201. Drug exposure and clinical effect of transdermal mirtazapine in healthy young cats: a pilot study

Abstract

Objectives The objective of this study was to measure drug exposure and clinical effects after administration of transdermal mirtazapine (TMZ) in healthy cats. Methods Phase I: seven healthy research cats received (1) 3.75 mg and 7.5 mg TMZ once aurally with 48 h serum sampling (serum samples were obtained via the jugular catheter at 0, 0.5, 1, 2, 5, 9, 12, 24, 36 and 48 h); (2) 7.5 mg TMZ and placebo daily aurally for 6 days then 48 h serum sampling; (3) 1.88 mg mirtazapine orally once with serum sampling at 1, 4 and 8 h. Phase II: 20 client-owned cats were enrolled in a randomized, double-blind, placebo-controlled, three-way crossover clinical effect study. Treatments consisted of 6 days of aural 7.5 mg TMZ or placebo gel at home, and 1.88 mg mirtazapine orally once in the clinic. Owners documented appetite, rate of food ingestion, begging activity and vocalization daily at home. On day 6, food consumed, activity and vocalization were documented in hospital, and trough and peak serum mirtazapine levels were obtained. Serum mirtazapine and gel concentrations were measured using liquid chromatography/tandem mass spectrometry. Results Phase I: administration of TMZ achieved measureable serum mirtazapine concentrations. Area under the curve0-48 of multidose 7.5 mg TMZ was significantly higher than single-dose 1.88 mg oral mirtazapine (OMZ) ( P = 0.02). Phase II: client-owned cats administered TMZ had a significant increase in appetite ( P = 0.003), rate of food ingestion ( P = 0.002), activity ( P = 0.002), begging ( P = 0.002) and vocalization ( P = 0.002) at home. Abstract

Rural communities in the US have increasing HIV burden tied to injection drug use, yet engagement in pre-exposure prophylaxis (PrEP) care has been low among people who inject drugs (PWID). Syringe service programs (SSPs) are widely implemented in Kentucky's Appalachian region, presenting an important opportunity to scale PrEP services. This paper examines PrEP awareness, interest and preferences among PWID attending community-based SSPs in Appalachia. Eighty participants were enrolled from two SSP locations. Eligibility included: ≥ 18 years old, current injection drug use and SSP use, and an indication for PrEP as defined by CDC guidelines. Participants completed a structured baseline interview. Predictors of PrEP awareness, interest and formulation preferences were examined. 38.8% reported baseline awareness of PrEP, 50% expressed high interest in PrEP, and 48.1%reported a preference for injectable PrEP. Significant bivariate predictors of PrEP interest included: current worry about health, higher perceived HIV risk, higher community HIV stigma, and higher enacted substance use stigma in the past year; in the adjusted model, enacted substance use stigma remained significant. Findings demonstrate substantial interest in PrEP among rural PWID. Intrapersonal and social determinant factors were associated with PrEP interest, which suggests the importance of multi-level intervention targets to increase PrEP uptake. [ABSTRACT FROM AUTHOR]

Abstract

Objective To explore the lived experience of people with myocardial ischaemia with no obstructive arteries. Design Qualitative study using semistructured interviews. Setting Telephone interviews with 17 participants living in the UK. Participants 17 people (2 males, 15 females; aged 31–69 years) with a presumed or confirmed diagnosis of myocardial ischaemia with no obstructive arteries, recruited via social media and online patient-led support forums. Results Five themes were generated. Theme 1 describes the wide range of experiences that participants described, particularly the frequency and intensity of symptoms, and the uncertainty and fear that symptoms commonly provoked. Theme 2 describes the major impact on social relationships, employment and other aspects of everyday life. Theme 3 illustrates challenging and traumatising experiences participants described around pathways to diagnosis and accessing medical support. Theme 4 highlights the lack of consensus and clarity that participants had been confronted with around treatment and management. Theme 5 describes coping and supportive strategies valued by participants. Conclusions This study provides insight into the challenges of living with myocardial ischaemia with no obstructive arteries. Findings highlight the significant psychological impact on people living with these conditions and the need for improvements in diagnosis, support and long-term management.

Abstract

Sexual assault is a global problem, with the risk highest among university students. Bystander intervention preventing sexual assaults has primarily been researched using quantitative methods to understand what factors influence it. However, both sexual assault and bystander intervention are complex with many subtle and overlapping issues that, when analyzed qualitatively, can offer new insights. The current study aimed to explore and develop a nuanced and comprehensive understanding of students’ perceptions of sexual assault and bystander intervention across two universities, one in the United Kingdom and one in Australia. Thirty-nine university students (19 in the United Kingdom; 20 in Australia) took part in one-to-one semistructured interviews. Using inductive thematic analysis, two overarching themes were identified: (a) navigating the complex dynamics of sexual assault; and (b) decisions to intervene or not to intervene. Findings suggest that the complexity and ambiguity around sexual assault can forestall bystander intervention. As such, increasing education, awareness, and discussions around sexual assault and bystander intervention is vital to increase awareness of the problem and mobilize action from bystanders to prevent sexual assault.

