Objective This paper discusses a method of local injection of autologous fat granules in repairing vaginal agenesis to further improve the effectiveness of the grafting of skin particles combined with oral mucosa. Methods This is a clinical study. Fat is suctioned out from the abdominal wall and is injected into the tissue space between the urethra and rectum. A cavity is formed by blunt dissection. Both oral mucosa and free skin grafts are cut into granules and mixed. A soft vaginal mold is evenly spread with the aforesaid mixed tissue. Then the soft mold is placed into the reconstructed vaginal cavity. After 14 days, the soft mold is taken out and replaced with a hard mold. Results There were 11 patients with vaginal agenesis treated with vaginal reconstruction. Follow up period varied from 6 months to 2 years and showed the reconstructed vagina as being soft and elastic while firm. Conclusion In skin grafting for treating vaginal agenesis using skin granules, the application of fat granule injection as a supporting structure can improve the degree of simulation. It is an upgrade based on the original method and is worthy of extending clinical practice.

Background The prognosis of breast cancer (BC) was associated with the expression of programmed cell death-1 (PD-1). Methods BC-related expression and clinical data were downloaded from TCGA database. PD-1 expression with overall survival and clinical factors were investigated. Gene set variation analysis (GSVA) and weighted gene correlation network analysis were performed to investigate the PD-1 expression-associated KEGG pathways and genes, respectively. Immune infiltration was analyzed using the ssGSEA algorithm and DAVID, respectively. Univariate and multivariable Cox and LASSO regression analyses were performed to select prognostic genes for modeling. Results High PD-1 expression was related to prolonged survival time (P = 0.014). PD-1 expression status showed correlations with age, race, and pathological subtype. ER- and PR-negative patients exhibited high PD-1 expression. The GSVA revealed that high PD-1 expression was associated with various immune-associated pathways, such as T cell/B cell receptor signaling pathway or natural killer cell-mediated cytotoxicity. The patients in the high-immune infiltration group exhibited significantly higher PD-1 expression levels. In summary, 397 genes associated with both immune infiltration and PD-1 expression were screened. Univariate analysis and LASSO regression model identified the six most valuable prognostic genes, namely IRC3, GBP2, IGJ, KLHDC7B, KLRB1, and RAC2. The prognostic model could predict survival for BC patients. Conclusion High PD-1 expression was associated with high-immune infiltration in BC patients. Genes closely associated with PD-1, immune infiltration and survival prognosis were screened to predict prognosis.

Objective In treating cervical esophageal fistula caused by trauma, tumor, surgery and other reasons, vacuum sealing drainage (VSD) used in order to speed up the healing and reduce the risk of recurrence. Methods In 24 cases of patients diagnosed with cervical esophageal fistula, the wound surface was opened completely to expose the fistula. Vacuum sealing drainage (VSD) devices were used to control infection, assisted by measures such as gastrointestinal decompression, prohibition of oral feeding, parenteral high nutritional support, and anti-inflammation, etc. Result 24 patients treated with vacuum sealing drainage (VSD) have been discharged from the hospital smoothly. After 3–6 months of follow-up and reexamination, all the cases were proved to heal well with no obvious sequelae and no recurrence of fistula. Conclusion For patients with cervical esophageal fistula, it is significant to apply the method of vacuum sealing drainage (VSD) for clinical treatment, which can effectively shorten the length of hospital stay, enable the patients to return to normal life earlier and enter the later stage of radiotherapy and chemotherapy as soon as possible. Hence it is worthy of clinical promotion.

The efficacy and safety of traditional Chinese medicine (TCM) foot bath combined with Shuganjieyu capsule (SGJYC) in the treatment of insomnia are controversial. Therefore, this study will systematically review the efficacy and safety of TCM foot bath combined with SGJYC in the treatment of insomnia, in order to provide strong evidence-based medical evidence for the treatment of insomnia with external therapy and internal therapy of TCM, and to provide more treatment decisions for clinicians to treat insomnia.

High-throughput next-generation sequencing (NGS) provides insights into genome-wide mutations and can be used to identify biomarkers for the prediction of immune and targeted responses. A deeper understanding of the molecular biological significance of genetic variation and effective interventions is required and ultimately needs to be associated with clinical benefits. We conducted a retrospective observational study of patients in two cancer cohorts who underwent NGS in a “real-world” setting. The association between differences in tumor mutational burden (TMB) and clinical presentation was evaluated. We aimed to identify several key mutation targets and describe their biological characteristics and potential clinical value. A pan-cancer dataset was downloaded as a verification set for further analysis and summary. Natural product screening for the targeted intervention of key markers was also achieved. The majority of tumor patients were younger adult males with advanced cancer. The gene identified with the highest mutation rate was TP53, followed by PIK3CA, EGFR, and LRP1B. The association of TMB (0–103.7 muts/Mb) with various clinical subgroups was determined. More frequent mutations, such as in LRP1B, as well as higher levels of ferritin and neuron-specific enolase, led to higher TMB levels. Further analysis of the key targets, LRP1B and APC, was performed, and mutations in LRP1B led to better immune benefits compared to APC. APC, one of the most frequently mutated genes in gastrointestinal tumors, was further investigated, and the potential interventions by cochinchinone B and rottlerin were clarified. In summary, based on the analysis of the characteristics of gene mutations in the “real world,” we obtained the potential association indicators of TMB, found the key signatures LRP1B and APC, and further described their biological significance and potential interventions.

Oxaliplatin (OXA) is a chemotherapy agent commonly used in the treatment of colorectal cancer (CRC). Sodium butyrate (NaB) has an antitumor effect.In total, 30 patients in stage III who completed 8 cycles of chemotherapy regimens were recruited for this study. The patients were divided into good and bad groups based on the chemotherapy efficacy. Gas chromatography-mass spectrometry (GC/MS) was used to detect microbial metabolites in stool samples from CRC patients. Cell counting kit-8 (CCK-8), Annexin-V APC/7-AAD double staining, Transwell assays, scratch-wound assays, and EdU assays were used to detect cell proliferation, apoptosis, invasion and migration, respectively. Fluoroelectron microscopy was used to observe the cell structures. To verify the inhibitory effect of NaB and OXA at animal level, a subcutaneous transplanted tumor model was established. Finally, 16S sequencing technology was used to detect intestinal bacteria. GC-MS was used to detect metabolites in mouse stools.NaB was a differential metabolite that affected the efficacy of OXA. NAB and oxaliplatin can synergically inhibit cell proliferation, migration and invasion, and induce cell apoptosis. Animal experiments confirmed the inhibitory effect of oxaliplatin and sodium butyrate on tumor in mice. In addition, the intestinal microbe detection and microbial metabolite detection in fecal samples from mice showed significant differences between butyrate-producing bacteria and NaB.NaB and OXA can synergistically inhibit the proliferation, invasion and metastasis of CRC cells and promote the apoptosis of CRC cells. NaB, as an OXA synergist, has the potential to become a new clinical adjuvant in CRC chemotherapy.
