Background: Despite its poor predictability of survival, random pattern skin flap has been the most commonly used reconstructive method until recently. To overcome its shortcomings, there have been numerous pharmacologic trials and research to enhance survivability. The authors tried Korean red ginseng extract (KRGE), a well-known herb remedy for its outstanding angiogenecity and vasculogenic properties. Methods: A total of 36 male SD rats weighing 300 to 350 g were used. Rats were divided into three groups, and 12 rats were distributed into each group. Group 1 was the control, where the rats were not given KRGE, group 2 contained rats given 100 mg/kg KRGE for 2 weeks, and group 3 contained rats given 200 mg/kg KRGE over the 2-week period. Skin flap survival was measured after 7 days postoperatively. Also, using micro-SPECT-CT, we evaluated vasculogenesis by direct visualization on the third postoperative day. By immunohistochemistry, we confirmed the increase of new vessel formation, and from polymerase chain reaction studies, we investigated the overproduction of numerous growth factors. Results: Compared to the control group, the experimental groups showed significantly lower necrotic areas of the flap. It was possible to confirm that the oral intake of red ginseng helps flap survival in a random pattern animal model, as demonstrated by the increasing growth factor production and angiogenesis. Conclusions: The results provide a valuable reference for further preclinical studies of KRGE in flap surgery and various reconstructive methods in the plastic surgical field. Fibrous dysplasia (FD) is a rare, non-hereditary skeletal disorder characterized by its chronic course of non-neoplastic fibrous tissue buildup in place of healthy bone. A myriad of factors have been associated with its onset and progression. Perturbation of cell–cell signaling networks and response outputs leading to disrupted building blocks, incoherent multi-level organization, and loss of rigid structural motifs in mineralized tissues are factors that have been identified to participate in FD induction. In more recent years, novel insights into the unique biology of FD are transforming our understandings of its pathology, natural discourse of the disease, and treatment prospects. Herein, we built upon existing knowledge with recent findings to review clinical, etiologic, and histological features of FD and discussed known and potential mechanisms underlying FD manifestations. Subsequently, we ended on a note of optimism by highlighting emerging therapeutic approaches aimed at either halting or ameliorating disease progression. [ABSTRACT FROM AUTHOR]

The purpose of this research was to apply mathematical models to predict the effect of kimchi powder on the growth of spoilage microflora in pork snack sticks. Kimchi powder was added to the pork sticks at concentrations of 0% (control, C), 1% ( T1), 2% ( T2) and 3% ( T3) of total content. Microbiological analysis was performed at the end of the drying process (day 0) and after 15, 30, 45, 60 and 90 days of storage at 25C. The obtained data were applied to the Baranyi growth model. Application of up to 2% kimchi powder to pork snack sticks had no effect on the initial and maximal bacterial counts, but 3% of kimchi powder reduced the lag time ( tlag) and extended the maximum specific growth rate (μmax) of total plate count, anaerobic bacteria and lactic acid bacteria significantly. Mathematical models developed on the basis of the Baranyi model were useful in predicting natural microflora in the pork snack sticks during storage. Practical Applications Many kinds of jerky have become diversified and industrialized recently and kimchi, fermented vegetable, has been known to have various antimicrobial effects. Kimchi powder-treated pork jerky has higher consumer acceptability because of its spicy taste. However, the possibility of contamination of microflora in terms of food safety can be high through manufacture and distribution in industry. The predictive model suggested in this study can be used to control the possibility of microfloral contamination in restructured pork jerky products supplemented with kimchi powder as basic information. Interleukin-33 (IL-33) is an IL-1 family protein that induces a type-2 immune response. IL-33 is constitutively expressed in epithelial cells and released in response to the cell damage or stimulation by an allergen. The secreted protein is activated when the N-terminal domain is cleaved by a protease, and the active form signals downstream immune cells, such as eosinophils, by binding to the heterodimeric ST2:IL-1RAcP receptor complex on the cell surface. The binding stimulates an inflammatory response, and the abnormal inflammatory response can cause allergic diseases such as atopic dermatitis and asthma. Inhibition of the interaction between IL-33 and ST2 is an attractive target to control the inflammatory disease at the upstream of the signaling. However, discovering the chemical moieties that bind to the protein–protein interaction interface is a challenging task due to the relatively wide and shallow binding pocket compared to the enzyme's active site. For the IL-33-specific binder discovery, a series of IL-33 mutants were designed, and an electrophile chemical library was screened. Herein, we described the backbone 1H, 15N, and 13C resonance assignments of three IL-33 (117–270) mutants. Based on the assignments, the binding site of a selected compound by this approach was determined by 2D NMR. Summary: Background: The limb-threatening large soft tissue defects that occur on the feet of type 2 diabetic patients have complex causes and are less likely to be