202. Efficacy of photodynamic therapy with methyl aminolevulinate in the treatment of superficial and nodular basal cell carcinoma: an open-label trial

Abstract

Photodynamic therapy with methyl aminolevulinate (MAL-PDT) is a non-invasive therapy for superficial and nodular basal cell carcinoma (BCC). We performed an open-label trial to evaluate efficacy, safety, tolerability and cosmetic outcome of MAL-PDT in selected patients with superficial and nodular BCCs. Ninety-four superficial and 24 nodular BCCs in 69 patients were treated with 2 to 8 MAL-PDT sessions. Efficacy, safety, tolerability and cosmetic outcome were evaluated at months 1, 3, 6 and 12 after the last MAL-PDT treatment and then every 3 months. One patient discontinued the study for reasons unrelated to study procedures. Complete clinical regression was detected in 84/94 (89.4%) superficial BCCs, and 12/23 (52.2%) nodular BCCs one month after 2 MAL-PDT sessions. No further clinical improvement was observed in either superficial or nodular BCCs with treatment continuation up to a maximum of 8 MAL-PDT sessions. Adverse effects were limited to mild local skin reactions, and cosmetic outcome was rated as excellent or good. Recurrence was observed in 2/84 (2.4%) successfully treated superficial BCCs at 6 and 12 months after treatment discontinuation. Based on the efficacy, tolerability, cosmetic outcome and recurrence rate, our results support the use of MAL-PDT for treatment of superficial BCC and for selected cases of nodular BCC.

Abstract

Inflammatory myopathies (IM) are acquired diseases of skeletal muscle comprising dermatomyositis (DM), polymyositis (PM), and inclusion-body myositis (IBM). Immunosuppressive therapies, usually beneficial for DM and PM, are poorly effective in IBM. We report the isolation and characterization of mesoangioblasts, vessel-associated stem cells, from diagnostic muscle biopsies of IM. The number of cells isolated, proliferation rate and lifespan, markers expression, and ability to differentiate into smooth muscle do not differ among normal and IM mesoangioblasts. At variance with normal, DM and PM mesoangioblasts, cells isolated from IBM, fail to differentiate into skeletal myotubes. These data correlate with lack in connective tissue of IBM muscle of alkaline phosphatase (ALP)-positive cells, conversely dramatically increased in PM and DM. A myogenic inhibitory basic helix-loop-helix factor B3 is highly expressed in IBM mesoangioblasts. Indeed, silencing this gene or overexpressing MyoD rescues the myogenic defect of IBM mesoangioblasts, opening novel cell-based therapeutic strategies for this crippling disorder.

Abstract

A review was done on studies concerning the effects of in-depth nonuniformity on optical absorption spectra of thin films. A relationship between the amplitude of interference fringes and depth profile of absorbing centers was established. These results were used to study photothermal spectra of amorphous silicon as well as surface/interface defects in alpha-Si:H. The analytical formula formed from the study was then applied in determining the shape of fringes.

Abstract

Background: Diabetic patients are considered less suitable than nondiabetic patients for β-blocker therapy because of the risk for worsened glucose and lipid metabolism and more severe hypoglycemic attacks. Objective: To compare the metabolic and cardiovascular effects of carvedilol with those of atenolol in diabetic patients with hypertension. Design: Randomized, double-blind, 24-week trial. Setting: University hospital clinic. Patients: 45 patients with non-insulin-dependent diabetes mellitus and hypertension. Intervention: After a 4- to 6-week run-in period during which placebo was given in a single-blind manner, patients were randomly assigned to carvedilol or atenolol. Measurements: An oral glucose tolerance test; assessment of insulin sensitivity and hormonal responses to insulin hypoglycemia; and assessment of lipid levels, blood pressure, left ventricular mass, and lipid peroxidation. Results: Changes in systolic and diastolic blood pressure and left ventricular mass index were similar with carvedilol and atenolol (P > 0.2). Fasting plasma glucose and insulin levels decreased with carvedilol and increased with atenolol. Responses to carvedilol were greater than those to atenolol, as follows: increase in total glucose disposal, 9.54 μmol/kg of body weight per minute (95% Cl, 7 to 11.9 μmol/kg per minute); decrease in plasma glucose response to oral glucose, 61 mmol/L x 180 minutes (Cl, -101 to -21 mmol/L x 180 minutes); decrease in insulin response to oral glucose, 6.2 nmol/L x 180 minutes (Cl, -9.8 to -2.6 nmol/L x 180 minutes); decrease in triglyceride level, 0.56 mmol/L (Cl, -0.75 to -0.37 mmol/L; P < 0.001); increase in high-density lipoprotein cholesterol level, 0.13 mmol/L (Cl, 0.09 to 0.17 mmol/L; P< 0.001); and decrease in lipid peroxidation, 0.25 μmol/L (Cl, -0.34 to -0.16 μmol/L). Conclusions: By improving glucose and lipid metabolism and reducing lipid peroxidation, carvedilol may offer advantages in patients with diabetes and hypertension.