212. Mechanisms of resolution of inflammation in paediatric neutrophilic lung disease

Abstract

Many paediatric airway diseases are characterised by persistent neutrophilic inflammation, which can lead to damage to the airways and lung parenchyma. One possible mechanism for the persistence of neutrophilic inflammation is failure of the normal active resolution of the inflammatory process. There is limited published literature on the role of inflammatory resolution in paediatric inflammatory lung disease. It is possible that targeting inflammatory resolution mechanisms and the ability to “switch off” inflammation may provide therapeutic targets in the future for these diseases. This thesis investigates the hypotheses that failure of mechanisms terminating acute inflammation are important in the pathophysiology of infective and inflammatory lung disease; and that the differences in prognosis between childhood inflammatory lung diseases: cystic fibrosis (CF) (both established and newly diagnosed by newborn screening (CF NBS)), bronchiectasis, primary ciliary dyskinesia (PCD) and persistent bacterial bronchitis (PBB), are related to the ability to resolve inflammation in each disease. A number of mechanisms and mediators important for inflammatory resolution are described in a cross-sectional study of bronchoalveolar lavage (BAL) and endobronchial biopsies (EBB): BAL CD25+FoxP3+ T regulatory cells by flow cytometry; annexin A1 (AnxA1) and its receptor ALX by RT-PCR of BAL and RTPCR and immunofluorescent staining of EBB; the transcription factor Lung Krüppel- Like Factor by immunofluorescent staining of EBB; BAL lipid mediators by liquid chromatography – mass spectrometry and the lipid enzyme 15-lipoxygenase by immunofluorescent staining of EBB. Findings were related to underlying diagnosis, clinical status and airway inflammatory status (BAL neutrophils, CXCL8 and IL-10). The main abnormality found was in the AnxA1 axis, where BAL AnxA1 mRNA levels were lower in neutrophilic lung disease as compared to controls. However this was related to disease severity rather than the CFTR defect. The ratio of BAL CXCL8: IL- 10 was higher in CF as compared to other neutrophilic lung diseases and thus there was evidence that the ability of IL-10 to resolve CXCL8 mediated inflammation was reduced in CF. Therefore there was some evidence for the importance of inflammatory resolution mechanisms studied in the paediatric neutrophilic airway, and limited evidence to suggest that the anti-inflammatory function of IL-10 is impaired in CF.

To evaluate potential airborne asbestos exposures during brake maintenance and repair activities on a P&H overhead crane, and during subsequent handling of the mechanic's clothing. Personal (n = 27) and area (n = 61) airborne fiber concentrations were measured during brake tests, removal, hand sanding, compressed air use, removal and reattachment of chrysotile-containing brake linings, and reinstallation of the brake linings. The mechanic's clothing was used to measure potential exposure during clothes handling. All brake linings contained between 19.9% to 52.4% chrysotile asbestos. No amphibole fibers were detected in any bulk or airborne samples. The average full-shift airborne chrysotile concentration was 0.035 f/cc (PCM-equivalent asbestos-specific fibers, or PCME). Average task-based personal air samples collected during brake maintenance, sanding, compressed air use, and brake lining removal tasks ranged from 0 to 0.48 f/cc (PCME). The calculated 30-minute time-weighted average (TWA) airborne chrysotile concentration associated with 5-15 minutes of clothes handling was 0–0.035 f/cc PCME. The results indicated that personal and area TWA fiber concentrations measured during all crane brake maintenance and clothes handling tasks were below the current OSHA 8-h TWA Permissible Exposure Limit for asbestos of 0.1 f/cc. Further, no airborne asbestos fibers were measured during routine brake maintenance tasks following the manufacturer's maintenance manual procedures. All short-term airborne chrysotile concentrations measured during non-routine tasks were below the current 30-minute OSHA excursion limit for asbestos of 1 f/cc. We explore the finding that households often expect their financial position to remain unchanged compared to other alternatives. A generalized middle inflated ordered probit (GMIOP) model is used to account for the tendency of individuals to choose "neutral" responses when faced with opinion‐based questions. Our analysis supports the use of a GMIOP model to account for this response pattern. Expectation indices based on competing discrete choice models are also explored. While financial optimism is significantly associated with increased consumption at both the intensive and extensive margin, indices which fail to take into account middle‐inflation overestimate the impact of financial expectations. The United States Supreme Court ruling that the Professional and Amateur Sports Protection Act (PASPA) was unconstitutional in May 2018 has changed the professional sport landscape. Currently, states are working to create and pass sports betting laws and capitalize on the financial opportunities, while professional leagues are looking to do the same. One way the leagues can further benefit financially from sports betting is selling athletes' biometric data (ABD) to data providers, such as Sportradar and Genius Sports. However, the dearth of legal precedent on protections of ABD and the lack of delineation of ownership of ABD within collective bargaining agreements poses obstacles for leagues to overcome before offering ABD in sports betting platforms. Therefore, the purpose of this paper is to discuss (1) the current state of sports betting in the United States, (2) the legal precedent around ABD, (3) the challenges that stem from legal precedent or laws, and (4) a solution for how leagues can sell ABD to data providers while being compliant with applicable laws. Abstract

