Abstract

Site-directed mutagenesis and transfection of a beta1-tubulin cDNA is used to determine whether amino acid substitutions other than H, F, or R at leucine 215 (L215) can produce paclitaxel resistance and whether all substitutions produce paclitaxel resistance by a similar mechanism. The results show that the L251 substitution might enhance the binding or effectiveness of paclitaxel and they also confirm the importance of the H6-H7 loop of beta-tubulin in microtubule assembly and resistance to antimitotic drugs.

Abstract

ATP-binding-cassette-transporter-A1 (ABCA1) plays a pivotal role in intracellular cholesterol removal, exerting a protective effect against atherosclerosis. ABCA1 gene severe mutations underlie Tangier disease, a rare Mendelian disorder that can lead to premature coronary artery disease (CAD), with age of CAD onset being two decades earlier in mutant homozygotes and one decade earlier in heterozygotes than in mutation non-carriers. It is unknown whether common polymorphisms in ABCA1 could influence age of symptom onset of CAD in the general population. We examined common promoter and non-synonymous coding polymorphisms in relation to age of symptom onset in a group of CAD patients (n = 1164), and also carried out in vitro assays to test effects of the promoter variations on ABCA1 promoter transcriptional activity and effects of the coding variations on ABCA1 function in mediating cellular cholesterol efflux. Age of symptom onset was found to be associated with the promoter − 407G > C polymorphism, being 2.82 years higher in C allele homozygotes than in G allele homozygotes and intermediate in heterozygotes (61.54, 59.79 and 58.72 years, respectively; P = 0.002). In agreement, patients carrying ABCA1 haplotypes containing the −407C allele had higher age of symptom onset. Patients of the G/G or G/C genotype of the −407G > C polymorphism had significant coronary artery stenosis (>75%) at a younger age than those of the C/C genotype (P = 0.003). Reporter gene assays showed that ABCA1 haplotypes bearing the −407C allele had higher promoter activity than haplotypes with the −407G allele. Functional analyses of the coding polymorphisms showed an effect of the V825I substitution on ABCA1 function, with the 825I variant having higher activity in mediating cholesterol efflux than the wild-type (825V). Abstract

Normal myocardial function in severe right ventricular volume overload hypertrophy. Am J Physiol Heart Circ Physiol 280: H11-H16, 2001.--Severe left ventricular volume overloading causes myocardial and cellular contractile dysfunction. Whether this is also true for severe right ventricular volume overloading was unknown. We therefore created severe tricuspid regurgitation percutaneously in seven dogs and then observed them for 3.5-4.0 yr. All five surviving operated dogs had severe tricuspid regurgitation and right heart failure, including massive ascites, but they did not have left heart failure. Right ventricular cardiocytes were isolated from these and from normal dogs, and sarcomere mechanics were assessed via laser diffraction. Right ventricular cardiocytes from the tricuspid regurgitation dogs were 20% longer than control cells, but neither the extent (0.171 [+ or -] 0.005 [micro]m) nor the velocity (2.92 [+ or -] 0.12 [micro]m/s) of sarcomere shortening differed from controls (0.179 [+ or -] 0.005 [micro]m and 3.09 [+ or -] 0.11 [micro]m/s, respectively). Thus, despite massive tricuspid regurgitation causing overt right heart failure, intrinsic right ventricular contractile function was normal. 244. Is endoprosthetic replacement of the proximal femur appropriate in the comorbid patient?

Abstract

Introduction: Patients with failed hip arthroplasty requiring extensive femoral reconstruction often present with a multitude of comorbidities. Many treatment options limit initial mobilisation relying on bone graft incorpora- tion. The use of endoprosthetic replacement (EPR), despite often being a "last resort", offers an expeditious solution with early mobilisation that is crucial in the comorbid individual. Many perceive that the surgical insult of EPR is associated with an increased mortality. The aim of this study was to report our experience of proximal femoral EPR as the treatment for failed arthroplasty or fracture fixation. Primary outcomes included mortality, complications, revision and function. Methods: Retrospective review of proximal femoral EPR undertaken at our institution for non-oncological indications between 2007 and 2015 identified 37 patients with a mean follow-up of 33 months. Patient case notes, demographics and radiographs were studied. Results: The 90-day mortality following proximal femoral EPR was 2.7%. 9 patients had died at the time of final follow-up (mean time to death was 33 months). The mean preoperative and postoperative Oxford Hip Score improved from 8 to 31 respectively (p<0.05). When considering revision for any cause, 5-year survival was 94.6%. 2 patients suffered periprosthetic joint infection and 1 patient required revision for prosthesis dislocation. Conclusions: We report a relatively low incidence of perioperative complications, with a mortality rate similar to other revision options in this high-risk cohort. Whilst further revision may not always be possible, this "last resort" technique is safe in the comorbid population presenting with significant proximal femoral bone deficiency. Abstract

This thesis research focuses on clast and crystal-specific studies to investigate the pre- and syn-eruptive magmatic processes of two supereruptions in the TVZ: the 1.21 Ma Ongatiti (>500 km3) and the 1.0 Ma Kidnappers (~1200 km3), together with the smaller (~200 km3) 1.0 Ma Rocky Hill eruption from the Mangakino Volcanic Centre (MVC). Crystallisation histories determined through SIMS U-Pb dating of zircons reveal that the paired Kidnappers and Rocky Hill eruptions were products of a common magmatic system, which built over ~200 kyr, in the time break after the Ongatiti eruption. U-Pb age spectra from the Ongatiti show a protracted crystallisation history (over ~250 kyr), in which the majority of zircon crystallised ~100 kyr prior to eruption in a crystal mush. Zircons then ascended with melt during accumulation of the final erupted magma body in the shallow crust. Zircons remained stable in the melt dominant body but underwent little further crystallisation. Zircons from all three systems record common geochemical processes governed by the fractionating assemblage (predominantly plagioclase and amphibole). In particular, the MREE/HREE ratios and Sr concentrations of zircons from the Ongatiti record imply two contrasting source regions governed by different proportions of crystallising amphibole. The in-situ major and trace element chemistry of glass shards and crystals from the Kidnappers fall deposit reveal that magma within the Kidnappers was stored in three discrete bodies, which were systematically tapped during the early stages of eruption. Temperature and pressure (T-P) estimates from amphibole and Fe-Ti oxide equilibria from each magma type are similar and therefore the three magma bodies were adjacent, not vertically stacked, in the crust. Amphibole model T-P estimates range from 770 to 840 °C and 90 to 170 MPa corresponding to pre-eruptive storage depths of ~4.0-6.5 km.