225. Production and biologic interactions of prostacyclin and platelet-activating factor in acute myocardial ischemia in the perfused rabbit heart

Abstract

This study showed that the 1-O-hexadecyl form of platelet-activating factor (PAF) is released along with 6-keto-prostacyclin1 alpha (6-keto-PGF1 alpha) in the perfusates of ischemic-isolated rabbit heart. During the early phase of the reperfusion, the release of PAF and 6-keto-PGF1 alpha was particularly increased (PAF, from 5.4 +/- 1.2 to 19.7 +/- 2.0 ng/min; 6-keto-PGF1 alpha, from 2.3 +/- 0.1 to 27.4 +/- 1.8 ng/min) and this event was coupled with the characteristic mechanical alterations of myocardial performance and perfusion pressure (PP). These changes were effectively antagonized by pretreatment of the hearts with both prostacyclin (PGI2, 20 ng/ml) and the PGI2-releaser defibrotide (400 micrograms/ml). The protecting activity observed with PGI2 was paralleled by a reduction in the rate of PAF release during reperfusion (from 19.7 +/- 2.0 to 6.8 +/- 0.2 ng/min). In defibrotide-treated preparations, inhibition of PAF formation was associated with a pronounced stimulation of 6-keto-PGF1 alpha generation, which was particularly marked on reperfusion (from 27.4 +/- 1.8 to 197.5 +/- 8.2 ng/min). Indomethacin (1 microgram/ml) ablated the antiischemic effect of defibrotide but did not potentiate the rate of formation of PAF despite inhibition of 6-keto-PGF1 alpha biosynthesis. From these results, we conclude that in a model of severe myocardial ischemia, in which the major determinants of cardiac performance are under control, generation of PAF and PGI2 appears to play an important biologic role in determination of the severity of myocardial ischemic damage.

248. Clinical trial on Aveeno Skin Relief Moisturizing Lotion in patients with itching accompanied by skin lesions and xerosis.

Abstract

Background: Itch associated with cutaneous xerosis is a common condition, which can be effectively treated with the use of emollients combined with hydration therapy. The aim of our study was to evaluate the efficacy and tolerability of a new lotion containing menthol and colloidal oatmeal in patients with itch and cutaneous xerosis.Patients and Methods: Fifty-four patients with pruritus of different origin associated with lesions as erythema, desquamation, scratching, and lichenization were treated with Aveeno Skin Relief Moisturizing Lotion once daily for 3 weeks. Efficacy and tolerability were evaluated by comparison of changes from baseline in clinical appearance, hydration, and pH.Results: After treatment, clinical examination, and the self-administered questionnaire revealed a significant improvement of cutaneous lesions including erythema, scaling, scratching lesions, lichenization, and pruritus in 52 of 54 treated patients (96%). Complete regression of cutaneous lesions and pruritus was achieved in 48 of 54 (88.9%) patients; whereas a partial remission was observed in 4/54 (7.4%) subjects and no improvement in 2/54 (3.7%) subjects. An increase of cutaneous hydration of 35.9% after 1 week treatment and of 45.7% after 3 weeks treatment was observed; those increments were associated with a simultaneous decrease of cutaneous pH: -4.0% after one week and -6.3% after 3 weeks treatment. The lotion was well tolerated by all patients.Conclusions. Based on our experience, Aveeno Skin Relief Moisturizing Lotion appears to be an effective moisturizing emollient that reduces pruritus in subjects with xerosis. [ABSTRACT FROM AUTHOR]

250. Elevated plasma fatty acid concentrations prolong cardiac repolarization in healthy subjects.

Abstract

Background: High fatty acid concentrations have been shown to stimulate sympathetic nervous system activity, which may modify ventricular repolarization and thus the Q-T interval on electrocardiogram recordings.Objective: The aim of this study was to investigate whether acute elevations of plasma fatty acid concentrations influence the corrected Q-T interval (Q-Tc), Q-Tc dispersion, and sympathetic nervous system activity in healthy nonobese subjects.Design: Thirty-two healthy subjects ( +/- SD: 48 +/- 7 y of age) received an infusion of 10% triacylglycerol emulsion plus heparin (a bolus of 200 U followed by 0.2 Umin[-1] x kg body wt[-1] for 180 min); on another occasion and in random order, the same subjects received a saline infusion.Results: Compared with the saline infusion, infusion of 10% triacylglycerol emulsion increased plasma fatty acids (P < 0.001) and was associated with an increase in mean blood pressure (P < 0.05), heart rate (P < 0.05), Q-Tc (P < 0.01), Q-Tc dispersion (P < 0.01), and plasma epinephrine (P < 0.005). Furthermore, individual changes in plasma epinephrine correlated with changes in Q-Tc (r = 0.60, P < 0.001) and Q-Tc dispersion (r = 0.53, P < 0.02) even after adjustment for age, sex, and body mass index (P < 0.03 for all correlations).
