Abstract

Purpose: To evaluate differences in the characteristics and outcomes of intensive care unit (ICU) patients over time. Methods: We reviewed all epidemiological data, including comorbidities, types and severity of organ failure, interventions, lengths of stay and outcome, for patients from the Sepsis Occurrence in Acutely ill Patients (SOAP) study, an observational study conducted in European intensive care units in 2002, and the Intensive Care Over Nations (ICON) audit, a survey of intensive care unit patients conducted in 2012. Results: We compared the 3147 patients from the SOAP study with the 4852 patients from the ICON audit admitted to intensive care units in the same countries as those in the SOAP study. The ICON patients were older (62.5 ± 17.0 vs. 60.6 ± 17.4 years) and had higher severity scores than the SOAP patients. The proportion of patients with sepsis at any time during the intensive care unit stay was slightly higher in the ICON study (31.9 vs. 29.6%, p = 0.03). In multilevel analysis, the adjusted odds of ICU mortality were significantly lower for ICON patients than for SOAP patients, particularly in patients with sepsis [OR 0.45 (0.35–0.59), p < 0.001]. Conclusions: Over the 10-year period between 2002 and 2012, the proportion of patients with sepsis admitted to European ICUs remained relatively stable, but the severity of disease increased. In multilevel analysis, the odds of ICU mortality were lower in our 2012 cohort compared to our 2002 cohort, particularly in patients with sepsis.

Abstract

The geometric characterization and evaluation of structural integrity are challenges for the preservation of historical buildings. The challenges are associated to the lack of constructive records, material diversity, access and contact restrictions, and sample extraction. Non-destructive tests are indicated in these cases. The objective of this paper is to evaluate the Ground Penetrating Radar (GPR) technique with 1.2 GHz antenna in the study of the determination of material thickness and discontinuities in the masonry shells of the Theatro Municipal do Rio de Janeiro, a heritage built in early 20th century, with high historical and cultural value. With paintings by renowned artists on their intrados, the domes and vault of brick masonry were at risk of collapse in the 1970s due to differential settlements in the supports and moisture, compromising their paintings. A strengthening intervention was carried out in 1976. Considering the masonry of bricks the main component, the electromagnetic wave propagation velocity was determined as 0,15 m/ns in one of the support walls, allowing to estimate the thicknesses of the elements ranging between 20 and 31 cm. Materials incorporated into the structure were identified: reinforcement, metal inserts and bricks. However, the frequency of the antenna did not allow the detection of damages. The application of the GPR to the characterization of materials and investigation of the integrity of the domes of the Theatro Municipal highlights the potential of this technology to study heritage and other constructions. Resumo A caracterização geométrica e avaliação da integridade estrutural são desafios para a preservação de edifícios históricos. Os desafios estão associados à escassez de registros construtivos, diversidade de materiais, restrições de acesso, contato e extração de amostras. Nesses casos são indicados ensaios não destrutivos. Este trabalho tem como objetivo avaliar a técnica do Ground Penetrating Radar (GPR), com antena de 1,2 GHz, na determinação de espessuras e descontinuidades nas cascas em alvenaria do Theatro Municipal do Rio de Janeiro, patrimônio construído no início do século XX, de elevado valor histórico e cultural. Com pinturas de artistas renomados em seu intradorso, as cúpulas e abóbada em alvenaria de tijolos estiveram risco de colapso na década de 1970 devido a recalques diferenciais e umidade, comprometendo as pinturas. O reforço da estrutura com concreto projetado foi realizado em 1976. Considerando a alvenaria de tijolos o principal componente, a velocidade de propagação de onda eletromagnética foi determinada em 0,15 m/ns em uma das paredes de apoio, permitindo estimar a variação entre 20 e 31 cm nas espessuras dos elementos estudados. Foram mapeados outros materiais incorporados à estrutura como armadura, chumbadores e tijolos, porém a frequência da antena não permitiu a detecção de danos. A aplicação do GPR para a investigação de integridade e caracterização de materiais das cúpulas do Theatro Municipal mostra o potencial dessa técnica para estudos de patrimônio histórico e de outras construções.

