One of the most important reasons for selecting public private partnerships (PPP) as a procurement method for infrastructure projects, is the ability to deliver the best value for money (VFM) under certain conditions. Several studies have identified and analysed the factors that contribute the value for money creation, and others have identified the most viable project success evaluation criteria. This paper aims to empirically examine the relationship between the PPP project ex-post performance and the value for money creation factors. 92 PPP experts from the UK and the UAE participated in this research via an online questionnaire. The questionnaire consisted of VFM factors, PPP performance indicators and several demographic questions. It was found, through the Pearson Correlation Coefficient and Regression analyses, that there was a significant and positive relationship between improved facilities factor to the users and time, quality, economic benefits and scope variation performance of PPP projects. The results of the analysis also showed clear output specification and early service delivery factors contribute only to the performance in terms of service and cost. Interestingly, the results of the analysis showed that optimisation of assets efficiency factor contributed negatively to the scope of variation of projects. The research also, through the regression analysis, showed that sector, experience and job position have no mediating role between PPP ex-post performance and VFM factors. This study confirmed the need for strict criteria in order to describe the achievement of success in PPP projects. These criteria ensure that the actual practice can be measured against the criteria which are set at the start of the project. This paper expanded empirically on the current literature on the relationship between the VFM and PPP project performance (specifically the ex-post performance).

In the wake of the Covid-19 pandemic, public private partnerships (PPP) can prove useful to close the healthcare investment gap and accelerate long-term recovery, by matching public and private money. Despite their extensive application in the delivery of major infrastructure projects globally, their performance remains contested. Drawing on a unique set of data of PPP contracts for healthcare investments in Italy, the paper explores the antecedents (namely, policies, institutions, and contracts), that have influenced the capacity of PPP contracts to deliver performance, measured in terms of value for money (VfM), and contract stability. Results show that although explicit policy goals have a clear contribution, VfM can benefit from competent central PPP institutions, which should play a hands-on role in the planning, design, and project management activities. At the same time, centralised PPP units, if not combined with local competent authorities and strong governance mechanisms, may increase the risk of contract renegotiation.

A model of investment in projects with uncertain construction time is presented in which both the cash-flows upon completion and construction progress are modeled as diffusions. The decision maker has three sequential options: (i) to start construction, (ii) to abandon the project during construction, and (ii) to start operations or abandon after completion. Existence of a solution and some of its qualitative properties are proved. A Markov-chain approximation is used to numerically approximate the solution is introduced. The model is applied to a real-world railway project for which it is found that it does not represent value for money (VfM) and is unlikely to do so in the next decade. A comparative statics analysis reveals that the presence of an abandonment option can increase project value substantially and can speed up investment. Finally, it is argued that the commonly-used benefit-to-cost ratio (BCR) is not an appropriate measure to summarize a project’s VfM. Instead, the value ratio (VR) is suggested as an alternative.

This paper aims to explore the primary factors that affect the perception of domestic tourists towards the service quality of budget hotels in Egypt. The factors were divided into three main constructs: physical quality, service quality and value for money. The primary findings reveal that the budget hotel location, cleanliness, maintenance, comfort level, hotel staff service, value for money room rates and food and beverage values are among the significant factors that influence domestic tourist satisfaction within predefined constructs. Primary Elements associated with the budget hotel stay were also ranked according to guest satisfaction.

