 times the background counts, and it was abruptly terminated by a distant lightning flash. In this work we reconstruct the aircraft path and event timeline. The glow‐terminating flash triggered a discharge from the aircraft wing that was recorded by a video camera operating onboard. Another count rate increase was observed 6 min later and lasted for 30 s. The lightning activity as reported by ground networks in this region was analyzed. The measured spectra characteristics of the emission were estimated. 165. A Multidisciplinary Infection Control Bundle to Reduce the Number of Spinal Cord Stimulator Infections

Abstract

To investigate the effect of a quality improvement project that resulted in an infection control bundle to reduce the number of spinal cord stimulator (SCS) infections. The study was performed in a single center for neuromodulation from January 1, 2014, through May 31, 2016. In response to a high number of surgical site infections (SSIs) after SCS surgery, a multidisciplinary team analyzed the surgical process and developed an infection prevention bundle consisting of five items: 1) showering and decolonization for five days prior to surgery and showering in the hospital on the morning of surgery; 2) performing the SCS implantation as the first in the daily operating room (OR) program; 3) maintaining a minimal number of people in the OR; 4) providing home care nurses with a folder with SCS wound care instructions including pictures; 5) giving oral specific wound care instructions to patients. The number of infections was calculated for the baseline, implementation, and sustainability phases. A total of 410 SCS surgeries were performed during the study period. In the preintervention phase, 26/249 (10.4%) SCS surgeries were infected. During the implementation and sustainability phase, 2/59 (3.4%) and 1/102 (1.0%) SCS surgeries were infected, respectively. The reduction in the number of infections in pre and postintervention phase was statistically significant (p= 0.003). Multidisciplinary measures to reduce SSIs reduced the number of SCS associated infections in our study setting.

Abstract

Renal artery embolization is performed before radical nephrectomy (RN) for renal mass in order to induce preoperative infarction and to facilitate surgical intervention through decrease of intraoperative bleeding. Moreover, in metastatic renal cancer it seems to stimulate tumour-specific antibodies, even if no established benefits in clinical response or survival have been reported. The role of preoperative renal artery embolization (PRAE) in management of renal masses has been often debated and its real benefits are still unclear. Nevertheless, in huge and complex renal masses, which are often characterized by a high and anarchic blood supply and rapid local invasion, radical nephrectomy can be challenging even for skilled surgeons. The aim of this prospective randomized study was to evaluate the effectiveness and safety of PRAE in complex masses by comparing perioperative outcomes of RN with and without PRAE.

Nucleic acid sequence-based amplification (NASBA; bioMérieux, Boxtel, The Netherlands) is a commercially available amplification procedure that uses RNA as the target. It makes use of the simultaneous enzymatic activities of avian myeloblastosis virus reverse transcriptase (AMV-RT), RNase H, and T7 RNA polymerase, under isothermal conditions. The constant temperature maintained throughout the amplification reaction allows each step of the reaction to proceed as soon as an amplification intermediate becomes available. 171. A pipeline to link meteorological information and TGFs detected by AGILE

Abstract

Terrestrial gamma ray flashes (TGFs) are brief (approximately hundreds of microseconds) intense gamma ray emissions coming from Earth's atmosphere (∼15 km above sea level), correlated with thunderstorms and atmospheric electric activity. Since their unexpected discovery in the early 1990s by the Burst And Transient Source Experiment/Compton Gamma Ray Observatory, TGFs have been further investigated by several satellites devoted to high‐energy astrophysics. The Astrorivelatore Gamma ad Immagini LEggero (AGILE) mission turned out to be particularly suitable to detect these events, due to a very wide energy range (up to 100 MeV), an optimized triggering system, and a unique low‐inclination near‐equatorial orbit (2.5°). We describe a detection system, developed for the AGILE satellite, whose aim is to provide real‐time meteorological information on each detected TGF. We take advantage of data acquired by geostationary satellites to promptly identify the associated storm and follow its evolution in space and time, in order to study its previous onset and development. Data from Low‐Earth Orbit meteorological satellites, such as the Global Precipitation Mission, as well as ground measurements from lightning detection networks, can be integrated in the pipeline. This system allows us a prompt characterization of the ground meteorological conditions at TGF time which will provide instrument‐independent trigger validation, fill in a database for subsequent statistical analysis, and eventually, on a longer term perspective, serve as a real‐time alert service open to the community. 172. Detection of multiple terrestrial gamma‐ray flashes from thunderstorm systems

