217. Surviving sepsis campaign international guidelines for the management of septic shock and sepsis-associated organ dysfunction in children

Abstract

Objectives To develop evidence-based recommendations for clinicians caring for children (including infants, school-aged children, and adolescents) with septic shock and other sepsis-associated organ dysfunction. Design A panel of 49 international experts, representing 12 international organizations, as well as three methodologists and three public members was convened. Panel members assembled at key international meetings (for those panel members attending the conference), and a stand-alone meeting was held for all panel members in November 2018. A formal conflict-of-interest policy was developed at the onset of the process and enforced throughout. Teleconferences and electronic-based discussion among the chairs, co-chairs, methodologists, and group heads, as well as within subgroups, served as an integral part of the guideline development process. Methods The panel consisted of six subgroups: recognition and management of infection, hemodynamics and resuscitation, ventilation, endocrine and metabolic therapies, adjunctive therapies, and research priorities. We conducted a systematic review for each Population, Intervention, Control, and Outcomes question to identify the best available evidence, statistically summarized the evidence, and then assessed the quality of evidence using the Grading of Recommendations Assessment, Development, and Evaluation approach. We used the evidence-to-decision framework to formulate recommendations as strong or weak, or as a best practice statement. In addition, 'in our practice' statements were included when evidence was inconclusive to issue a recommendation, but the panel felt that some guidance based on practice patterns may be appropriate. Results The panel provided 77 statements on the management and resuscitation of children with septic shock and other sepsis-associated organ dysfunction. Overall, six were strong recommendations, 49 were weak recommendations, and nine were best-practice statements. For 13 questions, no recommendations could be made; but, for 10 of these, 'in our practice' statements were provided. In addition, 52 research priorities were identified. Conclusions A large cohort of international experts was able to achieve consensus regarding many recommendations for the best care of children with sepsis, acknowledging that most aspects of care had relatively low quality of evidence resulting in the frequent issuance of weak recommendations. Despite this challenge, these recommendations regarding the management of children with septic shock and other sepsis-associated organ dysfunction provide a foundation for consistent care to improve outcomes and inform future research., Author(s): Scott L. Abstract

Sheaves are objects of a local nature: a global section is determined by how it looks locally. Hence, a sheaf cannot describe mathematical structures which contain global or nonlocal geometric information. To fill this gap, we introduce the theory of "gleaves", which are presheaves equipped with an additional "gluing operation" of compatible pairs of local sections. This generalizes the conditional product structures of Dawid and Studen\'y, which correspond to gleaves on distributive lattices. Our examples include the gleaf of metric spaces and the gleaf of joint probability distributions. A result of Johnstone shows that a category of gleaves can have a subobject classifier despite not being cartesian closed. Gleaves over the simplex category $\Delta$, which we call compositories, can be interpreted as a new kind of higher category in which the composition of an $m$-morphism and an $n$-morphism along a common $k$-morphism face results in an $(m+n-k)$-morphism. The distinctive feature of this composition operation is that the original morphisms can be recovered from the composite morphism as initial and final faces. Abstract

Contexte. L’ostéoporose est une maladie liée au vieillissement, silencieuse jusqu’à la survenue de fractures liées à la fragilité osseuse et entraînant une dégradation de la qualité de vie et une augmentation de la morbi-mortalité. L’ostéoporose est plus fréquente chez la femme que chez l’homme. Différentes classes thérapeutiques sont proposées et les indications sont variables. Objectif. L’objectif était de faire un point actualisé sur les différentes prises en soin de l’ostéoporose, en soins primaires, et selon les différents niveaux de prévention. Méthodes. Revue narrative de la littérature. Résultats. L’ostéodensitométrie est l’examen de référence utilisé en dépistage, en diagnostic ou en suivi du traitement : examen imparfait, car seule la densité osseuse est évaluée et non l’architecture de l’os. D’autres examens sont à l’étude. L’activité physique a une place importante dans la prévention et le traitement du fait de sa balance bénéfice-risque favorable. Les biphosphonates sont la classe thérapeutique la plus évaluée, avec une balance bénéfice-risque modeste. Conclusion. La balance bénéfice-risque des différentes interventions doit être connue afin de proposer une prise en soin adaptée à chaque patient(e) et choisie dans le cadre d’une décision médicale partagée.