Value can be defined as quality in relation to cost. The relationship between these two dimensions, in the context of a building's design, has to be properly understood if building owners (clients) are to be confident of achieving value for money in the outcome. This thesis proposes a cost-quality modelling technique that evaluates the cost and quality dimensions of early design options, enabling the relationship between them to be explored. The research has revealed problems associated with current practice in terms of early cost advice and the modelling techniques used for providing that advice. Inadequate consideration of the quality dimension was highlighted as a significant cause, due to its perceived elusiveness and complexity. The findings of the research, however, suggest that quality is amenable to a systematic treatment with respect to its representation and measurement. Results of a survey revealed that quality parameters, which are meaningful and necessary for early design generation and evaluation, are not as diverse as first believed. The results also revealed that quality can be examined and expressed in terms of its key contributing parameters. These findings, amongst others, have provided the basis for the development of a framework for quality benchmarks. A cost-quality modelling technique is proposed, based on the integration of the framework for quality benchmarks, a multi-criteria decision-making technique, and graphical and statistical methods. The proposed modelling technique has been applied to example building projects in order to test its sufficiency. Results show that the technique is able to guide the client and design team towards an improved solution in terms of cost and quality.

Objective: This study aimed to quantify heterogeneity in the value for money of precision medicine (PM) by application types across contexts and conditions and to quantify sources of heterogeneity to areas of particular promises or concerns as the field of PM moves forward., Methods: A systemic search was performed in Embase, Medline, EconLit, and CRD databases for studies published between 2011 and 2021 on cost-effectiveness analysis (CEA) of PM interventions. Based on a willingness-to-pay threshold of one-time GDP per capita of each study country, the net monetary benefit (NMB) of PM was pooled using random-effects meta-analyses. Sources of heterogeneity and study biases were examined using random-effects meta-regressions, jackknife sensitivity analysis, and the biases in economic studies checklist., Results: Among the 275 unique CEAs of PM, publicly sponsored studies found neither genetic testing nor gene therapy cost-effective in general, which was contradictory to studies funded by commercial entities and early stage evaluations. Evidence of PM being cost-effective was concentrated in a genetic test for screening, diagnosis, or as companion diagnostics (pooled NMBs, $48,152, $8,869, $5,693, p < 0.001), in the form of multigene panel testing (pooled NMBs = $31,026, p < 0.001), which only applied to a few disease areas such as cancer and high-income countries. Incremental effectiveness was an essential value driver for varied genetic tests but not gene therapy., Conclusion: Precision medicine's value for money across application types and contexts was difficult to conclude from published studies, which might be subject to systematic bias. This paper analyzes the renegotiation problem in the context of public-private partnership projects. Utilizing a game-theoretic approach, an equilibrium is found in which the government finds that accepting renegotiation can be efficient. A first indicator is proposed based the public sector comparator (PSC) that can be estimated by policymakers as an additional tool when deciding about renegotiation. A second more theoretical indicator is derived to analyze the economic and financial variables affecting renegotiation. This indicator is applied to four case studies in different countries (England, Taiwan, Portugal and China) and the results suggest that the model performs well.

This study aims to analyze the performance accountability of the Bengkalis District Social Servicethrough the principle of value for money. The type of research used is qualitative. Data collectiontechniques used are documentation and interviews. The data obtained are in the form of Performance Reports of Government Agencies of the Bengkalis Regency Social Service in 2020 and 2021. The results of this study indicate that in 2020 the economic calculation with a ratio value of 97.12% is categorized as economic, the efficiency calculation with a ratio value of 102.79% is said to be efficient. , and the calculation of effectiveness with a ratio value of 106.76%is categorized as effectiveness. In 2021 economic calculations with a ratio value of 88.91% are categorized as economical, efficiency calculations with a ratio value of 110.67% are categorizedas efficient, and effectiveness calculations with a ratio value of 99.14% are categorized as ineffective. Thus, the performance of the Bengkalis District Social Service in 2020 and 2021 in terms of economic, efficiency, and effectiveness (3E) elements has not met the value for money principle.