Abstract

The objective was to investigate the usefulness of Near Infrared Spectroscopy (NIRS) to predict bromatological traits concentrations of gamba grass (Andropogon gayanus Kunth). Random samples from different plant genetic backgrounds, climatic conditions over years, experimental sites, and harvests were used for spectral reading and reference analysis. Partial Least Square (PLS) regressions were used for developing the models with 239 samples for calibration and 119 for external validation. Crude Protein, Ash, and Dry Matter models had R2= 0.91, 0.91, and 0.90 and low standard errors of calibration (SEC) of 8.7, 3.8, and 5.5 g.kg-1 , respectively. Neutral and Acid Detergent Fibers, and Organic Matter had R2 = 0.85, 0.88, and 0.89 and SEC valuesof10.2, 9.4, and 7.9 g.kg-1 , respectively. In vitro digestibility of dry matter had R2= 0.79 and SEC 28.0 g.kg-1 . Acid Detergent Lignin had a poor fit with R2= 0.36.A high correlation between NIRS predictions and wet lab data was observed. Models were adequate and accurate for predicting all bromatological traits but ADL. Significative differences among 51 half-sib families were detected for quality traits predicted throughPLS models. Abstract

A cultura da soja representa grande parcela econômica na produção agrícola Brasileira. No Rio Grande do Sul (RS) a cada safra verifica-se um aumento da área ocupada com o cultivo. No ambiente de terras baixas a cultura abrange 5,4 milhões hectares correspondendo a cerca de 20% da área total do Estado. O uso de cultivares transgênicas que tem como principal vantagem o controle de insetos alvo, além da redução do número de aplicações de inseticidas ao longo do cultivo. Porém, o alto custo do valor da semente transgênica é considerado a principal desvantagem na adoção dessa tecnologia. O objetivo deste trabalho foi avaliar o desempenho de cultivares RR1 e RR2 em relação à ocorrência de lagartas e percevejos, bem como avaliar o custo de produção. O experimento foi instalado na Embrapa Clima Temperado nas safras 2014/2015 e 2015/2016 com as cultivares convencional (BMX Potência (RR1 6.7)) e as isolinhas Bt (BMX Ponta (IPRO 6.6) e BMX Vanguarda (IPRO 6.0)). A cultivar BMX Potência apresentou maior incidência de lagartas desfolhadoras e o do percevejo Nezara viridula, sendo a cultivar mais indicada, entre as avaliadas, para o ambiente de terras baixas do Sul do RS, devido à produtividade e lucratividade. The cassava cultivars BRS 417, BRS 418 and BRS 419 are the first specifically developed for flour and starch production in the Cerrado biome of Central Brazil. Abstract

Many masonry historical bridges are still under service, particularly in Europe. Their significant cultural value, key role in transportation network systems and deterioration condition requires assessing their safety with respect to different scenarios, particularly earthquakes, which masonry structures are inherently vulnerable. This assessment requires collecting detailed information such as geometry, material properties, boundary conditions and existing damages to construct reliable numerical models. However, their significant cultural value prevents conducting destructive and even minor-destructive tests. The structural assessment of Barcelos Bridge, a medieval stone masonry arch bridge in northern Portugal, by means of a numerical approach is discussed in the current article. In this regard, a 3D advanced finite element model was prepared, in which the external geometry and internal morphology were extracted from laser scanning and ground penetrating radar survey, respectively. Then, outcomes of indirect sonic tests were adopted to characterize material characteristics and dynamic properties (i.e. frequencies and mode shapes) obtained from ambient dynamic identification were employed to update the FE model. Two load scenarios including gravity and lateral transversal loadings were taken into account to assess the performance of the bridge. Thus, incremental nonlinear pushdown and pushover analyses were performed, which resulted in evaluating current safety level of the bridges and possible failure modes., (undefined)

