The objectives were to characterize the patient subgroup prescribed anti-IL-5 monoclonal antibodies and to report potential benefits.Methods: This is a retrospective case series study of 17 patients treated in a COPD subspecialty clinic. All patients had a diagnosis of COPD (post-bronchodilator FEV1/FVC < 0.7) and had been prescribed an anti-IL-5 biologic for at least 8 months. Acute exacerbations of COPD (AECOPDs) were collected as reported in electronic medical records.Results: All patients (17) enrolled were treated with biologics for ≥ 8 months, and 13 (76%) for ≥ 1 year. Patients were characterized by severe disease traits, FEV1 < 50% predicted, recurrent exacerbations (3.5 moderate-to-severe AECOPDs in the year before treatment), high peripheral blood eosinophil counts (≥ 250 cells/μL in the previous year), all on inhaled triple therapy, and only 1 patient with a diagnosis of asthma prior to smoking. There was a statistically significant decrease in the exacerbation rate compared with baseline after 8 and 12 months of anti-IL-5 treatment, respectively, yielding the equivalent of a 2– 3x reduction in exacerbation rate. Absolute FEV1 decreased, and the decline in FEV1 % of predicted reached statistical significance (p< 0.05); CAT score improved (p< 0.05).Discussion: This real-world evidence data aligns with existing studies suggesting the potential benefit of anti-IL-5 treatment for specific patients with COPD and therefore advocates for further investigation of RCTs on the use of anti-IL-5 biologics for well-characterized patients with COPD.Keywords: COPD, biologics, anti-IL5 therapy, blood eosinophil count, RWE, RWD

Abstract

JT-60SA is the largest tokamak before ITER, and operation is due to start in 2020. F4E (in the framework of the Broader Approach) and EUROfusion do contribute with several enhancement projects to the JT-60SA tokamak. The JT-60SA divertor cryopump system is one of such enhancements. The cryopump is separated into 9 identical units, distributed equally along the circumference and located in the volume in-vessel underneath the divertor dome. Each cryopump unit consists of four cryosorption panels cooled by 3.7 K supercritical helium, and a thermal radiation shield surrounding the cryopanels made of a base plate and an inlet baffle cooled at about 80 K. To have sufficient pumping capacity for helium, the cryopanels will be coated with activated charcoal. Daily regeneration of the cryopumps at 100 K is foreseen overnight. The paper will present the detailed design of the cryopumps and highlight the main results of the supporting design development. Thermo-mechanical analyses have covered a variety of load cases, reflecting operation and regeneration conditions, low cycle fatigue, electromagnetic forces under plasma disruption conditions, and accelerations under seismic events. Particularly challenging was to maximize performance in a given space limited by the size of the existing divertor cassettes. Vacuum flow simulations have been performed to predict pumping speed and capacities for hydrogenic gases and helium. It will be shown how the requirements put on the cryopumps have been considered in the final design.

Abstract

We present extractions of the nucleon nonsinglet moments utilizing new precision data on the deuteron F_{2} structure function at large Bjorken-x determined via the Rosenbluth separation technique at Jefferson Lab Experimental Hall C. These new data are combined with a complementary set of data on the proton previously measured in Hall C at similar kinematics and world datasets on the proton and deuteron at lower x measured at SLAC and CERN. The new Jefferson Lab data provide coverage of the upper third of the x range, crucial for precision determination of the higher moments. In contrast to previous extractions, these moments have been corrected for nuclear effects in the deuteron using a new global fit to the deuteron and proton data. The obtained experimental moments represent an order of magnitude improvement in precision over previous extractions using high x data. Moreover, recent exciting developments in lattice QCD calculations provide a first ever comparison of these new experimental results with calculations of moments carried out at the physical pion mass, as well as a new approach that first calculates the quark distributions directly before determining moments.