Cost‐effectiveness analysis is increasingly relevant in humanitarian action. The cost of response has increased exponentially in the past decade, alongside concurrent donor budget restrictions. However, there remains limited comprehension and application of these methods in this field. This paper documents methods developed for use within Action Against Hunger, an international humanitarian organisation, in response to a lack of understanding of this topic within the humanitarian community and limited evidence of the cost‐effectiveness of humanitarian action. These methods encompass costs to both implementing institutions and participating communities. Activity‐based cost analyses are conducted to assess resources per programme activity. Cost‐effectiveness is evaluated using successful programme outcomes, and uncertainty is appraised via sensitivity analysis. This paper aims to advance knowledge, stimulate discussion, and promote the adoption of cost‐effectiveness methods for building the evidence base for humanitarian action, including consideration of community costs, to enable analytical outputs that are useful for managers and policymakers alike. Customers' Perception of telecommunication companies is an important factor for service providers to enhance their services and seek advantages in a competitive market to gain customers' trust and loyalty. This paper represents a study of customer's perception for the three leading Mobile Service Providers „Zain”, „Ooredoo” and „Viva” in Kuwait's telecommunication market. Statistical analysis was done, using MINITAB and SPSS Software, to show the distribution of customers for the three mobile service providers and to form an idea about the perception of customers for the three providers. Different statistical techniques have been used and the results showed that Zain leads the market for the Network Quality, Viva has more advantage in customer service, and Ooredoo offers more services for customers. In addition, we have found that Age has a significant effect of which younger people prefer to use Viva, Gender, as well as the interaction between Gender and Age, has no significant effect on choosing the Mobile Service Provider, which gives an indication for the Mobile service providers about their targeted group age of customers. Public‒private partnerships (PPPs) are often promoted as a means to lower the costs and increase the quality and value for money (VfM) of public construction and infrastructure projects. While the increasing capital stock of PPPs warrants evaluation of their performance there has until now been limited systematic assessment of PPP versus conventional public procurement. This article contributes to the literature by presenting the findings of a systematic review of empirical peer‐reviewed studies comparing the costs, quality, and/or VfM of infrastructure PPP projects with conventional public procurement. The international literature suggests that PPPs are on average more costly and provide approximately similar VfM as conventional procurement. The number of empirical evaluations is limited, however, and evidence on the quality of infrastructure facilities is particularly scarce. While infrastructure PPPs continue to proliferate, systematic assessment of their performance is warranted to assist policy‐makers in choosing the procurement method that offers best value for taxpayers, users and society. Advocates of public‐private partnerships (PPPs) argue that they can deliver public infrastructure more efficiently than traditional procurement through timelier completion and superior value for money. Despite these claims comparative analysis of the performance of both procurement methods has received scant attention in the PPP literature to date. This paper addresses this issue by providing an in‐depth, case‐based comparison of PPP versus traditional procurement in the schools sector in Ireland. Through detailed semi‐structured interviews with key stakeholders and an examination of the available documentation, we assess whether the key objectives of using PPP have been achieved. Overall, we find no evidence that PPP leads to faster delivery of infrastructure when the overall procurement process from contract notice to delivery is accounted for. The Indian telecom sector is growing at a fast pace and has changed the market dynamics in a revolutionary way. Over the past few months, there has been competition among mobile service providers like Reliance Jio, Bharti Airtel, Idea Cellular and Vodafone in providing attractive offers to delight customers. Each of the major players is trying to differentiate themselves from the other by connecting with the customers at different levels. The primary objective of this study is to analyze factors affecting the preference of mobile service providers and their effect on overall customer satisfaction. The study used exploratory and conclusive research design. The data were collected from the existing users of mobile service of various telecom service providers. In the present study brand image, promotion, value for money and trust were found to be the most important factors associated with the choice of mobile service operators. But when these factors were assessed in terms of overall customer satisfaction, it was found that promotion is the most important factor followed by brand image, value for money and trust because promotion is more influential in customer's decision making. The term “value for money” (VFM) is a poorly understood concept in practice. There is a lack of clarity in its application, especially in the distinction between “value” and “money” when assessing VFM in investment evaluation and decision making. This article clarifies the ambiguities in VFM by conceptually deconstructing its meaning using a “value triangle model” and, on that basis, we propose a six-step VFM assessment procedure, which has been tested during VFM workshops on infrastructure projects. Departing from the existing concept of value in value management and value engineering literature, the value triangle model rests on Daniel Bernoulli’s concept of value as captured in his famous quotation: “The value of an item must not be based upon its price but rather on the utility which it yields” (Bernoulli, 1954, p. 24). We deconstruct value into three components using everyday language: useful purpose, beneficial outcomes, and important features, making it applicable to any VFM decision context. By making a clear distinction between value and money and, by treating value and value for money as separate notions, greater clarity of VFM is provided, thus enabling more effective measurement in practice. This paper examines the possibilities and limitations in pursuing value for money (VfM) in public procurement. There is ambiguity about the VfM concept and the methods that public procurers should be using. It is difficult for decision-makers to thoroughly understand the economic facts and valuations underlying VfM. The authors explain the conceptual VfM challenges and present a path to overcoming these with a lifecycle costing (LCC) approach in an indepth case study in the Finnish waste management context. Policy-makers and managers need to be aware that different stakeholders could have different views of what value for money (VfM) is. The authors present a new way of overcoming VfM challenges. This study examines major café experience attributes and their impacts on customers' attitudinal and behavioural outcomes in the New Zealand café industry. Based on an extended model of the consumption system approach, a set of hypotheses were developed and tested using hierarchical multiple regression. Using a sample of 205 respondents, the study found that service quality is the strongest predictor of all outcome variables (i.e., value for money, customer satisfaction, and customer loyalty intentions) which were predicted by either tangible or intangible café experience attributes. Background: Over the past decade, many hospitals have adopted hybrid operating rooms (ORs). As resources are limited, these ORs have to prove themselves in adding value. Current estimations on standard OR costs show great variety, while cost analyses of hybrid ORs are lacking. Therefore, this study aims to identify the cost drivers of a conventional and hybrid OR and take a first step in evaluating the added value of the hybrid OR. Methods: A comprehensive bottom-up cost analysis was conducted in five Dutch hospitals taking into account: construction, inventory, personnel and overhead costs by means of interviews and hospital specific data. The costs per minute for both ORs were calculated using the utilization rates of the ORs. Cost drivers were identified by sensitivity analyses. Results: The costs per minute for the conventional OR and the hybrid OR were €9.45 (€8.60-€10.23) and €19.88 (€16.10- €23.07), respectively. Total personnel and total inventory costs had most impact on the conventional OR costs. For the hybrid OR the costs were mostly driven by utilization rate, total inventory and construction costs. The results were incorporated in an open access calculation model to enable adjustment of the input parameters to a specific hospital or country setting. Conclusion: This study estimated a cost of €9.45 (€8.60-€10.23) and €19.88 (€16.10-€23.07) for the conventional and hybrid OR, respectively. The main factors influencing the OR costs are: total inventory costs, total construction costs, utilization rate, and total personnel costs. Our analysis can be used as a basis for future research focusing on evaluating value for money of this promising innovative OR. Furthermore, our results can inform surgeons, and decision and policy-makers in hospitals on the adoption and optimal utilization of new (hybrid) ORs.