Abstract

We identify the analogues of KMS state in topos theory. Topos KMS states can be viewed as classes of truth objects associated with a measure \mu^\rho (in one-to-one correspondence with an original KMS state \rho) which satisfies topos condition analogues to the ordinary KMS conditions. Topos KMS states can be defined both externally or internally according to whether the automorphism group of geometric morphisms acts externally on the topos structure, or the automorphism group is an object in the topos itself, in which case, it acts internally. We illustrate the formalism by a simple example on the Hilbert space $\Cl^3$.

Abstract

Topos theory has been suggested first by Isham and Butterfield, and then by Isham and Doering, as an alternative mathematical structure within which to formulate physical theories. In particular, it has been used to reformulate standard quantum mechanics in such a way that a novel type of logic is used to represent propositions. In recent years the topic has been considerably progressing with the introduction of probabilities, group and group transformations. In the present paper we will introduce a candidate for the complex quantity value object and analyse its relation to the real quantity value object. By defining the Grothendieck k-extension of these two objects, so as to turn them into abelian groups, it is possible to define internal one parameter groups in a topos. We then use this new definition to construct the topos analogue of the Stone's theorem.

Abstract

The Pediatric Acute Lung Injury and Sepsis Investigators (PALISI) Network originated over 20 years ago to foster research to optimize the care of critically ill infants and children. Over this period, PALISI has seen two major evolutions: formalization of our network infrastructure and a broadening of our clinical research focus. First, the network is unique in that its activities and meetings are funded by subscriptions from members who now comprise a multidisciplinary group of investigators from over 90 PICUs all over the United States (US) and Canada, with collaborations across the globe. In 2020, the network converted into a standalone, nonprofit organizational structure (501c3), making the PALISI Network formally independent of academic and clinical institutions or professional societies. Such an approach allows us to invest in infrastructure and future initiatives with broader opportunities for fund raising. Second, our research investigations have expanded beyond the original focus on sepsis and acute lung injury, to incorporate the whole field of pediatric critical care, for example, efficient liberation from mechanical ventilator support, prudent use of blood products, improved safety of intubation practices, optimal sedation practices and glucose control, and pandemic research on influenza and COVID-19. Our network approach in each field follows, where necessary, the full spectrum of clinical and translational research, including: immunobiology studies for understanding basic pathologic mechanisms; surveys to explore contemporary clinical practice; consensus conferences to establish agreement about literature evidence; observational prevalence and incidence studies to measure scale of a clinical issue or question; case control studies as preliminary best evidence for design of definitive prospective studies; and, randomized controlled trials for informing clinical care. As a research network, PALISI and its related subgroups have published over 350 peer-reviewed publications from 2002 through September 2022.

Abstract

Photosynthetic algae have evolved mechanisms to cope with suboptimal light and CO2 conditions. When light energy exceeds CO2 fixation capacity, Chlamydomonas reinhardtii activates photoprotection, mediated by LHCSR1/3 and PSBS, and the CO2 Concentrating Mechanism (CCM). How light and CO2 signals converge to regulate these processes remains unclear. Here, we show that excess light activates photoprotection- and CCM-related genes by altering intracellular CO2 concentrations and that depletion of CO2 drives these responses, even in total darkness. High CO2 levels, derived from respiration or impaired photosynthetic fixation, repress LHCSR3/CCM genes while stabilizing the LHCSR1 protein. Finally, we show that the CCM regulator CIA5 also regulates photoprotection, controlling LHCSR3 and PSBS transcript accumulation while inhibiting LHCSR1 protein accumulation. This work has allowed us to dissect the effect of CO2 and light on CCM and photoprotection, demonstrating that light often indirectly affects these processes by impacting intracellular CO2 levels.

245. Group Action in Topos Quantum Physics

Abstract

Topos theory has been suggested first by Isham and Butterfield, and then by Isham and D\"oring, as an alternative mathematical structure within which to formulate physical theories. In particular it has been used to reformulate standard quantum mechanics in such a way that a novel type of logic is used to represent propositions. In this paper we extend this formulation to include the notion of a group and group transformation in such a way that we overcome the problem of twisted presheaves. In order to implement this we need to change the type of topos involved, so as to render the notion of continuity of the group action meaningful.