Abstract

Background: CS1003 is a novel humanized IgG4 anti-PD-1 monoclonal antibody. In phase 1a dose-escalation, CS1003 showed a well-tolerated safety profile with no dose-limiting toxicity, and a wide therapeutic window at doses up to 10 mg/kg Q3W (maximum administrated dose, MAD) in patients (pts) with solid tumors. Two fixed-dose regimens were explored in the phase Ib portion of this ongoing clinical trial in pts with selected solid tumors. Method(s): Pts were enrolled in cohort A (200 mg Q3W) or cohort B (400 mg Q6W) to receive CS1003 intravenously. Safety, preliminary tumor activity (overall response per RECIST v1.1 by investigators) and PK were assessed. Result(s): As of 21 Mar 2020, 29 and 30 pts were enrolled in cohorts A and B, with a median treatment duration of 15.0 (range: 3.0-50.4) and 14.4 (range: 4.7-27.7) weeks, respectively. Following the first dose, the mean Cmax value of CS1003 in cohort B was around doubling of that in cohort A (112 vs 44 mug/ml) but lower than that (189 mug/ml) at the MAD, demonstrating sufficient safety margin. The mean Ctrough value of cohorts A and B were 9.2 and 18.8 mug/ml, respectively, expecting a complete PD-1 receptor occupancy throughout the dosing intervals with both regimens. The steady-state PK data of cohort B will be updated. In cohorts A and B, 97% (28/29) and 93% (28/30) pts had treatment-emergent adverse events; 41% (12/29) and 47% (14/30) pts had all-grade treatment-related adverse events (TRAEs), respectively. The TRAE profiles were comparable in both cohorts overall with two Grade (G) >= 3 events (G3 dermatitis and G4 Type 1 diabetes mellitus in cohort B), and the rest were G1/2. Among the 29 efficacy-evaluable pts in cohort A, there were 3 complete response (2 confirmed), 3 partial response (PR, 2 confirmed) and 5 stable disease (SD). 247. 98P Operational challenges of an Asian Pacific (APAC) academic oncology clinical trial.

Abstract

Background: Multinational academic trials can pose complex organisational issues. We describe our experience running the phase III ASCOLT trial (NCT00565708), studying the impact of aspirin in resected high-risk colorectal cancer. This is the first large academic adjuvant trial fully conducted in the APAC region. Method(s): ASCOLT is coordinated by the National Cancer Centre Singapore and funded largely by academic grants. The trial opened in 2008 and is nearing full accrual (1480 of 1587) at 58 sites in 12 countries/regions, including 5 middle income countries. Trial conduct logistics were analysed to identify barriers and enablers. Result(s): Diverse regulatory requirements necessitated >100 contracts and 49 ethics board reviews, leading to start up times of ~6 months per site. Consent forms were translated into 13 languages. The enrolment target and period were revised in 2017, and site numbers expanded due to lower than projected accrual and event rates. Other challenges include communication and logistical barriers, patient attrition, recruitment holds due to changes in local laws and limited funding for patient visits, affecting countries where healthcare is not publicly funded. Solutions included engaging local cooperative groups (e.g. the Australasian GastroIntestinal Trials Group in Australia and New Zealand) and 6 contract research organisations to manage sites, developing processes sensitive to local needs, transition to electronic data management, and a centralised drug dispensing system. Measures to overcome disparate experience across sites and to quality control include regular contact with the central team to troubleshoot (e.g. >200 data queries are raised a month) and central and on-site monitoring. 16 sites were closed due to resource or recruitment issues. Abstract

Quasielastic $^{12}$C$(e,e'p)$ scattering was measured at space-like 4-momentum transfer squared $Q^2$~=~8, 9.4, 11.4, and 14.2 (GeV/c)$^2$, the highest ever achieved to date. Nuclear transparency for this reaction was extracted by comparing the measured yield to that expected from a plane-wave impulse approximation calculation without any final state interactions. The measured transparency was consistent with no $Q^2$ dependence, up to proton momenta of 8.5~GeV/c, ruling out the quantum chromodynamics effect of color transparency at the measured $Q^2$ scales in exclusive $(e,e'p)$ reactions. Abstract

Positron beams, both polarized and unpolarized, are identified as essential ingredients for the experimental programs at the next generation of lepton accelerators. In the context of the hadronic physics program at Jefferson Lab (JLab), positron beams are complementary, even essential, tools for a precise understanding of the electromagnetic structure of nucleons and nuclei, in both the elastic and deep-inelastic regimes. For instance, elastic scattering of polarized and unpolarized electrons and positrons from the nucleon enables a model independent determination of its electromagnetic form factors. Also, the deeply-virtual scattering of polarized and unpolarized electrons and positrons allows unambiguous separation of the different contributions to the cross section of the lepto-production of photons and of lepton-pairs, enabling an accurate determination of the nucleons and nuclei generalized parton distributions, and providing an access to the gravitational form factors. Furthermore, positron beams offer the possibility of alternative tests of the Standard Model of particle physics through the search of a dark photon, the precise measurement of electroweak couplings, and the investigation of charged lepton flavor violation.