Background: Parkinson’s disease (PD) is a complex and debilitating condition that requires care from a multispecialty team. The Integrated Parkinson Care Network (IPCN) is an innovative pragmatic care model that focuses on integrated care, self-management support and technology-enabled care. Objective: This study aims to estimate the costs of the IPCN and assess whether benefits gained from the intervention offset its costs based on a single center experience. Methods: We conducted a return on investment (ROI) analysis of the IPCN from a societal perspective. The ROI for the IPCN was estimated as a ratio of the net savings and the intervention cost. The intervention cost was calculated as a sum of set-up and implementation costs. Cost savings was measured as the absolute reduction in the societal costs realized by PD patients. A positive ROI indicated that savings generated from the intervention offset its cost. Results: The total cost of the IPCN for 100 PD patients was C$135,669, or C$226 per patient per month. IPCN was associated with the reduction in societal cost of C$915 per patient per month (95%CI: –2,782, 951). The ROI per PD patient per month for the IPCN was 3.08 (95%CI: –0.60, 22.93), suggesting that for every C$1 invested in the IPCN, C$4.08 is gained through reduction in societal costs. The returns were greater among advanced PD patients. Conclusion: The IPCN has the potential to offer a good return on investment for PD patients, and its value for money is higher among advanced PD patients.

