The Nuss procedure, which is a minimally invasive approach for treating pectus excavatum, has better functional and cosmetic outcomes than other invasive procedures. Cardiac perforation is the most serious complication and several methods for the prevention of intraoperative events has been developed. Although most cardiac injuries are detected in the operating room, in the case described herein the patient experienced sudden hypovolemic shock during the postoperative recovery period. This indicates that special caution is mandatory even after successful execution of the Nuss procedure.

—Hochman and Lumb have raised an important point concerning the stability of likelihood ratios across various surgical procedures. Likelihood ratios, like the related properties of sensitivity and specificity, are usually assumed to be stable across populations with different pretest probabilities. The common example of variations in pretest probabilities results from variations in the prevalence of disease. However, for the clinical problem of assessing operative risk for cardiac patients, it is reasonable to think of the variations in pretest probabilities as variations in cardiac complication rates across categories of surgery. In performing the hypothesis-testing study of our modified index, which is reported elsewhere, 1 we were also concerned about the stability of likelihood ratios across the various surgical categories. To the Editor.— The report of a "Cardiac Arrest Following Fluorescein Angiography" by Eugene E. Cunningham, MD, and Venkataraman Balu, MD (242:2431, 1979), suggests a relationship between the intravenous fluorescein and the cardiac complication. It is well known that phenylephrine hydrochloride 10%, a frequently used concentration in eye drops for pupillary dilation, can precipitate cardiac arrhythmias and increases in blood pressure. Unfortunately, the article does not include a statement of the drops that were instilled before the angiogram. If phenylephrine or some other sympathomimetic drug were used, it would seem that these drugs would more likely be related to the cardiac arrest.

The condition reported here is exceedingly rare, and the case with its complications offers some interesting suggestions from a diagnostic point of view. The patient was seen several times in consultation with Dr. H. E. Markwort. C. K., a boy, aged 7 years, well nourished, with healthy, educated and fairly well to do parents, living in good surroundings in a modern home with plenty of sunshine, was taken sick, June 18, 1929, with chickenpox. MEDICAL HISTORY AND DIAGNOSIS BY DR. M'CASKEY. The following case was referred by Dr. G. M. LaSalle of Wabash, Ind., Sept. 20, 1902: A. T., laborer, aged 44. His health had always been good up to the time of his present illness, excepting that three years ago he had his nose broken on the left side by a vicious sheep which butted him with its horns. He was not entirely unconscious, but was severely stunned and the nasal bones badly broken, but he apparently recovered from the injury without incident.