corrected by free flap reconstruction compared to those in non-diabetic patients. We retrospectively analysed factors affecting the success of free flap transfer for necrotising soft tissue defects of the lower extremities in patients with type 2 diabetes. Methods: This study included 33 diabetic patients whose feet were treated with free flap transfers. All patients had limb-threatening large soft tissue defects with tendon or bone exposure. The operative results were divided into three groups at 1 month post-operatively: the complete healing group, and the complication group, with either partial necrosis requiring additional simple procedures or flap failure with total necrosis. Nine preoperative factors were analysed: (1) ankle brachial index, (2) HbA1c, (3) BMI, (4) the smoking factor, (5) atherosclerotic calcifications (6) serum creatinine levels (>1.28 mg dL−1 vs. <1.28 mg dL−1), (7) GFR, (8) wound infection and (9) wound defect size. Results: Of the 33 patients, 15 showed complete healing and 18 showed complications of the free flap (eight partial necrosis and 10 flap failure). No atherosclerotic calcifications were found in the patients in the complete healing group, although they were found in 12 patients in the complication group, and this difference was significant (p = 0.002). Patients with serum creatinine levels >1.28 mg dL−1 had significantly higher free flap transfer complication rates than those with serum creatinine levels <1.28 mg dL−1 (p = 0.038). Conclusions: This study analysed the risk factors of free flap reconstruction for limb-threatening large soft tissue defects on the feet of type 2 diabetic patients. Serum creatinine levels >1.28 mg dL−1 and atherosclerotic calcifications were confirmed as risk factors for flap survival. [Copyright &y& Elsevier]

Intravenous ferric carboxymaltose (IV-FCM) can effectively correct perioperative anemia in patients undergoing major surgeries. However, its efficacy and side effects in patients undergoing free flap-based breast reconstruction are yet to be investigated. At our institution, from year 2020, patients with breast cancer undergoing abdominal free flap-based breast reconstruction were injected 500 mg of IV-FCM immediately post-operation. Propensity-matched 82 IV-FCM injected (study group) and 164 historical control group patients were retrospectively analyzed for transfusion rates, changes in hematological parameters, and flap or donor-site related complications. The major and minor complication rates related to the operation site were similar between the two groups. There was no significant difference in the transfusion rate between the two groups (control 29.9% vs. study 32.9%, p = 0.71). However, the total amount of transfusion required was significantly higher in the historical control group (control—53.2% 1 pack, 42.6% 2 packs, 4.3% 3 packs of RBC vs. Study—66.7% 1 pack, 33.3% 2 packs, p = 0.02) than in the study group. Additionally, the historical control group showed a significantly higher drop in red blood cell count, hemoglobin, and hematocrit levels from postoperative days 1–2 and 2–3 compared to the study group. Titanium mesh is an alloplastic material widely used for the reconstruction of moderate-to-large skull defects. Repeated wound problems or infection following these reconstructions inevitably lead to the replacement of the cranioplasty material. Among the various alloplastic materials, polycaprolactone implants are usually used for the coverage of small defects such as burr holes. 1 Herein, we present a case of a large cranial defect successfully reconstructed with three-dimensional-printed polycaprolactone implant and a free latissimus dorsi musculocutaneous flap. Background: Microneedles are tiny needles, typically ranging from tens to hundreds of micrometers in length, used in various medical procedures and treatments. The tested medical device named "CELLADEEP Patch" a dissolvable microneedle therapy system (MTS), made of hyaluronic acid and collagen. And the iontophoresis technique is also applied in the system. The study aimed to evaluate the effectiveness of the "CELLADEEP Patch" in skin improvement. Methods: Ex vivo human‐derived skin tissue models were used in this study and they were divided into three different groups, namely, the Untreated Group, the Negative Control Group, and the Test Group respectively. The Untreated Group received no treatment measures, the Negative Control Group was exposed to ultraviolet B radiation (UVB) irradiation, and the Test Group was exposed to UVB irradiation and treated with "CELLADEEP Patch". Skin moisture content, transdermal water loss, and skin elasticity were evaluated by three clinical devices. Additionally, histological staining and related mRNA expression levels were also analyzed. Results: The results of skin moisture content, transdermal water loss, and skin elasticity evaluation consistently illustrated that the application of "CELLADEEP Patch" led to remarkable skin improvement. And the analysis of histological staining images also confirmed the effectiveness of the "CELLADEEP Patch", especially for increasing collagen density. Moreover, the upregulation of Collagen type 1 a (COL1A1) and hyaluronan synthase 3 mRNA expression and the decrease of Matrix metalloproteinase 1 (MMP‐1) and Interleukin‐1 beta (IL‐1β) mRNA expression reflected its wrinkle improvement, moisturizing and anti‐inflammation function. Conclusion: "CELLADEPP Patch", the MTS combined with the iontophoresis technique, exhibits its effectiveness in moisturizing, skin elasticity improvement, and anti‐inflammatory function when applied to ex vivo human‐derived skin tissue models in experiments. In this