Reflecting on Loader and White ’s (2018) suggestion that the labour of private security workers is difficult to ‘commodify’, this paper uses original empirical data to show that there are four elements to what private security workers ‘do’ for victims of domestic violence when contracted by domestic violence advocacy services: they provide (a) practical ‘target-hardening’ measures, (b) ‘expert’ advice on security, (c) forms of ‘security therapy’ as workers talk clients through their safety-based anxieties and (d) forms of ‘security theatre’ as workers provide the appearance of providing security despite the efficacy often being unclear or uncertain. Each of these elements have significant risks that can threaten the interests of victims and domestic violence services.

Public-private partnerships (PPPs) have proliferated and adapted to public development in recent decades; within it, the value for money (VfM) assessment defines the feasibility of the project procurement model as one of the essential components of PPP. However, evaluating the VfM in PPPs remains problematic. Given concerns about PPP profitability, a more integrated VfM evaluation is urgently needed to manage multiple indicators along the project lifecycle. Building information management (BIM), popular in architecture, engineering, and construction, provides resources that could support the VfM to a great extent. This paper uses a review approach to identify the current issues that are affecting VfM assessments and suggests that BIM, functioning throughout the PPP lifecycle, could support decision-making in VfM processes in order to satisfy service targets.

Background - Generation Z is the future larger group of consumers. Members of thisgeneration are key in the strategy of brands to remain competitive in the long-term. Buttheir loyalty is more difficult to get compared to other generations. It is assumed that itis linked to their specific expectations and the way brands handle them. Thus, theobjective of this study is to determine the real influence of these particular expectationson the two dimensions of Gen Z brand loyalty, attitudinal and behavioural.Method - Several hypotheses about the relationships between attitudinal loyalty,behavioural loyalty, ethical position, omnichannel experience, value for money,entertaining content and authenticity have been made. To confirm them, a questionnairetargeting French Gen Z and focusing on fashion brands has been sent via social mediaand answered by 150 respondents.Findings - The results of linear regressions show that value for money has a positiveinfluence on both attitudinal and behavioural loyalty. Also, omnichannel experienceinfluences behavioural loyalty positively while entertaining content influencesattitudinal loyalty positively. On the contrary, entertaining content has a negative impacton behavioural loyalty. So attitudinal loyalty as a moderating effect as its influence onbehavioural loyalty is positive. Ethical position and authenticity did not show anysignificant results.Conclusions - Value for money, omnichannel experience and entertaining content areespecially important to consider when it comes to Gen Z loyalty. Quite the reverse,ethics and authenticity are not as essential as suggested by many researchers andmarketers. Beyond this, the study contributes to knowledge by proving that brandloyalty should not be considered as a unit but as a multi-dimensional concept. Gen Zexpectations do not have the same influence on attitudinal and behavioural loyalty somarketers should be aware of that when building their strategy.

The evaluation of value for money (VFM) is an essential part of the pre-evaluation of public-private partnership (PPP) projects in China. It can be divided into qualitative evaluation and quantitative evaluation. However, there is no uniform weighting criterion for the indices of qualitative evaluation. To reasonably weigh the qualitative evaluation indices, this paper sorts and measures these indices through the analytic hierarchy process (AHP), using the public data on preapplied PPP aged care projects, and obtains the reference values. On this basis, several suggestions were put forward to standardize and develop VFM evaluation of PPP aged care projects, such as improve the scientific VFM evaluation system and realize whole-process performance appraisal. Based on actual project data, the qualitative evaluation is more objective than the analysis based on subjective experience. Therefore, our findings promote the application of VFM qualitative evaluation.