Abstract

Since their discovery, Terrestrial Gamma ray Flashes (TGFs) exhibited an evident correlation with thunderstorms and lightning activity. The fleeting nature of these events and the heavy absorption of gamma rays in the lowest atmospheric layers severely hamper the observation of this phenomenon, making us reveal just a small fraction of a probably much wider population. As each thunderstorm produces a large amount of lightning discharges during its lifetime, it is reasonable that even a large amount of TGFs are produced during the same event. However, detection of multiple TGFs coming from the same storm is difficult to perform, as it requires the constant monitoring of a spatially limited geographic region: this is not an easy task to perform for satellites on high‐inclination orbits that make them experience nonnegligible latitudinal shifts at each orbital passage over a certain region, preventing the monitoring of a limited geographic region throughout successive overpasses. In this perspective, the quasi‐equatorial (2.5°) orbit of the Astrorivelatore Gamma ad Immagini LEggero (AGILE) satellite ensures a minimal latitudinal shift when flying over the same region at successive passages, allowing for the follow‐up of thunderstorms in time. We exploit this feature of the AGILE satellite to search for multiple TGFs coming from the same geographic region and, in particular, from the same thunderstorm. We carry out this search on the AGILE TGF database (2009–2016), ending up with a sample of 79 systems producing more than one TGF, both during the same overpass and up to four overpasses after. Data acquired by geostationary meteorological satellites and cross correlation with radio sferics detected by World Wide Lightning Location Network are used to support this investigation. The AGILE satellite for the first time clearly establishes the multiple occurrences of TGFs from convective thunderstorms, both on timescales of minutes to several hours. Abstract

Alzheimer′s disease has recently emerged as a possible field of application for PDE4D inhibitors (PDE4DIs). The great structure similarity among the various PDE4 isoforms and, furthermore, the lack of the full length crystal structure of the enzyme, impaired the rational design of new selective PDE4DIs. In this paper, with the aim of exploring new insights into the PDE4D binding, we tackled the problem by performing a computational study based on docking simulations combined with molecular dynamics (D‐MD). Our work uniquely identified the binding mode and the key residues involved in the interaction with a number of in‐house catechol iminoether derivatives, acting as PDE4DIs. Moreover, the new binding mode was tested using a series of analogues previously reported by us and it was used to confirm their key structural features to allow PDE4D inhibition. The binding model disclosed within the current computational study may prove to be useful to further advance the design and synthesis of novel, more potent and selective, PDE4D inhibitors.

Abstract

This clinical report describes an adult patient referred for orthodontic treatment with mini-implants as anchorage to correct the root angulation of maxillary lateral incisors. The purpose of this report was to demonstrate the versatility of mini-implants placed in a vertical direction in esthetic areas. During orthodontic treatment, some aspects must be observed to preserve the interim restoration against the occlusal loads to avoid screw fracture. A fixed appliance was placed to correct the position of the maxillary anterior teeth and to complete the treatment. Acceptable esthetics and function were achieved. [ABSTRACT FROM AUTHOR]

Abstract

At the end of March 2015 the onboard software configuration of the Astrorivelatore Gamma a Immagini Leggero (AGILE) satellite was modified in order to disable the veto signal of the anticoincidence shield for the minicalorimeter instrument. The motivation for such a change was the understanding that the dead time induced by the anticoincidence prevented the detection of a large fraction of Terrestrial Gamma‐Ray Flashes (TGFs). The configuration change was highly successful resulting in an increase of one order of magnitude in TGF detection rate. As expected, the largest fraction of the new events has short duration (<100 μs), and part of them has simultaneous association with lightning sferics detected by the World Wide Lightning Location Network. The new configuration provides the largest TGF detection rate surface density (TGFs/km2/yr) to date, opening prospects for improved correlation studies with lightning and atmospheric parameters on short spatial and temporal scales along the equatorial region. Abstract