study, scalp tissues from Korean adults between 20 and 80 without skin disease were used. Scalp tissues were processed, and hair follicles were isolated and cultured with different treatments (including Bioscalp, Ultra Exo Booster, and Ultra S Line Plus) from Ultra V company. Over 12 days, observations and measurements of hair follicle characteristics were recorded at intervals (Days 0, 3, 6, 9, and 12). The study assessed the impact of these substances on hair follicle growth and morphology. Bioscalp, combined with Ultra Exo Booster and Ultra S Line Plus, showed significant hair elongation in ex vivo. Preservation of hair bulb diameter was observed, indicating potential for sustained hair growth by exosome‐based products. The hair growth cycle analysis suggested a lower transition to the catagen stage in test products from Ultra V compared to non‐treated groups. The research findings indicated that the tested formulations, especially the combination of Bioscalp, Ultra Exo Booster, and Ultra S Line Plus, demonstrated significant effectiveness in promoting hair growth, maintaining the integrity of the hair bulb, and reducing the transition to the catagen stage. Purpose: We aimed to compare breast-related complications between hypofractionated adjuvant postmastectomy radiation therapy (PMRT) and conventional fractionated radiation therapy (RT) in patients with breast cancer undergoing reconstruction by reconstruction surgery type.Methods and Materials: Data from a total of 396 patients with breast cancer who underwent breast reconstruction after mastectomy between 2009 and 2018 were retrospectively reviewed. All patients received adjuvant PMRT according to either a conventional fractionation or hypofractionation schedule. We analyzed breast-related complications according to the timing of breast reconstruction (immediate or delayed). In cases of delayed reconstruction, only PMRT delivered before final delayed reconstruction was included. A major breast complication was defined as a breast-related toxicity that required reoperation or rehospitalization after the end of RT.Results: The median follow-up time was 35.3 months (range, 8.8-122.7 months). Of all patients, 267 received immediate breast reconstruction and 129 received delayed breast reconstruction. In patients with immediate breast reconstruction, 91 were treated with conventional RT and 176 received hypofractionated RT. The occurrence of major breast-related complications did not differ significantly between the 2 fractionation regimens. Hypofractionated RT did not increase major wound problems (infection and dehiscence) compared with conventional RT. Furthermore, major contracture occurred significantly less frequently in hypofractionated RT. Of the patients who had delayed breast reconstruction, 48 received conventional RT and 81 received hypofractionated RT. There was no difference in the incidence of major breast complications between these 2 RT groups, and no difference in major breast complications were reported for either 1- or 2-stage delayed reconstruction. A time interval of >10 months between PMRT and final definitive reconstruction had a significantly lower incidence of major breast complications.Conclusions: Hypofractionated RT appears to be comparable with conventional fractionated RT in terms of breast-related complications in patients with breast cancer undergoing reconstruction, regardless of breast reconstruction type. Fibrous dysplasia (FD) poses a therapeutic challenge due to the dysregulated extracellular matrix (ECM) accumulation within affected bone tissues. In this study, we investigate the therapeutic potential of 1,25-dihydroxyvitamin D3 (1,25(OH)2D3) in managing FD by examining its effects on FD-derived cells in vitro. Our findings demonstrate that 1,25(OH)2D3 treatment attenuates the pro-fibrotic phenotype of FD-derived cells by suppressing the expression of key pro-fibrotic markers and inhibiting cell proliferation and migration. Moreover, 1,25(OH)2D3 enhances mineralization by attenuating pre-osteoblastic cellular hyperactivity and promoting maturation towards an osteocytic phenotype. These results offer valuable insights into potential treatments for FD, highlighting the role of 1,25(OH)2D3 in modulating the pathological properties of FD-derived cells. [ABSTRACT FROM AUTHOR]

Fibrous dysplasia (FD) is a rare bone disorder characterized by the replacement of normal bone with benign fibro-osseous tissue. Developments in our understanding of the pathophysiology and treatment options are impeded by the lack of suitable research models. In this study, we developed an in vitro organotypic model capable of recapitulating key intrinsic and phenotypic properties of FD. Initially, transcriptomic profiling of individual cells isolated from patient lesional tissues unveiled intralesional molecular and cellular heterogeneity. Leveraging these insights, we established patient-derived organoids (PDOs) using primary cells obtained from patient FD lesions. Evaluation of PDOs demonstrated preservation of fibrosis-associated constituent cell types and transcriptional signatures observed in FD lesions. Additionally, PDOs retained distinct constellations of genomic and metabolic alterations characteristic of FD. Histological evaluation further corroborated the fidelity of PDOs in recapitulating important phenotypic features of FD that underscore their pathophysiological relevance. Our findings represent meaningful progress in the field, as they open up the possibility for in vitro modeling of rare bone lesions in a three-dimensional context and may signify the first step towards creating a personalized platform for research and therapeutic studies.