Abstract

Les femmes qui immigrent au Québec sont souvent jeunes et susceptibles de donner naissance en contexte migratoire. Beaucoup de professionnels de la santé sont amenés à travailler avec cette clientèle diversifiée dans un moment important de leur vie. Ces femmes se retrouvent souvent confrontées à des pratiques d’accouchement ou de maternage différentes de leur pays d’origine. À cela s’ajoute parfois un contexte de vulnérabilité lié à des conditions antérieures à l’immigration, notamment dans le cas des réfugiés, ou à une situation de précarité socio-économique. Alors comment créer des ponts permettant autant aux arrivantes qu’aux professionnels de bien se comprendre et d’harmoniser les exigences cliniques aux besoins ? C’est une des questions à laquelle cet article tente de répondre en mettant en lumière la réflexion de deux femmes immigrantes originaires du Sénégal et du Brésil qui ont coanimé pendant plus de cinq ans un atelier interculturel dans le cadre d’un cours en pratique sage-femme à l’Université de Trois-Rivières (UQTR). Les rencontres avec ces futures professionnelles de la santé avaient d’abord pour objectif de les sensibiliser sur les expériences de la périnatalité en contexte migratoire. Or les échanges ont mené les deux intervenantes à un processus réflexif suscitant une prise de conscience de leur propre transformation identitaire. Le partage de leurs expériences leur a permis d’en faire le bilan et de s’en distancier. Les auteures, formées en ethnologie de l’interculturel, ont eu recours à l’approche auto-ethnographique pour analyser de manière approfondie cette expérience. Celle-ci a contribué à une meilleure reconnaissance et insertion dans la société d’accueil. Outre sa valeur pédagogique, la participation à la formation de professionnels est aussi, au final, une manière de cheminer.

Abstract

Background: In a surgical setting, COVID-19 patients may trigger in-hospital outbreaks and have worse postoperative outcomes. Despite these risks, there have been no consistent statements on surgical guidelines regarding the perioperative screening or management of COVID-19 patients, and we do not have objective global data that describe the current conditions surrounding this issue. This study aimed to clarify the current global surgical practice including COVID-19 screening, preventive measures and in-hospital infection under the COVID-19 pandemic, and to clarify the international gaps on infection control policies among countries worldwide. Methods: During April 2-8, 2020, a cross-sectional online survey on surgical practice was distributed to surgeons worldwide through international surgical societies, social media and personal contacts. Main outcome and measures included preventive measures and screening policies of COVID-19 in surgical practice and centers' experiences of in-hospital COVID-19 infection. Data were analyzed by country's cumulative deaths number by April 8, 2020 (high risk, >5000; intermediate risk, 100-5000; low risk, <100). Results: A total of 936 centers in 71 countries responded to the survey (high risk, 330 centers; intermediate risk, 242 centers; low risk, 364 centers). In the majority (71.9%) of the centers, local guidelines recommended preoperative testing based on symptoms or suspicious radiologic findings. Universal testing for every surgical patient was recommended in only 18.4% of the centers. In-hospital COVID-19 infection was reported from 31.5% of the centers, with higher rates in higher risk countries (high risk, 53.6%; intermediate risk, 26.4%; low risk, 14.8%; P < 0.001). Of the 295 centers that experienced in-hospital COVID-19 infection, 122 (41.4%) failed to trace it and 58 (19.7%) reported the infection originating from asymptomatic patients/staff members. Because of their low mitosis rate, hemangioblastomas usually have slow-growing and late manifestations, but may cause sudden neurological symptoms if tumor hemorrhage occurs. Few studies have evaluated the impact of pregnancy on the evolution of hemangioblastomas. Some authors have reported tumor growth in women with VHL disease, but no such association was observed by others. The influence of pregnancy on sporadic hemangioblastomas remains largely unexplored. We report here the case of a pregnant woman whose first manifestation of sporadic spinal hemangioblastoma was life-threatening, rapidly progressive dysautonomia. In addition, we discuss the role of pregnancy in the triggering of symptoms, as well as the possibility of medically indicated delivery for the management of these tumors. Abstract

The study of the interaction of the selective inhibitor of osteoclastic V-ATPase (2Z,4E)-5-(5,6-dichloro-2-indolyl)-2-methoxy-N-(1,2,2,6,6-pentamethylpiperidin-4-yl)-2,4-pentadienamide (SB 242784) with lipid vesicles together with the study of the compound behavior in aqueous solution is reported. The results reveal that with regards to membrane interaction their properties would remain very similar for both molecules, indicating that the differences in inhibition efficiencies are solely a consequence of the molecular recognition processes within the inhibition site in the V-ATPase.