The results of the study indicate that financial governance at the DPRD Secretariat of Pinrang Regency has implemented the principles of good governance. PurposeIn addition to lower value for money and the loss of trust in government, procurement corruption threatens to produce a vicious cycle whereby honest firms self-select out of public procurements, further increasing corruption and decreasing value for money. This paper aims to explore this vicious cycle hypothesis.Design/methodology/approachThis paper uses a survey of businesses registered with the procurement regulator in Honduras, a country with high grand corruption but low levels of administrative procurement corruption. The study uses the survey to test the effects of experiences and perceptions of corruption and personal connections on perceptions of fairness and intention to bid on future procurements.FindingsThis study finds that experiences with bribery and the perceived importance of personal and party connections undermine perceptions of fairness, particularly for firms bidding with Honduras’s public works agency. While firms that have not bid recently view the process as less merit-based than those that have, the study does not find that perceptions of fairness influence intention to bid in the future as the vicious cycle hypothesis would suggest.Social implicationsThis research suggests that even firms that are frustrated with procurement irregularities are willing to tolerate them to access government markets.Originality/valueThe study benefits from a unique survey of businesses on issues of corruption and connections in a challenging procurement environment.

Public long-term care (LTC) systems provide services to support people experiencing difficulties with their activities of daily living. This study investigates the marginal effect of changes in public LTC expenditure on care-related quality of life (CRQoL) of existing service users in England. The public LTC program for people aged 18 or older in England is called Adult Social Care (ASC) and it is provided and managed by local authorities. We collect data on the outcomes and characteristics of public ASC users, on public ASC expenditure, and on the characteristics of local authorities across England in 2017/18. We employ an instrumental variable approach using conditionally exogenous elements of the public funding system to estimate the effect of public ASC expenditure on user CRQoL. Our findings show that by increasing public ASC expenditure by £1000 per user, on average, local authorities increase user CRQoL by 0.0030. These results suggest that public ASC is effective in increasing users' quality of life but only to a relatively small extent. When combined with the other potential effects of LTC expenditure (e.g., on informal carers, mortality), this study can inform policy makers in the United Kingdom and internationally about whether social care provides good value for money.