Using the US Survey of Consumer Finances, we explore the empirical relationship between borrowing constraints and financial portfolio allocation by American households. To help motivate our empirical analysis we initially draw insights from a mean-variance model of optimal portfolio allocation with three tradable asset classes defined by increasing risk, and establish a link between borrowing restrictions and portfolio allocation in the presence of background risk. Our main contribution, however, lies in estimating the role that borrowing constraints play in shaping households' financial portfolio allocation. This is achieved using an ordered fractional probit model. In addition to revealing the significant empirical role played by household borrowing constraints in determining portfolio allocation, our analysis helps us to resolve ambiguity surrounding the behaviour of the medium-risk asset in our motivational theoretical framework. Further, the empirical distribution of medium-risk assets is found to be remarkably similar to that for high-risk assets, suggesting the presence of a more general 'risk puzzle', which our borrowing constraints measures partially explain. Abstract

The dusky-footed woodrat (Neotoma fuscipes) is an endemic North American rodent belonging to the family Cricetidae. We present here the first complete mitogenome within the Neotoma genus. The mitogenome is 16,199 bp in length, and has a structure and gene organization similar to other rodent species (e.g. Mus musculus, Peromyscus maniculatus and Microtus fortis calamorum). The overall base composition is A (35.2%), C (25.5%), G (12.3%), T (27.0%), with a GC content of 37.8%, similar to other rodents. This mitogenome serves as a foundation for future phylogenetic, phylogeographic and population genetic studies.

246. The role of fire in dinosaur-dominated ecosystems : examples from the Late Cretaceous of Alberta

Abstract

Wildfires are a major component of modern ecosystems, however the role of fire in deep time is still being explored. A database of Cretaceous charcoal localities highlights a reduced charcoal record in the Campanian and Maastrichtian compared to that expected given modelled atmospheric oxygen levels. Temporal distribution and relative abundances of charcoal have provided the first detailed information on fire activity spanning c. 11 million years of the Late Cretaceous in Alberta, Canada. A novel approach to charcoal quantification has been developed, allowing relative abundances to be calculated whilst compensating for charcoal fragmentation. The Campanian Oldman and Dinosaur Park Formations were investigated, (117 lithological units spanning 1.8Ma). Charcoal, dominated by gymnosperm wood, was distributed throughout the sedimentary succession in all lithologies, indicating that the Campanian wildfires were not restricted to specific environmental settings. This represents the first documented charcoal occurrence within the Dinosaur Park Formation. The Maastrichtian Horseshoe Canyon, Battle and Scollard Formations were investigated (60 samples spanning 9.5Ma). Greater charcoal abundances occurred in the lower part of the sedimentary succession but climatic variations exerted little or no control over wildfire occurrence. Samples from recent wildfires contained uncharred and partially charred plant debris addition to charcoal, highlighting a probable bias within the late Cretaceous assemblages. Elevated charcoal abundances were recorded in four vertebrate deposits within the Dinosaur Park Formation. Water flows and hyperconcentrated flows, responsible for the formation of these vertebrate deposits, may have been enhanced due to rainfall events influencing the burnt landscape, leading to post-fire destabilisation of slopes. Flooding events following wildfires may be the causal mechanism for the formation of other vertebrate assemblages. Thirteen samples from recent wildfires (Rodeo-Chediski fire -2002, Medano fire -2010 and the Scultz fire -2010) were investigated to further understand the composition of wildfire derived plant debris. Uncharred and partially charred plant debris was recorded in addition to charcoal, highlighting a probable bias within the Late Cretaceous assemblages. Relative abundances of charcoal could not be shown to 5 be influenced by proximity to burned area, vegetation stands, forest types or the size of channels. Further research into potential controls on charcoal distribution needs to be considered by wildfire researchers. This research has highlighted the previously unrecorded extent and importance of wildfire activity through the Late Cretaceous and demonstrated the need for greater investigation of sedimentary successions to prevent underestimation of past wildfire occurrence.