Introduction: In the end of 90's the adoption of mini-implants as Anchorage allowed a paradigm change influencing even the way of thinking orthodontic mechanics. The overlapping of the specialties of Orthodontics and Im-plantology started with orthodontic preparations for prosthetic implants insertion, aroused with the use of palatal implants and late with the introduction of mini-implants. The improvement of mini-implants insertion technique with the appearing of self-drilling screws has allowed orthodontists to plan and to place this precious Anchorage piece. Taking into account the versatility of positioning of these screws it was developed a concept that allows the construction of force action lines aiming at optimize the planning and predictability of orthodontic motion. Objective: To present some clinical results treatments conduct using Centrex System of orthodontic treatment, approximating the force line action of resistance center of units to be moved. Abstract

Objective: Early identification of craniofacial morphological characteristics allows orthopedic segmented interventions to attenuate dentoskeletal discrepancies, which may be partially disguised by natural dental compensation. To investigate the morphological characteristics of Brazilian children with Class III malocclusion, in stages I and II of cervical vertebrae maturation and compare them with the characteristics of Class I control patients. Methods: Pre-orthodontic treatment records of 20 patients with Class III malocclusion and 20 control Class I patients, matched by the same skeletal maturity index and sex, were selected. The craniofacial structures and their relationships were divided into different categories for analysis. Angular and linear measures were adopted from the analyses previously described by Downs, Jarabak, Jacobson and McNamara. The differences found between the groups of Class III patients and Class I control group, both subdivided according to the stage of cervical vertebrae maturation (I or II), were assessed by analysis of variance (ANOVA), complemented by Bonferroni's multiple mean comparisons test. Results: The analysis of variance showed statistically significant differences in the different studied groups, between the mean values found for some angular (SNA, SNB, ANB) and linear variables (Co - Gn, N - Perp Pog, Go - Me, Wits, S - Go, Ar - Go). Conclusion: Assessed children displaying Class III malocclusion show normal anterior base of skull and maxilla, and anterior positioning of the mandible partially related to increased posterior facial height with consequent mandibular counterclockwise rotation. Introduction: Evaluation of upper airway space is a routine procedure in orthodontic diagnosis and treatment planning. Although limited insofar as they provide two dimensional images of three-dimensional structures, lateral cephalometric radiographs have been used routinely to assess airway space permeability. Cone-Beam Computed Tomography (CBCT) has contributed to orthodontics with information concerning the upper airway space. By producing three-dimensional images CBCT allows professionals to accurately determine the most constricted area, where greater resistance to air passage occurs. Objectives: The purpose of this article is to enlighten orthodontists on the resources provided by CBCT in the diagnosis of possible physical barriers that can reduce upper airway permeability. Abstract

Abstract The authors reviewed the factors involved in the etiology, diagnosis and treatment of temporomandibular joint disorders (TMD). Although essential, specific criteria for inclusion and exclusion in TMD diagnosis have shown limited usefulness. Currently, the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) offer the best evidence-based classification for the most common TMD subgroups. The RDC/ TMD includes not only methods for physical diagnostic classification, comprised in Axis I, but also methods to assess the intensity and severity of chronic pain and the levels of non-specific depressive and physical symptoms, in Axis II. Although historically malocclusions have been identified as risk factors for the development of TMD--including those predominantly joint-related--in many cases the association established between these variables seems to have taken opposite directions. Regarding internal TMJ derangements, the results of studies on the induced shortening of the mandibular ramus, secondary to anterior articular disk displacement, indicate that repositioning the displaced disk in children or young adolescents may make more sense than previously imagined. The therapeutic use of dietary supplements, such as glucosamine sulfate, seems to be a safe alternative to the anti-inflammatory drugs commonly used to control pain associated with TMJ osteoarthritis, although evidence of its effectiveness for most TMD patients has yet to be fully established.