ilustraciones La garantía de los derechos de las personas en los estados modernos tiene más relación con la disponibilidad de infraestructura y de los servicios que dicha infraestructura posibilita, que la consagración normativa de derechos, que muchas veces no se pueden cumplir en el contexto colombiano, razón por la cual vemos problemas sistemáticos en la provisión de los servicios de conectividad vial, de salud, de educación, de un ambiente sano , entre otros . No obstante, lo anterior, se precisa que la consagración de derechos, dentro del ordenamiento jurídico colombiano, es necesario y fundamental, ya que sin la consagración normativa de dichos derechos sería problemático, alcanzar la eficacia de un derecho no expresamente reconocido. El presente trabajo busca mostrar una figura que puede ser una solución a dicha situación al generar alianzas entre el sector público y el sector privado para el desarrollo de infraestructura estratégica, teniendo en cuenta el desarrollo histórico, político, económico y jurídico de dichos esfuerzos, y no reduciendo el análisis a la consagración formal de dicha figura en la ley 1508 de 2012. Para el logro de lo anterior se delimita la definición de la figura en virtud del pronunciamiento de organismos internacionales y de definiciones de otras legislaciones, concluyendo con una definición propuesta por el autor. Posteriormente, se observa las condiciones económicas, políticas y sociales, que conllevaron a la adopción de dicha metodología de APP al Estado colombiano; se observa cómo se siguió las recomendaciones dadas por el consenso de Washington; como se intentó cambiar el modelo económico colombiano a través de la apertura económica liberal; y como se adoptaron nuevas estrategias para superar el rezago de infraestructura del país. Este contexto posibilitó adopciones normativas de orden constitucional y legal, por lo cual se expondrá los desarrollos que en estos campos se dieron para posibilitar una mayor eficacia de este tipo de asociaciones público-privadas. En el presente trabajo se observó los antecedentes de los esfuerzos público privado, estudiando la figura de la concesión; posteriormente se analizó la ley 1508 del 2012 en su exposición de motivos, su consagración normativa y la reglamentación dada en el decreto 1467 del 2012, mostrando que novedades trae dicha normatividad y que ventajas trae para cumplir con el propósito de desarrollar infraestructura de carácter estratégico, en el intento de consolidar una economía más competitiva, se realizó un estudio de la eficacia de la figura, encontrando que si se respetan principios fundamentales observados en las mejores experiencias internacionales de la figura, como es el caso Británico y el caso Chileno, tales como, “Valor por dinero” o “Asignación equilibrada de riesgos”, además de respetar principios de buena gerencia pública, se puede establecer que el mecanismo de asociación publico privada adoptado formalmente en la ley 1508 de 2012 es coherente y sólido para el desarrollo de infraestructura estratégica que requiere el país. This work seeks to show a figure that can be a solution to this situation by generating alliances between the public sector and the private sector for the development of strategic infrastructure, considering the historical, political, economic, and legal development of said efforts. And not reducing the analysis to the formal consecration of said figure in Law 1508 of 2012. Maestría Magíster en Derecho Para el correcto planteamiento metodológico y conceptual del presente trabajo es necesario indicarle al lector, que éste no busca simplemente mostrar la construcción dogmática y jurídica que entorno del modelo de asociación público privada se ha dado por parte de los cuerpos normativos, éste busca a través de una construcción teórico conceptual, que contextualice los alcances y límites de la figura, mostrar si la herramienta puede ser eficaz en el desarrollo de infraestructura estratégica y en qué medida esto sería posible, por lo cual no se quiere desligar por parte de este estudio, la relación que tiene la norma con sus efectos en la dinámica económica –social, sin querer con esto llegar a una simple casuística de la misma. Para lo anterior servirá de ayuda las experiencias internacionales más relevantes y como punto de referencia para realizar una comparación con el modelo colombiano, por lo cual se puede plantear que el presente estudio tiene una mirada conceptual – analítica, razón por lo cual se privilegiará un tipo de enfoque inductivo, que busca que desde lo particular se desarrollen las construcciones conceptuales que se requieren para el desarrollo de la figura y se muestren los vacíos que posee . With aid budgets shrinking in richer countries and more money for healthcare becoming available from domestic sources in poorer ones, the rhetoric of value for money or improved efficiency of aid spending is increasing. Taking healthcare as one example, we discuss the need for and potential benefits of (and obstacles to) the establishment of a national institute for aid effectiveness. In the case of the UK, such an institute would help improve development spending decisions made by DFID, the country’s aid agency, as well as by the various multilaterals, such as the Global Fund, through which British aid monies is channelled. It could and should also help countries becoming increasingly independent from aid build their own capacity to make sure their own resources go further in terms of health outcomes and more equitable distribution. Such an undertaking will not be easy given deep suspicion amongst development experts towards economists and arguments for improving efficiency. We argue that it is exactly because needs matter that those who make spending decisions must consider the needs not being met when a priority requires that finite resources are diverted elsewhere. These chosen unmet needs are the true costs; they are lost health. They must be considered, and should be minimised and must therefore be measured. Such exposition of the trade-offs of competing investment options can help inform an array of old and newer development tools, from strategic purchasing and pricing negotiations for healthcare products to performance based contracts and innovative financing tools for programmatic interventions.

