Background: In the United States, 5.7 million patients are admitted annually to intensive care units (ICU), with costs exceeding $82 billion. Although close monitoring and dynamic assessment of patient acuity are key aspects of ICU care, both are limited by the time constraints imposed on healthcare providers. Methods: Using the University of Florida Health (UFH) Integrated Data Repository as Honest Broker, we created a database with electronic health records data from a retrospective study cohort of 38,749 adult patients admitted to ICU at UF Health between 06/01/2014 and 08/22/2019. This repository includes demographic information, comorbidities, vital signs, laboratory values, medications with date and timestamps, and diagnoses and procedure codes for all index admission encounters as well as encounters within 12 months prior to index admission and 12 months follow-up. We developed algorithms to identify acuity status of the patient every four hours during each ICU stay. Results: We had 383,193 encounters (121,800 unique patients) admitted to the hospital, and 51,073 encounters (38,749 unique patients) with at least one ICU stay that lasted more than four hours. These patients requiring ICU admission had longer median hospital stay (7 days vs. 1 day) and higher in-hospital mortality (9.6% vs. 0.4%) compared with those not admitted to the ICU. Among patients who were admitted to the ICU and expired during hospital admission, more deaths occurred in the ICU than on general hospital wards (7.4% vs. 0.8%, respectively). Conclusions: We developed phenotyping algorithms that determined patient acuity status every four hours while admitted to the ICU. Current day pain assessment methods rely on patient self-report or by an observer like the Intensive Care Unit (ICU) nurses. Patient self-report is subjective to the individual and suffers due to poor recall. Pain assessment by manual observation is limited by the number of administrations per day and staff workload. Previous studies showed the feasibility of automatic pain assessment by detecting Facial Action Units (AUs). Pain is observed to be associated with certain facial action units (AUs). This method of pain assessment can overcome the pitfalls of present-day pain assessment techniques. All the previous studies are limited to controlled environment data. In this study, we evaluated the performance of OpenFace an open-source facial behavior analysis tool and AU R-CNN on the real-world ICU data. Presence of assisted breathing devices, variable lighting of ICUs, patient orientation with respect to camera significantly affected the performance of the models, although these showed the state-of-the-art results in facial behavior analysis tasks. To test the hypothesis that accuracy, discrimination, and precision in predicting postoperative complications improve when using both preoperative and intraoperative data input features versus preoperative data alone. Models that predict postoperative complications often ignore important intraoperative physiological changes. Incorporation of intraoperative physiological data may improve model performance. This retrospective cohort analysis included 52,529 inpatient surgeries at a single institution during a 5 year period. Random forest machine learning models in the validated MySurgeryRisk platform made patient-level predictions for three postoperative complications and mortality during hospital admission using electronic health record data and patient neighborhood characteristics. For each outcome, one model trained with preoperative data alone and one model trained with both preoperative and intraoperative data. Models were compared by accuracy, discrimination (expressed as AUROC), precision (expressed as AUPRC), and reclassification indices (NRI). Machine learning models incorporating both preoperative and intraoperative data had greater accuracy, discrimination, and precision than models using preoperative data alone for predicting all three postoperative complications (intensive care unit length of stay >48 hours, mechanical ventilation >48 hours, and neurological complications including delirium) and in-hospital mortality (accuracy: 88% vs. 77%, AUROC: 0.93 vs. 0.87, AUPRC: 0.21 vs. 0.15). Overall reclassification improvement was 2.9-10.0% for complications and 11.2% for in-hospital mortality. Purpose: Summarize ICU delirium prediction models published within the past five years. Methods: Electronic searches were conducted in April 2019 using PubMed, Embase, Cochrane Central, Web of Science, and CINAHL to identify peer reviewed studies published in English during the past five years that specifically addressed the development, validation, or recalibration of delirium prediction models in adult ICU populations. Data were extracted using CHARMS checklist elements for systematic reviews of prediction studies, including the following characteristics: study design, participant descriptions and recruitment methods, predicted outcomes, a priori candidate predictors, sample size, model development, model performance, study results, interpretation of those results, and whether the study included missing data. Results: Twenty studies featuring 26 distinct prediction models were included. Model performance varied greatly, as assessed by AUROC (0.68-0.94), specificity (56.5%-92.5%), and sensitivity (59%-90.9%). Most models used data collected from a single time point or window to predict the occurrence of delirium at any point during hospital or ICU admission, and lacked mechanisms for providing pragmatic, actionable predictions to clinicians. Conclusions: Although most ICU delirium prediction models have relatively good performance, they have limited applicability to clinical practice. Most models were static, making predictions based on data collected at a single time-point, failing to account for fluctuating conditions during ICU admission. Further research is needed to create clinically relevant dynamic delirium prediction models that can adapt to changes in individual patient physiology over time and deliver actionable predictions to clinicians.

Background: Acute kidney injury (AKI) is a common complication in hospitalized patients and a common cause for chronic kidney disease (CKD) and increased hospital cost and mortality. By timely detection of AKI and AKI progression, effective preventive or therapeutic measures could be offered. This study aims to develop and validate an electronic phenotype to identify patients with CKD and AKI. Methods: A database with electronic health records data from a retrospective study cohort of 84,352 hospitalized adults was created. This repository includes demographics, comorbidities, vital signs, laboratory values, medications, diagnoses and procedure codes for all index admission, 12 months prior and 12 months follow-up encounters. We developed algorithms to identify CKD and AKI based on the Kidney Disease: Improving Global Outcomes (KDIGO) criteria. To measure diagnostic performance of the algorithms, clinician experts performed clinical adjudication of AKI and CKD on 300 selected cases. Results: Among 149,136 encounters, identified CKD by medical history was 12% which increased to 16% using creatinine criteria. Among 130,081 encounters with sufficient data for AKI phenotyping 21% had AKI. The comparison of CKD phenotyping algorithm to manual chart review yielded PPV of 0.87, NPV of 0.99, sensitivity of 0.99, and specificity of 0.89. The comparison of AKI phenotyping algorithm to manual chart review yielded PPV of 0.99, NPV of 0.95 , sensitivity 0.98, and specificity 0.98. Conclusions: We developed phenotyping algorithms that yielded very good performance in identification of patients with CKD and AKI in validation cohort.