246. Review of the Topos Approach to Quantum Theory

Abstract

Topos theory has been suggested by D\"oring and Isham as an alternative mathematical structure with which to formulate physical theories. In particular, the topos approach suggests a radical new way of thinking about what a theory of physics is and what its conceptual framework looks like. The motivation of using topos theory to express quantum theory lies in the desire to overcome certain interpretational problems inherent in the standard formulation of the theory. In particular, the topos reformulation of quantum theory overcomes the instrumentalist/Copenhagen interpretation thereby rendering the theory more realist. In the process one ends up with a multivalued/intuitionistic logic rather than a Boolean logic. In this article we shall review some of these developments.

247. Non-Malleable Codes from the Wire-Tap Channel

Abstract

Recently, Dziembowski et al. introduced the notion of non-malleable codes (NMC), inspired from the notion of non-malleability in cryptography and the work of Gennaro et al. in 2004 on tamper proof security. Informally, when using NMC, if an attacker modifies a codeword, decoding this modified codeword will return either the original message or a completely unrelated value. The definition of NMC is related to a family of modifications authorized to the attacker. In their paper, Dziembowski et al. propose a construction valid for the family of all bit-wise independent functions. In this article, we study the link between the second version of the Wire-Tap (WT) Channel, introduced by Ozarow and Wyner in 1984, and NMC. Using coset-coding, we describe a new construction for NMC w.r.t. a subset of the family of bit-wise independent functions. Our scheme is easier to build and more efficient than the one proposed by Dziembowski et al., Comment: 12 pages

248. Approaches To Quantum Gravity

Abstract

One of the main challenges in theoretical physics over the last five decades has been to reconcile quantum mechanics with general relativity into a theory of quantum gravity. However, such a theory has been proved to be hard to attain due to i) conceptual difficulties present in both the component theories (General Relativity (GR) and Quantum Theory); ii) lack of experimental evidence, since the regimes at which quantum gravity is expected to be applicable are far beyond the range of conceivable experiments. Despite these difficulties, various approaches for a theory of Quantum Gravity have been developed. In this thesis we focus on two such approaches: Loop Quantum Gravity and the Topos theoretic approach. The choice fell on these approaches because, although they both reject the Copenhagen interpretation of quantum theory, their underpinning philosophical approach to formulating a quantum theory of gravity are radically different. In particular LQG is a rather conservative scheme, inheriting all the formalism of both GR and Quantum Theory, as it tries to bring to its logical extreme consequences the possibility of combining the two. On the other hand, the Topos approach involves the idea that a radical change of perspective is needed in order to solve the problem of quantum gravity, especially in regard to the fundamental concepts of `space' and `time'. Given the partial successes of both approaches, the hope is that it might be possible to find a common ground in which each approach can enrich the other.

249. Semiclassical analysis of the Loop Quantum Gravity volume operator: Area Coherent States

Abstract

We continue the semiclassical analysis of the Loop Quantum Gravity (LQG) volume operator that was started in the companion paper [23]. In the first paper we prepared the technical tools, in particular the use of complexifier coherent states that use squares of flux operators as the complexifier. In this paper, the complexifier is chosen for the first time to involve squares of area operators. Both cases use coherent states that depend on a graph. However, the basic difference between the two choices of complexifier is that in the first case the set of surfaces involved is discrete, while, in the second it is continuous. This raises the important question of whether the second set of states has improved invariance properties with respect to relative orientation of the chosen graph in the set of surfaces on which the complexifier depends. In this paper, we examine this question in detail, including a semiclassical analysis. The main result is that we obtain the correct semiclassical properties of the volume operator for i) artificial rescaling of the coherent state label; and ii) particular orientations of the 4- and 6-valent graphs that have measure zero in the group SO(3). Since such requirements are not present when analysing dual cell complex states, we conclude that coherent states whose complexifiers are squares of area operators are not an appropriate tool with which to analyse the semiclassical properties of the volume operator. Moreover, if one intends to go further and sample over graphs in order to obtain embedding independence, then the area complexifier coherent states should be ruled out altogether as semiclassical states.