Through the use of cost-consequence analysis (CCA), a recent evaluation of a Millennium Villages Project (MVP) in Ghana revealed it to have represented poor value for money (VFM), with comparator projects elsewhere seeming to deliver similar results at less than half the cost. However, complex integrated development programmes (IDPs) such as the MVP pose serious challenges for VFM assessments. IDPs target system-wide changes in resource-scarce contexts, making expensive foundational investments in infrastructure and other systems. The unit costs of benefits will tend to be high in the short or medium term. In contrast, many standalone projects, showing greater efficiency, may target similar outcomes, but do so by building upon existing prior investments. In this article, comparing three VFM approaches, we argue that CCA is the most appropriate for IDPs. However, its applications must be mindful of the contextual differences in which the comparator standalone projects and the IDP were implemented. [ABSTRACT FROM AUTHOR]

Background: Differences in payer evaluation and coverage of pharmaceuticals and medical procedures suggest that coverage may differ for medications and procedures independent of their clinical benefit. We hypothesized that coverage for medications is more restricted than corresponding coverage for nonmedication interventions.Methods: We included top-selling medications and highly utilized procedures. For each intervention-indication pair, we classified value in terms of cost-effectiveness (incremental cost per quality-adjusted life-year), as reported by the Tufts Medical Center Cost-Effectiveness Analysis Registry. For each intervention-indication pair and for each of 10 large payers, we classified coverage, when available, as either "more restrictive" or as "not more restrictive," compared with a benchmark. The benchmark reflected the US Food and Drug Administration label information, when available, or pertinent clinical guidelines. We compared coverage policies and the benchmark in terms of step edits and clinical restrictions. Finally, we regressed coverage restrictiveness against intervention type (medication or nonmedication), controlling for value (cost-effectiveness more or less favorable than a designated threshold).Results: We identified 392 medication and 185 procedure coverage decisions. A total of 26.3% of the medication coverage and 38.4% of the procedure coverage decisions were more restrictive than their corresponding benchmarks. After controlling for value, the odds of being more restrictive were 42% lower for medications than for procedures. Including unfavorable tier placement in the definition of "more restrictive" greatly increased the proportion of medication coverage decisions classified as "more restrictive" and reversed our findings.Conclusions: Therapy access depends on factors other than cost and clinical benefit, suggesting potential health care system inefficiency. The article deals with the theoretical background and concepts providing the basis for the use of forms of financing and acquisition of public goods and services through partnerships between the public and the private sectors (public- private partnership - PPP). The aim of this article is to introduce a new original methodology using a selection of qualitative and quantitative methods for evaluating investments and to form a complex output that will clearly and distinctly testify about the appropriateness of using the PPP method. For this purpose, processes which are normally used for business valuation - generators of values, property valuation, yield valuation, valuation based on market analysis, are combined and incorporated/recast in the current assessment methodology, which is based on the Public Sector Comparator and determinants of value for money (VFM), are being used. Although this new methodology was developed in response to the problematic situation regarding PPP projects' evaluation particularly in the Czech Republic, it can also be applied in other countries. Objective Peer support for people with dementia and carers is routinely advocated in national strategies and policy as a post-diagnostic intervention. However there is limited evidence to demonstrate the value these groups offer. This study looked at three dementia peer support groups in South London to evaluate what outcomes they produce and how much social value they create in relation to the cost of investment. Methods A Social Return on Investment (SROI) analysis was undertaken, which involves collecting data on the inputs, outputs and outcomes of an intervention, which are put into a formula, the end result being a SROI ratio showing how much social value is created per £1 of investment. Results Findings showed the three groups created social value ranging from £1.17 to £5.18 for every pound (£) of investment, dependent on the design and structure of the group. Key outcomes for people with dementia were mental stimulation and a reduction in loneliness and isolation. Carers reported a reduction in stress and burden of care. Volunteers cited an increased knowledge of dementia. Conclusions This study has shown that peer groups for people with dementia produce a social value greater than the cost of investment which provides encouraging evidence for those looking to commission, invest, set up or evaluate peer support groups for people with dementia and carers. Beyond the SROI ratio, this study has shown these groups create beneficial outcomes not only for the group members but also more widely for their carers and the group volunteers.
